  • Maxillary first molar, the "6-year molar", is the tooth largest in volume and most complex in root and root canal anatomy. Therefore, maxillary first molar is possibly the most treated, least understood. It is the posterior tooth with the highest endodontic failure rate and unquestionably one of the most important teeth. The earliest permanent posterior tooth to erupt, the mandibular first molar seems to be the most frequently in need of endodontic treatment.(omitted)

  • The reliability of blade root fixing section is required to endure the centrifugal force and vibration stress for the last stage blade of steam turbine in thermal power plant. Most of the domestic steam turbine last stage blades have finger type roots. The finger type blade is very complex, so the inspection had been performed only on the exposed fixing pin cross-section area due to the difficulty of inspection. But the centrifugal force and vibration stress are also applied at the blade root finger and the crack generates, so the inspection method for finger section is necessary. For the inspection of root finger, inspection points were decided by simulating ultra-sonic path with 3D modeling, curve-shape probe and fixing jig were invented, and the characteristics analysis method of ultrasonic reflection signal and defect signal disposition method were invented. This invented method was actually executed at site and prevented the blade liberation failure by detecting the cracks at the fingers. Also, the same type blades of the other turbines were inspected periodically and the reliability of the turbine increased.

Surgical extrusion of a maxillary premolar after orthodontic extrusion: a retrospective study

  • Objectives: Crown-root fracture and cervical caries in maxillary premolars constitute a challenge in cases of subgingival placement of restoration margins. Surgical extrusion has been practiced successfully in permanent anterior teeth. The aim of the present retrospective study was to assess the clinical outcome of surgical extrusion after orthodontic extrusion in maxillary premolars. Materials and Methods: Twenty-one single, tapered root maxillary premolars with subgingival crown-root fracture or caries were included. Presurgical orthodontic extrusion was performed on all teeth to prevent root resorption. Extent of extrusion and rotation was determined based on crown/root ratio. The postoperative splinting period was 7 to 14 days. Clinical and radiographic examination was performed at an interval of 1, 2, and 3 months. Results: After the mean follow-up of $41.9{\pm}15.2months$, failure was observed as increased mobility in 3 of 21 cases. No significant difference was observed in the outcome of surgical extrusion based on tooth type, age, sex, $180^{\circ}$ rotation, or time for extraction. Furthermore, marginal bone loss was not observed. Conclusion: Surgical extrusion of maxillary premolars can be a possible therapeutic option in cases of subgingival crown-root fracture.

  • This paper presents the root cause failure analysis of the circulating water pump in the 560 MW thermal power plant. A fractured austenitic stainless-steel shaft operated for 24 years was examined. Fracture morphology was investigated by micro and macro-fractographic analysis. The metallurgical analyses including chemical analysis, metallography and hardness testing were performed. The analysis reveals that the pump shaft was fractured due to the reverse bending load with combination of rotating bending load. Corrective actions for plant operator was recommended based on the analysis.

  • The following thesis provides an explanation for the definition of the MIL-HDBK-2155 : Failure Reporting, Analysis and Corrective Action System (FRACAS), which systemizes the collection and analysis of failure data and the feedback process of the results. It also presents a plan based on MIL-HDBK-2155 for the collection and analysis of operating specifications on weapon systems. The collection and analysis of failure data and the feedback process utilizing FRACAS contributes to identifying improvement requirements during equipment operation as well as finding and eliminating the root cause of the failures. The objective of applying FRACAS to weapon systems is to receive source data feedback for reliability enhancements and performance improvements during operation. This is done by recognizing weaknesses in the design or operation by identifying the type of failures that might occur, and by performing Failure Modes, Effects and Criticality Analysis(FMECA) and Failure Tree Analysis(FTA).

  • This paper presents a case history of piping failures on power plant. The root cause of the failure was defined to set the optimal countermeasures. The failure comes from transient vibration and the 1st stress increased at the hydraulic oil supply system of control valves for high pressure steam turbine.

  • In this paper, the dent printing phenomena and critical dent size which lead to premature failure of deep grooved ball bearing for automobiles are presented. The results of newly assembled test bearings showed that the ball dent size strongly affect the bearing life and inner ring raceways were the major failed parts in spite of using the dented balls. The dent printing phenomena are a root cause of premature failure, which was not reported until now. And there exist the critical ball dent size for given rolling bearings and operating conditions. So, the initial condition of tribologically mating parts should be treated as important as the failed body itself.

  • Basic function of calorifier system is to supply warm water to the vessel. The heater used in the calorifier system plays a very important role in its reliability. The failure mechanism of heater are compared with accelerated life test. The main cause of failed heater is pitting corrosion occurred between the surface of heater and spacer. To prevent the corrosion failure from heater, material of spacer replaces metal(SUS 304) with polymer (Acryl). The life of redesigned heater can guarantee 2.47years of B10 life under the worst condition.

고강도 볼트 카드늄 취성파괴 사례연구 (The Case Study on Cadmium Embrittlement Failure of High Strength Bolt)

  • It happened fractures on special bolt which supported main landing gear actuator up-lock rod of aircraft. Cracks were initiated mainly from the center hole and the external thread of the special bolt. To find out failure root causes, metallographic, fractographic analyses as well as test work were carried out. From the fractographic study by SEM work, fracture occurred by a brittle intergranular type failure. The fracture could be occurred primarily by solid-metal-induced embrittlement due to cadmium embrittler penetrated into the flaw existed after machining work for center hole and thread on the bolt during baking treatment processing to eliminate hydrogen. For its successful application, cadmium EP bolts require proper and adequate baking treatment after electroplating, and make no more drilled center hole on the bolt to prevent same failure.

Push-out bond strengths of fiber-reinforced composite posts with various resin cements according to the root level

  • PURPOSE. The aim of this study was to determine whether the push-out bond strengths between the radicular dentin and fiber reinforced-composite (FRC) posts with various resin cements decreased or not, according to the coronal, middle or apical level of the root. MATERIALS AND METHODS. FRC posts were cemented with one of five resin cement groups (RelyX Unicem: Uni, Contax with activator & LuxaCore-Dual: LuA, Contax & LuxaCore-Dual: Lu, Panavia F 2.0: PA, Super-Bond C&B: SB) into extracted human mandibular premolars. The roots were sliced into discs at the coronal, middle and apical levels. Push-out bond strength tests were performed with a universal testing machine at a crosshead speed of 0.5 mm/min, and the failure aspect was analyzed. RESULTS. There were no significant differences (P>.05) in the bond strengths of the different resin cements at the coronal level, but there were significant differences in the bond strengths at the middle and apical levels (P<.05). Only the Uni and LuA cements did not show any significant decrease in their bond strengths at all the root levels (P>.05); all other groups had a significant decrease in bond strength at the middle or apical level (P<.05). The failure aspect was dominantly cohesive at the coronal level of all resin cements (P<.05), whereas it was dominantly adhesive at the apical level. CONCLUSION. All resin cement groups showed decreases in bond strengths at the middle or apical level except LuA and Uni.
