SPF Evaluation and Safety of Sunscreen Products (자외선 차단 제품의 SPF 측정과 안전성)

  • Many methods of in vitro SPF evaluation have been developed in order to measure SPF of sunscreen products. In this study, we compared two spectrophotometric in vitro measurements using the skin cast made of Luviset CAP- $X^{R}$ resin and Transpor $e^{R}$ tape as substrate with two in vivo measurements using guinea pig and human. And we also observed the radiation reaching on the earth's surface in Seoul for one year and the change of the primary skin irritancy along the change of SPF value. According to the our research, the first, in vivo measurement using guinea pig had the close agreement with SPF measurement using human and showed the tendancy that guinea pig SPF was lower than human SPF. The second, there was the good relationship between in vitro SPF by using Transpor $e^{R}$ tape and in vivo SPF, and it is possible to predict human SPF from Protection Index(PI) by using tape. The third, it is thought that SPE 20 is reasonable value because of UV intensity in Seoul and the primary skin irritancy of higher SPF than 25. So we propose that it is important to distinguish the sunscreen product having higher than approximate 25 from the category of general cosmetics including lower SPF sunscreen product.oduct.

  • In this study, we evaluate the correlation between in vitro and in vivo determination of SPF of sunscreen products containing various ingredients depending on emulsification system. For in vitro approach, we determined SPF by the method of Diffey and Robson using an TransporeTM tape(3M Health care, USA) and SPF 290-analyzer(Optometrics Co. USA). SPF values and standard deviations are calculated and displayed after completion of the run. In vivo SPF values are determined according to KFDA (the Korea Food and Drug Administration) method in panels of Fitzpatrick's skin type II or III. We investigated the difference in SPF data of sunscreen ingredient according to emulsification system. The in vivo SPF data is high in water-in oil(W/O) emulsion than in oil-in water(O/W) emulsion samples. The difference may be due to the particular behavior in each vehicles and its presence on skin surface may produce a different sunscreen film. We obtained the corrlation coefficient between in vitro and in vivo SPF data for O/W (R-squre=0.72 )and W/O emulsion(R-squre=0.77). From these results, we suggest the improvement of methodology using Transpore$^{TM}$ tape as substrate to increase the predictability of in vitro method.d.

  • In recent years, the safety of sunscreens has been challenged based on the reports of their adverse effect on users; dermatitis, allergic contact dermatitis and photo allergic contact dermatitis. The unscientific idea that higher SPF sunsreen is good for health mealeads many users to tend to prefer higher SPF sunscreen. In the toxicological aspect, the need to investigate the safety of sunscreens is steadily increasing. However, there were few studies on the correlation between sun protection factor (SPF) and the safety of sunscreens. The objective of this study was to assess whether there was a correlation between SPF and the safety of sunscreens. We measured in vitro SPF index using homosalate as a standard and examined the toxicity tests such as primary skin irritation tests, ocular irritation test and skin sensitization test. Homosalate (HS), octyl methoxycinnamate (OMC), octyl salicylate (OS), octocrylene (OC) as UVB organic filter and benzophenone-3 (BP3), butyl methoxy dibenzoil methane (BMDM) as UVA organic filter, and titanium dioxide (TD), zinc oxide (ZO) as inorganic filters were used. The skin irritation indexes in rabbits treated with HS, OMC, OS, BP3, and BMDM were significantly increased as SPF indices were increased. Neither ocular irritation in rabbits nor skin sensitization in guinea pigs were increased. It suggests that there might be a good correlation between SPF and the skin irritation indices of organic UV filters and skin irritation might be one of most sensitive index to assess the safety of sunscreens.

Clinical Significance of S-Phase Fraction in Small Cell Lung Cancer (소세포 폐암에서 S-Phase Fraction의 임상적 의의)

  • Background: DNA content analysis of human solid tumor is now widely performed by flow cytometric study. One of the most interesting and potentially important observation in this field is that proliferative activity(S-Phase fraction of cell cycle) may profoundly affect the prognosis. Method: S-Phase fraction(SPF) have been measured by flow cytometric method using tumor cells isolated from paraffin embedded tissue. To evaluate the prognostic significance, SPF of small lung cancer cell was assessed in 42 patients who died after receiving anticancer chemotherapy. Results: 1) Mean survival time of patients with small cell lung cancer was 190(${\pm}156$) days. Survival time were shortened, when TNM stage and PS scale were advanced. 2) Mean value of SPF of patients with small cell lung cancer was 27.4(${\pm}8.5$)%. SPF had nothing to do with advance of TNM stage and PS scale. 3) In each identical TNM stage, there were not statistic significance between SPF and survival times. 4) There was a tendency like that higher SPF, better chemotherapeutic response. Conclusion: We could not find statistic significance between SPF and survival times, but SPF was a good predictive factor for chemotherapeutic response.

Development of a novel in vitro SPF evaluation method (새로운 in vitro SPF측정법 개발)

  • We have recently developed a novel in vitro SPF evaluation method with a high correlation coefficient (r=0.93) to in vivo SPF values(test sunscreen formulas : cress m, foundation). In this method, the in vitro SPF value was determined by the ratio of the time taken to achieve the minimal erythema dose with and without the sunscreens applied. We also reviewed the dilution and thin-layer methods for in vitro SPF evaluation, and investigated the relationship between irradiance(3$\times$10-3 2.5x 10-5W/cm2) and the degree of erythema. The degree of erythema was similar if t he radiation exposure(40mJ/cm2) was kept constant. It was obtained more than 0.90 correlation coefficient when it was compared the results by our new method with those from SPF-290 analyzer (Optometries, U.S.A.).

Relationship between S-Phase Fraction and Survival Time in Patients with Primary Squamous Lung Cancer (편평상피성 폐암에 있어서 S-Phase Fraction과 생존기간과의 관계)

To evaluate the prognostic significance, SPF of squamous lung cancer cell was assessed in 21 patients who died without any specific treatment. Results: 1) Mean survival time of squamous lung cancer patients was 225(${\pm}162$) days. Survival time were shortened, when TNM stage and PS scale were advanced. 2) Mean value of SPF of squamous lung cancer patients was 23.4(${\pm}11.3$)%. SPF had nothing to do with advance of TNM stage and PS scale. 3) Mean survival time of high SPF group(more than 20% of cell proliferation cycle) and low SPF group were 153(${\pm}99$) days and 342(${\pm}180$) days(p<0.01). In each identical TNM stage and PS scale, there were also statistic significant differences in mean survival time between high and low SPF group. Conclusion: On multivariate analysis including TNM stage and performance status, SPF was the significant and independent prognostic factor in the primary squamous lung cancer patients group.

  • The present study was undertaken to develop the in vitro sunprotection factor(SPF) measurement method having good correlation with in vivo method. As in vitro method, 8% homomentyl salicylate, P3 reference standard and sunscreens on market were measured using SPF 290 analyzer and were compared the SPF with labed value. In vitro SPF of 8% HMS and P3 reference standard were 4.59 $\pm$ 0.12 and 14.94 $\pm$ 0.83. There are good correlation, correlation factor were 0.9506 and 0.9769 respectively, between the in vitro and in vivo SPF for the sunscreen creams and lotions examined. Correlation factors of makeup base & liquid goundation, lotion labled with “shake before use”, compact powder were 0.8812, 0.8632 and 0.5984. The best sample applied method of compact powder was 1:0.8 mixture with cream base. These results suggest that the in vitro test method could be used as an alternative method for SPF.

Adult Trauma Patients with Isolated Thoracolumbar Spinous and Transverse Process Fractures May be Managed Conservatively to Improve Emergency Department Throughput

  • Purpose: Traumatic vertebral injuries have a prevalence of 4-5% at level I centers. Studies have demonstrated that isolated thoracolumbar transverse process fractures (iTPF) rarely require brace or surgical interventions. We hypothesized that similarly isolated thoracolumbar spinous process fractures (iSPF) would have less need for bracing and operative interventions than SPFs with associated vertebral body (VB) fractures (SPF+VB). We performed a similar analysis for iTPF compared to transverse process fractures associated with VB injury (TPF+VB). Methods: In this single-center, retrospective study from 2012 to 2016, patients were classified into iSPF, SPF+VB, iTPF, and TPF+VB groups. Data including the fracture pattern, neurologic deficits, and operative intervention were obtained. The primary outcome studied was the need for bracing and/or surgery. A statistical analysis was conducted. Results: Of 98 patients with spinous process fractures, 21 had iSPF and 77 had SPF+VB. No iSPF patients underwent surgery, whereas 24 (31.17%) SPF+VB patients did undergo surgery (p=0.012). In the iSPF group, three patients (15%) received braces only for comfort, whereas 37 (48.68%) of the SPF+VB group required bracing (p=0.058). Of 474 patients with transverse process fractures, 335 had iTPF and 139 had TPF+VB. No iTPF patients underwent surgery, whereas 28 (20.14%) TPF+VB patients did (p≤0.001). Of the iTPF patients, six (1.86%) were recommended to receive braces only for comfort, while 68 (50.75%) of the TPF+VB patients required bracing (p<0.001). Conclusions: No patients with iSPF or iTPF required surgical intervention, and bracing was recommended to patients in these groups for comfort only. It appears that these injures may be safely managed without interventions, calling into question the need for spine consultation.

In vitro SPF Measurement of Sunscreen Agents in Cosmetics (화장품에서 자외선 차단제의 in vitro SPF 측정)

  • This study evaluated SPF (Sun Protection Factor) of various quntity and kind of sunscreen ingredients which is used in O/W and W/O emulsion by systematic and quantitative approach. Octylmethoxycinnamate(OMC), Buthylmethoxydibenzoylmethan (BMDM), Octocrylene (OC), Octylsalicylate(OS), Octyl Triazone (OT), Titanium dioxide (TD) are used for the experiment. As a result, when different chemical sunscreen ingredients are added to OMC, the synergy effect of SPF was high in order of BMDM, OC, OS, OT. There was no significant difference in O/W and W/O emulsions. It can be a guide to use sunscreen ingredients effectively when the relation between the results of in vitro SPF and in vivo SPF is comprehended.

Studies on the Correlation between SPF index and Primary Irritation Index of Octylmethoxycinnamte (Octylmethoxycinnamate의 SPF 지수와 피부자극지수와의 상관성 연구)

In the toxicological aspect, the need to investigate the safety of sunscreens is steadily increasing. However, there were few studies on the correlation between sun protection factor (SPF) and the safety of sunscreens. The objective of this study was to assess whether there was a correlation between SPF and the skin irritation index of sunscreens. Octyl methoxycinnamate (OMC) was selected, which was the most frequently used (about 92% in korea and about 90% in the world) in sunscreens. In vitro SPF test was performed using the standard samples recommended by KFDA guideline. We examined primary and cumulative skin irritation and eye irritation of OMC standard sample as well as homosalate. As in vitro SPF was increased, skin irritation index was increased in primary skin irritation test ($r^2$=0.986) and cumulative skin irritation test ($r^2$=0.939). In addition, OMC was non-irritating and non-sensitizing. In this results, there was a good correlation between SPF index and skin irritation index.
