Paranasal sinusitis, especially chronic is one of the most common diseases in the field of otolaryngology. It is similar to Bee Yeun(鼻淵) in oriental medicine. Most cases of sinusitis are due to anatomical abnormalities within ostiomeatal unit or disturbed mucocilliary flow. The ostiomeatal unit is the first place of contact with bacteria and allergens during aspiration, and it can be obstructed easily by minute mucosal swelling due to anatomical narrowness. Therefore the treatment of paranasal sinusitis is not easy and often leads to recurrences in spite of long term treatment or surgical therapy. We studied 83 patients who had visited our hospital with complaints of nasal symptoms; they had been diagnosed as having paranasal sinusitis through an endoscopy or CT scan in another hospital and were diagnosed as the paranasal sinusitis through a PNS series. The results were as follows: 1. Age and sex distribution: The most common occurence was found between 6-10 and 1-5 years old. The Males Were 52($62.7\%$) and Females were 31($37.3\%$). 2. By residence 58 cases lived in apts: 25 lived in houses. 3. The longest duration of disease varied from a lower of 13 to a higher of 36 months in 22 cases and from 7 to 12 months in 21 cases. 4. The most common complication & past history with otolaryngologic or allergic disease were adenoid or tonsil hypertrophy & tonsillectomy and adenoidectomy (21 cases). In decreasing order the others were atopic dematitis, otitis media with effusion and allergic rhinitis. 5. Distribution of paranasal sinus disease was most common in both maxillary sinuses in 52 cases. 12 cases showed a normal PNS X-ray series but these had been diagnosed as paranasal sinusitis with an endoscopy or CT scan in another hospital. 6. Common sinusitis - related symptoms were from highest incidence to lowest nasal obstruction, postnasal drip rhinorrhea, frontal headache, cough with sputum. 7. The most administered of prescription was Gamibangpoongtongsungsan and Sunbangpaedoksan extract. 8. In 26 cases the subjects showed significant improvement symptoms in PNS X-ray series : In 21 case showed partial improvement symptoms.
Background &Objectives : Rhinitis is an inflammation of nasal mucosa and the major symtoms are watery rhinorrhea, sneezing, itchy nose, and nasal obstruction. Rhinitis is classified into allergic rhinitis and nonallergic rhinitis. Allergic rhinitis is an immune reaction by allergen, and vasomotor rhinitis which is nonallergic and noninfectious is hypersensitive reaction. The incidence of allergic rhinitis has increased and the rate of vasomotor rhinitis is high. However there have been no studies about vasomotor rhinitis compared with allergic rhinitis. And there have been no studies so far performed on the effect of Tongkwansan. Therefore this study is aimed to find out the effects of Tongkwansan on allergic rhinitis and vasomotor rhinitis. Materials and Methods : Fifteen BALC/c mouses were divided into three groups : normal group, control group and sample group. To induce the allergic rhinitis in control group and sample group, mouses were sensitized intrapertioneally 0.1% ovalbumin solution three times at intervals of 1 week. Then intranasal sensitization was performed by diffusing 0.1 % ovalbumin solution 3 times at intervals of 2 days. After that time, mouses in the sample group were oral administration treated by Tongkwansan for 28 days. We observed changes in the segment of IL-4, IL-5, $IFN-{\gamma}$, Total IgE, and ovalvumin specific IgE in blood. We used the statistical methods of ANOVA test(p<0.05). Results : There were no significant changes statistically in $IFN-{\gamma}$, IL-4, and IL-5 in blood(p<0.05). There were also no significant changes statistically in Total IgE, OVA-specific IgE in blood(p<0.05). Conclusion : According to above results, it is supposed that Tongkwansan has no significant effects on allergic rhinitis. But it is supposed that Tongkwansan has significant effects on vasomotor rhinitis which is nonallergic and noninfectious
Objectives : The purpose of this study is to investigate for rhinitis of middle school students and to confirm the effect of oriental medical treatments on rhinitis. We proceeded with this oriental medical treatment programs in collaboration with community health center. Methods : We have given oriental medical treatment, such as herbal medicines(pillet type Gamisinicheongpye-um and ointment) and intradermal acupuncture therapy at Hapgok (LI4), Yeonghyang (LI20), Indang, Bi-ik points, to patients for 9 weeks. Results of this treatment program were evaluated on a 4-point-scale depends on symptoms ; 'severe ()', 'moderate (++)', 'mild (+)' and 'no symptom (-)'. Results : After oriental medical treatments, rhinitis symptoms (sneezing, rhinorrhea, nasal obstruction, nasal itching and dysosmia) were improved significantly compared to before treatments (p<0.05). Conclusions : We confirmed rhinitis treated with oriental medicine should significant improvement and suggest that it is necessary to expand oriental medical treatment programs in collaboration with community health center.
There is much confusion in the field of Rhinitis regarding how to best measure disease severity objectively, Therefore, we aimed to establish a new adequate scoring system for Rhinitis, that should be based on comparison analysis of various scoring systems. We report as follows. We researched for data relating to severity scoring systems for rhinitis in Entrez PubMed from 1995 to 2005 and in Kiss Kstudy. Results and Conclusions: Properties of severity scoring systems were validity, sensitivity of change and ease of use. The essential items of severity scoring systems were subjective symptoms. The criterion of severity were divided into subjective symptoms and complication and Quality of Life. Intensity items are nasal obstruction, rhinorrhea, sneezing, itching, Postnasal drip, nasal mucosa swelling, nasal mucosa color, complication. Subjective symptoms is difficulty of Life. The significant items of severity scoring system are nasal symptoms. The whole score does with the maximum 30 scores. As it were, we assumed nasal symptoms accounted for around 80% of each total score, with complication and difficulty of Life representing 20%.
Objectives: The purpose of this study is to analyze both treatment and adverse events of Korean medicine treatment for whiplash associated disorder patients during pregnancy. Methods: We investigated the medical records of 44 pregnant patients who had got into car accidents from April 1, 2018 to May 31, 2019. Results: 100 percent of the inpatients and 88 precent of the outpatients were treated by acupuncture. 79 percent of the inpatients and 72 precent of the outpatients were treated by chuna, and 86 percent of the inpatients and 64 precent of the outpatients were treated by pharmacopuncture. Out of 32 patients with low back pain, 94 percent were treated by acupuncture, 47 percent by phramacopuncture and 38 precent by chuna. 90 percent of 31 patients with cervical pain were treated by acupuncture, 71 percent were treated by pharmacopuncture and 77 percent were treated by chuna. There were adverse events like cough, rhinorrhea, dilatation of cervix and worsen low back pain during or after the treatment. Conclusions: Korean medicine treatment including chuna and pharmacopuncture can be applied to pregnant women injured by traffic accident.
Objective : The purpose of this study is to summarize the differential diagnosis between allergic and nonallergic rhinitis and suggest management. Methods : We reviewed the current update of medical history and physical examination for allergic and nonallergic rhinitis. Then we analyzed clinical characteristics according to onset age, gender incidence and state of nasal symptoms, etc. Results : 1. Patients with nonallergic rhinitis tend to develop symptoms at a later age(>35 years of age), and there is a female-to-male incidence ratio for nonallergic rhinitis of 2:1 to 3:1. 2. Patients with nonallergic rhinitis report nasal congestion, nasal rhinorrhea and more often report postnasal drip rather than sneezing and itching, which are predominant symptoms of allergic rhinitis. And the nasal mucosa in nonallergic rhinitis usually looks normal. 3. Patients with nonallergic rhinitis have few complaints of concomitant symptoms of allergic symptoms and the absence of other atopic diseases in the patient or in the family supports the diagnosis of nonallergic rhinitis. 4. Common triggers of nonallergic rhinitis are nonspecific irritant exposures and many patients with nonallergic rhinitis find that antihistamines have no benefit. Conclusions : The differential diagnosis between allergic and nonallergic rhinitis is clinical and relies on a detailed medical history and physical examination.
Purpose: Recently there are scientific evidences for the health effects of Asian dust-storms. Particularly in 2002, the daily average of $PM_10$ exceeded 600 and 700 $\mu{g}/m^3$ in March and April respectively. We examined the effects of Asian dust‐storms on perceived symptoms and preventive behaviors among children, adults, and elders. Method: The subjects of this study were 425 children, 444 adults and 60 elders. A questionnaire survey was carried out on the children and the adults by teachers and on the elders by interviewers to obtain information on demographic variables, perceived symptoms and preventive behaviors, previous respiratory disease, environmental tobacco smoke, and proximity between the house and the road during Asian dust-storms. We analyzed using descriptive statistics, $X^2$ test and multiple logistic regression models. After adjustment for potential confounders, we estimated the odds ratios and 95% confidence intervals of the children and the elders for perceived symptoms and preventive behaviors. Result: The elders had a significantly higher prevalence of respiratory symptoms than the other groups. The children had a significantly higher prevalence than the adults in getting URI (1.51), coughing (1.68), rhinorrhea (1.46), fever (2.39) and medication for allergy or asthma (1.90). The children had better behaviors than the elders in closing windows (3.57), taking less outdoor recreational activity (2.59) and staying home (2.60). Conclusion: This study suggested that children and elders are susceptible to the effects of Asian dust on health. Health educators should inform these populations about the influence of Asian dust and provide health education for preventive behaviors.
Objective: The purpose of this case study was to report the effect of Korean medicine treatment on a patient diagnosed with a nasal polyp. Methods: A 62-year-old female patient with a nasal polyp visited the clinic sixteen times from September 11th, 2018 to December 18th, 2018. The patient was treated with Eunhwayeongyo-tang, Haeyeol-tang, Hongsamkyungok-go extract, and acupuncture during the treatment period. We evaluated the improvement of symptoms by nasoendoscopy. Results: After the Korean medicine treatment, the nasal polyp was completely eradicated. Hyperemia of the nasal mucous membrane and rhinorrhea were also relieved. Conclusion: The study findings suggested that Korean medicine treatment (Eunhwayeongyo-tang, Haeyeol-tang, and Hongsamkyungok-go extract, and acupuncture) might be effective for treatment of patients diagnosed with nasal polyps.
Objective : The endoscopic endonasal transsphenoidal approach is a widely-used method for the surgical treatment of pituitary adenomas. We aimed to evaluate the results of endoscopic surgery by comparing preoperative classification methods and investigating their relationship with postoperative resection and remission rates and complications. Methods : We retrospectively reviewed the medical records of 236 patients (118 males) who underwent surgery for pituitary adenomas. Preoperative Knosp classification, tumor size (TS), suprasellar extension (SSE), postoperative resection and remission rates, and complications were evaluated. Results : The follow-up period was 3 months to 6 years. The patients' ages ranged between 16 and 84 years. Endocrinologically, 114 patients (48.3%) had functional adenoma (FA), and 122 patients (51.7%) had non-functional adenoma (NFA). Among the FA group, 92 (80.7%) showed remission. A statistically significant difference was found between patients with and without remission in terms of the Knosp, TS, and SSE classifications (p<0.01). Knosp, TS, and SSE classification grades were found to be correlated with the resection rates (p<0.01). Meningitis was seen in seven patients (3.0%), diabetes insipidus in 16 (6.9%; permanently in two [0.9%]), and rhinorrhea in 19 (8.1%). Thirty-six patients (15.3%) developed pituitary insufficiency and received hormone replacement therapy. Conclusion : The resection categories and remission rates of FAs were directly proportional to the adenoma sizes and Knosp grades, while the degree of suprasellar growth further complicated resection and remission rates. Adenoma sizes less than 2 cm and SSEs less than 1 cm are associated with favorable remission and resection rates.
Background : Palivizumab is an intravenous monoclonal antibody which is used in the prevention of respiratory syncytial virus (RSV) infection. It is currently recommended for infants who are at high-risk for RSV infections due to preterm birth or other medical conditions such as congenital heart disease. Palivizumab is a humanized monoclonal antibody directed against an epitope in the antigenic site A of the protein F of RSV particles. Palivizumab is given once a month via intramuscular (IM) injection throughout the duration of the RSV season. Since palivizumab is known to have preventive effects against RSV infection for children with older siblings, the insurance coverage for palivizumab was expanded in October 2016. Methods : The electronic medical records of children under 2 years old who have older siblings who visited or were admitted to the Severance Hospital from October 2015 to May 2016 and from October 2016 to May 2017 were reviewed retrospectively. The data were then divided into two groups depending on the pilivizumab administration. Results : A total of 67 patients were enrolled in this study. The effectiveness in the reduction of hospitalization was statistically significant (p=0.009). Palivizumab decreased respiratory symptoms such as cough, rhinorrhea, and fever in patients with older siblings (p 0.05). Conclusions : In this study, palivizumab administration was effective in preventing RSV infection in infants with older siblings. Expanding palivizumab-prophylaxis administration to infants with older siblings may be effective in the prevention of upper respiratory infections.
