Chest Wall Reconstruction Using Latissimus Dorsi Myocutaneous Flap (광배근피판을 이용한 외상성 흉벽 결손의 재건 1례)

  • Chest wall reconstruction Is difficult because of the physiological and anatomical functions of thoracic cavity where the thoracic cavity must be stabilized in order to prevent paradoxical motion, and adequate protection must be provided to the mediastinal structures. A 37-year-old male patient with full-thickness defect on the left anterior thoracic cavity due to traffic accident was treated. Emergency operation was performed for debridement and bleeding control, and on postoperative 25 days, chest wall reconstruction was performed as a second-stage operation. Left anterior chest wall was reconstructed with latissimus dorsi myocutaneous flap which had thoracodorsal arterial pedicle, and split-thickness skin grift from the left thigh was done. After chest wall reconstruction, spontaneous self-respiration was possible without ventilatory support. The pulmonary function test performed at postoperative 3 months revealed 80% of predicted values (FVC, FEV1.0). The postoperative result of chest wall reconstruction using latissimus dorsi myocutaneous flap was excellent anatomically, physiologically and aesthetically.

  • Superoxide dismutase which is metalloenzyme that decomposes superoxide radicals into hydrogen peroxide and molecular oxygen. Vitreoscilla has FeSOD. Expression of FeSOD to paraquat was largely constitutive. This suggests that the basal level of FeSOD is sufficient to provide protection against superoxide generated during normal aerobic metabolism. Induction of SOD by iron supports that insertion of the active site metal into the corresponding apoprotein. The effect of paraquat on induction by iron seemed that iron brought the synergism effect in SOD activity with paraquat. It suggests that the relief of growth inhibition is due to protection against the lethality of O$_2$afforded by the elevated SOD. There may be control of FeSOD activity posttranslationally. Posttranslation control of enzyme function is particularly feasible for a metalloenzyme, for which conversion of apo- to holoenzyme may be the rate-limiting or regulatory step.

  • The following results obtained through studies related on nose. 1. Nose is called mingtang(明堂) mianwang(面王), as it emphasize the meaning of viewing diagnosis(望診) and is called yuanpinzhimen(元牝之門), tianpinzhimen(天牝之門), qiqizhimen(七氣之門) as it emphasize the meaning of the entrance of atmosphere flow. Because it's position is the middle of face, we call tianzhongzhiyue(天中之嶽), zhongyue(中嶽). 2. As the organ of body, nose comes under the category of lung-iron(肺金), functionally comes under the category of heart-fire(心火). Functionally we can smell oder, organically nose is the pathway of respiration and to be resonator. 3. Wuzangliufu(五臟六腑) is arranged closely around the nose. The essence of wuzangliufu(五臟六腑) reflect nose, so nose becomes the standard of coloring diagnosis(色診). 4. We call nose is feihou(肺候), but it is not connected directly between the lung channel and nose. In case of lung symptom(肺證), first of all, nose become to appear the condition of lung, the lung channel and nose is close functionally. 5. The view of channels, nose is a connection with channels of yangming(陽明), bladder(膀胱), du(督脈) directly. By the connecting of each three channels, spleen stomach heart and kidney(脾胃心腎) are connected with indirectly. As the result of the relation, nose helps for body to breathe and to circulate qi(氣) and smell. And so the nature of wuzangliufu(五臟六腑) appear to the nose. 6. Particularly, the yangming channel(陽明經), and the bladder channel(膀胱經) are important. Because air-qi(天氣) reach to kidney lung through nose, ground-qi(地氣) reach to spleen stomach through mouth and yingwei(營衛) revolve through the bladder channel(膀胱經). As the result nose becomes the essential organ of circulation of yinewei(營衛), ascent and descent of qi(氣).

Health Improvement and Treatment Effect of Probiotics (프로바이오틱스의 건강증진과 질병치료 효능)

  • Background : The human being and the bacteria have accomplished a balance and have coexisted each other during long time. Probiotics have evolved with the human host to exist and the result they have operated profitably to human host. So it requires that the functions of probiotics are expounded in view of Traditional Korean Medicine. Aim : Suggest the functions of probiotics in view of Traditional Korean Medicine. Method : The author's research has been performed to review the related references. Results : Probiotics assist the absorption of the lactose, proteins and minerals and product several kinds of vitamins, organic acids. Probiotics suppresses the growth of noxious bacteria and the production of harmful substances or gases. They absorbed and discharge the bile acid, and thus help us maintain the optimal level of blood cholesterol concentration. They can reinforce the immune response of the mucous membrane and control the hypersensitivity immune reaction such as asthma, atopy on the other hand. Probiotics have right functions as above and so can be applied widely in treatment of various disease and symptom. Conclusion : Considering the functions of probiotics in view of Traditional Korean Medicine, they participate in our spleen-earth-system (digestion and synthesis) and liver-wood-system (regulation of digestion, metabolism, internal secretion etc.), assist the function of lung-metal system(respiration and regulation of water metabolism) and regulate wi-chi (reinforce/control immune system). Consequently, hereafter there would be a necessity of control a circumstance in treatment of various diseases under these categories that probiotics should be able to do their right functions inside the human body.

Effect of Rope-skipping Exercise on the Enhancement of Cardiopulmonary Function (줄넘기 운동 훈련이 심폐기능 항진에 미치는 효과)

  • In an attempt to observe the effects obtained by the regular physical training, nine soldiers performed regularly the rope-skipping for nine weeks. All subjects were healthy and did not experience any special military training Programs. During the course of the training, their cardiopulmonary functions were measured in the resting and the Post-exercise recovery periods, and the values were compared with ones of the pre-trained. The test exercises loaded to the subjects were rope-skipping and step-rising & falling. The results obtained were as follows: 1) By the training, heart rates decreased very significantly in the resting and post-exercise recovery periods. And the effects began to bring out at the early stage, about the 7th day. 2) As the duration of the training increased, the systolic blood pressures decreased meaningfully in the resting and recovery periods. 3) Only in the early recovery phase after the exercise of the rope·skipping, the respiration rates decreased significantly by the training. 4) The lighter the intensity of the test exercise loaded was, the more prominent the effect of the physical training on the cardiopulmonary functions was. The above results suggest that the 9 week training of the rope-skipping would bring about the enhancement of the cardiopulmonary functions.

Long Term Follow Up of Maxilla Reconstruction Following the Ablative Cancer Surgery (악성종양 절제술 후 상악 재건의 장기 추적관찰)

  • Purpose: The objective of this study was to evaluate the outcomes of using the free flap in the reconstruction of maxillary defects. Methods: 27 consecutive cases of maxillary reconstruction with free flap were reviewed. All clinical data were analyzed, including ideal selection of flap, time of reconstruction, recurrence of cancer, postoperative complications, flap design, and follow-up results. The main operative functional items, including speech, oral diet, mastication, eye globe position and function, respiration, and aesthetic results were evaluated. Results: Among the 24 patients who underwent maxillary reconstruction with the free flap, 14 patients underwent immediate reconstruction after maxillary cancer ablation, and 10 patients underwent delayed reconstruction. There occurred 1 flap loss. Recurrences of the cancer after the reconstruction happened in 2 cases. Postoperative complications were 3 cases of gravitational ptosis of the flap, 2 cases of the nasal obstruction, and 1 case of fistula formation. Out of 27 free flaps, there were 15 latissimus dorsi myocutaneous flaps, 5 radial forearm, 4 rectus abdominis myocutaneous flaps, 1 scapular flap, 2 fibula osteocutaneous flap, respectively. Flaps were designed such as 1 lobe in 9 cases, 2 lobes in 9 cases, and 3 lobes in 5 cases. Among the 14 patients who had intraoral defect or who had palatal resection surgery, 2 patients complained the inaccuracy of the pronunciation due to the ptosis of the flap. It was corrected by the reconstruction of the maxillary buttress and hung the sling to the upper direction. All of the 14 patients were able to take unrestricted diets. In 6 patients who had reconstruction of inferior orbital wall with rib bone graft, they preserved normal vision. Aesthetically, most of the patients were satisfied with the result. Conclusion: LD free flap is suggested in uni-maxilla defect as the 1st choice, and fibular osteocutaneous flap and calvarial bone graft to cover the larger defect in bi-maxilla defect.

Bacterial Abundances and Enzymatic Activities under Artificial Vegetation Island in Lake Paldang (팔당호에 설치된 인공식물섬에서의 세균 수와 체외효소 활성도의 변화)

  • For analyzing function of a microbial ecosystem which was created under the artificial vegetation island (AVI) installed at Lake Paldang, zooplankton and bacterial numbers and exoenzyme activities (${\beta}$-glucosidase and phosphatase) were measured biweekly from 3 November 2()()1 to 20 April 2002 at AVI site and control site. Under the AVI, the water quality was worse than control site in term of comparing the environmental parameters. But, zooplankton number of AVI site was 25 times higher than that of control site. Respiratory active bacterial numbers were 3-8 times higher at AVI site. In addition, enzymatic activities were higher at AVI site than those of control site. These results suggest that the zooplankton-phytoplankton-bacteria relationships are closely coupled with each other and organic materials are eliminated by respiration of zooplankton and bacterial activities.

Association of Nose Size and Shapes with Self-rated Health and Mibyeong (코의 크기 및 형태와 자가건강, 미병과의 상관성)

  • Mibyeong (sub-health) is a concept that represents the sub-health in traditional East Asian medicine. Assuming that the nose sizes and shapes are related to respiratory function, in this study, we hypothesized that the nose size and shape features are related to the self-rated health (SRH) level and self-rated Mibyeong severity, and aimed to assess this relationship using a fully automated image analysis system. The nose size features were evaluated from the frontal and profile face images of 810 participants. The nose size features consisted of five length features, one area feature, and one volume feature. The level of SRH and the Mibyeong severity were determined using a questionnaire. The normalized nasal height was negatively associated with the self-rated health score (SRHS) (partial ρ = -0.125, p = 3.53E-04) and the Mibyeong score (MBS) (partial ρ = -.172, p = 9.38E-07), even after adjustment for sex, age, and body mass index. The normalized nasal volume (ρ = -.105, p = 0.003), the normalized nasal tip protrusion length (ρ = -.087, p = 0.014), and the normalized nares width (ρ = -.086, p = .015) showed significant correlation with the SRHS. The normalized nasal area (ρ = -.118, p = 0.001), the normalized nasal volume (ρ = -.107, p = .002) showed significant correlation with the MBS. The wider, longer, and larger the nose, the lower the SRHS and MBS, indicating that health status can be estimated based on the size and shape features of the nose.

Oxygen Consumption of Sea Squirt Halocynthia roretzi Depending on the Water Temperature and Body Size (수온과 크기에 따른 멍게(Halocynthia roretzi)의 산소 소비)

  • The oxygen consumption rate (OCR) based on the water temperature and body size of the sea squirt Halocynthia roretzi was examined to provide quantitative information about the metabolic response of the species. OCRs were measured using a closed flow-through respirometer at four different water temperatures (10, 15, 20 and 25℃) and two different body sizes (21.4±1.1 g and 150.5±1.3 g, wet weight) with triplicates of each treatment. OCR increased as water temperature increased at both body sizes, but decreased as body size increased regardless of the water temperature (P<0.001). The effect of body size evaluated as a power function ranged from 0.8055 to 0.8884. The highest Q10 values in the small and large size groups ranged from 15 to 20℃ and 20 to 25℃, respectively. The metabolic daily energy loss rate via respiration at all tested temperatures ranged from 56.2 to 106.1 J g-1 d-1 in the small-size group and from 44.5 to 92.0 J g-1 d-1 in the large-size group. Our results indicate that the metabolic response of H. roretzi highly depends on fluctuating water temperature at a given life stage.

Anatomy and Histology of the Olfactory Organ of Asian Swamp Eel Monopterus albus (드렁허리 Monopterus albus 후각기관의 해부 및 조직학적 특성 연구)

  • The anatomy and histology of the olfactory organ of Monopterus albus was investigated using stereo microscopy, light microscopy, and scanning electron microscopy. The external structure of the olfactory organ exhibited closed anterior and posterior nostrils parallel to the skin surface. The interior structure consisted of a pipe-like chamber, and lower and upper accessory nasal sacs. The olfactory chamber was composed of the sensory and non-sensory epithelium, and an unidentified organ. The sensory epithelium of the pseudostratified epithelial layer was composed of olfactory receptor neurons, supporting cells, basal cells, and lymphatic cells; and the non-sensory epithelium of the stratified squamous layer contained stratified epithelial cells and mucous cells with acidic mucopolysaccharides. The presence of intraepithelial blood capillaries and abundant dermal vascularization in the sensory epithelium of the olfactory chamber may provide strong histological evidence that respiration occurs through the olfactory epithelium.
