A Survey about the recognition regarding the Korean acupuncture method and research direction (한국 침법에 대한 인식도 및 연구방향에 관한 설문 조사)

  • Obejectives : This survey was accomplished to find out how Korean medical doctors think that the recognition regarding the Korean acupuncture method and research direction Methods : The survey questions were developed by the consensus from the professors who major in acupuncture and moxibustion. The questionnaire was given the 2731 Korean medical doctors at the mending education site; In addition, it was given to 793 doctors working at the 105 Korean medical hospitals listed on the National Korean Medical Hospital Associations Address book by postal mail; and also it was given to the 142 public health care Korean medical doctors who attended the conference for the municipal and provincial representatives of the public health service Korean medical doctors. Total of 1277 questionnaires were retrieved out of the 3666 subjects. Results : 1. Eleven hundred eighth seven out of 1277 Korean medical doctors think motion and bone and joint disease which has an effect in acupuncture treatment(92.9%) 2. Nine hundred fourteen out of 1277 Korean medical doctors used "Body acupuncture method"(71.5%) and 902 out of 1277 Korean medical doctors think of the korean acupuncture method which is "Sa Am acupuncture method"(70.6%) 3. Eight hundred forty out of 1277 Korean medical doctors want verification which leads a clinical research to know that acupuncturist have acupuncture and moxibustion treatment(65.7%). Conclusion : Korean medical doctors think motion and bone and joint disease which has an effect in acupuncture treatment Korean medical doctors most used "Body acupuncture method" but think of the korean acupuncture method which is "Sa Am acupuncture method". Korean medical doctors want verification which leads a clinical research to know that acupuncturist have acupuncture and moxibustion treatment.

A Literature Study on the Korean Acupuncture for Headache and Vertigo (두통과 현훈 치료관련 침구문헌 비교연구)

  • Background : The varieties of manifestations referred to headache and vertigo have been long complicated physicians in the efficient diagnosis and treatment of these diseases. Objectives : Establish a distinctive and efficient acupuncture method for the treatment of headache and dizziness based in literature research. Method : We reviewed four Korean medical literature, "治腫指南Guide to Swollen Sore Treatment", "東醫寶鑑Treasured Mirror of Eastern Medicine", "鍼灸經驗方Experiential Prescriptions of Acupuncture and Moxibustion", and "舍岩鍼法Essential Rhymes on Acupuncture and Moxibustion by Master Sa-am", and analyzed the therapeutic characteristics in the treatment of headache and vertigo. Result : 1. "治腫指南Guide to Swollen Sore Treatment" inserted needle in GV20百會 and waited until the skin erupts and heat burns, then applied in those regions affected. 2. "東醫寶鑑Treasured Mirror of Eastern Medicine" mostly applied moxibustion on headache, however, needling acupoints such as GV20百會, GV24神庭, GV23上星, GV22顖會 were also considered important. 3. "鍼灸經驗方Experiential Prescriptions of Acupuncture and Moxibustion" used moxa cones on headache in order to disperse heat and direct Gi downward. 4. "校勘 舍岩道人鍼法Essential Rhymes on Acupuncture and Moxibustion by Master Sa-am" defined the cause of dizziness, wood excavates earth and earth kills water. Conclusions : After previous study on eye, oral, glottal, labial, dental diseases and stroke, we could also find various efficient methods according to headache and vertigo, through literature research of Korean medical classics. This study will concurrently result in establishing distinctive therapeutic method characteristic of Korea.

Two Case Reports of Facial Flushing of Women in Their Twenties Treated by Sasang Constitutional Medicine (사상방을 이용한 20대 여성 안면홍조 환자 치험 2례)

  • Objectives: The purpose of this study is to report the effect of Sasang constitutional medicine and acupuncture on facial flushing of women in their twenties. Methods: Two patients who had facial flushing were treated with Sasang constitutional medicine and acupuncture. In the first case, the patient diagnosed with Taeeumin was treated by Chungpesagan-tang and Sa-Am Liver-sedating Acupuncture. In the second case, the patient diagnosed with Soeumin was treated by Palmulgunja-tang, Sa-Am Spleen-tonifying Acupuncture and Seungyangsungi-tang which is not belong to Sasang constitutional medicine. The progress of symptoms was evaluated by NRS, frequency of facial flushing, duration time of facial flushing, amount of sweating accompanied by facial flushing. Results: After treatment, the patients' symptoms of facial flushing were improved. Conclusions: These case studies show that Sasang constitutional medicine and acupuncture are effective for the patient in their twenties who had facial flushing.

A Clinical Study on the Effects of Eo-Hyeol Bang for Patients with Lumbago Due to Traffic Accident (교통사고로 인한 요통환자의 어혈방(瘀血方) 치료효능에 관한 임상적 연구)

  • Objective : The purpose of this study is to observe the effects of Eo-Hyeol Bang(瘀血方) for the patients with low back pain due to traffic accident. Methods : Eo-Hyeol Bang based on Sa-Am Acupuncture treatment(four needle technic), consists of Taibai(SP3), Taiyuan(LU9), Quchi(LI11). In following research, 60 patients were divided into Group A and Group B of each 30 persons. Only Group A was treated with Eo-Hyeol Bang, Group B was treated with common acupuncture treatment. Results : In VAS, Group A is $5.77{\pm}1.96$ and Group B is $4.53{\pm}2.40$ (P=0.033). In Improvement index, Group A is $0.3191{\pm}0.1597$ and Group B is $0.2267{\pm}0.1297$ (P=0.017). Conclusions : As a result of each P-value, we knew that Eo-Hyeol Bang is more effective than another acupuncture technic.

The Literary Study on The Written Date of and The Background of Sa-Ahm's 5 Element Acupuncture Method (사암침법(舍巖鍼法)의 저작시기(著作時期) 및 형성배경(形成背景)에 관(關)한 연구(硏究))

  • The Sa-Ahm monk, who was believed to have existed in the middle of Cho-Seon Dynasty in Korea, have created an original Sa-Ahm's acupuncture method and written down, "Book of Sa-Ahm's 5 Element Acupuncture Method" using not only laws of creation and suppression in 5 element theory but also its main and other connected meridians according to the daignosis of visceras and bowels. Also the written time and background of its theory have not been known, and after obtaining Ji-San's book on Sa-Ahm's acupuncture method, I studied on them. The results are as follows : 1) The treatment chapter of Book of Sa-Ahm's 5 Element Acupuncture Method was written down by Sa-Ahm himself, and the chapter for Medical Experience was found to be written down by Ji-San. 2) It seemed that Sa-Ahm adopted the main ideas of "Eui Hak Jeong Jon(醫學正傳)" for the section of physiology, pathology and classification of disease symptoms the contents of which covered 40 of 43 parts, and the ideas of "Dong Eui Bo Gam(東醫寶鑑)" for YunGi treatise(運氣篇), and SangHan part(傷寒門) in classification of diease symptoms was intensified by the ideas of "Eui Hak Ip Moon(醫學入門)", and the contents of apoplexy, diseases characterized by acute diarrhea and vomiting, beriberi, diseases of oral cavity were intensified by the ideas of "Chim Ku Kyeong Hearn Bang(鍼灸經驗方)". 3) The written date of the Book of Sa-Ahm's 5 Element Acupuncture Method has been established as 1644 AD(22nd year of In Jo king of Cho-Seon Dynasty) after studying preface, contents and acknowledgement of Heo Im's "Chim Ku Kyeong Hearn Bang(鍼灸經驗方)". 4) The philosophic background of "Sa-Am Acupuncture Method(舍巖鍼法)" seems to be derived from the theories of "I Ching(周易)" and Yin Yang 5 Element Theory(陰陽五行學說), and to be influenced by such medical scholars as Tan Ke(丹溪), Woo Bak(虞搏), and Heo Im(許任).

  • Objectives: This study aims to identify effects of Korean medical treatment for stress urinary incontinence patient. Methods: The patient was treated by moxibustion at Shuidao (ST28), Sa-Am acupuncture lung tonification, Bojungikkitang-gamibang and Boshimdan for 2 weeks. These treatments are Korean medical treatment, and non-invasive. Results: After 2 weeks of Korean medical treatment, the symptoms of stress urinary incontinence disappeared. Conclusions: This case report shows that non-invasive Korean medical treatments are effective against stress urinary incontinence.

The Effects of Sa-am Acupuncture Simpo-jeongkyeok Treatment on the Blood Pressure, Pulse Rate, and Body Temperature

  • Objectives: The present study evaluated the effects of sa-am acupuncture (SAA) simpo-jeongkyeok (SPJK) treatment on the blood pressure (BP), pulse rate (PR), and body temperature (BT) of patients with hwa byung (HB). Methods: This patient assessor blind, randomized, placebo controlled trial included 50 volunteers, divided randomly into two groups. The treatment group underwent SPJK (PC9, LR1, PC3, KI10) while the control (sham) group received minimal needle insertion at non acupoints. The BP in both arms, PR, and BT at several acupoints were measured before and after treatment at the $1^{st}$, $2^{nd}$, $3^{rd}$, and $4^{th}$ visits and before treatment at the follow-up visit. We analyzed data by using the repeated measured analysis of variance (RM ANOVA), Mann-Whitney U, and wilcoxon signed rank tests; differences at P < 0.05 were considered significant. Results: No significant differences in the systolic blood pressure (SBP), diastolic blood pressure (DBP) and PR between the treatment and control group were observed at each visit. However, the decrease in the SBP for the treatment group before and after each visit was significantly higher than it was in the control group. The SBP in both arms in the treatment group was decreased between visits 1 and 2, 1 and 3, 1 and 4, and 1 and follow-up. The DBP in both arms and in the right arm between visits 1 and 3 in the treatment group showed decreases. A minimal BT increase for treatment at CV06 and CV12 and a minimal BT decrease for treatment at CV17 and (Ex) Yintang were found. Patients in the treatment group who visited more frequently experienced a greater decrease in the PR, but that effect was not maintained. Conclusion: The results suggest that SAA SPJK treatment has instant positive effects on the BP, PR, and BT in patients with HB, but the effects on the BP and PR are not maintained.

A Literature Study on the Korean Acupuncture for Oral, Glottal, Labial and Dental diseases (구설순치(口舌脣齒)질환에 대한 한국 침구서적의 치료법 비교 연구)

Acupuncture has been widely used in Korea throughout the history and provides an efficient method in the treatment of them Objectives : Establish a distinctive and efficient acupuncture method for the treatment of oral, glottal, labial and dental diseases based in literature research Method : We reviewed four Korean medical literature, "Guide to Swollen Sore Treatment", "Treasured Mirror of Eastern Medicine", "Experiential Prescriptions of Acupuncture and Moxibustion", and "Essential Rhymes on Acupuncture and Moxibustion by Master Sa-am", and analyzed the therapeutic characteristics in the treatment of oral, glottal, labial and dental diseases Result : 1. According to "Guide to Swollen Sore Treatment", we could noted frequent application of blood letting methods in the affected area, then rinse ones mouth with salty water and further application of taro plaster. Also found acupuncture methods utilizing heated mole cricket or silkworm in the sublingual region. Regarding herbal method, Realgar was rubbed in the affected area. Mainly used acupuncture points in the treatment of oral, glottal, labial and dental diseases were GV20, GB20, LU5, and auricular anterior hairline 2. In "Treasured Mirror of Eastern Medicine", blood letting method in the sublingual region and burning needle searing method were the most frequently applied in the treatment of oral, glottal, labial and dental diseases. Moxibustion was also applied in the treatment of labial and dental diseases. Particularly, said to apply 3 moxa cones in ear zones of both sides, when no medicine is effective. This demonstrates how emphasized the importance of moxibustion in this kind of disease. Mainly used acupuncture points were GV16, CV24, LI4, EX-HN12, and EX-HN13 3. In "Experiential Prescriptions of Acupuncture and Moxibustion", no other methods beside wrist. Superstitious methods like applying moxibustion on the tooth picture drawn on the roofing tile need further confirmation. Mainly used acupuncture points were LI4, ST36, and HT7. 4. "Essential Rhymes on Acupuncture and Moxibustion by Master Sa-am", identified the cause of diseases according to visceral pattern identification. Stomach and Spleen in charge of vocal, lingual, and labial disease, Kidney for dental disease, and further scrutinizing identification according to Liver, Heart, Stomach, Lung, and Kidney manifestations. Used supplementation and draining needling methods of self meridians and other correlating meridians. Conclusions : After previous study on stroke and eye diseases, we could also find various efficient methods according to oral, glottal, labial and dental diseases, through literature research of korean medical classics. This study will concurrently result in establishing distinctive therapeutic method characteristic of Korea.

  • Background : Stroke occurrences are prevalent and require intensive care for its treatment and rehabilitation. Acupuncture has been widely used in Korea throughout the history and provides an efficient method in the treatment of it. Objectives : To establish a distinctive and efficient acupuncture method for the treatment of stroke based in literature research. Method : We reviewed four Korean medical literature, $ll$治腫指南Guide to Swollen Sore Treatment$gg$, $ll$東醫寶鑑Treasured Mirror of Eastern Medicine$gg$, $ll$鍼灸經驗方Experiential Prescriptions of Acupuncture and Moxibustion$gg$, and $ll$舍岩鍼法Essential Rhymes on Acupuncture and Moxibustion by Master Sa-am$gg$, and analyzed the therapeutic characteristics in the treatment of stroke. Result : 1. In $ll$治腫指南Guide to Swollen Sore Treatment$gg$, various types of treatment can be found. Besides acupuncture, cupping, moxibustion with moxa tube, and bath therapy using Duchesneae Indicae Herba and Sal was applied. Needling in sublingual and vocal region, and also points like GB20, GB31, LI15, BL60, GV20, GV20, TE4, GB39, LU5, ST36, GB30 were frequently inserted. 2. In $ll$東醫寶鑑Treasured Mirror of Eastern Medicine$gg$, moxibustion treatment were in top priority. Points like GV20, LI15, LI11, GB31, ST36, GB39, LI4, GB20 pertinent to Governor, Conception, Gallbladder, Large Intestine, and Stomach meridian were most frequently needled. Selection of adjacent point was widely applied. 3. $ll$鍼灸經驗方Experiential Prescriptions of Acupuncture and Moxibustion$gg$ has some similarity compared to $ll$東醫寶鑑 Treasured Mirror of Eastern Medicine$gg$ in considering the moxa in top priority. But selected points far from the disease site. Main points were LI4, ST36, GB39, PC5, GV20, LI11, LR3, BL40, HT7. 4. $ll$校勘 舍岩道人鍼法Essential Rhymes on Acupuncture and Moxibustion by Master Sa-am$gg$ having the same content orders with $ll$鍼灸經驗方Experiential Prescriptions of Acupuncture and Moxibustion$gg$, it may had some influence from it. But the differences are also apparent. It emphasized in using mother-supplementing child-draining method, experiential prescriptions and visceral pattern identification. Conclusions : We could find various efficient methods through literature research of medical classics. This will not be limited in stroke alone but also will be applied in other diseases. This study will concurrently result in establishing distinctive therapeutic method characteristic of Korea.

A Study on the Sasang Constitutional application of Clinical Acupuncture (임상 침법의 사상의학적 활용)

  • Objective : This study aims to search Four-constitution Medicine based acupuncture by investigators and its effect on constitution-specific symptoms. Method : Among 463 oriental medical doctors who are the members of society of Four-constitution Medicine, 191 results are collected. The items of the questionnaire are sex, age, work experiences, working type, constitution diagnosis & prescription, the use of constitution acupuncture and so on. Result : The proportion of acupuncture treatment applied constitution-based acupuncture to patients are "Must Use" 12.6%, "Almost Use" 14.1%, "Sometimes Use" 42.9% and "Never Use" is 1.0%. The average ratio of patients treated by constitution-based acupuncture to all patients is 47%. 1. Digestive tract disease is found to be the most effective disease with constitution based acupuncture. Musculoskelectal problems, cardiovascular disease, respiratory disease, neuropsychiatric disease and urogenital disease are followed. On the oriental medicine, spleen system disease is the most frequently applied and heart system disease, neuropsychiatric disease, liver system disease, locomotorium disease and pulmonary system disease are followed. 2. Acupuncture therapeutic measures are different according to disease. For example, Sa-am acupuncture is the most widely used for spleen system disease, locomotorium disease, heart system disease, neuropsychiatric disease, pulmonary system disease. Eight constitution medicine acupuncture is effective for ophthalmic & otolaryngologic disease. Conclusion : This study is to investigate constitution-based acupuncture technique. In conclusion, the most widely used technique is Sa-am acupuncture and it is the most successful in treating digestive tract disease.
