A Case of Relapsing Polychondritis Involved Tracheobronchial Tree (기관과 기관지를 침범한 재발성 다발성 연골염(Relapsing Polychondritis) 1예)

  • Relapsing polychondritis is a systemic disorder characterized by recurrent inflammation and degeneration of cartilaginous tissue throughout the body. The association with HLA-DR4 and the occurrence of antibodies to type II collagen and other autoantibodies suggest that an immunologic mechanism is involved in its pathogenesis. The eyes, ears, nose, larynx, trachea and articular areas are commonly involved. Airway narrowing or collapse from respiratory tract involvement occurs in up to 50% of patients with relapsing polychondritis. Treatment consists of administration of corticosteroids and other anti-inflammatory and immunosuppresive drugs. We experienced a case of relapsing polychondritis involving the tracheobronchial tree, nose and ears in a 49-year-old woman. The patient was clinically and histologically diagnosed as relapsing polychondritis according to McAdam's and Damiani's criteria. We report this case with a review of the literature.

  • Camera for QUasars in EArly uNiverse (CQUEAN) is an optical CCD camera system made by Center for Exploration of the Origin of the Universe (CEOU). CQUEAN is developed for follow-up observation of red sources such as high-redshift quasar candidates ($z{\geq}5$), gamma-ray bursts (GRB), brown dwarfs and young stellar objects. The CQUEAN is composed of a science camera with deep-depletion CCD chip which is sensitive at around $1{\mu}m$, a set of custom-made wide-band filters for detection of quasar candidates at z~5, and a guide camera. A focal reducer was developed to secure $4.8^{\prime}{\times}4.8^{\prime}$ field of view, and an in-house user software for efficient data acquisition. CQUEAN was attached to 2.1m Otto Struve Telescope in McDonald Observatory, USA, in August 2010. About 1000 quasar candidates including 3 confirmed with follow-up spectroscopy, have been observed so far, and many high-z galaxy cluster candidates, GRBs and supernovae were also observed. And monitoring of HBC 722, a young stellar object, is under way since 2011. Further enhancement of CQUEAN including the introduction of narrow-band filters is planned.

Approaches to Improving Production Efficiencies of Transgenic Animals

  • Transgenic animals are very useful for scientific, pharmaceutical, and agricultural purposes. In livestock, transgenic technology has been used forthe genetic alteration of farm animals, the production of human proteins inlarge quantities in the milk of transgenic farm animals, and the generation of animals with organs suitable for xenotransplantation. To date, the transfer of foreign genes into farm animals has been performed mainly by microinjection of DNA into the pronuclei of fertilized eggs. However, the overall success rate of transgenic animals in livestock so far has been disappointingly low, eg., the efficiency is 0∼5% in swine, and less than 1% in sheep and cattle, compared with the rate in mice where 5% microinjected develop into transgenic animals. Recently, McGreath et al. (2000) have succeeded in producing the gene targeted sheep by the use of nuclear transfer from cultured somatic cells transfected with a foreign gene in vitro. However, we may need plenty of time until currently employ this method for gene transfer to farm animals. We have been studying to exploit the method for improving production efficiencies of transgenic animals with emphasis of its application to farm animals. The present paper describes three approaches that we have made in our laboratory to improve production efficiencies of transgenic animals, based on the DNA microinjection method. 1. Co-injection of restriction enzyme with foreign DNA into the pronucleus for elevating production efficiencies of transgenic animals. 2. Efficient selection of transgenic mouse embryos using EGFP as a marker gene. 3. Phenotypes of tansgenic mice expressing WAP/hGH-CAG/EGFP fusion gene produced by selecting transgenic embryos. 4. Efficient site-specific integration of the transgene targeting an endogenous lox like site in early mouse embryos.

A Study about the Mediating Effects of Website Attitude and Trust in Continuously using the Website (웹사이트의 계속적 이용에 있어 신뢰와 태도의 매개효과 연구)

  • In the past days, most of studies about users' behavior in online environments have been mainly focused on the adoption of website and internet shopping or shopping mall. But the generic model of website usage behavior has not been made and the leading theory of website usage behavior has not been existed. The purpose of this study consists of two parts: (1) to find the determinant variables of the website users' continuous using Intention in a certain website, (2) to examine the relative influences of the variables influencing the users' behavior in online environments. The framework of this study is based on the combination of some theories - Davis(1989)'s TAM, Fishbein(1975)'s TRA, Hoffman et al.(1997), Schneiderman(2000), Yoon(2002), Lee and Turban(2001), Sultan et al.(2002), Shankar et al.(2002a, 2002b), McKnight et al.(2002), Chen and Dhillon(2003), Heijden(2003) and so on. Major findings of this research are summarized as fellows: First, determinant of website continuous using intention was affected by 1) the-attitude and 2) the trust of the website except of perceived usefulness. Second, the website attitude was affected by the perceived usefulness, design, experience, reputation. But perceived ease of use did not affect the website attitude. Third, the website trust was affected by the perceived usefulness, perceived ease of use, design, experience of website. But the reputation of website did not determine the trust of Website in this research model.

STUDY ON VERTICAL DISPLACEMENT OF SOFT TISSUE UNDER DISTAL EXTENSION PARTIAL DENTURE BASE BY FUNCTIONAL IMPRESSION (유리단 국소의치의 기능 인상에 의한 연조직의 수직적 변위량에 관한 연구)

  • Distal extension partial dentures are supported by both the relatively rigid teeth and the resilient mucosa. So impression techniques of residual alveolar ridge in case of distal extension partial denture have particular importance in order to broad distribution of the masticatory force. McLean recognized the need for recording the tissues supporting distal extension partial denture base in functional form to equalize the resilient and non-resilient support, and this was called functional impression. Many investigators proposed various techniques of the functional impression for a distal extension partial denture, but only a little studies were performed about displacement of soft tissue under distal extension partial denture base. The purpose of this study is to investigate the amount of vertical displacement of the soft tissue under distal extension partial denture base by different functional impression techniques. Impression techniques used were Z.O.P. Impression, Selective Tissue Placement Impression, Functional Relining Impression. Measurement of the vertical displacement of soft tissue were made with Depth Gauge and Measuring Platform. A Anatomic Impression was used as a control. The results were tested statistically using 3 way ANOVA and Scheffe test. The followings were the results obtained from this study. 1. The greatest amount of soft tissue displacement was observed in the center of the retromolar pad. 2. No significant differences were found between the crest of alveolar ridge and the buccal shelf area. 3. The greatest soft tissue displacement was observed in Functional Relining Impression using Iowa wax, and the least displacement was observed in Selective Tissue Placement Impression using murcaptan rubber base. 4. No significant differences were found between finger pressure and biting pressure in Z.O.P. Impression, but greater displacement was observed by biting pressure than finger pressure in Functional Reling Impression.

Individualized Traditional Korean Acupuncture for Knee Osteoarthritis : a Protocol for a Randomized Controlled Trial

  • Objective : To test the hypotheses that individualized traditional Korean acupuncture improves pain and disability in patients with osteoarthritis of the knee and that benefits remain after stopping treatment more so than is the case for standardized minimal acupuncture. Design : Randomized single blind controlled trial with two intervention arms (individualized traditional Korean acupuncture, standardized minimal acupuncture) of six weeks' duration and three months follow-up. Setting : Acupuncture interventions were applied by two training doctors in the Department of Acupuncture and Moxibustion in a 1000-bed hospital. Assessment of the result was performed in a university-based laboratory. Participants : 50 patients with symptoms of knee osteoarthritis as diagnosed by an orthopedist. Intervention : Individualized traditional Korean acupuncture or standardized minimal acupuncture for six weeks. Main outcome measures: Primary outcome measure was pain as measured by the visual analogue scale. Secondary measures of pain and disability included the Western Ontario and McMaster Universities (WOMAC) index, Short Form-36 (SF-36), Lequesne Functional Index (LFI) score and Korean version of Health Assessment Questionnaire (KHAQ). Discussion : This paper presents detail on the rationale, design, methods and operational aspects of the trial.

  • Herniate disc disease is one of the biggest problem in claim of insurance as well as in medical. Herniate disc disease have recently increased, and it is ranked 8th in claim reasons recently. As an underwriter and physical therapist, I want to study interrelationship of insurance and disc disease. I think it is necessary to know about knowledge of medical, so this study is given some space to structure of spine, cause of herniated disc disease, role of disc, methods of classification of disable (McBride method and AMA method), and spine disability stage. disc surgery is divided laparoscope disc surgery and spine surgery. I analysis it some factors- gender, age, occupation, re-surgery, and state of after surgery - through searching medical papers. I suggest below conclusion to underwriter because it can be useful to make questionnaire, and underwriter can expect prognosis. conclusion The negative factors of disc surgery (compare to other cases) were as follows: 1. endoscope disc surgery: $20{\sim}40year$ old man, hospitalization period more than 5 days. 2. spine surgery: $45{\sim}70year$ old woman, hospitalization period more than 15 days. 3. re-surgery experience: exist 4. working condition: a person who draws a small income, non-regular worker, working period is less than 1 year. 5. method of surgery: pedicle screw fixation. spine fusion surgery, artificial disc surgery. 6. post surgery condition: appearance of muscle weakness, paralysis, reference pain, lordosis, kyphosis, and complication. smoker or take a drink.

Detector Mount Design for IGRINS

  • The Immersion Grating Infrared Spectrometer (IGRINS) is a near-infrared wide-band high-resolution spectrograph jointly developed by the Korea Astronomy and Space Science Institute and the University of Texas at Austin. IGRINS employs three HAWAII-2RG Focal Plane Array (H2RG FPA) detectors. We present the design and fabrication of the detector mount for the H2RG detector. The detector mount consists of a detector housing, an ASIC housing, a Field Flattener Lens (FFL) mount, and a support base frame. The detector and the ASIC housing should be kept at 65 K and the support base frame at 130 K. Therefore they are thermally isolated by the support made of GFRP material. The detector mount is designed so that it has features of fine adjusting the position of the detector surface in the optical axis and of fine adjusting yaw and pitch angles in order to utilize as an optical system alignment compensator. We optimized the structural stability and thermal characteristics of the mount design using computer-aided 3D modeling and finite element analysis. Based on the structural and thermal analysis, the designed detector mount meets an optical stability tolerance and system thermal requirements. Actual detector mount fabricated based on the design has been installed into the IGRINS cryostat and successfully passed a vacuum test and a cold test.

  • A Protomodel Space Infrared Cryogenic System (PSICS) cooled by a stirling cry-ocooler has been designed. The PSICS has an IR sensor inside the cold box which is cooled by a stirling cryocooler with refrigeration capacity of 500mW at 80K in a vacuum vessel. It is important to minimize the heat load so that the background thermal noise can be reduced. In order to design the cryogenic system with low heat load and to reduce the remained heat load, we have performed numerical analyses. In this paper, we present the design factors and the results obtained by the thermal analysis of the PSICS.

Association Between Leisure Time Physical Activity, Cardiopulmonary Fitness, Cardiovascular Risk Factors, and Cardiovascular Workload at Work in Firefighters

  • Background: Overweight, obesity, and cardiovascular disease risk factors are prevalent among firefighters in some developed countries. It is unclear whether physical activity and cardiopulmonary fitness reduce cardiovascular disease risk and the cardiovascular workload at work in firefighters. The present study investigated the relationship between leisure-time physical activity, cardiopulmonary fitness, cardiovascular disease risk factors, and cardiovascular workload at work in firefighters in Hong Kong. Methods: Male firefighters (n = 387) were randomly selected from serving firefighters in Hong Kong (n = 5,370) for the assessment of cardiovascular disease risk factors (obesity, hypertension, diabetes mellitus, dyslipidemia, smoking, known cardiovascular diseases). One-third (Target Group) were randomly selected for the assessment of off-duty leisure-time physical activity using the short version of the International Physical Activity Questionnaire. Maximal oxygen uptake was assessed, as well as cardiovascular workload using heart rate monitoring for each firefighter for four "normal" 24-hour working shifts and during real-situation simulated scenarios. Results: Overall, 33.9% of the firefighters had at least two cardiovascular disease risk factors. In the Target Group, firefighters who had higher leisure-time physical activity had a lower resting heart rate and a lower average working heart rate, and spent a smaller proportion of time working at a moderate-intensity cardiovascular workload. Firefighters who had moderate aerobic fitness and high leisure-time physical activity had a lower peak working heart rate during the mountain rescue scenario compared with firefighters who had low leisure-time physical activities. Conclusion: Leisure-time physical activity conferred significant benefits during job tasks of moderate cardiovascular workload in firefighters in Hong Kong.
