Purpose: In this study analysis was done of utilization of rooming-in care in South Korean hospitals in order to examine the factors related to mothers and hospitals that affect rooming-in care. Methods: With the involvement of 254,414 mothers who gave birth across 953 hospitals, the analysis used the health insurance qualification data of the National Health Insurance Corporations and Health Insurance Review and Assessment Service (2006). Factors associated with rooming-in care were analyzed using a GEE logistic regression analysis to consider factors related to both mothers and hospitals. Results: Only 45.1% of the mothers used rooming-in care. The results of the regression analysis revealed that individual factors of the mothers were not associated with rooming-in care, whereas group factors of the hospitals were. Rooming-in care use was primarily related to small hospital, location of hospital, and higher nurse staffing level. Conclusion: The findings of this study indicate that the utilization of rooming-in care is not associated with factors an individual mother, but rather with the group factors of the hospitals. Thus, a policy-based approach considering both of these types of factors is required to enhance the utilization of rooming-in care.
The purpose of this study was to compare maternal attitude and self confidence for infant care of primiparas of rooming-in and not rooming-in. The subjects were 128 primiparas who had delivered at eight general hospital in Seoul. 67 primiparas were in three rooming-in facilities and 61 primiparas were in five not rooming-in facilities. The data were collected from primiparas using Cohler's Maternal Attitude Scale and Pharis' Self Confidence Scale at postpartal 1 or 2 weeks. The results of this study were as follows : 1. The mean of maternal attitude was 100.32 and the range was from 82 to 138. The score of primiparas in rooming-in(100.94) was higher than those of not rooming-in(97.43). There was a significant difference in maternal attitudes between rooming-in group and not rooming-in group(P=.001). There were no significant differences in maternal attitude according to age and infant sex. But types of feeding were related to maternal attitude (P=.017). 2. The mean of self confidence for infant care was 119.55 and the range was from 58 to 173. The mean of primiparas in rooming- in(123.10) was higher than those of not rooming-in (115.86). There was a significant difference in self confidence for infant care between rooming-in group and not rooming- in group (P=.040). No significant differences existed in self confidence for infant care according to age, infant sex, and types of feeding. 3. The rate of breast feeding was 64.2% in rooming-in group and 34.4% in not rooming-in group at postpartal 1 or 2 weeks. There was a significant difference in breast feeding between the two groups(P=.004). In conclusion, rooming-in facilities provided primiparas with more positive maternal attitude and greater self confidence for infant care and increased the rate of breast feeding.
Purpose: The objective of this study was to identify the effectiveness of continued breast feeding and maternal identity between rooming-in and not rooming-in groups of mothers in post partum care. Method: The subjects were selected by convenient sampling, with 95 women in childbirth completing a questionnaire. The data were collected on the 2nd day after delivery by questionnaire, and after the 4th and 8th weeks by telephone interview. Measures used in this study included general characteristics, breast feeding related characteristics, and the maternal identity scale revised by Koh(1996). The data were analyzed with the SPSS program, and the Chi-test and t-test were used to identify the effect on breast feeding and maternal identity. Results: Continued breast feeding up to 4 weeks after deliverly was not statistically significant between rooming-in and not rooming-in group, but beyond 8 weeks of deliverly, it was significantly different in rooming-in group(p<.01). Maternal identity was significantly higher in rooming-in group than in not rooming-in group(<.01). Conclusion: Mothers preferred rooming-in system because it helped breast feeding and baby care. Rooming-in system provides self confidence in taking care of baby, knowledge about breast feeding and stimulates early phase lactation. We recommend rooming-in system in hospitals to promote breast feeding and maternal identity. Nurses are encouraged to discuss prenatal breast feeding before delivery and rooming-in care should be planned individually for continuation of breast feeding and formation of maternal identity.
Purpose: In Kyung Hee East-West Neo Medical Center, Seoul, Korea, efforts to raise rooming-in care success rate have been undertaken since when the hospital was established in 2006. We intended to analyze our experience over the past 3 years of period and to discuss the advantages of rooming-in. Methods: We analyzed the rooming-in practice rate, failure rate, and the breast feeding rate. Subjects were 860 normal healthy neonates from June 2006 to June 2009. Results: Among these 860 cases, 83 babies were required separation out of rooming-in in the middle of the course. Among these 83 cases, 70 cases had to stop the course due to poor condition of babies and 13 cases due to maternal condition. 70 cases of infant's causes consist of 68 cases of NICU admission and 2 cases of poor feeding support. The other 13 cases of separation include refusal by maternal condition. Therefore the success rate of rooming-in for the last 3 years was 90.3%, that is 777 cases among the total 860 cases. The percentage of exclusive breast feeding was 64%, that of mixed feeding with breast and formula feeding was 25%, and formula feeding only was 11%. Conclusion: We experienced successful rooming-in care for the last 3 years. Nursery facilities should educate and encourage the advantages of rooming-in, including the good formation of attachment between mother and infant, emotional stability, protection from infection, and increased breast feeding rate so that rooming-in care can be fully established.
본 연구는 모자동실 유형에 따른 산모의 피로도와 모유수유율에 차이가 있는지를 확인하기 위해 시도되었다. 서울소재 일 종합병원의 모자동실제 정책이 전일모자동실제에서 부분모자동실제로 전환된 후 두 제도 간에 산모의 피로도와 모유수유율에 차이가 있는지 비교하기 위해 수행되었으며, 평가내용은 출산 후 산모의 피로도와 모유수유율을 비교분석하여 관련성을 파악하는 것이다. 전일모자동실제의 산모 64명을 대상으로 자체 평가한 2012년 10월의 분석결과와 부분모자동실제로 전환운영한 2014년 6- 7월까지의 산모 43명을 대상으로 수집한 자료를 비교분석하였다. 자료처리 및 분석방법은 SPSS 22.0을 이용하여 빈도, 백분율, $x^2-test$, t-test, 피어슨상관관계분석을 통해 4개의 가설을 검정하였다. 그 결과 '전일모자동실군이 부분모자동실군보다 하루 수유횟수가 많은 것'으로 나타났으며, '전일모자동실군과 부분모자동실군 간에 모유수유율은 차이가 없었다'. '전일모자동실군이 부분모자동실군보다 피로도가 높았으며', '산모의 피로도와 입원기간동안의 모유수유율은 관련이 없게' 나타났다. 따라서 본 연구 결과를 토대로 모자동실제 유형에 따라 모유수유율에는 차이가 없으므로 산모의 피로도가 높은 전일모자동실제로 운영하는 것 보다 산모와 신생아의 상태를 고려하여 개별화된 부분모자동실제로 운영하는 방안을 고려해 볼 수 있다.
Purpose: The purpose of this study was to compare maternal perceptions between two groups of postpartum women, women who chose to have their babies room-in and women who did not (non rooming-in group). Methods: Data collection was conducted in 37 hospitals from August 10 to September 20, 2008. The participants were 209 mothers opting for rooming-in and 128 mothers for non rooming-in. The women completed a questionnaire which included the Edinburgh postpartum depression scale, maternal attachment inventory, and postpartum self-evaluation questionnaire. Results: There were significant differences in education level, income and antenatal education between the rooming-in and non rooming-in groups. The rooming-in group also showed higher levels of satisfaction with medical services. More important, the rate of breast feeding for the rooming-in group was higher than for the non rooming-in group. There was no difference between the two groups for postpartum depression, maternal attachment and mothers' confidence with infant care tasks. Conclusion: These findings suggest that rooming-in system has more advantages compared to non rooming-in and that it can help to promote breast feeding. The authors recommend that the rooming-in system be expanded nationwide. The results of this research can be used to assist the development of future rooming-in system expansion strategy.
This study aims to provide space organization of the Postpartum Care Center and basic data for architectural planning in the reasonable size. Nurturing method of infants at independent Postpartum Care Center combines rooming-in and non rooming-in and space organization is planned accordingly. Types of the existing floor plans are classified as a type centered on maternity protection room, a type of the separate space for multipurpose, a near type to the infants' room. New types of plane are presented as follows a core type concentrate upon nurse room, the infants' room and the same environmental and balanced corridors between the both. When an average capacity of 20 at Postpartum Care Center is planned, proper suitable floor space of Postpartum Care Center is above $753.9m^2$ and $37.69m^2$ per postpartum woman is required. It is indicated that an average floor space per postpartum woman of the reviewed facilities is smaller than one of the Postpartum Care Center in a plan and plane distributed by function too lack in nursing space. Expansion of space for the Postpartum Care Center is generally required, also the enlargement of nursing space is necessary to be harmonized with the management of a medical facilities and those mutual cooperation.
Purpose: This study was done to examine the attitude and awareness of nurses about rooming-in for new mothers and their infants. Methods: Data were collected from 462 nurses from 40 hospitals from August 10 to September 20, 2008 using a questionnaire, and the collected data were analyzed using descriptive statistics, t-test and ANOVA. Results: The mean score for attitude and awareness of nurses about rooming-in was 3.02 (range=2.13~3.80), which indicates that the nurses had positive opinions of the system. The factors with the highest and lowest scores were awareness of a successful implementation method (mean=3.35) and awareness of the advantages and disadvantages of the system (mean=2.73), respectively. The attitude and awareness of nurses about rooming-in differed significantly with age, religion, education level, parity, type of hospital, hospital nurse staffing grade (bed-to-nurse ratio), and presence of a rooming-in system. Conclusion: This results of the study show that attitudes and awareness of nurses to rooming-in differed significantly according to personal factors (age, parity, religion, educational level) and work characteristics (hospital type, hospital nurse staffing grade, presence of rooming-in system). These findings can be used as basic data in determining optimal strategies for a system of rooming-in in the future.
목적: 신생아실에서 산모와 격리되어 관리되는 신생아와 비교해 봄으로써 모자동실이 신생아의 정서적 안정에 미치는 효과를 비교해 보고자 한다. 방법: 2007년 7월 1일부터 10월 31일 사이에 출생한 정상 신생아 중 첫 3일간 모자동실을시행한 신생아 24명(모자동실군), 신생아실에서 관리한 24명(신생아실군), 총 48명에 대하여 브라젤튼의 신생아행동평가척도 중 자극척도와 자기 안정화 척도를 관찰하였으며, 대사이상검사를 위해 천자침으로 발뒤꿈치를 찌른 후 울음을 그칠 때까지의 시간을 측정하였다. 결 과: 모자동실군은 자극화척도에서 신생아실군에 비해 높은 점수를 나타내어 외부자극에대해 매우 안정되어 있었으며($6.8{\pm}1.7$ 대 $4.2{\pm}2.1$, P<0.001), 자기 안정화 척도도 높은 점수를 나타내어 자극된 상태에서 빨리 안정화하는 것을 관찰할 수 있었다($5.9{\pm}0.3$ 대 $4.5{\pm}1.8$, P=0.001). 천자침을 찌른 후 울음을 그칠 때까지의 시간도 모자동실군에서 훨씬 짧은 것을 알 수 있었다($17{\pm}15.1$ 초 대 $115.3{\pm}98.5$초, P<0.001). 결론: 모자동실을 하는 신생아는 신생아실에서 격리되어 관리하는 신생아에 비해 생후 수일동안의 기간에도 불구하고 외부 자극에 대해 안정적이며, 자극된 상태로부터 빨리 안정화되는 것으로 나타나 정서적으로 안정되어 있음을 알 수 있었다.
The purpose of this study is to evaluate environmental characteristics of maternity rooms. The method of this study is a field survey on 8 samples of postpartum care centers in Seoul. The plan, colors, materials, furniture and environmental characteristics of maternity rooms are analyzed. The characteristics of maternity rooms environment were categorized into four items; comfort, privacy, communication and dwelling. The results are as follows: Western-style and rooming-separation system of maternity rooms are used. Maternity rooms are generally good for dwelling quality but insufficient for communication. There are a lack of supply to control a temperature Individually in maternity rooms. It demands to make the type of one-sided public space between maternity room and living room for privacy. All of the maternity rooms surveyed are furnished with TV, radio, and telephone but, to improve communication with visitors, it is recommended that more convenient supplies such as audio and video system, chairs, and table be equipped. There are needs for sky-light windows in maternity rooms. It is necessary to research more about the space of living room, nursing room and service area, and we need more study about baby, nurser and owner' spaces.
