Assessment of Autonomic Function in Functional Headache by Heart Rate Variability (심인성(心因性) 두통(頭痛)의 심박변이도(HRV) 분석에 의한 자율신경기능 평가)

  • Objectives : This study sought to define autonomic functional difference on heart rate variability between headache patients and normal subjects Methods : We determined, heart rate variability of 52 headache patients and 30 normal subjects by means of three time domain measures: Mean PR(mean pulse rate), SDNN(standard deviation of all normal R-R intervals), RMSSD(the square root of the mean of sum of the squares of differences between adjacent normal R-R intervals), and five frequency domain measures: TP(total power), VLF(very low frequency), LF(low frequency), HF(high frequency). Results : 1) RMSSD, TP, HF, HF Norm of HRV decreased with aging and LF Norm, LF/HF increased with aging in headache patients. 2) There was no significant differences between migraine and tention headache in any standard index of HRV, whereas, HRV of total headache patients were smaller than those of normal subjects. 3) In the gender comparison, SDNN, RMSSD, HF of tention headache patients were the smallest and then migraine patients, normal subjects in order with man, whereas, LF Norm, LF/HF of normal subjects were the smallest with man. RMSSD of migraine patients were the smallest and then tention headache patients, normal subjects in order with women. 4) In the term of history of headache patients, SDNN, RMSSD were getting lower as long term. 5) There was no significant differences of HRV in physical symptom of headache patients. Conclusions : All of these results show that standard index of HRV of headache patients were significantly different with normal subjects.

The heart rate variability(HRV) of the headache patients caused by Chiljungsang (칠정상(七情傷)으로 인한 두통 환자의 심박변이도(Heart Rate Variability)에 관한 고찰)

  • Objective: To treat psychogenic headache patients, doctors have to amplify on the headache caused by emotional stress to patients, and assist the patients to cope with difficulties. So, we investigated HRV of the headache patients caused by Chiljungsang and would like to apply to the clinical treatment. Method: Our study measured time and frequency domain HRV indicies(5-min resting study) of 123 headache patients caused by emotional stress. Standardized tests of HRV allow a quantitative estimation of autonomic nervous system function. Results & Conclusions: 1. The study classed as aspects of the head pain showed the differences in RMS-SD(square root of mean squared difference of successive NN intervals) band, HF(high frequency) band significantly. 2. The male headache patients showed higher all the indicies except heart rate compared to the female patients, significantly in SDNN(standard deviation of NN interval), TP(total Power), HF band. 3. As the patients grow older, SDNN, RMS-SD band was lower and LF(low frequency) band, LF/HF ratio higher significantly. The beginning age lower, SDNN, RMS-SD band was higher significantly. The duration of the disease longer, LF band, LF/HF ratio was higher significantly.

Emotion Recognition Method of Competition-Cooperation Using Electrocardiogram (심전도를 이용한 경쟁-협력의 감성 인식 방법)

  • Attempts have been made to recognize social emotion, including competition-cooperation, while designing interaction in work places. This study aimed to determine the cardiac response associated with classifying competition-cooperation of social emotion. Sixty students from Sangmyung University participated in the study and were asked to play a pattern game to experience the social emotion associated with competition and cooperation. Electrocardiograms were measured during the task and were analyzed to obtain time domain indicators, such as RRI, SDNN, and pNN50, and frequency domain indicators, such as VLF, LF, HF, VLF/HF, LF/HF, lnVLF, lnLF, lnHF, and lnVLF/lnHF. The significance of classifying social emotions was assessed using an independent t-test. The rule-base for the classification was determined using significant parameters of 30 participants and verified from data obtained from another 30 participants. As a result, 91.67% participants were correctly classified. This study proposes a new method of classifying social emotions of competition and cooperation and provides objective data for designing social interaction.

Analysis of Twenty-Four Hours Heart Rate Variability among Patients with Major Depressive Disorder (주요우울장애 환자에서 24시간 심박변이도 분석)

  • Objective : There have been few comprehensive studies on the analysis of 24-hour HRV of major depressive disorder (MDD). The purpose of this study was to compare the autonomic nerve system of patients with a MDD with healthy patients and to examine the physiologic and clinical effects of 24-hour HRV by analyzing whether the HRV demonstrates the level of depressive symptoms after improving the symptoms in patients with a MDD. Methods : The 24-hour HRV was measured in patient groups with a MDD (n=16) and control groups (n=16). The patients with a MDD received the follow up test for two months after the treatment. Results : There were significant differences among the indexes (SDNN, rMSSD, SDNN index, and pNN50) of time-domain analysis and the indexes (TP, VLF, LF, HF, and ULF) of frequency-domain analysis of HRV between patient and control groups. The means of RR, SDNN, SDANN, and TP increased after two month of the treatment, comparing them with before the treatment, but there were no statistical significance. Conclusion : The results of 24-hour HRV analysis indicated significant decrease of HRV indexes among MDD patients which may suggest decrease of parasympathetic nervous functions.

Heart Response Effect by 1/f Fluctuation Sounds for Emotional Labor on Employee (1/f 수준 별 음악 자극이 감정 노동 종사자의 심장 반응에 미치는 효과)

  • This study identified heart response of participants while listening to sounds which have 1/f fluctuations with exponent ${\alpha}$ gradient. The participants were engaged in emotional stress work. Prior studies related to 1/f fluctuation sound have reported that sound source can alleviate psychological and physiological state of users. Subjects of this study were exposed to sound with three levels of ${\alpha}$ gradient. Heart response of subjects were measured with Photoplethysmography(PPG) sensor simultaneously. The dependent variables of this study were beat per minute(BPM), very low frequency percent of pulse rate variability (VLF percent), the standard deviation of all normal RR intervals (SDNN), and high frequency power(HF power). Subject showed arousal response when exposed to sound with exponent ${\alpha}$ gradient of 3 whereas the sound with exponent ${\alpha}$ gradient of 1 and 2 resulted in relax effect. The characteristic of 1/f fluctuation sounds can be applied to alleviate stress for employers under emotional labor.

Effects of Thoracic Flexibility Exercise Program on Pain, Heart Rate Variability, and Depression in Patients with Chronic Low Back Pain (흉추 가동성 운동프로그램이 만성 요통 환자의 통증과 심박 변이도 및 우울증에 미치는 영향)

  • Purpose : This study aimed to investigate the effects of a thoracic flexibility exercise program on pain, heart rate variability (HRV), and depression in patients with chronic low back pain. Methods : Fifty patients with chronic low back pain participated in this study. Each participant performed a pre-test and post-test. The outcome measures evaluated were a visual analog scale, HRV test results (automatic activity, standard deviation normal to normal [SDNN], low frequency [LF], high frequency [HF], and LF/HF ratio), and the Beck Depression Inventory. Patients underwent conventional physiotherapy for 35 minutes per session. In addition, a thoracic flexibility exercise program (trunk rotation exercise, McKenzie exercise, and supine thoracic extension exercise) were performed 3 times per week for 4 weeks. Results : After the training period, the visual analog scale results showed a significant decrease in the post-test compared to the pre-test. Automatic activity, SDNN, LF, HF, and LF/HF ratio significantly increased in the post-test compared to the pre-test. The Beck depression inventory results significantly decreased in the post-test compared to the pre-test. Conclusion : This study demonstrated that a thoracic flexibility exercise program improves a visual analog scale, HRV (automatic activity, SDNN, LF, HF, and LF/HF ratio), and the Beck Depression Inventory in patients with chronic low back pain.

A Report of the Symptoms Accompanied with Acne and Heart Rate Variability Analysis of Acne Patients (여드름 환자의 동반증상 연구 및 환자 특성별 HRV 분석)

  • Objectives: The purpose of this study is to investigate the symptoms accompanied with acne and HRV pattern of the acne patients. Methods: We reviewed 60 acne patients(age 15-38) who visited ${\bigcirc}{\bigcirc}$ hospital from January 2010 to October 2011. We investigated the symptoms accompanied with acne of 60 patients and evaluated HRV of 41 acne patients. The data were analyzed by Mann-Whitney test using SPSS/PC ver 13.0 program. Results: 1. Prevalence of female post-adolescent acne was 66.7% in this study. The symptoms accompanied with acne were dischezia(35%), gastric discomfort(28.3%) in all patients and dysmenorrhea(40%), leukorrhea(38%), irregular menstrual period (24%) in female patients. 2. SDNN, RMSSD, VLF and HF of female were significantly lower than male. 3. SDNN, TP and VLF of un-dysmenorrhea group were significantly lower than dysmenorrhea group. 4. PSI of under 6 months group was significantly higer than over 6 months group. 5. In postadolescent acne patients SDNN, RMSSD, TP, LF and HF were significantly lower and PSI was significantly higher than adolescent acne patients. Conclusions: There are many female post-adolescent acne pateints and the assessment of the symptoms accompanied with acne and stress degree is helpful in treating acne. The further research and development about the diagnostic and therapeutic methods of acne are needed in oriental obstetrics and gynecology field.

Characteristics of Autonomic Nervous System Responses Induced by Anger in Individuals with High Trait Anxiety (분노유발에 따른 특성불안자의 자율신경계 반응 특성)

  • Individuals with high trait anxiety try to suppress their anger expression, thus there are limits in measuring their anger using subjective behavioral evaluation. In order to overcome this limitation, this study attempted to identify the difference in the autonomic nervous system responses induced by anger in individuals with high trait anxiety. Participants were divided into two groups, anxiety and control groups. Electrocardiogram (ECG), respiration (RESP), electrodermal activity (EDA), and skin temperature (SKT) were measured while participants were presented with an anger-inducing stimulus. Heart rate (HR), standard deviation of NN interval (SDNN), root mean square of successive difference (RMSSD), low frequency (LF), high frequency (HF), LF/HF ratio, respiration rate (RR), skin conductance level (SCL), and maximum skin temperature (maxSKT) were calculated before and after presenting the stimulus. Anxiety group reported greater anger by the anger-inducing stimulus compared to the control group. Anxiety group also showed significant increase in SDNN and LF, and decrease in HF, LF/HF ratio, and RR. These results suggest that the autonomic nervous system responses may be used as objective indicators of anger experiences in individuals with high trait anxiety.

Evaluation of Horticultural Therapy on the Emotional Improvement of Depressed Patients by Using Heart Rate Variability (심박변이도를 이용한 우울증 환자의 정서개선에 미치는 원예치료 효과 분석)

  • To evaluate the effect of horticultural therapy (HT) on the emotional improvement of depressed patients, computer-based heart rate variability (HRV) was compared with self-report scale (SRS) known as existing subjective evaluation method. SRS included four test areas: mental stress scale (MSS), physical stress scale (PSS), Beck anxiety inventory (BAI), and Beck depression inventory (BDI). HRV was itemized into four parameters: standard deviation of the N-N intervals (SDNN), square root of mean squared difference of successive N-N intervals (RMSSD), total power (TP), and low-frequency/high-frequency ratio (LF/HF ratio). Thirty patients with depression at the same mental hospital participated in this study. 15 patients of the treatment group received HT once a week for three months, but the control group did not during the same period. As a result, the emotional improvement in treatment group was clearly identified through HRV as well as SRS. The significant difference was shown at three test areas (MSS, BAI, and BDI, $p$ < 0.001) in SRS and at one parameter (total power, $p$ < 0.05) in HRV. There was noticeable increase in SDNN, RMSSD, and LF/HF ratio in treatment group after HT activity, but no significant difference. Although all parameters of HRV did not show significance, the possibility of HRV as an objective evaluation method to HT was recognized in this study. These results also implied that HT was efficient in the mental and physical regeneration of the depressed patients in both subjective and objective evaluation methods.

Autonomic Nervous System response affected by 3D visual fatigue evoked during watching 3D TV (3D TV 시청으로 유발된 시각피로가 자율신경계 기능에 미치는 영향)

  • As technology in 3D industry has rapidly advanced, a lot of studies primarily focusing on visual function and cognition have become vigorous. However, studies on effect of 3D visual fatigue on autonomic nervous system have not less been conducted. Thus, this study was to identify and determine the effect that might have a negative influence on sympathetic nervous system, parasympathetic nervous system, and cardiovascular system. Fifteen undergraduates (female: 9, mean age: $22.53{\pm}2.55$) participated and were sat on a comfortable chair, viewing a 3D content during about 1 hour. Cardiac responses like SDNN(standard deviation of RR intervals), RMS-SD(root mean squared successive difference), and HF/LF ratios extracted from the measured PPG(Photo-PlethysmoGram) before viewing 3D were compared to those after viewing 3D. The results showed that after subjects watched the 3D, responses in sympathetic nervous system and parasympathetic nervous system were activated and deactivated, respectively relative to those before watching the 3D. The results showed that HF/LF ratio, Ln(LF), and Ln(HF) after viewing 3D were significantly reduced relative to those before viewing 3D. No significant effects were observed in SDNN and RMS-SD. Results obtained in this study showed that visual fatigue induced by watching 3D adversely influenced autonomic nervous system, and thereby reduced heart rate variability causing sympathetic nervous acceleration.
