The Change of Shoulder Flexion by Degree of Freedom in The Individuals (운동자유도에 따른 어깨관절 굴곡각도의 변화)

  • Objective : The purpose of this study was conducted to find the change of shoulder flexion by degree of freedom between the Proprioceptive Neuromuscular Facilitation(PNF) pattern, the flexion- abduction- external rotation, and not PNF pattern, the flexion- exteral rotation(except abduction)movement. Method : PNF pattern applied on the unilateral upper extremity in all subjects were the flexion- abduction- external rotation, and not PNF pattern, and twenty-six normal subjects(13 male, 13 female) were tested by Cybex Norm Testing & Rehabilitation System. Results : PNF pattern showed significant increase of flexion than not PNF pattern, the flexion-exteral rotation(except abduction)movement, and also the female group was significantly increased than male group. Conclusion : The results suggested that the application of PNF pattern to the unilateral upper extremity had affected on the shoulder flexion by degree of freedom.

Physical Therapy Intervention for High School Baseball Players with Internal Impingement Syndrome : Comparison of the effects of eccentric training and concentric training (내부 충돌증후군을 가진 고등학교 야구선수들을 위한 물리치료적 중재법 : 편심성 훈련과 동심성 훈련의 효과 비교)

  • Purpose : The purpose of this study was to suggest a more effective method by comparing the effects of changes in pain intensity, muscle strength, and athletic performance after applying a 6-week eccentric training program (ET-MWM) or concentric training program (CT-MWM) with MWM for high school baseball players with shoulder internal impingement (SII). Methods : A total of 75 participants were randomly assigned to each group and divided into two groups, "ET-MWM group (n=35)" and "CT-MWM group (n=32)" according to the intervention method. Pain intensity, muscle strength (external rotation, internal rotation), and athletic performance were first measured before the intervention, and after the intervention 3 times a week for a total of 6 weeks, both groups were re-measured in the same way. Visual analog scale (VAS) was used for pain intensity, biodex dynamometer for muscle strength (60 °/sec.), and Kerlan-Jobe orthopedic clinic shoulder & elbow score (K-KJOC) for athletic performance. Results : As a result of analyzing the homogeneity of the pre-intervention characteristics and initial measurement variables of the study subjects, there was no significant difference between the two groups in all variable values. Pain intensity (VAS) was significantly reduced in the ET-MWM group than in the CT-MWM group (p<.05). In addition, the maximum muscle strength of external rotation & internal rotation of the shoulder (60 °/sec.) and athletic performance (K-KJOC) were significantly increased in the ET-MWM group than in the CT-MWM group (p<.05). Conclusion : Compared with the CT-MWM training program, the ET-MWM training program reduced shoulder joint pain and further increased the muscle strength required for throwing motion in high school baseball players. As the result showed better athletic performance improvement, the ET-MWM training program can be clinically recommended as a more effective intervention.

Comparison of Lumbopelvic Rotation Angle during Active Straight Leg Raise in Patients with Chronic Low Back Pain with and without Lumbar Segmental Instability (만성요통환자의 요추부 불안정성 유무에 따른 능동 하지직거상 시 요골반부 회전각의 차이)

  • PURPOSE: This study aimed to compare the degree of lumbopelvic rotation during the active straight leg raise (ASLR) test in chronic low back pain with and without lumbar segmental instability. METHODS: A total of 71 patients with chronic low back pain were recruited for this study. The subjects who tested positive for more than three of the five lumbar segmental instability tests (prone lumbar instability, lumbar passive extension test, anterior posterior mobility test, passive straight leg raise, age) were categorized into the lumbar segmental instability positive group. Patients who tested positive for less than three of the five tests were categorized into the lumbar segmental instability negative group. The lumbopelvic rotation was measured three times during ASLR and a mean was determined. Subjective heaviness during the ASLR was measured on 6 point scale. RESULTS: There was a statistically significant difference in the lumbopelvic rotation angle between the groups with and without lumbar segmental instability (p<.01). There was no significant difference in the subjective heaviness during ASLR. The mean lumbopelvic rotation angle during ASLR was $13.54{\pm}2.86^{\circ}$, and $8.81{\pm}2.47^{\circ}$ in the positive and negative groups, respectively (p<.01). The cut-off value of the lumbopelvic rotation during was $10.5^{\circ}$, the sensitivity was 82.9%, and the specificity was 80.6%. CONCLUSION: These results suggest that lumbopelvic rotation is more prevalent in patients without lumbar segmental instability. Clinically, this important when diagnosing chronic low back pain with lumbar segmental instability, as the lumbopelvic rotation angle during the ASLR test can be used to aid in diagnosis.

  • Study on spin-rotation relaxation of nuclear spins located on a methyl group can reveal valuable dynamic information related to the internal rotation of methyl group itself. Toward this end we have measured methyl carbon-13 spin-rotation of methyl group itself. Toward this end we have measured methyl carbon-13 spin-rotational relaxation rate in toluene and 2-chloro-p-xylene over the temperature range of 179-363K. To interpret the temperature dependence of measured spin-rotational relaxation rate we have revised the temperature dependence of measured spin-rotational relaxation rate we have revised the expression derived thus far by other authors and reproduced experimental data on the basis of the newly derived expression. The results confirmed that our expression leads to better agreement with experimental data than the previous one over observed temperature range, especially at high temperature.

Rotational Motion of Shoulder in Normal Volunteer and SLAP Lesions (정상 성인과 SLAP 병변이 있는 환자의 견관절 회전운동)

  • Purpose: The purpose of this study is to measure the internal and external rotation of shoulder and compare with normal volunteer and patients diagnosed as SLAP lesion. Materials and Methods: Thirty-eight cases (group 1) who had SLAP lesions at shoulder arthroscopy and fourty young volunteers(group 2) were analyzed retrogradely with medical record, intra-operative arthroscopic photo & video for SLAP lesions and the ROM of shoulder Under the interscalene anesthesia, the range of motion of internal rotation and external rotation were measured on flxed scapula and 90 degree abduction of shoulder, and the same method for group 2. We analyzed the results with two sample T-test and Wisconsin signed ranks test. Results: There was a significant difference between group 1 and group 2 for the ROM of shoulder. (IR; p<0.001,ER; p<0.001).For the group 1, internal and external rotation with the arm abducted 90 averaged 50 and 64 degrees and for the group 2, internal and external rotation averaged 77 and 90 degrees with significant difference(IR; p<0.001,ER; p<0.001).Conclusion: The range of motion of shoulder, especially internal and external rotation significantly decreased in SLAP lesions. Our results suggest that a limited rotational motion of shoulder closely related with SLAP lesion.

The Influence of Trunk Rotation Exercise and PNF Exercise on Gait in the Individuals with Malalignment Syndrome (체간 양측성 회전 운동과 PNF 운동이 부정렬 증후군을 가진 성인의 보행에 미치는 영향)

  • Purpose : The aim of this study was to identify of bilateral trunk rotation(BTR) exercise and PNF exercise on gait in the individuals with malalignment syndrome. Methods : Subjects were 32 that were divided 2 groups in 20's generation. Interventions were trunk ratation exercise and PNF exercise. We used Medex for trunk rotation exercise. BTR group received exercise for three-sets (10min/set) along with stretching exercise ten-minutes, 3 times per week. PNF group took turns the D1 pattern in upper extremity and the D1 pattern in the opposite side of lower extremity for three-sets (10min/set). The measurement were force metatarsal 1 (FM 1), impulse metatarsal 1 (IM 1), force heel lat (FHL), impulse heel lat (IHL) by using footscan (RS scan). Statistical method was repeated measurement of ANOVA and p value was 0.05. Results : BTR and PNF group were significantly different in time(FM 1, IM 1, FHL, IHL). As different of right/left, BTR and PNF exercise were significantly different in FM 1, IM 1, FHL. Conclusion : BTR exercise was good exercise for malalignment but needs expensive equipment, for example, Medex. PNF exercise doesn't need expensive equipment but good method in malalignment syndrome person for gait ability. If PNF exercise is more experiment, PNF exercise could use variety for more patients.

The Effect of External-rotation Reducing Exercise on the Gait of Patient with Hemiplegia (편마비 환자의 보행에 대한 외회전 감소운동의 효과)

  • In this paper, the effects of the external-rotation exercise on the gait of hemiplegia was investigated. 26 patients with hemiplegia due to cerebrovascular accident participated in this study. Subjects were randomly assigned to experimental group(n=12) and control one(n=14). 12 subjects of the experimental group were received the external rotation reducing exercise on the mat with seating and supine posture after the general therapeutic exercise for hemiplegia. 14 subjects of the control group were received only general therapeutic exercise. The effects of external rotation reducing exercise were evaluated by measurements of gait velocity, cadence, stride length, step length and foot angle using ink-foot prints. The collected data were analyzed statistically based on Wilcoxon, Mann-Whitney and correlation analysis. After treatment of three weeks, it turned out that external rotation reducing exercise has the significant effect on foot angle(p<0.01). However the exercise has no statistically significant direct effect on the gait velocity, cadence, stride length and step length.

The Effects of Joint Mobolization on Neck Myofacial Pain Syndrome (경부근막동통증후군 환자의 가동범위에 대한 연구)

  • Purpose : To identify the effects of Hold-Relax of proprioceptive neuromuscular facilitation (PNF) on the functional improvement of patients with neck myofascial pain syndrome. Methode : The present research investigated 60 patients with neck myofascial syndrome, dividing them into a group doing Hold-Relax of proprioceptive neuromuscular facilitation and a group doing Stretching, This study examined degree of recovery from neck pain by comparing their neck myofascial pain syndrome before and after the treatment, and compared two groups to find difference in the degree of recovery from myofascial pain syndrome. Results : 1. For Hold-Relax of proprioceptive neuromuscular facilitation group, the pain rating score (PRS) were significantly decreased for six weeks treatment. 2. For Stretching group, the pain rating score (PRS) did not indicate significant decrease for six weeks treatment. 3. For Hold-Relax of proprioceptive neuromuscular facilitation group, range of motion of flexion, left rotation, right rotation indicated significant difference after pre test and after two week but no significant difference after four week. 4. For Stretching group, range of motion of flexion, left rotation, and right rotation except extension indicated significant difference after pre test and after two week but no significant difference after four week. Conclusion : The Hold-Relax of proprioceptive neuromuscular facilitation is considered to be effective in increasing joint range of motion of patient and decreasing pain. Accordingly physical therapists has to learn a wide range of patterns and to develop various treatment techniques.

The Surgical Treatment of Acute Acromioclavicular Joint Dislocation Using C-C Sling Method and Modified Phemister Operation (급성 견봉쇄골관절 탈구에서 변형된 Phemister 술식과 C-C sling 술식의 결과 비교)

  • Purpose: The purpose of this study was to compare the method of using coraco-clavicular (C-C) sling with modified Phemister operation by postoperative clinical results, radiologic analysis and complications. Materials and Methods: 33 patients of acromioclavicular joint dislocation were included in this study. Thirteen patients were treated with simple C-C sling method and twenty patients were treated with modified Phemister operation. The assesment of clinical and radiological evaluation were performed and the final results were examined by using the Weitzman's classification. Results: In the final result of C-C sling method group, forward elevation $161^{\circ}$, external rotation $70^{\circ}$, internal rotation T8 level, Visual Analogue Scale (VAS) 83.3 points were checked. In modified Phemister operation group, forward elevation $155^{\circ}$, external rotation $67^{\circ}$, internal rotation T6 level, VAS 83.8 points were checked. In coracoclavicular distance of C-C sling method group, pre-operation 12.82 mm and last follow up 8.37 mm were checked. In modified Phemister operation group, pre-operation 12.8 mm and last follow up 7.7 mm were checked. In functional evaluation by the Weitzman criteria, C-C sling group had excellent 8, good 1, fair 1 and modified Phemister group had excellent 13, good 4, fair 3. Conclusion: C-C sling method would be the better than the Modified Phemister operation because of short operation time and smaller skin incision.

The Comparative Study on Effect of Jungsongouhyul Phamacopuncture and Electroacupuncture in Patients with Acute Traumatic Shoulder Pain (급성 외상성 어깨 통증 환자에 대한 중성어혈약침과 전침의 효과에 관한 비교 연구)

  • Objectives : The purpose of this study is to report the effect of Jungsongouhyul phamacopuncture and electroacupuncture on patients with acute traumatic shoulder pain. Methods : The patients with acute traumatic shoulder pain were diagnosed by blood stasis and divided two groups. One group was treated byJungsongouhyul phamacopuncture. The other group was treated by electroacupuncture. Each group was treated 3 times a week for 2 weeks. The results were measured by visual analog scale(VAS), range of abduction and internal rotation. Results : Two groups had no significant differences before treatment. Jungsongouhyul phamacopuncture group had effects on VAS, range of abduction and internal rotation significantly. Electroacupuncture group also had effects on VAS, range of abduction and internal rotation significantly. But there was no significant differences between two groups. Conclusions : Jungsongouhyul phamacopuncture and electroacupuncture have effect on patients with acute traumatic shoulder pain significantly.
