Comparison of Clinical and Structural Outcomes of Open and Arthroscopic Repair for Massive Rotator Cuff Tear

  • Background: Management of massive rotator cuff tears can be challenging because of the less satisfactory results and a higher retear rate regardless of the use of open or arthroscopic repair technique. Methods: We retrospectively analyzed 102 cases of massive rotator cuff tear treated with either open or arthroscopic repair. Open repair was performed in 38 patients; and arthroscopic repair, in 64 patients. The mean age at the time of surgery was 59.7 years in the open group and 57.6 years in the arthroscopic group. Results: The Constant score increased from the preoperative mean of 55.9 to 73.2 at the last follow-up in the open repair group and from 53.8 to 67.6 in the arthroscopic repair group (p<0.001 and <0.001, respectively). The University of California at Los Angeles (UCLA) score increased from a preoperative mean of 17.7 to 30.8 at the last follow-up in the open group and from 17.5 to 28.7 in the arthroscopic group (p<0.001 and <0.001, respectively). No statistically significant difference in the Constant and UCLA scores was observed between the two groups at the last follow-up (p=0.128 and 0.087, respectively). Retear was found in 14 patients (36.8%) in the open group and 39 patients (60.9%) in the arthroscopic group (p=0.024). Conclusions: Open and arthroscopic repairs of massive rotator cuff tears may provide satisfactory clinical results with no significant difference. However, a significantly lower retear rate was observed for the open repair group compared with the arthroscopic repair group.

Does the Use of Injectable Atelocollagen during Arthroscopic Rotator Cuff Repair Improve Clinical and Structural Outcomes?

  • Background: Since the establishment of biological augmentation to improve the treatment of rotator cuff tears, it is imperative to explore newer techniques to reduce the retear rate and improve long-term shoulder function after rotator cuff repair. This study was undertaken to determine the consequences of a gel-type atelocollagen injection during arthroscopic rotator cuff repair on clinical outcomes, and evaluate its effect on structural integrity. Methods: Between January 2014 and June 2015, 121 patients with full thickness rotator cuff tears underwent arthroscopic rotator cuff repair. Of these, 61 patients were subjected to arthroscopic rotator cuff repair in combination with an atelocollagen injection (group I), and 60 patients underwent arthroscopic rotator cuff repair alone (group II). The visual analogue scale (VAS) for pain and the Korean Shoulder Society (KSS) scores were evaluated preoperatively and postoperatively. Magnetic resonance imaging (MRI) was performed at 6 months postoperatively, to assess the integrity of the repair. Results: VAS scores were significantly lower in group I than in group II at 3, 7, and 14 days after surgery. KSS scores showed no significant difference between groups in the 24 months period of follow-up. No significant difference was obtained in the healing rate of the rotator cuff tear at 6 months postoperatively (p=0.529). Conclusions: Although a gel-type atelocollagen injection results in reduced pain in patients at 2 weeks after surgery, our study does not substantiate the administration of atelocollagen during rotator cuff repair to improve the clinical outcomes and healing of the rotator cuff.

Periodic Replacement Policies with Minimal Repair Cost Limit

  • Periodic replacement policies are proposed for a system whose repair cost, when it fails, can be estimated by inspection. The system is replaced when it reaches age T (Policy A), or when it fails for the first time after age T (Policy B). If it fails before reaching age T, the repair cost is estimated and minimal repair is then undertaken if the estimated cost is less than a predetermined limit L; otherwise, the system is replaced. The expected cost rate functions are obtained, their behaviors are examined, and ways of obtaining optimal T and L are explored.

Numerical study of bonded composite patch repair in damaged laminate composites

  • The present study deals with the repair of composite structures by bonding composite patches. The composite structure is a carbon/epoxy laminate with stacking sequence [45/-45/0/90]S. The damaged zone is simulated by a central crack and repaired by bonding symmetrical composite patches. The repair is carried out using composite patches laminated from the same elemental folds as those of the cracked specimen. Three-dimensional finite element method is used to determine the energy release rate along the front of repaired crack. The effects of the repair technique used single or double patch, the stacking sequence of the cracked composite patch and the adhesive properties were highlighted on the variations of the fracture energy in mode I and mixed mode I + II loading.

  • In order to predict the future needs of the aircraft repair parts, each military group develops and applies various techniques to their characteristics. However, the aircraft and the equipped weapon systems are becoming increasingly advanced, and there is a problem in improving the hit rate by applying the existing demand prediction technique due to the change of the aircraft condition according to the long term operation of the aircraft. In this study, we propose a new prediction model based on the conventional time-series analysis technique to improve the prediction accuracy of aircraft repair parts by using machine learning model. And we show the most effective predictive method by demonstrating the change of hit rate based on actual data.

Minimally invasive distal biceps tendon repair: a case series

  • Background: Distal biceps tendon repairs are commonly performed using open techniques. A minimally invasive distal biceps tendon repair technique using a speculum and hooded endoscope was developed to improve visualization, reduce soft-tissue dissection, and minimize complications. This paper describes the technique and reports the outcomes of 75 minimally invasive distal biceps tendon repairs. Methods: The operation reports and outcomes of 75 patients who underwent distal biceps tendon repair using this technique between 2011 and 2021 were retrospectively reviewed. Results: Median time to follow-up was 12 months (interquartile range [IQR], 6-56 months). Primary outcomes were function as measured by the Disabilities of Arm, Shoulder and Hand Score (DASH) questionnaire, and rate of complications. Median DASH score was 1.7 of 100 (IQR, 0-6.8). There were 2 of 75 (2.7%) re-ruptures of the distal tendon. There were no cases of vascular injury, proximal radius fracture, or posterior interosseous nerve, median, or ulnar nerve palsy. Conclusions: In this series, minimally invasive distal biceps repair was safe and effective with a low rate of major complications. Recovery of function, as indicated by low DASH scores, was satisfactory, and inconvenience during recovery was minimized. Level of evidence: IV.

콘크리트 단면복구용 보수재료의 굵은 골재 최대치수 및 조립률에 따른 재료적 특성 (Material Properties Depending on the Maximum Aggregate Size and Fineness Modulus for Concrete Repair Materials)

  • 본 연구에서는 보수재료에 굵은 골재를 사용하게 되면서 얻을 수 있는 효과를 검토하기 위해 굵은 골재의 최대치수 및 조립률을 다양화 하면서 콘크리트의 특성을 파악하는 기초 연구를 수행하였다. 그 결과 콘크리트의 슬럼프는 굵은 골재 최대치수가 커질수록 증가하였고, 공기량은 최대 골재 치수가 10 mm 이상인 배합군에 비해 최대 골재 치수가 5 mm 의 골재를 사용한 배합에서 공기량이 1.88~2.35 배 높게 측정되었다. 압축강도는 굵은 골재의 실적률에 많은 영향을 받는 것으로 나타났다. 실적률이 가장 높은 최대치수 20 mm 의 굵은 골재를 사용한 배합에서 압축강도가 가장 높게 측정되었으며 실적률이 가장 낮은 배합의 압축강도보다 45% 이상 높게 나타났다. 건조수축 측정결과 굵은 골재 최대치수 및 조립률의 변화에 따라 굵은 골재의 실적률이 높아지면서 건조 수축이 가장 낮게 측정되었고, 실적률이 가장 낮은 배합에서 가장 크게 나타났다. 본 연구를 통해 콘크리트 구조물 보수용 재료에 사용되는 굵은 골재의 최대치수 및 조립률을 다양화해서 입도를 조정하게 되면 강도 및 작업성이 우수해지고 건조수축을 줄일 수 있는 것을 확인 할 수 있었다.

  • This paper is concerned with cost analysis model in periodic maintenance policy. Generally periodic maintenance policy in which item is repaired periodic interval times. And in the article minimal repair is considered. Mimimal repair means that if a unit fails, unit is instantaneously restored to same hazard rate curve as before failure. In the paper periodic maintenance policy with minimal repair is as follows; Operating unit is periodically replaced in periodic maintenance time, if a failure occurs between minimal repair and periodic maintenance time, unit is replaced by a new item until tile periodic maintenance time comes. Also unit undergoes minimal repair at failures in minimal-repair-for-failure interval. Then total expected cost per unit time is calculated according to scale parameter of failure distribution. Maintenance cost factors are included operating, fixed, minimal repair, periodic maintenance and new item replacement cost. Numerical example is shown in which failure time of system has weibull distribution.

  • In this paper, we consider a replacement model following the expiration of warranty. In other words, this paper proposes the optimal replacement policy for a repairable system following the expiration of payable renewing replacement-non-renewing minimal repair warranty. The expected cost rate per unit time from the user's perspective is used to determine the optimality of the replacement policy. Thus, we derive the expressions for the expected cycle length and the expected total cost to obtain the expected cost rate per unit time. Finally, the numerical examples are presented for illustrative purpose.
