본 연구를 통해 역이온 영동(reverse iontophoresis)을 이용한 무채혈 혈당 측정시스템의 patch sensor에 사용되는 효소 고정형 수화젤의 생체적합성 여부를 확인하고자 하였다. 아크릴레이트 계열의 단량체를 사용하여 일정한 unit의 효소가 고정된 수화젤을 합성하였다. 합성된 수화젤의 물성분석을 위해 FT-IR spectrometer를 이용하여 구조분석을 하고 DSC를 이용하여 열적 안정성을 확인하였다. 또한, UV-Vis spoctrophotometer를 이용하여 표준품 대비 50% 이상의 효소 활성도를 확인하였다. 표면의 효소 고정화 확인을 위해 SEM을 이용하여 확인한 결과 효소가 고정화되어 있음을 확인하였다. 수화젤은 환자의 피부에 직접적으로 접촉됨으로써 접촉 시 무해성을 평가하기 위하여, ISO-10993에 의하여 세포독성(cytotoxicity), 피내반응(intracutaneous reactivity), 피부자극(skin irritation) 및 감작성(maximization sensitization) 시험을 실시하였고 이를 통해 생체적합성이 우수하다는 것을 확인하였다.
Recent technical advancement allows noninvasive measurement of blood glucose. In this literature, we reviewed various noninvasive techniques for measuring glucose concentration. Optical or electrical methods have been investigated. Optical techniques include near-infrared spectroscopy, Raman spectroscopy, optical coherence technique, polarization, fluorescence, occlusion spectroscopy, and photoacoustic spectroscopy. Electrical methods include reverse iontophoresis, impedance spectroscopy, and electromagnetic sensing. Ultrasound, detection from breath, or fluid harvesting technique can be used to measure blood glucose level. Combination of various methods is also promising. Although there are many interesting and promising technologies and devices, there need further researches until a commercially available non-invasive glucometer is popular.
The objective of this work is to study transdermal delivery of levodopa using iontophoresis and evaluate various factors which affect the transdermal transport. Levodopa is unstable in aqueous solution, and, in order to establish a stable condition for levodopa for the duration of experiment, we investigated the stability of levodopa in aqueous solutions of different pHs with/without the addition of dextrose or the application of current. Using stable aqueous solution, we have studied the effect of pH, polarity and penetration enhancer (ethanol) on transdermal flux and compared the results. We also investigated the iontophoretic flux from hydroxypropyl cellulose (HPC) hydrogel. In vitro flux study was performed at $33^{\circ}C$, using side-by-side diffusion cell. Full thickness hairless mouse skin and rat skin were used for this work. Current densities applied were 0.4 or $0.6mA/cm^2$ and current was off after 6 hour application. Stability study showed that levodopa solution with a pH 2.5 or 4.5 maintained the initial concentration of levodopa for 24 hours with the addition of 5% dextrose. However, at pH 9.5, levodopa was unstable and 30 to 40% of levodopa degraded within 24 hours, even with the addition of 5% dextrose. Hydrogel swollen with dextrose added levodopa solution maintained about 97% of the initial concentration of levodopa for 13 days, when stored in $4^{\circ}C$. The application of current did not affect the stability of levodopa in hydrogel. Flux study from levodopa solution with pH 2.5 showed that cathodal delivery of levodopa was higher than passive or anodal delivery. When the pH of the donor solution was 4.5, anodal delivery of levodopa was higher than passive or cathodal delivery. These results seem to indicate that electroosmosis plays more dominant role than electrorepulsion in the flux of levodopa at pH 2.5, and the reverse situation applies for pH 4.5. The passive flux was unexpectedly high for the ionized levodopa. Similar to the results from aqueous solution, cumulative amount of levodopa transported trom HPC hydrogel by cathodal delivery was significantly higher than passive or anodal delivery. The treatment of 70% ethanol cotton ball by scrubbing increased passive, anodal and cathodal flux, with the largest increase for anodal flux. These results indicate that iontophoretic delivery of zwitterion such as levodopa is much complicated than that can be expected from small ionic molecules with single charge. The results also indicate that the balance between electroosmosis and electrorepulsion plays a very important role in the transport through skin.
본 연구는 동맥혈압의 조절에 결정적 역할을 하는 상부연수 복외측부에 존재하는 심맥관계 활동과 관련 있는 신경세포들에 대해 iontophoresis 법으로 투여한 신경흥분전달물질의 작용기전을 규명하고자 하였다. 이를 위해서 $\alpha$-chloralose로 마취한 고양이를 실험동물로 하여 연수의 복외측부에 multibarrel 전극을 삽입하여 심맥관계 활동과 관련 있는 신경세포의 활동을 세포외기록법으로 확인한 후 세포에 iontophoresis법으로 투여한 glutamate, GABA(\ulcorner-aminobutyric acid) 및 bicuculline의 효과를 관찰하여 다음과 같은 결과를 얻었다. 1) 자발적으로 활동전압을 보이지 않는 세포에서 glutamate를 10초 주기로 5초간 iontophoresis할 때 주기적인 세포활동을 기록할 수 있었고 이 활동은 동시에 가한 GABA에 의하여 억제되었다. 2) 자발적으로 활동하고 있는 세포들에 대하여 glutamate를 투여하였을 때 활동전압의 빈도가 증가하였고 GABA에 의해서 억제되었다. 3) GABA억제제인 bicuculline을 단독으로 투여시 활동전압 빈도의 증가는 뚜렷하지 않았으나 GABA와 동시 투여시에는 GABA의 억제작용을 차단하였다. 4) GABA의 작용은 RVLM(rostral ventrolateral medulla) 세포의 주기적 활동과 basal discharge 모두를 감소시켰으나 주기적 양상이 없어진 세포는 드물었다. 5) 말초에 가한 유해자극에 의하여 유발된 신경세포 활동이 GABA에 의하여 억제되었다. 이상의 결과로부터 $\alpha$-chloralose로 마취한 고양이에서 GABA는 RVLM에 존재하는 심맥관계 신경세포들에 대하여 억제적으로 작용하나 bicuculline의 투여후에도 이들 세포들의 주기적 활동이 유지되는 점으로 보아 RVLM세포의 자발적 흥분에 대하여 감압반사의 흥분이 기여하는 바가 크지는 않을 것으로 사료된다.
EIS (Electro interstitial scan, 전기체간스캔법)는 전극을 이용해 미세전류를 인체에 인가하고 그에 따른 전기적 반응을 분석하여 생리적인 정보를 얻는 방법으로, 비침습적이고 간단한 검사가 가능하다는 장점이 있다. 특히 당뇨병 진단을 위한 스크린용으로 적합하다는 연구들이 진행되어 왔으나 대부분 진단 원리에 대한 구체적인 논의가 이루어지지 않았다. 본 연구에서는 EIS 방법이 당뇨병 스크리닝 및 임상에 유용하게 활용될 수 있을지 분석해 보기위해 당뇨병 환자와 정상인을 대상으로 EIS 장비의 원 신호인 전압 변동 데이터를 특정경로에서 측정하였다. 전압 신호의 특징점을 추출하고 두 그룹 사이의 AUC (Area under the curve)를 계산한 결과 7개의 변수들이 60% 이상의 분류 정확도를 보였다. 또한 이 변수들을 k-NN 분류기로 학습한 결과, 왼쪽 손에서의 전압 변동 크기를 기준으로 분석했을 때 분류 정확도를 76.2%까지 높일 수 있었다. EIS 기반의 전압신호 분석법으로 비침습적인 당뇨병 스크리닝의 가능성을 보였다.
