Analysis of Changes in Patient Costs in 7Diagnosis-Related Groups through Time Series Analysis - Focusing on the Characteristics of Medical Institutions - (시계열 분석을 통한 7개질병군 포괄수가제의 환자 비용 변화 분석 -의료기관의 유형별 특성을 중심으로-)

  • Objectives : This study analyzed the trends of patient costs in 7diagnosis-related groups(DRG) since July 2013 when the government made it mandatory for all hospitals and clinics. Methods : Data were collected from the 7DRG score chart published by the Ministry of Health and Welfare(MoHW) from July 2013 to January 2017. The average value of the weekday relative value scale was multiplied by unit price, referred to as'- "patient costs by disease group"-' and they were analyzed by time series. Results : Patient costs had increased among all patients with a comprehensive disease. Small and medium-sized hospitals (hospitals and clinics) showed a slight increase in patient costs. Conclusions : Enforcement of the Korean diagnosis-related groups has led to management crisis in small and medium-sized hospitals and deterioration medical service quality. To solve this problem, The weekday relative value scale of small and medium-sized hospitals should be increased significantly.

  • The purpose of this study is to set a standard for sale prices of supplying apartment houses about decision factors in sale price of supplying apartment houses using Analytic Hierarchy Process. This is done by modeling decision factors in sale price of supplying apartment houses as hierarchy. According to the modeled hierarchy, the relative importance of supplying factors are determined using a survey of a group of real estate experts. In addition, through Analytic Hierarchy Process, the relative importance of phased sale prices of supplying apartment is analyzed in order to set a standard to estimate competitive sale prices of newly supplying apartment houses.

Consumers' acceptance and resistance to virtual bank: views of non-users (인터넷전문은행 수용 의도와 저항에 관한 연구: 소비자, 혁신, 환경 특성을 중심으로)

  • Convergence between technology and financial services is ubiquitous and widespread. Virtual banks represent an important aspect of financial markets that can generate value added for consumers and enhance the quality of financial services. This study explores the effect of innovation characteristics (relative advantage, compatibility, and perceived risk), consumer characteristics (status quo bias), and social mechanisms (network externality: complementarity, numbers of peers) on consumers' adoption intention and resistance to virtual banks. This study adopted an innovation resistance model with two dependent variables: adoption intention and resistance to virtual banks. An online self-administered survey was conducted and 532 or non-users of virtual banks aged 20 to 69 years old were analyzed. Frequency analysis, descriptive analysis, and hierarchical multiple regression indicated that status quo bias, relative advantage, perceived risk, complementarity, and number of peers insignificantly influence the adoption intention regarding virtual banks. Furthermore, status quo bias, relative advantage, perceived risk, and number of peers insignificantly influence the resistance to virtual banks. Female respondents have a lower adoption intention and higher resistance to virtual banks than male respondents. The findings suggest that the innovation resistance model can be useful in understanding consumers'adoption and resistance behavior as well as reveal that innovation characteristics, consumer characteristics, and social mechanism are important antecedent variables of the innovation adoption decision.

The Impairment of Thyroid Hormones Homeostasis after Short-Term Exposure to Di(2-ethylhexyl)phthalate in Adolescent Male Rats

  • Di(2-ethylhexyl)phthalate (DEHP) could induce metabolic disorders through interfering with thyroid homeostasis. Therefore, we evaluated the effects of short term to environmental relevant doses of DEHP on thyroid hormones. Four week old Sprague-Dawley (SD) rats were treated with vehicle (corn oil), and DEHP 0.75, 7.5, and 150 mg/kg/day. The rats were treated with once daily by oral gavage and were sacrificed with after 1 week. They were measured body weight and relative thyroid weight, serum thyroid hormones and histological changes of thyroid. There was no difference in body weight between the control and DEHP exposed rats. Relative thyroid weight in DEHP 150 mg/kg/day treated group was significantly lower than control. Serum thyroxine levels was decreased in rats exposed to 0.75 and 150 mg/kg/day DEHP. No histological changes were observed in the thyroid of rats administered DEHP compared to control. Exposure to DEHP at environmental relevant levels, even short-term exposure, can cause hypothyroidism in adolescent rats even the exposure period is relative short.

Early Detection of Intravenous Infiltration Using Multi-frequency Bioelectrical Impedance Measurement System: Pilot Study

  • The early detection of intravenous (IV) infiltration is necessary to minimize the injury caused by the infiltration, which is one of the most important tasks for nurses. For detecting early infiltration in patients receiving invasive vein treatment, bioelectrical impedance was measured using multi-frequency bioelectrical impedance. The impedance decreased significantly at infiltration, and then decreased gradually over time after infiltration. The relative impedance at 20 kHz decreased remarkably at infiltration, and then gradually decreased thereafter. In addition, the impedance ratio increased temporarily at infiltration and then gradually decreased over time. Furthermore, the impedance at each frequency decreased quantitatively over time. This indicates that IV solution leaking from the vein due to infiltration accumulates in the subcutaneous tissues. Moreover, slopes of log Z vs. log f differently decreased with increasing log f, indicating that the impedance exhibits different responses depending on the frequency.

  • Korean e-Government web site has been evaluating as a distinguished government, ranked 5th of the 119 countries in the "Global e-government readiness index report 2005" by UN in 2006. However we have to focus poor rate of utilization in proportion to its high evaluation of e-Government ranking. These reason probably did not reflect the requirements of user to build and operate e-Government web site. In this paper we will provide the important factors to consider to build and operate e-Government web site. For this, we analyze the relative importance for evaluation factors of e-Government web site using AHP(Analytic Hierarchy Process).

3-Dimensional Micro Solder Ball Inspection Using LED Reflection Image

  • This paper presents an optical technique for the three-dimensional (3D) shape inspection of micro solder balls used in ball-grid array (BGA) packaging. The proposed technique uses an optical source composed of spatially arranged light-emitting diodes (LEDs) and the results are derived based on the specular reflection characteristics of the micro solder balls for BGA A vision system comprising a camera and LEDs is designed to capture the reflected images of multiple solder balls arranged arbitrarily on a tray and the locations of the LED point-light-source reflections in each ball are determined via image processing, for shape inspection. The proposed methodology aims to determine the presence of defects in 3D BGA shape using the statistical information of the relative positions of multiple BGA balls, which are included in the image. The presence of the BGA balls with large deviations in relative position imply the inconsistencies in their shape. Experiments were conducted to verify that the proposed method could be applied to inspection without sophisticated mechanism and productivity problem.

Numerical Modeling and Experiment for Single Grid-Based Phase-Contrast X-Ray Imaging

  • In this work, we investigated the recently proposed phase-contrast x-ray imaging (PCXI) technique, the so-called single grid-based PCXI, which has great simplicity and minimal requirements on the setup alignment. It allows for imaging of smaller features and variations in the examined sample than conventional attenuation-based x-ray imaging with lower x-ray dose. We performed a systematic simulation using a simulation platform developed by us to investigate the image characteristics. We also performed a preliminary PCXI experiment using an established a table-top setup to demonstrate the performance of the simulation platform. The system consists of an x-ray tube ($50kV_p$, 5 mAs), a focused-linear grid (200-lines/inch), and a flat-panel detector ($48-{\mu}m$ pixel size). According to our results, the simulated contrast of phase images was much enhanced, compared to that of the absorption images. The scattering length scale estimated for a given simulation condition was about 117 nm. It was very similar, at least qualitatively, to the experimental contrast, which demonstrates the performance of the simulation platform. We also found that the level of the phase gradient of oriented structures strongly depended on the orientation of the structure relative to that of linear grids.

  • The interest about Artificial Intelligence through the AlphaGo Match in Korea has been increasing rapidly. So far, very little has been done in Artificial Intelligence. The aim of this paper is to reveal technology convergence and to assess the country profile in the field of artificial intelligence(AI). Technology convergence map was created after extracting USPTO patent grants and Web of Science data and generating matrics in the field of AI. Several Indicators were obtained by extracting and calculating SCOPUS Data that KISTI has. According to USPTO patent grants, it shows that AI technology has a strong relationship with several sectors such as cost/price determination, image analysis, and surgery, etc. Also, AI has a active convergence with some fields of Electrical and Electronic Engineering, BioTechnologies, and Medicine etc. According to country profile analysis, Korea reaches a global average growth index. However, in terms of specialization index (SI) and average of relative citations (ARC), there is a large gap between Korea and research leading countries.

A Study on the Cost-Effective Security System for SME Hospital Acceptability in Convergence Medical Environment (의료융합 환경에서 수용성을 고려한 비용 효율적 보안체계구축 방안 연구: 중소의료기관을 중심으로)

  • As industries converge in recent years, the impact of the use of convergence tools among industries on the security of the organization is increasing. However, organizational members lack the ability to adapt to introduction of new system because they are operating business around existing systems, and thus, security considerations are an environment that will inevitably emerge as a follow-up priority. In this study, we studied cost-effective options for factors that should be considered first in order to establish a security system for small and medium-sized healthcare institutions in the healthcare convergence industry. Specifically, the current legal system was considered and the security status was analyzed through prior research, and the necessary security solution/system was derived from small and medium-sized healthcare institutions. In addition, it analyzed relative priorities for cost-efficient deployment of security systems to those involved in small and medium-sized healthcare institutions in actual business environments and presented measures to establish a overall security.
