  • Objectives: This study was performed to analyze randomized controlled trial, which studied the effect of Saenghwa-tang treatment on postpartum prolonged lochia and uterine subinvolution. Methods: Researchers searched for randomized controlled trial of based on postpartum prolonged lochia, uterine subinvolution and Saenghwa-tang. The paper search was conducted through 6 online databases on August 10, 2023. Results: 8 studies were included after selection and exclusion criteria. 5 studies compared Saenghwa-tang alone with western medicine. 3 studies compared combined treatment of Saenghwa-tang and western medicine, with western medicine alone. Comparing with control group, the treatment group showed statistically significant improvement on total effective rate, uterine involution, serum fibrinogen, D-dimer, viscosity of blood and plasma, Erythrocyte aggregation, and various symptoms. Conclusions: This study suggests that Saenghwa-tang has benefit for treating prolonged lochia and uterine subinvolution. For reliable evidence, further research is needed to establish safety of Saenghwa-tang, standardize diagnosis criteria and specify the treatment course.

  • Objectives: We took breast milk samples and analyzed them using HPLC and LC/MS/MS, to evaluate the effects of taking Saenghwa-tang during breastfeeding on breast milk. Methods: The study participants were 20 lactating women who admitted in Korean medical postpartum care center. Breast milks were collected from paticipants who have been administrated Saenghwa-tang for more than 3 days. We used HPLC and LC/MS/MS for the determinations of amygdalin, liquiritins, 6-gingerol, decursin and decursinol angelate in Saenghwa-tang. Results: 1. Participants' $Mean{\pm}S.D$ (standard deviation) of age is $31.05{\pm}1.96$, and 15 participants had normal delivery and 5 participants had cesarean delivery. 12 participants were primipara and 8 participants were multipara. $Mean{\pm}S.D$ of lactating date is $9.4{\pm}0.94$. 2. Using HPLC, we learned LOQ level peak that matches the peak retention time of standard components of Saenghwa-tang was not detected from 20 breast milk samples. 3. Using LC/MS/MS, decursin of Angelicae Gigantis Radix was detected from HMSP 02, HMSP 04, HMSP 06, HMSP 11, and the each concentrations are 16, 2, 64, 11 ppb. Liquiritin of Glycyrrhizae Radix was not detected from HMSP 13~HMSP 18. Conclusions: Data obtained by this approach shows that this method is reliable and suitable for determining the safety of taking Saenghwa-tang during breastfeeding.

Results: 1. We selected total 48 studies. There were 40 clinical studies (25 case reports and 15 case series) and 8 animal experimental studies. 2. Of the 40 clinical studies, other treatments were used together with herbal medicine : acupuncture (28), chuna manipulation (3), and pharmacopuncture (5). 3. Of the 25 case reports, the most common symptoms were musculoskeletal symptoms (8), followed by mental disorders including postpartum depression (5). Various prescriptions of herbal medicine were used depending on the diseases or symptoms. 4. In 15 case series, 665 subjects in 11 case series had taken Saenghwa-tang-gagam within 1~2 weeks after childbirth, and the average duration of Saenghwa-tang-gagam administration were $9{\pm}4.97$ days. 498 subjects in 9 studies were breast-feeding, and there were no side effects reported. 5. All experimental studies were about the recovery, the immunity and the breast-feeding associated puerperal characteristics. Also, 3 studies used Saenghwa-tang as an intervention. Conclusions: Saenghwa-tang can be commonly used for the purpose of prevention and care of the postpartum symptoms within the first 1~2 weeks after childbirth for 10 days. Also, various prescriptions of herbal medicine may be used depending on the diseases or symptoms, and can be conducted with other treatments like acupuncture, cupping, moxibustion, pharmacopuncture and chuna manipulation.

  • Objectives : The aim of this study was to investigate the effect of Omibaekchul-san on postpartum edema in women hospitalized at korean medical postpartum care center in comparison with Saenghwa-tang. Methods : The body composition data was collected from 102 women, who took postpartum care in postpartum care center attached to Won-kwang University Jeonju Korean Medical Hospital, from March 2012 to July 2013. The data was analyzed by Body Composition Analyzer (Inbody 720). Specifically the data was composed of the Extra Cellular Fluid/Total Body Fluid (ECF/TBF), Total Body Water (TBW), Intra Cellular Water (ICW), Extra Cellular Water (ECW), Body Weight, Body Mass Index (BMI), Waist Hip Ratio (WHR), and the ECF/TBF of each body parts such as the upperlimb, lowerlimb and trunk. The body composition was measured twice during 2~8.5 days from childbirth and once again after 7~13.5 days from childbirth by Body Composition Analyzer. Results : The Omibaekchul-san -treated group showed a significant decrease of ECF/TBF, ECW, TBF, Body weight, BMI, ECF/TBF of each body parts and increase of WHR in comparison with the data of the Saenghwa-tang-treated group. In two groups, only the change of ICW was unsignificant. Conclusions : Statistically this study suggests that the Omibaekchul-san will help the quality of life of women who are in postpartum edema.

Among 6 cases, Korean medicine treatment including herbal medicine, acupuncture, electroacupuncture, moxibustion was used alone. Ikgibohyeol-tang-gami-bang, Gyejibokryeong-hwan, Saenghwa-tang-gami-bang were prescribed in 2 cases each. Angelicae Gigantis Radix and CV4 were most frequently used herb and acupoint. All subjects were successful in becoming pregnant. Conclusion: This review shows that Korean medicine treatment can be effective in infertile women with uterine myoma. However, the number of included studies is small, so the level of evidence needs to be raised through more clinical studies in the future.

  • Objectives: This study was performed to analyze the interventions of Oriental Medicine which had been commonly used for postpartum disease and postpartum care. Methods: We searched research on the interventions for postpartum disease and postpartum care in 4 domestic search engines. After that, we conducted eligibility screening based on inclusion and exclusion criteria. Results: 1. We selected total 50 studies. There were 2 randomized controlled trial (RCT), 5 non-RCT, 35 case reports, 8 case series within the 6~8 weeks after childbirth. 2. Of the 35 case reports, several interventions were used : acupuncture (22), moxibustion (11), cupping therapy (7), pharmacopuncture (5), chuna manipulation (4), herbal medicine (34). The most common symptoms were musculoskeletal symptoms (8), followed by postpartum depression (7). Various prescriptions and acupoints of oriental medicine were used depending on the diseases or symptoms. 3. Of the 8 case series, 382 subjects in 5 case series had taken Saenghwa-tang-gagam. And Acupuncture, moxibustion, cupping therapy (5), pharmacopuncture (1) were used as an intervention. 4. The most commonly used acupoint is 腎兪 (BL23) in the pain including postpartum back pain and 三陰交 (SP6), 關元 (CV4) in the postpartum care. 關元 (CV4) is the most commomly used moxibustion point not only the postpartum disease but also the postpartum care. Conclusions: In clinical studies of oriental medicine related to postpartum disease and postpartum care, pain-related clinical studies that belong to or progress to Sanhupung were the most common (30%), and among them, postpartum low back pain studies were the most common (20%). Based on this, we believe that large-scale clinical studies with high quality using oriental interventions including chuna and pharmacopuncture are needed to establish guidelines for the management of pain treatment including postpartum back pain.
