  • The effect s of social support on post-stroke depression were investigated. Social support was classified into three categories : primary caregiver's, significant other's, and professional support. And primary caregiver's and significant other's support were divided into their relation, living state (which means they living together or not), frequency which they met at, and perceived satisfaction about the support, respectively. Professional support was divided into number of professionals who take care of, frequency they met at, and perceived satisfaction. The subjects were 254 stroke patients who had been discharged and were taken follow-up care at the out patient department. The depression was measured using CES-D. Out of the primary caregiver's support, only relation and perceived satisfaction were identified to affect the post-stroke depression. The patients who primarily their spouse takes care of were less depressed than those who their adult children take care of. None of the significant other's support affects the post-stroke depression. Only perceived satisfaction of the professional support was found to affect the post-stroke depression. So it was found that the quality, not the amount, of care was important to post-stroke depression. These results also support the claim that the facilities and teaching programs for primary caregivers are necessary.

  • Consequence-Based Engineering (CBE) is a new paradigm proposed by the Mid-America Earthquake Center (MAE) to guide evaluation and rehabilitation of building structures and networks in areas of low probability - high consequence earthquakes such as the central region of the U.S. The principal objective of CBE is to minimize consequences by prescribing appropriate intervention procedures for a broad range of structures and systems, in consultation with key decision makers. One possible intervention option for rehabilitating unreinforced masonry (URM) buildings, widely used for essential facilities in Mid-America, is passive energy dissipation (PED). After the CBE process is described, its application in the rehabilitation of vulnerable URM building construction in Mid-America is illustrated through the use of PED devices attached to flexible timber floor diaphragms. It is shown that PED's can be applied to URM buildings in situations where floor diaphragm flexibility can be controlled to reduce both out-of-plane and in-plane wall responses and damage. Reductions as high as 48% in roof displacement and acceleration can be achieved as demonstrated in studies reported below.

Regardless of the extent of seriousness of handicaps, they have to sustain various difficulties in their life time and pose challenging problems to their homes, community and nation. In the spectrum of their problems, measures such as treatment, rehabilitation and welfare work all occupied important part but most significant measure is the prevention. Such handicaps items from health care of maternal and child, mostly causes throughout the process of pregnancy, delivery and upbringing. And then this study is focused on the pregnant history of the mothers with handicapped children. The objective of this study is to identify the basic factors which can create a handicapped child during pregnancy and is to prevent the birth of such a child. For this study, 301 mothers who have handicapped children were selected, now receiving various training at 5 rehabilitation and educational facilities. And questionnaires distributed to them during the period of Oct, 1983 to Dec, 14. The collected responses have been analized in terms of significant numbers, percentage and metic average. Their related results are as follows: 1. The sexual ratio between the male and female was 58.5% compare 41.5%. The average age of the handicapped children was 10.1 years old and that of mothers at the time of the children's birth was 28.9 old years. They were born as 2.14th in their family, the ratio of antenatal care for pregnant mothers was 36.2% and 22.9% were abnormally delivered. 37.9% out of the total were born from mothers with ages above 35 years and as their first baby. 2. The time of confirming the handicapped nature of children was during babyhood by 97.1% and discovery of these misfortunes were made by chance. As for causes of those-handicapped status 20.9% disease from complication, 15.3% from difficulties experience at the time of delivery, 11.3% from heredity and 10% were results of drug abuse during pregnancy, and 49.8% of the handicapped children were mentally retarded.

  • PURPOSE: This study was designed to evaluate the current state of training in assistive technology(AT) for pediatric physical therapists(PPTs) and pediatric occupational therapists(POTs), in addition, investigate therapists' preferred training methods and contents. The eventual purpose was to suggest the essential fundamental factors to adapt the assistive technology in clinics. METHODS: The subject of this study were 167 therapists(98 PPTs and 69 POTs) who work at general hospitals, welfare center, facilities for the disabled, and special education school in Seoul and Gyeonggi province. Frequency analysis and Chi-squared test were used. RESULTS: Significant numbers of PPTs(66 therapists, 67.3%) and POTs(50 therapists, 72.5%) answered that they had received training in AT. More than 48% answered no training experience or low adequacy in each classification scheme for questions. The most difficulty in the training was indicated by lack of education opportunity(90 therapists, 53.9%). The most required device training was seating and position device training(43 therapists, 21.9%) for PPTs and orthosis and prosthesis device training(29 therapists, 21.0%) for POTs. In assistive technology service, PPTs needed evaluation to AT device training(69 therapists, 35.2%) and POTs needed evaluation to disabilities for AT training(41 therapists, 29.7%). Both therapists answered that the most effective training is continuous education(52 therapists, 31.1%) and college education(48 therapists, 28.7%). CONCLUSION: Our findings indicate that PPTs and POTs need more opportunities for training in AT. For effective clinical app lication of AT, there should be continuous education such as on-the-job training, mentoring program, technical manual, and college education.

  • In a broad sense, ‘After-care SYSTEM’ for discharged prisoners mean legal actions of prisoners who have been released from lawful detention In its narrow sense, mean preventive protection and observation activities under regular guidance and supervision against those released from penal facilities after a certain period of detention Therefore, they should not be viewed as objects of mere concern or social work programs but preventive protection should he provided to them as part of national criminal policy After-care system is in the following two ways, The one is based on individual prisoner's request and consent, which is called 'Voluntary After-care system', The other is the one which is not based in personal request or consent but is based on obligation, which is named 'Compulsory After-care system In Korea, however no Compulsory After-care system is in practice Voluntary After-care system is to be carried out 6 method in the following by existing Probation, Parole Law. (1) offer of board and lodging (2) allowance of Traveling expense (3) allowance of occupation instrument or lending rehabilitation fund (4) training of occupation and vocational guidance (5) self-reliance support for After-care probationer (6) guidance of good deed And then to establish the society without offenders is the ideal of human beings, but criminal acts don't fade away, so in the field of the science of criminology, the importance of correctional system has become greater. The correctional idea has moved from severe punishment to educational rehabilitation for the goal of protecting both offender and security from the threat of crime in to day Some it is required that Compulsory After-care system is most important system in effective measures, and that existing Probation, Parole Law in Korea is renewed into Compulsory After-care system in the future.

  • Purpose: This study aimed to describe Korean nursing home nurses' perceptions on dysphagia management and their working experiences of older adults with dysphagia. Methods: Using a purposive sampling design, 23 eligible nurses were interviewed as four focus groups from 4 facilities out of nursing homes. The qualitative data from focus group interviews were analyzed using thematic analysis to classify common themes into larger categories. Results: Two main themes on the attributes of caring nursing home residents with dysphagia were 'Need for nursing resources' and 'Need for support'. In addition, four subthemes emerged as a result of analysis: 'need for nursing protocols for assessing and managing dysphagia', 'need for emergency care skills of nurses and nurse assistants', 'need for prompt and accurate management by cooperation of physicians and therapists', and 'need for partnership upon comprehension of visiting family members concerning dysphagia'. Conclusion: Nursing home nurses fully recognized the importance of dysphagia management; however, they experienced substantial barriers due to lack of adequate nursing protocols or partnership with family caregivers, insufficient training for emergency care, and deficient support from medical staff. Development of nursing guidelines tailored to the nursing home context and based on partnership among medical experts and family caregivers is needed.

  • Objective: This study was conducted to provide basic data on physical function improvement activities of elderly care facilities by identifying the degree of physical improvement activities of elderly caregivers. Design: Descriptive correlational study. Methods: The instrument of this study consisted of 12 questions on the general characteristics of the subject. For the assessment of improvement in physical function activities, the measuring tool used for the elderly consisted of a total of 20 questions. For data analysis, Pearson's correlation coefficient & Spearman's rho and multiple regression were used. Results: The higher the age of the subjects, the lower their educational background (r=-0.273, p<0.05), and the higher the probability of having a religion (r=-0.258, p<0.05), the more stable the employment type (r=-0.333, p<0.05). The higher the level of education, the higher the monthly income (r=0.187, p<0.01), and the shorter the career (r=-0.204, p<0.05). The more stable employment, the more unstable duty (r=-0.245, p<0.05), and the more unstable work, the higher the monthly income (r=-0.206, p<0.05) and the longer the career, the higher the monthly income (r=0.247, p<0.05). The more stable the employment, the more activities to improve physical function were found (r=0.341, p<0.05), and the more unstable the duty, the more activities to improve physical function were found (r=0.321, p<0.05), and the higher monthly income, the less physical function improvement activities (r=-0.196, p<0.05). Conclusions: It is necessary to provide regular services by a dedicated physical therapist for physical function improvement activities in order to improve the quality of life of the elderly in the future.

  • Objective: The purpose of this study was to compare the differences in the length of hospital stay between hemorrhage stroke survivors with health insurance and those with medical care after controlling all factors except for the type of medical insurance by using the propensity score matching (PSM) method. Design: Retrospective cohort study. Methods: Data from the Korean National Centers for Disease Control and Prevention's In-Depth Discharge Injury Survey between the years 2006 and 2012 were used for analysis. A total of 4,538 cases were defined as persons with hemorrhagic stroke (I60-I62) based on the block of categories in the International Classification of Diseases (10th). In order to analyze the inpatient period differences depending on the type of health care, which reflects one's socio-economic level, the chi-square and t-test was conducted. Results: Frequency and percentage were presented, and regression analysis was used to determine the factors affecting the inpatient period. Age, severity of disease, treatment outcome, and post-discharge status were no longer statistically significant after matching. The inpatient period of the persons receiving medical aid benefits was found to be significantly longer than those with national health insurance (p<0.05). Conclusions: The factors influencing the inpatient period of hemorrhagic stroke survivors were treatment outcomes, severity of disease, hospital admission process, and the type of health care. It is necessary for systematic and comprehensive governmental management for persons with hemorrhagic stroke to be transferred to long-term care facilities.
