Influence of Sleep-Related Breathing Disorders on Changes of Cardiovascular Function (수면과 관련된 호흡장애가 심혈관계의 기능 변화에 미치는 영향)

  • The data collected to date indicate that sleep-related breathing disorders, including sleep-disordered breathing(sleep apnea) and underlying respiratory system diseases, are one of the important risk factors for cardiovascular dysfunction. Sleep-disordered breathing(sleep apnea) is now recognized as one of the leading causes of systemic hypertension, cardiac arrhythmias, coronary heart disease, pulmonary hypertension, right heart failure, and stroke. Sleep may exert a profound effect on breathing in patients with underlying respiratory system disease including bronchopumonary diseases, chest wall abnormalities, central alveolar hypoventilation syndromes or respiratory neuromuscular disorders. Chronic hypoxia and hypercapnia in these patients may accelerate the development of long term cardiovascular complications such as cardiac arrhythmias, pulmonary hypertension, and right heart failure(cor pulmonale). Several recent studies reported that sleep-related breathing disorders are associated with long-term cardiovascular morbidity and mortality. Careful assessment of respiratory and cardiovascular function in these patients is critical. Aggressive and highly effective treatment of sleep-related breathing disorders using tracheostomy, mechanical ventilation, nasal continuous positive airway pressure therapy(nCPAP), intercurrent oxygen therapy or other interventions can reduce the prevalence of cardiovascular dysfunction and the long-term mortality.

Pandemic Influenza A/H1N1 Viral Pneumonia without Co-Infection in Korea: Chest CT Findings

  • Background: To evaluate chest CT findings of pandemic influenza A/H1N1 pneumonia without co-infection. Methods: Among 56 patients diagnosed with pandemic influenza A/H1N1 pneumonia, chest CT was obtained in 22 between October 2009 and Februrary 2010. Since two patients were co-infected with bacteria, the other twenty were evaluated. Predominant parenchymal patterns were categorized into consolidation, ground glass opacity (GGO), and mixed patterns. Distribution of parenchymal abnormalities was assessed. Results: Median age was 46.5 years. The CURB-65 score, which is the scoring system for severity of community acquired pneumonia, had a median of 1. Median duration of symptoms was 3 days. All had abnormal chest x-ray findings. The median number of days after the hospital visit that Chest CT was performed was 1. The reasons for chest CT performance were radiographic findings unusual for pneumonia (n=13) and unexplained dyspnea (n=7). GGO was the most predominant pattern on CT (n=13, 65.0%). Parenchymal abnormalities were observed in both lungs in 13 cases and were more extensive in the lower lung zone than the upper. Central and peripheral distributions were identified in ten and nine cases, respectively. One showed diffuse distribution. Peribronchial wall thickening was found in 16 cases. Centrilobular branching nodules (n=7), interlobular septal thickening (n=4), atelectasis (n=1), pleural effusion (n=5), enlarged hilar and mediastinal lymph nodes (n=6 and n=7) were also noted. Conclusion: Patchy and bilateral GGO along bronchi with predominant involvement of lower lungs are the most common chest CT findings of pandemic influenza A/H1N1 pneumonia.

The Histopathologic Effects of Jungcheonwhadamgangki-tang on the Experimental Asthma induced by Ovalbumin (정천화담항기탕(定喘化痰降氣湯)이 ovalbumin으로 유발된 천식(喘息)에 미치는 영향(影響))

  • Objectives: In order to evaluate the effect of jungcheonwhadamgangki-tang on Balb/c mouse of allergy-sensitive to bronchial asthma induced by ovalbumin. Methods: The changes of diameter lumen of trachea which was upper respiratory organ, weight and gross appearance of lung, histology of lung and trachea, numbers of inflammatory cells in the bronchoalveolar lavage fluld(BALF) were observed. Results : The results are obtained as follows. 1. The diameters of trachea lumen were significantly increased in Jungcheonwhadam gangki-tang treated group as compared with control group. 2. Inflammatory cells including neutrophil and eosinophil in BALF were significantly. in Jungcheonwhadamgangki-tang treated group as compared with control group. 3. Weight of lung and black spots, which resulted from infiltration of inflammatory were significantly decreased in Jungcheonwhadamgangki-tang treated group as compared with control group.4. Hypertrophy of mucous membrane of trachea and bronchus and bronchioles in the lung, peritracheal, peribronchus and peribronchiolar inflammatory cell infiltration, and mucoid exudate deposit in the lumen were significantly decreased in Jungcheonwhadamgangki-tang treated group as compared with control group. Conclusions : It is considered that Jungcheonwhadamgangki-tang has somewhat favorable effect on the bronchial asthma because the bronchial asthma specific series of abnormalities in respiratory system were decreased after oral administration of Jungcheonwhadamgangki-tang in this study. In future, it is needed that the toxicological and dosage specific study of Jungcheonwhadamgangki-tang to use against bronchial asthma with safe.

  • Churg-Strauss syndrome(CSS) is a systemic vascular disorder that has an unknown cause with multiorgan involvement and diverse presentations. The three main histologically distinct phases were necrotizing vasculitis, tissue eosinophilia and extravascular granulomas. A diagnosis of CSS can be made on four or more of the following six criteria : 1) asthma, 2) peripheral eosinophilia >10% on the differential leukocyte count, 3) mononeuropathy (including multiple) or polyneuropathy, 4) paranasal sinus abnormalities, 5) nonfixed pulmonary infiltrates and 6) biopsy evidence of extravascular eosinophils in the skin, the nerves, or the lungs. CSS has a good prognosis with systemic steroid therapy. The 5 year survival is approximately 70%. We experienced a 66-year-old man who presented with cough, sputum, edema and numbness in both legs. He presented with all of the 6 CSS criteria. A nerve and muscle biopsy confirmed the diagnosis. Here, we report this case with a review of the relevant literatures.

The Changes of Respiratory Mechanics by a Bronchodilator Inhalation Under the Variable Level of PEEP in Patients with Acute Respiratory Distress Syndrome (급성호흡곤란증후군에서 기도확장제 투여 전후에 호기말양압 수준의 변화가 호흡역학에 미치는 영향)

  • Background : Reduced lung compliance and increased lung resistance are the primary lung mechanical abnormalities in acute respiratory distress syndrome (ARDS). Although there is little information regarding the mechanisms responsible for the increases in the respiratory resistance of ARDS, bronchodilators have been frequently administered in mechanically ventilated ARDS patients. To determine the effect of a bronchodilator on the respiratory mechanics depending on the level of applied positive end-expiratory pressure (PEEP), the changes in the respiratory mechanics by salbutamol inhalation was measured under the variable PEEP level in patients with ARDS. Materials and Methods : Fifteen mechanically ventilated paralyzed ARDS patients (14 of male, mean age 57 years) were enrolled in this study. The respiratory system compliance, and the maximum and minimum inspiratory resistance were obtained by the end-inspiratory occlusion method during constant flow inflation using the CP-100 pulmonary monitor (Bicore, Irvine, CA, USA). The measurements were performed at randomly applied 8, 10 and 12 cm $H_2O$ PEEP before and 30 mins after administrating salbutamol using a meter-dose-inhaler (100ug${\times}$6). Results : 1) The maximum inspiratory resistance of the lung was higher than the reported normal values due to an increase in the minimal inspiratory resistance & additional resistance. 2) The maximum inspiratory resistance and peak airway pressure were significantly higher at 12cm $H_2O$ of PEEP compared with those at 10cm $H_2O$ of PEEP. 3) Salbutamol induced a significant decrease in the maximum and the minimum inspiratory resistance but no significant change in the additional resistance only was observed at 12cm $H_2O$ of PEEP(from $15.66{\pm}1.99$ to $13.54{\pm}2.41$, from $10.24{\pm}2.98$ to $8.04{\pm}2.34$, and from $5.42{\pm}3.41$ to $5.50{\pm}3.58cm$ $H_2O$/L/sec, respectively). 4)The lung compliance did not change at the applied PEEP and salbutamol inhalation levels. Conclusion : The bronchodilator response would be different depending on the level of applied PEEP despite the increased respiratory resistance in patients with ARDS.

  • Asthma is an inflammatory disease of airways that is induced by Th2 cytokines and inhibited by Th1 cytokines. In this study we wanted to investigate the effect of SCT and BJT on eosinophilla and cytokines of BALF in a mouse model established airway inflammmation. Asthma was induced to male c57/bl6 mice. Allergen-specific antibody responses, cytokine$(IL-4,\;IL-5,\;INF-{\gamma})$, and eosinophil inflammation of the airways were investigated on the BALF and splenocyte. SCT and BJT effectively induced $INF-{\gamma}$ and inhibited IL-4, IL-5 as well as eosinophilic inflammation when SCT and BJT were administered. Total Ig E level in the BALF decreased. SCT was more effectiveness than BJT. It is considered that SCT and BJT have faborable effect on the asthma because the asthma specific series of abnormalities in respiratory system were decreased.

  • A 42-year-old man was diagnosed with cancer of the right lower lung lobe with a posteparterial type of tracheal bronchus, in which the posterior segmental bronchus of the right upper lobe arose from the distal bronchus intermedius. A mass involved the distal bronchus intermedius, requiring a right lower bilobectomy with an additional posterior segmental resection of the right upper lung lobe. Thus, we performed a right lower bilobectomy and sleeve anastomosis of the posterior segmental bronchus of the right upper lobe to the proximal bronchus intermedius, sparing the pulmonary parenchyma of the same lobe.

  • A hiccup is caused by involuntary, intermittent, and spasmodic contractions of the diaphragm and intercostal muscles. It starts with a sudden inspiration and ends with an abrupt closure of the glottis. Even though a hiccup is thought to develop through the hiccup reflex arc, its exact pathophysiology is still unclear. The etiologies include gastrointestinal disorders, respiratory abnormalities, psychogenic factors, toxic-metabolic disorders, central nervous system dysfunctions and irritation of the vagus and phrenic nerves. Most benign hiccups can be controlled by traditional empirical therapy such as breath holding and swallowing water. However, though rare, a persistent hiccup longer than 48 hours can lead to significant adverse effects including malnutrition, dehydration, insomnia, electrolyte imbalance, and cardiac arrhythmia. An intractable hiccup can sometimes even cause death. We herein describe a patient with non-small cell lung cancer who was severely distressed by a persistent hiccup.

  • Sarcoidosis is a multi-systemic syndrome of an unknown etiology, and it is characterized by the formation of multiple noncaseating granulomas that disrupt the architecture and function of the tissues in which they reside. The most commonly affected organs are lung, skin and lymph nodes. Overt clinical involvement of the nervous system is uncommon and this occurs in about 5% of all patients during the course of their disease. The most common manifestations are granulomatous leptomeningitis, cranial nerve palsy, electrolyte or other endocrinologic abnormalities, but isolated memory impairment is a rare manifestation. This is a case of 59 years-old male with recent memory impairment, and he was previously diagnosed with pulmonary sarcoidosis by transbronchial lung biopsy. The brain MRI imaging revealed the leptomeningeal and parenchymal involvement of sarcoidosis. He was treated with high dose corticosteroid and his memory function was improved to nearly a normal level. We report here on a case of successful treatment of pulmonary sarcoidosis combined with neurosarcoidosis with using high dose corticosteroid, and the patient presented with recent memory impairment.

  • Objectives : In order to study the effect of oral administration of Jungcheonhwadamgangki-tang against the asthma. Method : Asthma was induced to Balb/c mouse with ovalbumin by using method of Hatfield et al. It was observed the changes numbers and morphology of the mast cells in the trachea, numbers of mucous secretory cell in the bronchus, morphology of the bronchus, ultramicroscopical appearance of surface of trachea and number of cilia and mucous secretory cells by scanning electron microscope. Result : 1. Degranulation and decreasing of the numbers of mast cells were significantly decreased in the Jungcheonhwadamgangki-tang extract group as compared with control group. 2. Hypertrophy of mucous membrane of bronchus In the lung, infiltration of inflammatory cells, increasing of mucous secretory cells in the bronchus were significantly decreased in the Jungcheonhwadamgangki-tang extract group as compared with control group. 3. Shedding, decreasing of cilia cells and increasing of mucous secretory cells in the surface of the trachea were significantly decreased in the Jungcheonhwadamgangki-tang extract group as compared with control group. Conclusion : It is considered that Jungcheonhwadamgangki-tang has somewhat favorable effects on the asthma because the asthma specific series of abnormalities in respiratory system were decreased after oral administration of Jungcheonhwadamgangki-tang in this study.
