An Analysis of Hypertension Status and Related Factors in Korean Early Adults (한국 청년층의 고혈압 현황 및 관련 요인 분석)

  • The purpose of this study is to identify awareness of hypertension prevalence among Korean youth and related causes. Data from the Korean National Health and Nutritional Examination Survey, conducted in 2016 by the Korea Centers for Disease Control and Prevention, were used to apply weights for complex sample analysis. A total of 1,075 data age 19~35 Korean youth were used for analysis. The analysis demonstrated that 25.7% of age 19~35 Korean youth were in the prehypertension stage and 6.1% were in the hypertension stage. Those aware of hypertension was 0.3%. This revelation necessitates establishment of preventive measures by way of physical examination among youth to ensure no youth are left untreated in terms of health management. This study indicated that diabetes affects hypertension significantly among males (p<0.0001), with low income (p=0.034) and obesity (p<0.0001). Consequently, they should be managed as high-risk, and it is imperative to educate them about changes in lifestyle such as walking or less dining-out, rather than engaging in specific sports activities or nutrient intake.

Component Analysis of Laryngeal Cancer Incidence Dynamics in Kazakhstan from 1999 to 2014

  • Background: In this study, we examined epidemiological aspects of dynamic changes in incidences of laryngeal cancer in male and female populations in Kazakhstan. Materials and Methods: Primary data were for registered patients with malignant laryngeal tumors in the whole country during the period of 1999-2014. Evaluation of changes in laryngeal cancer incidence in the population of Kazakhstan was performed using component analysis. Results: It was determined that the number of patients with laryngeal cancer in the whole country is decreasing although with conflicting impacts of different factors. Despite population growth (all - ${\Delta}_P=+66.1%$, men - ${\Delta}_P=+70.9%$ and women - ${\Delta}_P=+46.4%$), and aging (all - ${\Delta}_A=+45.1%$, men - ${\Delta}_A=+54.3$ and women - ${\Delta}_A=+22.2$), the reduction in risk of developing the disease (all - ${\Delta}_R=-165.6%$, men - ${\Delta}_R=-170.9%$ and women - ${\Delta}_R=-141.0%$) was overwhelming. Conclusions: This investigation was the first epidemiological study of dynamics of laryngeal cancer by component analysis in population of Kazakhstan. Implementation of the results of the study is recommended in management of anti-cancer activities for laryngeal cancer.

  • Thorough preparations and investigations must be done in order to prevent the hazards which will happen when workers work on the roof of the rolling stocks. Some industrial disasters can be prevented by solving the risks involved in the working of the work processes. Falling accidents tend to happen from the roof of the trains when workers work in the high position. So through the analysis of the 4M( Man. Machine, Media, Management), this paper analyzed the risk factors among the roof works and all problems of the maintenance. Also this paper suggests new devices or new methods to prevent falling disasters and to relieve the mental senses of uneasiness.

  • The contents of aircraft system safety assessment vary depending on factors such as the complexity of the system, how critical the system is to flight safety, what volume of experience is available on the type of system and the novelty and complexity of the technologies being used. If the system safety assessment is to substantiate that the developed products are 'safe enough' to be taken into use, then the system safety assessment should be planned and managed to provide the necessary assurance that all relevant hazards and failure conditions have been identified and that all significant combinations of hazards and failures which could cause those conditions have been considered. The assessment must assist the designer and management in making decisions. It must make clear what the critical features of each system are and upon which special manufacturing techniques, inspection, testing, crew drills and maintenance practice they are critically dependent. This paper has prepared to study on promoting the efficiency of aircraft system safety assessment and to present how to compile system safety assessment strategy.

Relationships between Work Postures and Upper Extremity Cumulative Trauma Disorders in Medical Insurance Bill Reviewers (의료보험 심사 업무의 작업자세(Work Postures) 특성과 누적외상성질환(CTDs) 발생에 관한 연구)

  • The purpose of this study is to investigate the relationship between the work postures and upper extremity cumulative trauma disorders(CTDs) in female medical insurance bill reviewers(n=448). This study included diagnosis for CTDs, anthropometry and job analysis (workstations and posture). The characteristics of subject were 33.6 years of average age and 8.7 years of average work duration. The results were as fallows. Ergonomic conditions of workstation were unsuitable compared with anthropometry result. The height of work surface(79.5 cm) and chair(43.0 cm)were high. work space was small, and legs space under the table was limited. Work postures were awkward compared with recommended neutral postures. Neck flexion($21.0-36.0^{\circ}$), elbow elevation, shoulder abduction ($46.0-47.0^{\circ}$). wrist radial and ulnar deviation, forearm supination and pronation, and wrist repeated motion(12.7-21.5 freq./min) were analyzed as hazardous risk factors by job analysis. The prevalence of CTDs was 32.8 % by medical diagnosis. The prevalence rate of CTDs in hazardous work posture group was significantly greater than safe work posture group both neck-shoulder (relative prevalence = 5.2, p<0.001) and wrist-hand (relative prevalence = 2.5, p<0.05).

Pattern and Management of Dyslipidemia in Type 2 Diabetes Patients in Korea (제 2형 당뇨환자에서 지질이상 유형 및 관리)

  • Dyslipidemia is an important CHD risk factor in diabetic patients. We conducted this study to assess the pattern of dyslipidemia in type 2 diabetes patients, to examine the demographic and clinical factors associated with dyslipidemia and to evaluate attaining within the lipid target goals and treatment strategies. A retrospective analysis was conducted among patents diagnosed type 2 diabetes at outpatient clinic in endocrinology between January 2003 and December 2004. Clinical history and physical examination were reviewed and laboratory data including blood glucose, HbAlc, lipid levels were recorded sequentially at least 1 year. In 882 patients with type 2 diabetes, 437 patients (49.6%) have dyslipidemia and 73% of them (319 patients) received lipid-lowering agents. 244 patients (94 males, 150 females, mean age 60 years old) were susceptible to analyses. The most frequent pattern of dyslipidemia is high LDL level and high TG levels (28%). Metabolic syndrome and macrovascular complication were significant negative independent association with lipid levels within the target goals (p<0.05). Only 15.2% (19 males, 18 females) attained within the lipid tar- get goals. Patients with diabetic dyslipidemia need maximization of lipid-lowering agents, increasing the fibric acid derivatives prescription and the effort to correction of low HDL and/or high TG.

Pharmaceutical Care for Medication Safety in Critically Ill Neonates (신생아중환자의 안전한 약물사용을 위한 약료서비스)

  • Objective: This study aimed to investigate pharmaceutical care for critically ill neonates and suggest targeted strategies compatible with the Korean health-system pharmacy. Methods: Articles that reported pharmacy practices for critically ill neonates were reviewed. Pharmaceutical care practices and roles of neonatal pharmacists were identified, and criteria were developed for neonates in need of specialized care by clinical pharmacists. Results: Neonatal pharmacists play many roles in the overall medication management pathway. For clinical decision support, multidisciplinary ward rounds, clinical pharmacokinetic services, and consultation for pharmacotherapy and nutrition support were conducted. Prevention and resolution of drug-related problems through review of medication charts contributed to medication safety. Pharmaceutical optimization of intravenous medication played an important role in safe and effective therapy. Information on the use of off-label medicine, recommended dosage and dosing schedules, and stability of intravenous medicine was provided to other health professionals. Most clinical practices for neonates in Korea included therapeutic drug monitoring and nutrition support services. Reduction in medication errors and adverse drug reactions, shortening the duration of weaning medicines, decreasing the use and cost of antimicrobials, and improvement in nutrition status were reported as the outcomes of pharmacist-led interventions. The essential criteria of pharmaceutical care, including for patients with potential high-risk factors for drug-related problems, was developed. Conclusion: Pharmaceutical care for critically ill neonates varies widely. Development and provision of standardized pharmaceutical care for Korean neonates and a stepwise strategy for the expansion of clinical pharmacy services are required.

Neuroleptic Malignant Syndrome in Children and Adolescents : A Review (소아 청소년에서의 신경이완제 악성증후군에 대한 종설)

  • Neuroleptic malignant syndrome (NMS) is a severe iatrogenic complication of treatment with antipsychotic medication. The aim of this review is to provide the clinical characteristics and treatments of children and adolescents with NMS. Searches were conducted in Medline, Korean studies Information Service System (KISS), and Research Information Service System (RISS). Sixteen case reports and two review articles were selected in Medline, and two Korean cases reported in department of emergency medicine and pediatrics were selected from RISS. Heterogeneous and atypical presentations of NMS were observed in children and adolescents. Some noticeable differences were observed between adult patients and child patients with NMS. In addition, symptom presentations related to atypical antipsychotic agents differed from those of typical ones. In treatment, bromocriptine and benzodiazepine were recommended for management of symptoms. In particular, electroconvulsive therapy (ECT) was a useful treatment option. For prevention and early detection of NMS in children and adolescents, evaluation of risk factors and understanding of diagnostic features of NMS are very important.

The Relationship between Smoking, Alcohol Drinking, and Parenting Behavior Perceived by Adolescents (청소년이 인지하는 부모양육행동과 흡연 및 음주와의 관련성)

  • Objectives: The purpose of this study was to analysis the health behaviors differences of adolescents depending on whether parenting behavior perceived by adolescents. Methods: The data which is used in this study was extracted from the 2011 second korean adolescents' penal data collected by the National Youth Policy Institute. Multivariate logistic regression analysis was performed in order to find how degree parents relationship factors are associated with adolescents' health behavior after adjusting sex, smoking(alcohol drinking) friends, and other significant variables. The data was processed by SAS 9.1. Results: Respondent's general characteristics such as smoking(alcohol drinking)friends, parents' job were positively relative to smoking and alcohol drinking experience. Also Parents's affection and experience of being abused from parents were significantly associated with smoking(alcohol drinking) experience. Conclusions: Negative vertical attitude for children is important for the health risk behaviors of adolescents. To increase effectiveness of the health policy for the adolescents in korea, it is important to adapt new strategy to include the positive empowerment of adolescents' social networks except for smoking(alcohol drinking)friends and policy support to turn negative vertical attitude for children into positive attitude for children.

Impacts of Daily Habits, Health Status, and Health Promoting Behavior on Prevalence of Irritable Bowel Syndrome among Female Adolescents (여고생의 생활습관, 건강상태 및 건강증진행위가 과민성 장증후군 유발에 미치는 영향)

  • Purpose: This study was to investigate impacts of daily habits, health status and health promoting behavior on prevalence of irritable bowel syndrome (IBS) among female high school students. Methods: The survey was conducted from 526 female students in high school. Rome III criteria was used to diagnose IBS. Data of daily habits, health status, and health promoting behavior were collected through self-administered structured questionnaires. Results: The prevalence of IBS was 25.7%. Compared to the non-IBS, the IBS group scored significantly lower in exercise time, sleeping time, four dimensions of health status (physical, emotional, social, spiritual health) and management of relationship and stress in health promoting behavior. Risk factors influencing IBS were exercise time, physical health and emotional health status. Conclusion: The results of this study suggest that nursing interventions and education programs are important to be developed, while considering exercise, health status and health promoting behaviors for female adolescents in order to manage their IBS.
