The Effect of Action Observation Training on Upper Motor Function in Stroke Patients : A Multiple Bbaseline Design (동작관찰훈련이 뇌졸중 환자의 상지운동기능에 미치는 영향 : 다중기초선연구)

  • Purpose: The discovery of mirror neuron system may positively affect functional recovery; therefore, rehabilitation is needed that is practical for use in clinical settings. The purpose of this study was to examine the effect of action observation training on upper motor function in people who had suffered strokes. Methods: Three elderly patients with stroke, aged to years, were recruited from a stroke rehabilitation center. A nonconcurrent, multiple baseline subject approach was taken, with an A-B-A treatment single-subject experimental design, and the experiment was conducted for 3 weeks. The action observation training was repeated 5 times in 5 days during the intervention period. The arm function, including WMFT, BBT, and grip and pinch strength, was evaluated in each subject 5 times during the baseline period, the intervention period, and the baseline regression period. Results: The results of the evaluation for each subject were presented as mean values and video graphs. The WMFT scores of 2 subjects were improved during the intervention period in comparison with the baseline period, and this improvement was maintained even during the regression baseline period. The BBT and the grip and pinch strength were not improved. Conclusion: Based on these results, we suggest that the action observation training for 5 sessions was effective in improving upper limb function of stroke patients but was not effective in improving hand dexterity or grip and pinch strength.

Genotoxicity Study of ChondroT (ChondroT의 유전독성 연구)

  • Objectives This study was performed to observe the genotoxic effect of the ChondroT. Methods To evaluate the genotoxicity of ChondroT, an experiment of bacterial reverse mutation test, in vitro mammalian chromosomal aberration test and mammalian erythrocyte micronucleus test in mouse was conducted. Results TA98, TA100 and TA1537 strains in the absence of metabolic activation system (S9 mix), the number of revertant colonies being greater than 2-fold of the respective negative control value. Both in -S9 mix and +S9 mix, the frequencies of aberration cells with structural aberration and numerical aberrations of chromosome were less than 5%. There was no increase of polychromatic erythrocyte with one or more micronuclei at any dose of test substance compared to the negative control group (p<0.05). Conclusions In TA98, TA100 and TA1537 strains in the absence of metabolic activation system (S9 mix), the number of revertant colonies was greater than 2-fold of the respective negative control value, showing positive results. ChondroT was considered to be non-clastogenic to Chinese hamster lung (CHL/IU) cells under the present experimental condition. and ChondroT was determined not to induce an increased frequency of micronuclei in the bone marrow cells of male ICR mice under the present experimental condition.

A Study on the Determinants of Convalescent Rehabilitation Medical Service Needs at Regional Level (지역별 회복기 재활 의료서비스 필요도 결정요인 분석 연구)

  • Background: Based on the increase in the needs for convalescent rehabilitation medical services in Korea, this study aims to calculate the needs for rehabilitation services and examine its determinants for 229 regions. Methods: Claim data from the Health Insurance Review and Assessment Service were used to estimate patients who need to receive rehabilitation services, and data from various sources were also used for analysis. The number of cases and incidence rates of hospitalization related to convalescent rehabilitation were calculated to estimate the needs for services by region, and the results were visualized via a map. Multivariate regression and fixed effects regression using panel data were performed to identify the determinants of regional variation of the incidence rate. Results: First, the incidence rate of rural areas such as Jeolla-do, Gyeongsang-do, and Chungcheong-do was higher than urban areas (metropolitan cities). Second, the population, proportion of the elder, medical aid recipients, financial independence, traffic deaths, smoking, diabetes rate, and medical infrastructure correlated significantly with the incidence rate. Third, 'rho' values which mean the fraction of variance due to individual terms in panel data regression models were 0.965 and 0.976, respectively. Conclusion: The incidence rate of hospitalizations was correlated with most independent variables in this study and there is a gap between urban and rural areas. These regional disparities are fixed in our society. An improved regional convalescent rehabilitation system is suggested to cover the entire area including rural areas with a high rate of aging.

Therapeutic Effects of Acupuncture for Shoulder Impingement Syndrome: A Systematic Review and Meta-Analysis (어깨충돌증후군에 대한 침치료의 효과: 체계적 문헌고찰 및 메타분석)

  • Objectives This review was conducted to evaluate the therapeutic effects of manual acupuncture (MA) for shoulder impingement syndrome (SIS). Methods We searched 12 electronic databases (DBpia, Korean studies Information Service System [KISS], Oriental Medicine Advanced Searching Integrated System [OASIS], Research Information Sharing Service [RISS], China National Knowledge Infrastructure [CNKI], CINAHL, Clinical Key, Cochrane Library, Embase, JAMA, PubMed, Web of Science) to find randomized-controlled clinical trials (RCTs) investigating therapeutic effects of MA for treating SIS. Shoulder Pain and Disability Index scores and numeric pain rating scale or visual analogue scale were analyzed as the main evaluation criteria. Results Among 181 studies, 169 were screened and only 12 RCTs were eligible in our review. Finally, 11 RCTs could be statistically analyzed. MA was more effective than sham treatment and physical therapy in terms of reducing pain (p=0.003, p=0.0007 each). Electroacupuncture (EA) showed more significant effect than physical therapy (PT) for improving shoulder pain (p<0.00001) and shoulder functionality (p<0.00001). Conclusions These results suggest that MA and EA could be superior option for treating SIS than sham treatment or PT. However this review has its limitations due to the small sample size and lack of well-designed RCTs that were included in the study. Further well-designed RCTs are necessary to provide high-level evidence.

Electroacupuncture for Rotator Cuff Disorder: A Systematic Review and Meta-Analysis (회전근개 질환의 전침 치료에 대한 체계적 문헌고찰 및 메타분석)

  • Objectives This study aims to evaluate the efficacy and safety of electroacupuncture for rotator cuff disorder. Methods We searched nine online databases (PubMed, Embase, Cochrane Library, Chinese Academic Journals, Korean studies Information Service System, Rsearch Information Sharing Service, ScienceON, KMbase, Oriental Medicine Advanced Searching Integrated System) and two related journals up to April 2024 to identify randomized controlled trials that applied electroacupuncture to rotator cuff disorder. Selected studies were analyzed for risk of bias using the Cochrane risk of bias tool, and a meta-analysis was performed with RevMan version 5.4.1. Results Out of 175 studies, eleven randomized controlled trials were selected for final analysis. Most studies showed that electroacupuncture had effect on rotator cuff disorder. In the meta-analysis, electroacupuncture combined with rehabilitation treatment was significantly more effective than rehabilitation treatment alone in improving visual analog scale (p<0.00001). Almost studies did not report any side effects or adverse reactions to electroacupuncture treatment. Conclusions This systematic review suggests that electroacupuncture is an effective treatment for pain management in rotator cuff disorder. However, the lack of adverse effect reporting and a high risk of bias indicate the need for high-quality randomized controlled trials from various countries.

  • Gait analysis is essential for leg diagnosis and rehabilitation for the patients, the handicapped and the elderly. The use of 3D motion capture device for gait analysis is very common for gait analysis. However, this device has several shortcomings including limited workspace, visibility and high price. Instead, we developed gait estimation system using gyroscopes. This system provides gait information including the number of gaits, stride and walking distance. With four gyroscope (one for each leg's thigh and calf) outputs, the proposed gait modeling estimates the movements of the hip, the knees and the feet. Complete pedestrian localization is implemented with gait information and the heading angle estimated from the rate gyro and the magnetic compass measurements. The developed system is very useful for diagnosis and the rehabilitation of the pedestrian at the hospital. It is also useful for indoor localization of the pedestrians.

  • This paper describes a development of rehabilitation training system for the postural balance control. A new rehabilitation training system, designated as a virtual cycling system, was developed to improve postural balance control by combining virtual reality technology with an unfixed bicycle. In this experiment, 20 normal adults were tested to investigate the influencing parameters of postural balance control. In order to evaluate the usefulness and the training effects of the system, several parameters including path deviation, cycling velocity, cycling time, center of pressure, and head movement were evaluated and analyzed quantitatively.   • Objectives : This study was performed to examine the hypothesis that the structural imbalance affect cardiac function and autonomic reflex system and to investigate the possibility of the chiropractic care for cardiovascular system. Methods : 78 of Dong-Guk University students with structural imbalance were recruited for the investigation from March to June 2007. Heart rate variability, Buss and Durkee Hostility inventory(BDHI) and physical examinations to evaluate psoas muscle contracture were performed. Results : Left psoas muscle contracture was associated with decrease of LF/HF ratio(p=0.048). Conclusion : Left side contracture of psoas muscle showed a tendency to decrease sympathetic activity.

Interactive Rehabilitation Support System for Dementia Patients

  • This paper presents the preliminary study of an interactive rehabilitation support system for both dementia patients and their caregivers, the goal of which is to improve the quality of life(QOL) of the patients suffering from dementia through virtual interaction. To achieve the virtual interaction, three kinds of recognition modules for speech, facial image and pen-mouse gesture are studied. The results of both practical tests and questionnaire surveys show that the proposed system had to be further improved, especially in both speech recognition and user interface for real-world applications. The surveys also revealed that the pen-mouse gesture recognition, as one of possible interactive aids, show us a probability to support weakness of speech recognition.

  • Proprioception means the ability to perceive the sensation of position and movement of body. As it is transmitted to central nervous system and used in feed-back or feed-forward motor control, proprioception allows us to keep our normal movement and normal balance activity. However, the conditions such as injury, disease, aging and fatigue can damage the proprioceptiive sensation of position, movement and lead to a functional impairment and additional damages in musculoskeletal system, because they alter the amount of proprioceptive ability that transfer into the central nervous system. The purpose of this study was to identify the definition and the function of proprioception, to look into variations in injury, disease, aging and fatigue that can be easily met in clinical application and eventually to provide valuable aid for assessment and treatment.
