Purpose: Diabetic foot wound (DFW) is known as a major contributor of nontraumatic lower extremity amputation. We aimed to evaluate overall amputation rates and risk factors for amputation in patients with DFW. Materials and Methods: From January 2014 to December 2017, 141 patients with DFW were enrolled. We determined rates and risk factors of major amputation in DFW and in DFW with peripheral arterial occlusive disease (PAOD). In addition, we investigated rates and predictors for amputation in diabetic foot ulcer (DFU). Results: The overall rate of major amputation was 26.2% in patients with DFW. Among 141 DFWs, 76 patients (53.9%) had PAOD and 29 patients (38.2%) of 76 DFWs with PAOD underwent major amputation. Wound state according to Wagner classification, congestive heart failure, leukocytosis, dementia, and PAOD were the significant risk factors for major amputation. In DFW with PAOD, Wagner classification grades and leukocytosis were the predictors for major amputation. In addition, amputation was performed for 28 patients (38.4%) while major amputation was performed for 5 patients (6.8%) of 73 DFUs. Only the presence of osteomyelitis (OM) showed significant difference for amputation in DFU. Conclusion: This study represented that approximately a quarter of DFWs underwent major amputation. Moreover, over half of DFW patients had PAOD and about 38.2% of them underwent major amputation. Wound state and PAOD was major predictors for major amputation in DFW. Systemic factors, such as CHF, leukocytosis, and dementia were identified as risk factors for major amputation. In terms of DFU, 38.4% underwent amputation and the presence of OM was a determinant for amputation.
Background: Early non-small cell lung cancer (NSCLC) that abuts adjacent structures requires careful evaluation due to its potential impact on postoperative outcomes and prognosis. We examined stage I NSCLC with invasion into adjacent structures, focusing on the prognostic implications after curative surgical resection. Methods: We retrospectively analyzed the records of 796 patients who underwent curative surgical resection for pathologic stage IA/IB NSCLC (i.e., visceral pleural invasion only) at a single center from 2008 to 2017. Patients were classified based on tumor abutment and then reclassified by the presence of visceral pleural invasion. Clinical characteristics, pathological features, and survival rates were compared. Results: The study included 181 patients with abutting NSCLC (22.7% of all participants) and 615 with non-abutting tumors (77.3%). Those with tumor abutment exhibited higher rates of non-adenocarcinoma (26.5% vs. 9.9%, p<0.01) and visceral/lymphatic/vascular invasion (30.4%/33.1%/12.7% vs. 8.5%/22.4%/5.7%, respectively; p<0.01) compared to those without abutment. Multivariable analysis identified lymphatic invasion and male sex as risk factors for overall survival (OS) and disease-free survival (DFS) in stage I NSCLC measuring 3 cm or smaller. Age, smoking history, vascular invasion, and recurrence emerged as risk factors for OS, whereas the presence of non-pure ground-glass opacity was a risk factor for DFS. Conclusion: NSCLC lesions 3 cm or smaller that abut adjacent structures present higher rates of various risk factors than non-abutting lesions, necessitating evaluation of tumor invasion into adjacent structures and lymph node metastasis. In isolation, however, the presence of tumor abutment without visceral pleural invasion does not constitute a risk factor.
This study aims to propose an optimal asset allocation that minimizes the risk of insufficient realized replacement rates compared to the OECD average replacement rate. To do this, we set the shortfall risk of replacement rates and calculates an asset allocation plan to minimize this risk based on the period of enrollment, the income level and additional contribution. We consider stocks and deposits as investment assets, using Monte Carlo simulation with a GBM model to generate return distributions for stocks. Our result show that, for individuals with a enrollment period of less than 30 years, participants should invest a minimum of 70-80% of their funds in risky assets to minimize the shortfall risk. However, the proportion of funds that need to be invested in risky assets declines significantly when participants contribute an additional premiums. This effect is particularly pronounced among low-income individuals. Therefore, to achieve OECD average replacement rates, the government needs to incentivize participants to invest more in risky assets, while also providing policies to encourage additional contributions, especially for the low-income population.
Although the incidence of breast cancer in Asia remains lower than in North America, Western Europe, and Oceania, rates have been increasing rapidly during the past few decades, and Asian countries now account for 40% of breast cancer cases diagnosed worldwide. Breast cancer mortality has also increased among Asian women, in contrast to decreased mortality in Northern America, Western Europe, and Oceania. These increased rates are associated with higher prevalence of breast cancer risk factors (e.g., reduced parity, delayed childbirth, increased obesity) that have accompanied economic development throughout the region. However, Asian regions (western, south-central, south-eastern, and eastern) and countries differ in the types and magnitude of changes in breast cancer risk factors, and cannot be viewed as a single homogeneous group. The objective of this paper was to contrast the heterogeneous epidemiology of breast cancer by Asian regions and countries, and to suggest potential avenues for future research.
In this study, the risk rates of different contamination sources of the Foreign material in a minienvironment were analyzed through CFD simulation. From the results, the ambient contamination sources mainly affect wafers in the FOUP, whereas the internal contamination sources mainly affect wafers laid on the robot arm in the minienvironment. And the purging plenum system is very useful in protecting the wafers in the FOUP from Foreign materials transferred from the FFU. However, this system is unable to protect the wafers on the robot arm from internal Foreign materials and the wafers in the FOUP from sources of the interface between the FOUP and the minienvironment.
A traffic accident analysis method was developed and tested based on the highway alignment risk indices using geographic information systems(GIS). Impacts of the highway alignment on traffic accidents have been identified by examining accidents occurred on different alignment conditions and by investigating traffic accident risk indices(TARI). Evaluative criteria are suggested using geometric design elements as an independent variable. Traffic accident rates were forecasted more realistically and objectively by considering the interaction between highway alignment factors and the design consistency. And traffic accident risk indices and risk ratings were suggested based on model estimation results and accident data. Finally, forecasting traffic accident rates, evaluating the level of risk and then visualizing information graphically were combined into one system called risk assessment system by means of GIS. This risk assessment system is expected to play a major role in designing four-lane highways and developing remedies for highway sections susceptible to traffic accidents.
The age-specific anti-hepatitis A virus (HAV) seroprevalence rates in South Korea have changed markedly since the last 2030 years with an improvement in the socio-economic, housing, and environmental-sanitation conditions. These changes are characterized by very low anti-HAV seropositive rates among individuals less than 30 years of age; however, nowadays, most adolescents and young adults at an increased risk of developing symptomatic HAV infections. The Korea Center for Disease Control Sentinel Surveillance System has recently revealed an increase in the incidence of hepatitis A infection since 2001 and has revealed a potential endemic nature of the hepatitis A infection. Hepatitis A vaccines that were introduced in 1997 in Korea have made the current anti-HAV IgG positive rates in children (less than 10 years of age) approximately 50% of the rates observed in Seoul in 2006. However, in the same year, a few children were diagnosed as having anti-HAV IgG antibodies in Busan. This suggests the presence of some difference in the vaccination policy among doctors practicing in Seoul and Busan. Thus, the current recommendation of vaccinating 12-year-old child with HAV vaccination should be emphasized and a new strategy should be developed for the vaccination program to cater to the adolescents and young adults who are not immune, as well as for persons who are at a high risk for hepatitis A viral infection such as military personnel and hospital and day care center employees. Further, urgent hepatitis A vaccinations are also needed in patients with chronic liver diseases.
The estimation of radioactivity level is vital for population health risk assessment and geological point of view and can be evaluated as rate of exhalation and source concentration (226Ra, 232Th and 40K). The present study deals with the soil samples for investigation of radionuclides content and exhalation rates of radon -thoron gas from different sites in northern Haryana, India. Absorbed dose and associated index estimated in the present study are the measures of environmental radioactivity to inhalation dose. Effective doses received by different tissues and organs by considering different occupancy and conditions are also measured. Exhalation rates of radon and thoron are measured with active scintillation monitors based on alpha spectroscopy namely scintillation radon (SRM) and thoron (STM) monitors respectively. Sample height was optimized before measurement of thoron exhalation rate using STM. Average values of radon and thoron exhalation are found 16.6 ± 0.7 mBqkg-1h-1 and 132.1 ± 2.6 mBqm-2s-1 respectively. Also, a simple approach was also adopted, to evaluate the thoron exhalation which accomplished a lot of challenges, the results are compared with the data obtained experimentally. The study is useful in the nationwide mapping of radon and thoron exhalation rates for understanding the environmental radioactivity status.

The military has adopted and implemented risk assessment since 2013 to reduce fatalities and accident rates at construction sites. However, risk assessment is carried out as a voluntary preventive activity and is maintained as a superficial administrative process for owners. Risk assessment must comply with the Occupational Safety and Health Act and the Enforcement Guideline of Risk Assessment. However, the scope of the assessment covers many industries, including manufacturing and distribution, making it difficult to apply to construction sites. As a result, the guidelines are interpreted and applied differently at each construction site. This study aims to improve the risk assessment system so that it is applicable to construction projects and proposes the improvement of the risk assessment system by analyzing the current status of serious accidents in military construction projects.
Classification as a tool to extract information from data plays an important role in science and engineering. Among various classification methodologies, support vector machine has recently seen significant developments. The central problem this paper addresses is the accuracy of support vector machine. In particular, we are interested in the situations where fast rates of convergence to the Bayes risk can be achieved by support vector machine. Through learning examples, we illustrate that support vector machine may yield fast rates if the space spanned by an adopted kernel is sufficiently large.
