The Effect of Repetitive Insertion and Pullout of Spinal Screws on Pullout Resistance : A Biomechanical Study (척추 수술에 사용되는 나사못의 반복 삽입과 인출이 인장항력에 미치는 영향 : 생체 역학적 연구)

  • Object : The clinical uses of screws are increasing with broader applications in spinal disorders. When screws are inserted repeatedly to achieve optimal position, tips of screw pitch may become damaged during insertion even though there are significant differences in the moduli of elasticity between bone and titanium. The effect of repeated screw insertion on pullout resistance was investigated. Methods : Three different titanium screws(cortical lateral mass screw, cancellous lateral mass screw and cervical vertebral body screw) were inserted into the synthetic cancellous material and then extracted axially at a rate of 2.4mm/min using Instron(Model TT-D, Canton, MA). Each set of screws was inserted and pulled out three times. There were six screws in each group. The insertional torque was measured with a torque wrench during insertion. Pullout strength was recorded with a digital oscilloscope. Results : The mean pullout force measurements for the cortical lateral mass screws($185.66N{\pm}42.60$, $167.10N{\pm}27.01$ and $162.52 N{\pm}23.83$ for first, second and third pullout respectively : p=0.03) and the cervical vertebral body screws($386.0N{\pm}24.1$, $360.2N{\pm}17.5$ and $330.9N{\pm}16.7$ : p=0.0024) showed consecutive decrease in pullout resistance after each pullout, whereas the cancellous lateral mass screws did not($194.00N{\pm}36.47$, $219.24N{\pm}26.58$ and 199.49N(36.63 : p=0.24). The SEM after insertion and pullout three times showed a blunting in the tip of the screw pitch and a smearing of the screw surface. Conclusions : Repetitive screw insertion and pullout resulted in the decrease of pullout resistance in certain screws possibly caused by blunting the screw tip. This means screw tips suffer deformations during either repeated insertion or pullout. Thus, the screws that have been inserted should not be used for the final construct.

  • Objectives : This study is to examine the influence of exorcise program to body shape and body composition of women. Exercise program is composed muscular resistance exercise and aerobic exorcise with 70min/time, 5day/week for 4weeks and designed for the reinforcement of muscular strength and the decease the decrease of obesity. Methods : The subjects of this study are 26 females who are from twenty until thirty-nine years old and are divided into two groups according to Obese Rate. normal weight group is 18 females under a BMI of 23 and overweight group is 8 females over a BMI of 23. we measured body compositon and body shape with body composition analyzer(inbody 3.0) measured body compositions are body weight, Lean body mass, base metabolic rate, body fat, %body fat, waist hip ratio, body mass index. measured 6body shade are upper arm circumference, chest circumference, waist circumference, hip circumference, thigh circumference. the measurements had been taken before exercise of each group, after 4 weeks, after 8 weeks, after 12 weeks. Body compositon and body shape of the measured data were analyzed by two-way repeated ANOVA followed by Dunnett's Post hoc test using SPSS. Differences were considered significant at p<0.05. Results and Conclusions : The weight was reduced at a significant level in overweight group after 4 weeks. Body fat percent was reduced at a significant level in normal group alter 4 weeks and overweight group after 8 weeks. Waist-hip ratio was reduced at a significant level in normal and overweight group after 4 weeks. Lean body mass and body metabolic rate were reduced but did not satisfy statistical significance. Circumference of all body parts showed an decrease after 12 weeks exercise program. Upper Arm, Chest, Waist, Hip Circumference were reduced at a significant level in overweight group after 4 weeks, but In normal group only Waist Circumference was reduced at a significant level after 4 weeks. Aerobic and Circuit weight training for 12 weeks reduced body weight and body fat percent, waist circumstance statistically significant without loss of lean body mass and body metabolic rate. Also these change was more effective in overweight group than in normal group.

  • The aeration of an overtopping wave on a vertical structure generated by a plunging wave was investigated through laboratory measurements of void fraction. The overtopping wave occurring after wave breaking becomes multi-phased and turbulent with significant aeration, so that the void fraction of the flow is of importance. In this study, fiber optic reflectometer and bubble image velocimetry were employed to measure the void fraction, velocity, and layer thickness of the overtopping flow. Mean properties were obtained by ensembleand time-averaging the repeated instantaneous void fractions and velocities. The mean void fractions show that the overtopping wave is very high-aerated near the overtopping wave front and relatively low-aerated near the deck surface and rear free surface of the wave. The flow rate and momentum of the overtopping flow estimated using the measured data show that the void ratio is an important parameter to consider in the multiphase flow. From the similarity profiles of the depth-averaged void fraction, velocity, and layer thickness, one-dimensional empirical equations were obtained and used to estimate the flow rate and momentum of the overtopping flow.

  • Geological sequestration of carbon dioxide ($CO_2$) is one of the most effective strategies far long-term removal of greenhouse gas from atmosphere. This paper reviews three projects for the $CO_2$ sequestration in geological formation. A unique $CO_2$ injection into a marine aquifer has been successfully monitored with repeated surface seismic measurements in the North Sea Sleipner West field. The seismic images reveal the extent and internal shape of the $CO_2$ bubble. Massive miscible $CO_2$ has been injected into a complex fractured carbonate reservoir at the Weyburn oil filed. High-resolution time-lapse P-wave data were successfully obtained to map the features of $CO_2$ movements within the two thin zones of different lithology. From the time-lapse crosswell EM imaging at the Lost Hills oil field in central California, U.S.A., the replacement of brine with $CO_2$ has been confirmed through a decrease of conductivity. The conductivity image was successfully compared with induction logs observed in the two wells.

The Effect of Exercise Intensity on Muscle Activity and Kinematic Variables of the Lower Extremity during Squat

  • Objective: The purpose of this study was to determine how exercise intensity affects muscle activity and kinematic variables during squat. Method: Fifteen trainers with >5 years of experience were recruited. For the electromyography (EMG) measurements, four surface electrodes were attached to both sides of the lower extremity to monitor the rectus femoris (RF) and biceps femoris. Three digital camcorders were used to obtain three-dimensional kinematics of the body. Each subject performed a squat in different conditions (40% one-repetition maximum [40%1RM], 60%1RM, and 80%1RM). For each trial being analyzed, three critical instants and two phases were identified from the video recording. For each dependent variable, one-way analysis of variance with repeated measures was used to determine whether there were significant differences among the three different conditions (p<.05). When a significant difference was found, post hoc analyses were performed using the contrast procedure. Results: The results showed that the average integrated EMG values of the RF were significantly greater in 80%1RM than in 40%1RM during the extension phase. The temporal parameter was significantly longer in 80%1RM than in 40%1RM and 60%1RM during the extension phase. The joint angle of the knee was significantly greater in 80%1RM than in 40%1RM at flexion. The range of motion of the knee was significantly less in 80%1RM than in 40%1RM and 60%1RM during the flexion phase and the extension phase. The angular velocity was significantly less in 80%1RM than in 40%1RM and 60%1RM during the extension phase. Conclusion: Generally, the increase of muscle strength decreases the pace of motion based on the relation between the strength and speed of muscle. In this study, we also found that the increase of exercise intensity may contribute to the increase of the muscle activity of the RF and the running time in the extension phase during squat motion. We observed that increased exercise intensity may hinder the regulation of the range of motion and joint angle. It is suitable to perform consistent movements while controlling the proper range of motion to maximize the benefit of resistance training.

After retrospective evaluation of the SETUP rate change during the treatment of head and neck cancer patient with Helical Tomotherapy (두경부환자의 토모테라피 치료시 SETUP 변화율에 대한 후향적 평가)

  • Purpose : Retrospective evaluation of setup changes using the corrected position during helical tomotherapy Materials and Methods : Head and neck cancer patients were randomly sampled and summarized into 3 groups: Group 1(32) Brain, Group 2 2(28)Maxillar, Nasal cavity, Group 3 (35) Nasopharynx(NPX), Tongue, Tonsil, and Oropharynx(OPX). In 3 groups, the statistical tests based on repeated measurements among 30 times of the duration of treatment by applying X, Y, Z axis errors, roll, weight changes, and vectors as variables. Results : The statistical test results showed that there was no difference between x-axis (p = 0.458) and y-axis (p=0.986) and in roll (p = 0.037), weight change (p <0.001), and the vector (p <0.001). In addition, the pattern between the three groups based on the fraction revealed no difference in x-axis (p = 0.430) and roll (p = 0.299) but a difference in y-axis (.023), weight change (p = 0.001), and vector (p = 0.028). Conclusion : The results of the retrospective evaluation found the change in the group 3 with respect Y, Z, weight, and vector and a larger random error during the treatment including low neck.

Activation of Paraspinal, Abdominal, and Hip Muscles During Various Low Back Stabilization Exercises in Males and Females

  • Many muscles of the trunk and hip are capable of contributing to the stabilization and protection of the lumbar spine. To have optimal effectiveness, a training program should include dynamic back/stomach/hip exercises. This study was designed to assess the L5 level paraspinal, external abdominal oblique, and gluteus maximus muscle activities during various low back stabilization exercises. Participants were 26 healthy adults (13 males, 13 Females), aged 21 to 28 years. The surface electromyography (EMG) was recorded from the L5 level paraspinal, external abdominal oblique, and gluteus maximus muscles. The recorded signal was averaged and normalized to the maximal electromyographic amplitude obtained during the maximal voluntary contraction. The measurements were taken during 3 low back stabilization exercises. One-way analysis of variance with repeated measures was used to examine the difference, and a post hoc test was performed with least significant difference. A level of significance was set at p<.05. The significance of difference between men and women, and between the electromyographic recording sites was evaluated by an independent t-test. The EMG activity for the externus oblique and gluteus maximus muscles had significant differences among 3 exercises (p<.05). In males, the EMG activity for the external abdominal oblique muscle had significantly increased differences during exercises 1 and exercise 2 (p<.05). The gluteus maximus muscle had significantly increased differences during exercise 2 and exercise 3 (p<.05). In females, the multifidus muscle had significantly increased difference during exercise 3 (p<.05), the external abdominal oblique muscle had significantly increased difference during exercise 1 (p<.05). and the gluteus maximus muscle had significantly decreased difference during exercise 3 (p<.05). The results were that the external abdominal oblique muscle was apparently activated during the curl-up exercise in females and males, and the multifidus muscle was apparently activated during the bridging exercise in females and during the sling exercise in males and females.1)In comparison of the %MVC between males and females, exercise 2 and exercise 3 apparently activated of the multifidus and gluteus maximus muscles in both males and females (p<.05). The EMG activity of the gluteus maximus muscle of the males significantly increased during exercise 2 and exercise 3 (p<.05). The EMG activity the multifidus muscle of the females was significantly increased during exercise 2 and exercise 3 (p<.05). More research is needed to understand the nature of motor control problems in the deep muscles in patients with low back pain.

A Comparison of Change in Thickness for Lower Trapezius Muscle During Lower Trapezius Muscle Isometric Exercise and Reliability of Ultrasound Imaging (하승모근 등척성 운동방법에 따른 근두께 변화량 비교 및 초음파 영상의 신뢰도 연구)

  • The lower trapezius muscle is an important stabilizer and primary mover of the scapula. The potential use of ultrasound imaging to evaluate scapular muscle function warrants investigation. The purpose of this study is to use ultrasound imaging for determining the effectiveness of 4 different isometric exercises for maximally activating the lower trapezius muscles in healthy subjects. Twenty-eight (14 men and 14 women) volunteers were recruited for this study. Thickness measurements of the lower trapezius muscles were recorded during 4 exercises: latissimus pulldown (LP), prone V-raise (PV), prone row (PR), and modified prone cobra (MP). Lower trapezius muscle thickness was measured 3 times by 2 investigators at a point 3 cm lateral to the lateral edge of the T8 spinous process. The order of 4 exercise execution was randomized for each participant. To identify statistical significance, one-way ANOVA with repeated measures was used with the significance level of .05. Intraclass correlation coefficient (ICC) for intra-reliability was .86~.98 and inter-rater reliability .83~.96 for the lower trapezius, respectively (p<.01). Thickness changes in the lower trapezius muscles between the relaxed and contracted states in men were as follows: LP ($7.37{\pm}2.68mm$, 182%), MP ($4.69{\pm}1.74mm$, 167%), PV ($4.52{\pm}1.47mm$, 149%), and PR ($3.84{\pm}1.72mm$, 133%). In women the values were as follows: LP ($4.64{\pm}1.24mm$, 163%), MP ($2.79{\pm}.81mm$, 131%), PV ($2.78{\pm}.85mm$, 129%), and PR ($2.21{\pm}1.26$ mm, 100%). Thickness of the lower trapezius muscles significantly differed between exercises in both the gender (p<.01). The LP was the most effective exercise for increasing the activation of the lower trapezius muscle in both the gender. We recommend performing the LP exercise for strengthening the lower trapezius muscles.

Diet intervention to improve cardiovascular risk factors among Iranian postmenopausal women

  • Cardiovascular disease is a leading cause of death and disability and remains so in the future. The aim of this study was to detect the impact of a 6-month diet intervention on cardiovascular risk factors in postmenopausal Iranian women. It was a randomized controlled trial that carried out in the East Health Clinic in Ahvaz-Iran. This study started on June 2007 and was completed on May 2008. A total 64 healthy postmenopausal women recruited and randomly assigned to the intervention group (35) and control (29). Over the six months follow-up, the intervention group received five educational sessions (two face to face and three lecture discussion classes with slide demonstration) at the first month. These sessions were about menopause, cardiovascular disease and healthy diet. Every participant in the intervention group received one face to face education session at the $3^{rd}$ month, and also received a telephone call at the end of each month starting with the second month to remain on the diet. Pre-intervention and post-intervention anthropometric measurements, blood pressure, blood lipids and dietary intake were assessed. Data analyzed using the SPSS version 15. Descriptive (means and SD), univariate (paired-t test, Chi-Square and Independent T-test) and multivariate (GLM Repeated Measure) statistics were used. Participants in the diet group had significantly lower weight (-0.9 kg), body mass index (-0.4 $kg/m^2$), and fasting blood sugar (-4.5 mg/dl). The diet group significantly increased their daily intake of fiber (+2.3 g, P=0.05), decreased their intake of sodium (-28 mg, P=0.04), and consumption of fruit and vegetable ${\geq}$ 5 serving a day (80%, P=0.03) compared to the control group. Post menopausal women are at a greater risk for cardiovascular disease. Healthy diet using educational intervention can be an effective means of reducing cardiovascular risk in postmenopausal women.

A Study on Development of Patent Information Retrieval Using Textmining (텍스트 마이닝을 이용한 특허정보검색 개발에 관한 연구)

  • The patent information retrieval system can serve a variety of purposes. In general, the patent information is retrieved using limited key words. To identify earlier technology and priority rights repeated effort is needed. This study proposes a method of content-based retrieval using text mining. Using the proposed algorithm, each of the documents is invested with characteristic value. The characteristic values are used to compare similarities between query documents and database documents. Text analysis is composed of 3 steps: stop-word, keyword analysis and weighted value calculation. In the test results, the general retrieval and the proposed algorithm were compared by using accuracy measurements. As the study arranges the result documents as similarities of the query documents, the surfer can improve the efficiency by reviewing the similar documents first. Also because of being able to input the full-text of patent documents, the users unacquainted with surfing can use it easily and quickly. It can reduce the amount of displayed missing data through the use of content based retrieval instead of keyword based retrieval for extending the scope of the search.
