Background: The management of cleft lip and palate aims at improving the patient's aesthetic and functional outcomes. Delaying primary repair can disrupt the patient's functional status. Long-term follow-up is essential to evaluate the need for secondary repair or revision surgery. This article presents the epidemiology of cleft lip and palate, including comprehensive patient characteristics, the extent of delay, and secondary repair at our institutional center, the Bandung Cleft Lip and Palate Center, Faculty of Medicine, Universitas Padjadjaran, Bandung, Indonesia. Methods: This retrospective study aimed to determine the epidemiology and recurrence rates of cleft lip and palate at the Bandung Cleft Lip and Palate Center, Indonesia, from January 2007 to December 2021. The inclusion criteria were patients diagnosed with cleft lip and/or palate. Procedures such as labioplasty, palatoplasty, secondary lip and nasal repair, and alveolar bone grafting were performed, and data on recurrence were available. Results: In total, there were 3,618 patients with cleft lip and palate, with an age range of 12 months to 67 years. The mean age was 4.33 years, and the median age was 1.35 years. Males predominated over females in all cleft types (60.4%), and the cleft lip was on the left side in 1,677 patients (46.4%). Most cases were unilateral (2,531; 70.0%) and complete (2,349; 64.9%), and involved a diagnosis of cleft lip and palate (1,981; 54.8%). Conclusion: Delayed primary labioplasty can affect daily functioning. Primary repair for patients with cleft lip and palate may be postponed due to limited awareness, socioeconomic factors, inadequate facilities, and varying adherence to treatment guidelines. Despite variations in the timing of primary cleft lip repair (not adhering to the recommended protocol), only 10% of these patients undergo reoperation. Healthcare providers should prioritize the importance of the ideal timing for primary repair in order to optimize physiological function without compromising the aesthetic results.

This study presents a unique way to improve consumer-oriented service for assistive orthosis products. Satisfaction and important factors were analyzed based on responses from disabled people who have used the orthoses. We surveyed 185 disabled people with QUEST (Quebec User Evaluation of Satisfaction with assistive Technology), and we conducted in-depth interviews with 25 individuals. The average satisfaction score was 3.68 (orthoses satisfaction: 3.78; service satisfaction: 3.52). For each item, the highest score was effectiveness, and the lowest one was cost. As important factors for each item, the participants selected cost, comfort, repair, and service. The fee system for orthoses needs rational revision, and a delivery system should be constructed for follow-up services such as repair.
A 57-year-old man who simultaneously underwent an operation for repair of rotator cuff and a revision operation for nonunion of a Pilon fracture presented with osteomyelitis of the humeral head on the $19^{th}$ day after surgery due to MRSA (Methicillin-resistant Staphylococcus aureus) infection. Infection was controlled after administration of appropriate intravenous antibiotic therapy and performance of several surgical procedures. However, devastating defects at the humeral head and the rotator cuff remained. No case of short term MRSA induced osteomyelitis has been reported.

We experienced three cases with chronic shoulder pain due to a knot impingement after arthroscopic rotator cuff tear repair and treated with arthroscopic revision surgery. Ultrasonography is commonly used for an imaging scan and an useful diagnostic tool to follow up after rotator cuff repair recently. We also could diagnose three cases with the knot impingement using ultrasonography obviously. And we report these cases with a review of current literature.

The maintenance and repair work of sidewalk block pavement is a construction that requires a large amount of budget to be invested every year. It is important to establish an appropriate standards for estimating construction cost to ensure proper budgeting and quality. In this study, the standards for estimating the cost of maintenance and construction work for sidewalk blocks that can be applied to the construction volume classified according to the site conditions, construction type, and equipment use differentiated from new construction was established. As a result, the daily construction volume was presented by reflecting excavator and truck as equipment in the combination of paver and common worker. The re-installation was applied by separating the construction volume of sections with general blocks and induction/raised blocks based on the installation of sidewalk blocks after demolition. Generally if block cutting is necessary, the precision construction conditions using a cutting machine were taken into consideration to secure the construction quality. In addition, it has been revised to apply classified construction volume into A and B-Type depending on the park and site conditions.
Object of this paper are numerical and experimental evaluation of Korean Standard Train Loading(L-loading scheme) with respect to UIC's and real train loading, quantitative formulation of the real train types in South Korea. These objects are require to changing environment of train operation, for example, high density traffic and gradual train speed increase. For the reasonable repair, reinforcement and rehabilitation of existing railway infrastructures, statistical analysis of the loading effect during the long term experiment in conventional lines have to conduct. Statistical quantitative formulation of the loading case need to consistency numerical evaluation of the railway safety. Hereafter, those results will be core technical data for the economy enhancement of international line construction. Further, these results are using to make track maintenance criterions for transcontinental, speed up railway and revision of standard train leading diagram.

Penetrating wounds to the face are cosmetically devastating and can be life-threatening. If the foreign body causing the penetrating wound is a piece of wood, small remnants might be left behind after the initial treatment. A 33-year-old male patient presented to the emergency center after a piece of lumber pierced his face as a passenger in a traffic accident. The patient's vital signs were stable, and emergency surgery was performed to remove the foreign body and repair the soft tissue. No noteworthy complications were seen after open reduction and internal fixation of the facial bone fractures. Seven months after the accident, the patient underwent scar revision along with full-thickness skin grafting for post-traumatic scars. After the surgery, pus-like discharge which was not previously present was observed, and the graft did not take well. A residual foreign body, which was the cause of graft failure, was found on computed tomography and the remaining foreign body was removed through revision surgery. The patient is receiving outpatient follow-up without any complications 6 months after surgery. This case demonstrates the importance of performing a careful evaluation to avoid missing a residual foreign body, especially if it is of wooden nature.
Objective : The authors performed a retrospective study to assess the clinical and radiological outcome in symptomatic lumbar spondylolysis patients who underwent a direct pars repair surgery using two different surgical methods; pedicle screw with universal hook system (PSUH) and direct pars screw fixation (DPSF), and compared the results between two different treated groups. Methods : Forty-seven consecutive patients (PSUH; 23, DPSF; 15) with symptomatic lumbar spondylolysis who underwent a direct pars repair surgery were included. The average follow-up period was 37 months in the PSUH group, and 28 months in the DPSF group. The clinical outcome was measured using visual analogue pain scale (VAS) and Oswestry disability index (ODI). The length of operation time, the amount of blood loss, the duration of hospital stay, surgical complications, and fusion status were also assessed. Results : When compared to the DPSF group, the average preoperative VAS and ODI score of the PSUH group were less decreased at the last follow-up; (the PSUH group; back VAS : 4.9 vs. 3.0, leg VAS : 6.8 vs. 2.2, ODI : 50.6% vs. 24.6%, the DPSF group; back VAS : 5.7 vs. 1.1, leg VAS : 6.1 vs. 1.2, ODI : 57.4% vs. 18.2%). The average operation time was 174.9 minutes in the PSUH group, and 141.7 minutes in the DPSF group. The average blood loss during operation was 468.8 cc in the PSUH group, and 298.8 cc in the DPSF group. The average hospital stay after operation was 8.9 days in the PSUH group, and 7 days in the DPSF group. In the PSUH group, there was one case of a screw misplacement requiring revision surgery. In the DPSF group, one patient suffered from transient leg pain. The successful bone fusion rate was 78.3% in the PSUH group, and 93.3% in the DPSF group. Conclusion : The present study suggests that the technique using direct pars screw would be more effective than the method using pedicle screw with lamina hook system, in terms of decreased operation time, amount of blood loss, hospital stay, and increased fusion success rate, as well as better clinical outcome.

The specialized structure of the upper eyelid ensures complete closure of the eye and eyeball sealing. An upper eyelid injury can cause various symptoms associated with eyeball trauma, not just scar formation or eyelid deformity. In this report, we describe a case of lagophthalmos observed after wound repair in a patient with a crushing injury caused by a grinder. Several surgical techniques are used to treat lagophthalmos or scar contracture. In most cases, a releasing procedure is performed after 6 months of initial repair. However, if the patient has severe symptoms that are not relieved by conservative care, early revision is inevitable. We describe a case of early lagophthalmos successfully resolved with pentagonal wedge resection, fat redistribution, and full-thickness skin grafting. After the revisional surgery, we observed that the patient regained the ability to completely close the injured eyelid, with restoration of function and favorable cosmetic outcomes. Pentagonal wedge resection to release a retracted structure, fat redistribution to prevent readhesion, and full-thickness skin grafting for enough amount of skin to regain upper eyelid function is useful for scar release and lagophthalmos following crushing injuries of the upper eyelid.
The maintenance and improvement of housing become more important than quantitative supply. So this paper suggests the directions to solve the current issues of housing improvement policies. In 2015, the housing improvement for the low-income has been expanded with the revision of housing benefit policy. And each related government departments are steadily carrying out various housing improvement programs. Under the current improvement policies, individual houses are being repaired according to the criteria of the target selection. And the repair allowance of the housing benefit is limited to the ownership households except the rented. Hereafter, it has to considered the review of the detailed goal setting and application problems in housing improvement policy and to consider the extension to the rented households and the raise of the income criteria of targets. Also, it needs to carry out the improvement of several neighboring houses at the same time for synergy effect other than individual house, to change the selection way of repair contractors and to secure the quality and advancement of the improvement.
