Characterization of Five Shu Acupoint Pattern in Saam Acupuncture Using Text Mininig (텍스트마이닝을 통한 사암침법 오수혈 사용 패턴 분석)

  • Background : Saam acupuncture were composed by applying the elemental concepts from the Five Phase theory - the relationships between the cycles such as Saeng(Sheng, 'nourishing' or 'creating') and Geuk(Ke, 'suppressing' or 'controlling') - onto the Five Phase points and 12 channels to compensate for the imbalance in each of the 12 main energy traits. Objective : The present study is aimed to find out the characteristics of Five Phase points pattern in Saam acupuncture. Methods : We analysed the characteristics of five elements of the Five Phase points in Korean medical texts such as Saamdoinchimguyogyeol, Dongeuibogam and Chimgugyeongheombang in mid Chosun Dynasty. Using non-negative factorization(NNMF) methods, we extracted the feature matrix of five elements of Five Phase points in each classic medical text. Results : In Saam acupuncture, two characteristics were most prominent: (1) "Self" component of Five elements, (2) "Mother" and "Grandmother" component of Five elements. Conclusions : Saam acupuncture used the combination of Five-Shu acupoint based on ZangFu pattern identification. Our findings suggest that grasping the characteristics of Five Phase points combinations can improve the understanding the selection of the relevant acupoints based on the ZangFu pattern identifications.

A Study on the Formation Process of Saam Acupuncture Method (사암침법(舍岩鍼法) 처방 형성 과정 고찰)

  • Objectives : The authors performed this study to further understand Saam acupuncture method in an aspect of formation process. Methods : We were finding Saam's idea in a preface of the book. And we analyzed Saam acupuncture patterns to understand their formation. Results & Conclusion : We assume that Saam acupuncture patterns has formed over time. We were able to split their patters into three phases. That is Primitive-phase, Basic-phase and Practical-phase. We think they are traces of Formation Process. In the first, Saam found DongHaeng-acupoints that have in common Five Element between the five viscera and acupoint. And then, he compounded other meridian's DongHeng-acupoints to control self meridian. Finally acupoints of self meridian were added.

Literature Review and Network Analysis on the Pain Disease Approach of Saam Acupuncture Method (사암도인침법의 통증 질환 접근법에 대한 고찰)

  • Objectives : Saam acupuncture initiated by Saamdoin in $17^{th}$ century is one of the most widely adopted acupuncture techniques used by Korean medical doctors in clinic. Our study aimed to analyze the application of the Saam acupuncture method to pain diseases based on the literature data. Methods : Based on the contents described in "(Do Hae Kyo Kam) Saam's acupuncture method", the texts related to pain condition were analyzed. The frequency of prescription of Saam acupuncture method was analyzed, and then the relationships between each acupoint were visualized by network analysis and hierarchical cluster analysis for the quantitative aspect. Results and conclusions: In our study, Lung tonifying and Liver tonifying acupuncture were the most frequently used method for the treatment of pain disease. As the acupoints, BL66 and SI5 were used the most frequently. It was found that visceral pattern identification was considered as the most important factor in the selection of the Saam acupuncture method. Network analysis and hierarchical clustering analysis showed that each acupoint was closely related to other acupoints, and most of them were connected more closely according to the method of Saam acupuncture operation. The experiential prescriptions of Saam acupuncture were classified as an independent group. In the future, fundamental research on the principle of Saam acupuncture method is needed for the various diseases, and research for the clinical efficacy and the mechanism of Saam acupuncture method should be preceded.

A Literature Study on the Combination of acupoints of HANGYEOK and YEOLGYEOK in the Saam Acupuncture Method (사암침법 한격과 열격의 경혈구성에 관한 고찰)

  • Objectives: The objective of this study is to review the methods of combining acupoints of HANGYEOK(Cooling Mode) and YEOLGYEOK(Heating Mode) in Saam Acupuncture Method. Methods: We searched and investigated the journals and literatures by OASIS(Korea Institute of Oriental Medicine) and relevant searching engines with the key word "Saam, Hangyeok, Yeolgyeok". Unfortunately there was no relevant articles on this theme except six literatures. Six literatures with the key words relevant to Saam and acupuncture were selected and analyzed according to each composition of acupuncture prescription especially focusing on HANGYEOK and YEOLGYEOK. Results: Analyzing HANGYEOK and YEOLGYEOK of 12 meridians, two different methods emerged. In HANGYEOK to treat Heat Pattern, one method is to tonify Water points and directly sedate Fire points. The other is also to tonify Water point, but sedate Earth points instead of Fire points. It is assumed that Earth sedation method is to eradicate the underlying Fire. In only Heart meridian and Pericardium meridian there is one method to treat Heat Pattern: Water tonification and Fire Sedation. In YEOLGYEOK to treat Cold Pattern, there is one method to tonify Fire points and sedate Water points. In Pericardium meridian and Triple Energizer meridian there are some different combination of acupoints depending on literatures. Conclusions: Considering HANGYEOK and YEOLGYEOK in Saam Acupuncture Method, two methods are mainly discussed in HANGYEOK; Water Tonification Fire Sedation and Water Tonification Earth Sedation. To remove the underlying Fire, it is regarded that Water Tonification Earth Sedation is appropriate and close to Saam's thought.

A Study on the Fixed- and Transformed-pattern of Saam Acupuncture Treatment (사암침법(舍巖鍼法)의 정형(定型)과 변형(變型)에 관(關)한 고찰(考察))

  • Objectives : The authors performed this study to further understand Saam acupuncture treatment in an aspect of the use of two patterns i.e. fixed- and transformed-pattern. Methods : The authors did documentary survey based on Do Hae Kyo Kam Sa Am Do In Chim Beop. 1. We investigated the frequency of the use of fixed- and transformed-pattern as well as the examples of use. 2. We surveyed related literatures and classified transformed-pattern. We did study on the principles of prescriptions of fixed- and transformed-pattern and researched the utilization. Results and Conclusions : 1. Fixed-pattern occupys 35.3% and transformed-pattern takes much more part. 2. Fixed-pattern is based on Nangyoung 69nan, and has the prescription of two enhancement and two inhibition by adaptation of Sangsaengsanggeuk(相生相剋) into self meridian and other meridian, and suggests the importance of the order of needling. 3. There are main points and assistant points in tonifying and suppressing of Fixed-pattern, accordingly, it has a system like Gunshinjwasa(君臣佐使). 4. Transformed-pattern is classified into following 3 types: first, the use of Nangyoung 75nan; second, the substitution of Junghuyl(井穴) with Hyunghyul(滎穴) or Haphyul(合穴) according to Nangyoung 73nan; third, the substitution of the same Osuhyul(五輸穴) in other meridians.

Telephone Survey for Developing Clinical Trial Protocol on Individualized Acupuncture Treatment for Knee Osteoarthritis (퇴행성 슬관절염의 맞춤형 침구 임상시험 프로토콜 개발을 위한 전화조사)

  • Objectives : This study was accomplished in order to find out how Korean medical doctors take acupuncture prescription for knee osteoarthritis in clinical practice Methods : The survey questions were developed by the consensus from 4 professors and 10 residents who major in acupuncture & moxibustion for developing clinical trial protocol on individualized acupuncture treatment for knee osteoarthritis. 206 persons having more than 10 years experience were randomly selected from a list of Korean medical doctors. Telephone interviews with them were conducted by the well-trained interviewers between May 21 and May 27 in 2005. Results : 139 of 206 Korean medical doctors made an acupuncture prescription prior to treatment of knee osteoarthritis. 103 of 139 Korean medical doctors used both local and remote points, and 26 of them used remote points only, and 10 of them used local points only. In case of doctors who use remote points, the five element (45 of 129) or Saam (28 of 129) acupoints were used dominantly, and its was based on the pattern identification of the Jangbu organs related with the pain (56 of 129) or of the pain location (35 of 129). Conclusion : This study showed that Koran medical doctors prefer to use remote points such as five element or Saam acupoints based on the pattern identification of the Jangbu organs related with the pain or the pain location. These result may be useful for developing protocols for clinical trial on acupuncture on knee osteoarthritis.

E-mail Survey for Developing Clinical Trial Protocol on Individualized Acupuncture Treatment for Knee Osteoarthritis (무릎 관절염의 맞춤형 침구 임상시험 프로토콜 개발을 위한 전자우편 설문 조사)

  • Objectives : This survey was accomplished to find out how Korean medical doctors take acupuncture prescriptions for knee osteoarthritis in real clinical practice. Methods : The survey questions were developed by the consensus from 4 professors and 10 residents who major in acupuncture & moxibustion for developing clinical trial protocol on individualized acupuncture treatment for knee osteoarthritis. The questionnaires were distributed via e-mail to 3,306 members of Korea Oriental Medical Association from March 15th to March 23rd in 2005.84 members completed answers, and the computerized data were analyzed by ISP statistical program. Results : 1. 68 out of 84 Korean medical doctors used pattern diagnosis. 2. 61 out of 84 Korean medical doctors used both local and remote points, 20 doctors remote points only, and 3 doctors local acupuncture points only. 3. In case of doctors who use remote acupuncture points only, the acupuncture prescription principle was Saam or five element acupuncture (66%), along the meridian pathway (14%), Eight constitutional acupuncture (11%), Taegeuk acupuncture (2%), and miscellaneous (18%). Conclusion : In our e-mail survey, Korean medical doctors who experienced more than 10 year practice answered that they use five element acupuncture or Saam acupuncture according to meridian pathway theory as the most common principle of their acupuncture treatment prescription.

A Literature Study on the Korean Acupuncture for Eye diseases (국내침구서적의 안질환(眼疾患)치료에 관한 문헌 연구 - "치종지남(治腫指南)" "동의보감(東醫寶鑑)" "침구경험방(鍼灸經驗方)" "교감(校勘) 사암도인침법(舍岩道人鍼法)"의 비교연구 -)

  • Background : Eye diseases refer to a wide range of disconveniences from conjunctivitis, pterygium, glaucoma to even blindness. Acupuncture has been widely used in Korea throughout the history and provides an efficient method in the treatment of them. Objectives : Establish a distinctive and efficient acupuncture method for the treatment of eye diseases based in literature research. Method : We reviewed four Korean medical literature, "Guide to Swollen Sore Treatment[治腫指南]", "Treasured Mirror of Eastern Medicine[東醫寶鑑]", "Experiential Prescriptions of Acupuncture and Moxibustion[鍼灸經驗方]", and "Essential Rhymes on Acupuncture and Moxibustion by Master Saam[舍岩鍼法]", and analyzed the therapeutic characteristics in the treatment of eye diseases. Result : 1. According to "Guide to Swollen Sore Treatment[治腫指南]", various methods were applied in the treatment of eye diseases. We can cite salt water washing method after needling, pricking bloodletting method using three-edged needle, surgery method using bent needle and lance needle, or sore treatment using sliced bean-curd and ground Aristolochiae Fructus among others. Acupuncture points like GV20[百會], BL1[睛明], EX-HN5[太陽], GB20[風池], GV24[神庭], GB1[瞳子髎], and GB15[臨泣] were mostly needled. 2. In "Treasured Mirror of Eastern Medicine[東醫寶鑑]", pricking bloodletting method were most frequently used in comparison to single acupuncture or moxibustion methods. Applied points were GV20[百會], BL1[睛明], LI4[合谷], EX-HN5[太陽], GB37[光明], BL18[肝兪], GB20[風池], BL2[攢竹], GB1[瞳子髎], and ST36[三里]. Also selections of adjacent points were considered important. 3. In respect to treatment methods "Experiential Prescriptions of Acupuncture and Moxibustion [鍼灸經驗方]" has some similarity to "Treasured Mirror of Eastern Medicine[東醫寶鑑]" as pricking bloodletting method were mostly used. Also focused on normal Qi flow through meridian. Points like BL18[肝兪], BL1[睛明], LU5[尺澤], EX-HN5[太陽], LI4[合谷] were used. 4. "Essential Rhymes on Acupuncture and Moxibustion by Master Saam[校勘舍岩道人鍼法]" considered visceral pattern identification method fundamental in the diagnosis and treatment of eye diseases. Specifically, Liver, Heart, Stomach, Lung, Kidney identification methods are presented. Combined both corresponding and connecting meridians supplementation and draining methods according to mother-child relation. Also Saam master's own experiential prescriptions are noted. Conclusions : After previous study on stroke, we could also find various efficient methods according to eye diseases, through literature research of korean medical classics. This study will concurrently result in establishing distinctive therapeutic method characteristic of Korea.

A Study on the Recognition and Actual Condition of Korea Medical Doctors in Oriental Medical Care of Hypertension (국내 임상한의사의 고혈압 한방치료 인식 및 실태조사)

  • Objectives : The study was accomplished to find out how korean medical doctor take oriental medical service for hypertension patients in clinical practice Methods : Survey questions were developed based on consensus of 2 clinician, 2 oriental internal medicine specialist and 1 acupuncture specialist. The list of the Korean oriental medical doctors is provided by the Association of the Korean Oriental Medicine. The questionnaire were distributed via E-mail to 9,465 members of Korean oriental medical doctors from 19th August 2008 to 11th September 2008. One thousand sixty three(11.24% of 9,465) members completed answer and the computerized data were analyzed by SAS statistical program Results : Fifty-one percent of Korean oriental medical doctors has experienced hypertension treatment. The most common medical diagnosis method was pattern diagnosis(64.2%). Saam and five element acupuncture were as frequent as 32.3% of acupuncture prescription principle. The most common acupuncture points were region of shoulder and back acupoints inclued $GB_{21}$, $GV_{14}$(25%). The most common herbs prescription principle were 'constitution prescription'(35.4%), 'pattern prescription'(12.6%). The most common opinion for the revitalization of oriental medical service were 'medical insurance system of herbal medicine'(28.8%) and 'research of oriental medical treatments in hypertensive patients'(26.8%). Conclusions : This survey provides unique insight into the perception, treatment method of the Korea medical doctor at oriental medical service for hypertension patient. This study provides a wealth of information and a fresh raft of questions which will inform future research and policy-making.
