한국형 환자안전문화 측정도구 개발 및 평가 (Development and Validation of the Korean Patient Safety Culture Survey Instrument for Hospitals)

  • Purpose: This study aimed to develop a survey instrument to assess the Patient Safety Culture in Korean hospitals and evaluate its validity and reliability. Methods: A preliminary instrument was developed through a literature review, focus group interviews, content validity testing, and pretesting for face validity. A total of 467 hospital employees participated in the psychometric testing. Validity and reliability assessments included content validity, construct validity, criterion-related validity, and internal consistency. Results: The Korean Patient Safety Culture Survey Instrument comprised 35 items across seven factors: leadership, patient safety policy and procedure, patient safety improvement system, teamwork, non-punitive environment, patient safety knowledge and attitudes, and patient safety priority. These seven factors contributed 60.98% of the variance of the total scale. Cronbach's alpha for internal consistency was .93; the seven factors ranged from .66 to .91. Conclusion: The results of this study showed that the Korean Patient Safety Culture Survey Instrument is reliable, valid, and suitable for measuring patient safety culture in Korean hospitals.

유아교육기관 교사의 안전교육 실시와 관련된 교육적 진단요인: PRECEDE 모형을 근간으로 (A Diagnostic Study of Teachers' Safety Education Activities in Early-child Care Centers: Based on the PRECEDE Model)

  • Objectives: The purpose of this study was to examine teachers' safety education activities to determine the significant educational diagnosis variables and to identify their needs of safety education in early-child care centers based on the PRECEDE model. Methods: A total of 304 teachers in early-child care centers participated in this study selected by a multi-stage stratified sampling method considering 11 regions in Seoul, Korea. Self-report type questionnaires were posted to all teachers in 220 early-child care centers by ground mailing service and the 304 teachers completed the questionnaires. The participants' responses were anonymously coded into and analyzed in SPSS program. Results: 'Scratch or bite' was the most frequent accident type(78.3%) and the frequent accident places were 'classroom(88.8%)' and 'playground(67.8%)'. The most frequently conducted safety education activities were 'reminding children their safe behaviors at the beginning and the end of daily class' and the next was 'saving a special time for safety education.' For educational diagnosis factors, related to safety education activities, teachers' safety education activity was more frequent when teachers' safety knowledge was high(p<.001), when teachers had good application skills of their knowledge to their teaching activities(p<.001), when they had strong needs on safety training opportunities(p<.05), and their interests on safety education(p<.001). For enabling factors, class preparation by safety education guide-book review(p<.001), by development of educational materials(p<.001), and by search for the related reference (p<.001), and by participation to safety education training programs for teachers(p<.01) were the significant enabling factors on teachers' safety class activities. For the reinforcing factors, the center-wide support of safety education brochures to children (p<.001), the concerns of centers utilizing safety education specialists(p<.001), and the concerns about safety information collection out of centers(p<.001) were significant factors related with teachers' safety education activities. Conclusions: The significant educational and institutional factors on teachers' safety education activities were teachers' concerns on safety education, their interests on safety knowledge, and the strong concerns on child safety education from the centers.

  • Recently, many safety measures are developing for the prevention of human error, which is main factors of railway accident. For the efficient management of human factors, many expertise on design, conditions, safety culture and staffing are required. But current safety management activities on safety critical works are focused on training, due to the limited resource and information. In order to establish railway human factors management, a systematic review model is required. Based on system engineering and nuclear industry model, a program review model is proposed in this study. The model includes operating experience review, task analysis, staffing and qualification, human reliability analysis, huma-system interface design, procedure development, training program, verification and validation, implementation and monitoring. Results can be applied for the review of safety measures relating to human factors.

제조업 근로자의 작업환경안전에 영향을 미치는 요인에 관한 연구 (A Study on the Factors Influencing Safeness of Work Environment in Manufacturing Industry)

  • The manufacturing industry is the one with the most frequent industrial disasters, and it is important to study the safety climate recognized by workers in the industry in order to actively prevent industrial accidents. However, little research has been performed on the safety climate in workplaces felt by the workers in the manufacturing industry. The purpose of this study is to identify the factors affecting work environment safeness based on practical analysis via survey, and to establish the relation between safety performance factors and safeness of work environment. The survey was performed for workers in the manufacturing industry(n=228), and structural equations model analysis by using the AMOS 7.0 was performed in order to identify the fit of the research model and the causation among factors. According to the results of the analysis, the reliability and the model fit were appropriate for interpretation, and safety participation was shown to affect safeness of work environment more than safety compliance. The results of this study may serve as the reference for taking a measure to improve the level of safeness of the work environment felt by workers in the manufacturing industry.

  • This paper presents an assessment system of safety level of 22.9kV grade high voltage electrical facilities considering environmental factors. The assessment system was developed based on the following procedure. Firstly, assessment structure was determined by consulting standards regarding inspection and diagnosis of electrical facilities. Secondly, contents of items and sub items of assessment system were developed. Thirdly, in order to quantify the importance of the assessment system, the weight was calculated using Analytic Hierarchy Process(AHP). Lastly, assessment table of safety level was developed including environmental factors such as period of use and load factor. The developed system can evaluate the safety level of high voltage facilities in an objective way. Therefore it can be applicable to electrical safety management system based on Internet of Things(IoT).

혈액투석실 간호사의 환자안전에 대한 위험요인 인식과 역량이 안전간호활동에 미치는 영향 (Effect of Dialysis Nurses' Perception of Patient Safety Risk Factors and Patient Safety Competency on Safety Nursing Activities)

  • Purpose: The study was conducted to investigate the correlation between perception of patient safety risk factors, patient safety competency, and safety nursing activities of nurses in hemodialysis units and identify factors affecting patient safety activities. Methods: The participants were 146 nurses from 16 hemodialysis units located in Gyeongsangnam-do. Data were analyzed using descriptive statistics, independent t-test, one-way ANOVA, Pearson correlation coefficient, and multiple regression analysis using the SPSS, version 24.0. Results: The mean safety nursing activity score was 3.47±0.38. safety nursing activities of the participants were significantly correlated with patient safety competency. The characteristics showing significant differences in safety nursing activities were educational level, hospital type, hospital work experience, number of hemodialysis treatment per day, number of hemodialysis treatment per nurse, educational experience of patient safety, presence of a patient safety incident report registration system, and direct registration of patient safety incident report. The multiple regression analysis revealed that the factors influencing safety nursing activities were patient safety incident report, patient safety competency, and number of daily hemodialysis treatment (<5~7 times/day) per nurse (R2=.34). Conclusion: The results of this study suggest that the safety nursing activities of hemodialysis unit nurses should be intensified. In addition, the registration system of patient safety incident report and nurses' competency on patient safety should be improved, and the number of hemodialysis per nurse should be fewer than 7 times per day.

신체장애인의 주거만족도 평가 요소 도출에 관한 연구 (A Study on Deriving Evaluation Factors for Housing Satisfaction of the Physically Handicapped)

  • This study is a literature study aimed to derive the evaluation factors for the survey of housing satisfaction of the physically handicapped. To attain the objective, the present study compared the influential factors of housing satisfaction for normal people and those for the physically handicapped found in the precedent researches, and analyzed them to draw out the evaluation factors for housing satisfaction of the physically handicapped. And then it drew out sub-factors and elements for the housing satisfaction of the physically handicapped. The evaluation factors for housing satisfaction of the physically handicapped are divided mainly into physical, social-psychological and economic factors. First, physical factor consists of 'functionality (usability)', 'safety', 'convenience', 'comfort', 'aesthetics', 'accessibility (mobility)', 'identifiability (recognition)'. Of them, 'functionality (usability)' factor consists of 3 sub-factors and 8 elements. 'Safety' was subdivided into securing safety and public order/crime prevention. and structural safety, and 7 elements were drawn out from 'safety'. 'Convenience' is sub-categorized into convenient usage and simple operation, and 9 elements were derived. 'Comfort' was classified into 2 sub-factors and 9 elements. 'Aesthetics' was classified into 2 sub-factors and elements. 'Accessibility (mobility)' was divided into 2 sub-factors and elements. 'Identifiability (recognition)' was classified into 2 sub-factors and 5 elements. Second, social-psychological factor includes 'sociality' and 'independence' factor. One sub-factor and one element were drawn from each of both factors. Third, 'economic factor' consists of 'economy'. One sub-factor and elements were drawn out from the factor. It is expected that the 10 evaluation factors, their sub-factors and elements that have been derived from this study can be used as the basic data and/or reference in designing residential space for the physically handicapped and making policy for it after verifying them in following studies. The present study finds itself significance in that the findings here can ultimately improve the housing welfare and life quality of the physically handicapped.

치기공과 학생의 임상실습 환경과 안전에 관한 연구 (A Study about Clinical Training Environment and Safety of Dental Technology Students)

  • Purpose: The intention of the study is to reveal the factors that influence the safety-behavior and safety-accident of the students of dental laboratory science. We intend to use the study as a basic data of searching effective ways to heighten the safety-behavior of clinical training and to prevent safety-accident. Methods: The survey was conducted on dental technology students. The collected data was analyzed by the statistical program SPSS 21.0. The results were analyzed by reliability, frequency, t-test, correlation, multiple regression. To test for significance on each item, p<0.05 has been decided as a standard. Results: The results of the study showed that the safety of the students was influenced by the school year, the leader of clinical training, clinical training environment and the experience of safety education. The safety-accident turned out to be influenced by the school year of the student and the safety behavior. Conclusion: Active leader of clinical training, clinical training environment that enables the safety-behavior, and the offering of the systematic safety education were the most important factors to heighten the safety behavior of the students and prevent the safety-accident. These factors were expected to not only induce the safety-behavior but also prevent the safety-accident as well.

중소 정형외과병원 간호사의 환자안전문화인식, 환자안전지식과 환자안전간호수행 (Perceptions of Patient Safety Culture, Safety Care Knowledge and Activity among Nurses at an Orthopedic Hospital)

  • Purpose: The purpose of this study was to identify the factors affecting the perceptions of patient safety culture, safety care knowledge, and safety care activity among nurses at orthopedic hospitals. Methods: Data were collected during Feb. 16 and Feb. 26, 2017, from 195 nurses of 9 small to medium sized orthopedic hospitals. Questionnaires about patient safety culture, safety care knowledge and activity were used. Data were analyzed by descriptive statistics, t-test, ANOVA, $Scheff\acute{e}$ test, Pearson's correlation coefficient, and stepwise multiple regression. Results: The safety care activity was positively correlated with perceptions of patient safety culture (r=.50, p<.001) and knowledge (r=.48, p<.001). Factors that had influence on the safety care activity were the patient safety culture (${\beta}=.30$, p<.001), age (${\beta}=.27$, p<.001), and knowledge of the safety activity (${\beta}=.21$, p=.004). The patient safety care activity was explained 36.6% by those factors. Conclusion: To enhance the patient safety care activity, it should be provided the environment and open communication for the perceptions of patient safety culture and the in service education program for safety care knowledge.

건축현장작업의 Human Factors 적용에 관한 연구 (A Study on the Application of Human Factors in Construction Work)

  • In current domestic construction field, several factors influencing the safety for field workers are various due to the size and complexity of construction works involved. Among the factors, the age is the important one, because the average age of workers is getting older due to the 3D phenomena in construction fields. The safety for workers of all ages is important, but especially safety for the old, is more important. Thus, the fundamental techniques for placement of field workers with adequate safety corresponding to complexity and hard works are investigated through analyzing fatigue and heart rate of individual worker, especially for the old.
