흡연이 치주수술을 포함한 치주치료의 결과에 미치는 영향 (The influence of smoking on the outcome of surgical periodontal therapy -2-year retrospective study)

  • The purpose of this study was to examine the effect of smoking on the clinical, radiographic outcome of surgical peridontal therapy. The outcome of periodontal surgery was evaluated in 51 systemically healthy subjects that had received maintenance care. The study subject consisted of 26 smokers and 25 non-smokers. The average age of smoking patients was 51 years old and non-smoking patients was 48 years old. Changes of probing pocket depth(PPD) and radiographic bone height, and number of missing teeth compared between smokers and non-smokers during maintenance period after surgical therapy. The clinical parameters were less favorable in the smoking group compared with non-smoking group. The number and percentage of missing teeth were greater in smokers(21.6%) than non-smokers(l2.4%), especially in molars. The mean presurgery PPD was similar in smokers and non-smokers, molars and premolars, but significantly decreased at least 2 years after surgery. The mean PPD reduction was significantly greater in non-smokers than smokers. Both in the smoking and non-smoking group, the mean PPD reduction was significantly greater in premolars than molars. The radiographical evaluation was also less favorable in the smoking group than non-smoking group. The radiographic evaluation of bone height in smokers showed bone loss. On the contrary, bone height of non-smokers showed bone gain during the period of maintenance. But there was no significant difference between molars and premolars. The clinical and radiographic outcomes of the smoking group was less favorable than those of the non-smoking group. Therefore , smoking seems to influence on the clinical and radiographic outcomes of surgical periodontal therapy.

Effect of Suction Nozzle Modification on the Performance and Aero-acoustic Noise of a Vacuum Cleaner

  • The suction nozzle of a vacuum cleaner was modified to enhance the power performance and to reduce the airflow-induced acoustic noise. The suction power efficiencies of the vacuum cleaner were measured for various nozzles; (1) original nozzle, (2) original nozzle with modified trench height, (3) original nozzle with modified connecting chamber, and (4) a combination of (2) and (3). In addition, the suction pressure and sound pressure level around the suction nozzle were measured to validate the reduction of acoustic noise. The power efficiency and mean suction pressure increased when the trench height of the suction nozzle was increased. This was attributed to the suppression of the flow separation in the suction channel. Modification of the connecting chamber in the original nozzle, which had an abrupt contraction from a rectangular chamber into a circular pipe, into a smooth converging contraction substantially improved the suction flow into the connecting pipe. When both modifications were applied simultaneously, the resulting suction nozzle was more effective from the viewpoints of aerodynamic power increase and sound pressure level reduction.

Evaluation of Dynamic Properties of Trackbed Foundation Soil Using Mid-size Resonant Column Test

  • A mid-size RC test apparatus (MRCA) equipped with a program is developed that can test samples up to D=10 cm diameter and H=20 cm height which are larger than usual samples used in practice. Using the developed RC test apparatus, two types of crushed trackbed foundation materials were tested in order to get the shear modulus reduction curves of the materials with changing of shear strain levels. For comparison purpose, large repetitive triaxial compression tests (LRT) with samples of height H=60cm and diameter D=30 cm were performed also. Resilient modulus obtained from the LRT was converted to shear modulus by considering elastic theory and strain level conversion and were compared to shear modulus values from the MRCA. It is found from this study that the MRCA can be used to test the trackbed foundation materials properly. It is found also that strain levels of $E_{v2}$ mostly used in the field should be verified considering the shear modulus reduction curves and proper values of $E_{v2}$ of trackbed foundation must be used considering the strain level verified.

Foodborne Pathogen Reduction을 위한 항균제의 새로운 Delivery System인 Aerosolization

  • Aims: As a preliminary experiment on new sanitizer delivery tools, the efficacy of aerosolizedsanitizer on foodborne pathogens was investigated in larger model chamber system.Methods: Peroxyacetic acid and hydrogen peroxide were aerosolized in a model system againstartificially inoculated target microorganisms on laboratory media. Cultures of 4 different foodborne pathogens were inoculated and affixed onto 3 different heights (bottom, wall, and ceiling), and 3different orientations (face-down, vertical, and face-down) inside a commercial semi-trailer cabinet(14.6 x 2.6 x 2.8 m). Sanitizer was aerosolized into 2 m droplet size fog and treated for 1 h atambient temperature.Results: Populations of Bacillus cereus, Listeria innocua, Staphylococcus aureus, and Salmonellatyphimurium were reduced by an average of 3.09, 7.69, 6.93 and 8.18 log units per plate, respectively.Interestingly, L. innocua, Staph. aureus, and Salm. typhimurium showed statistically not different (P$\leq$ 0.05) reduction patterns relative to height and orientation that were never expected in a sprayingsystemConclusion and significance: Aerosolized sanitizers diffuse like gaseous sanitizers, so it has greatpotential for use in commercial applications.

Current Concepts in the Mandibular Condyle Fracture Management Part II: Open Reduction Versus Closed Reduction

  • In the treatment of mandibular condyle fracture, conservative treatment using closed reduction or surgical treatment using open reduction can be used. Management of mandibular condylar fractures remains a source of ongoing controversy in oral and maxillofacial trauma. For each type of condylar fracture,the treatment method must be chosen taking into consideration the presence of teeth, fracture height, patient'sadaptation, patient's masticatory system, disturbance of occlusal function, and deviation of the mandible. In the past, closed reduction with concomitant active physical therapy conducted after intermaxillary fixation during the recovery period had been mainly used, but in recent years, open treatment of condylar fractures with rigid internal fixation has become more common. The objective of this review was to evaluate the main variables that determine the choice of an open or closed method for treatment of condylar fractures, identifying their indications, advantages, and disadvantages, and to appraise the current evidence regarding the effectiveness of interventions that are used in the management of fractures of the mandibular condyle.

Cone-beam computed tomographic evaluation of dimensional hard tissue changes following alveolar ridge preservation techniques of different bone substitutes: a systematic review and meta-analysis

  • Purpose: This study was conducted to evaluate and compare the effects of different graft materials used in alveolar ridge preservation on dimensional hard tissue changes of the alveolar ridge, assessed using cone-beam computed tomography (CBCT) scans. Methods: A systematic electronic search of MEDLINE and the Cochrane Central Register of Controlled Trials and a manual search were conducted from November 2019 until January 2020. Randomized controlled trials were included if they assessed at least 1 variable related to vertical or horizontal hard tissue changes measured using CBCT scans. After a qualitative analysis of the included studies, subgroups were formed according to the graft material used, and a quantitative analysis was performed for 5 outcome variables: changes in vertical alveolar bone height at 2 points (midbuccal and midpalatal/midlingual) and changes in horizontal (buccolingual) alveolar bone width at 3 different levels from the initial crest height (1, 3, and 5 mm). Results: The search resulted in 1,582 studies, and after an independent 3-stage screening, 16 studies were selected for qualitative analysis and 9 for quantitative analysis. The metaanalysis showed a significantly (P<0.05) lower reduction of alveolar ridge dimensions for the xenogenic subgroup than in the allogenic subgroup, both vertically at the midbuccal aspect (weighted mean difference [WMD]=-0.20; standard error [SE]=0.26 vs. WMD=-0.90; SE=0.22) as well as horizontally at 1 mm (WMD=-1.32; SE=0.07 vs. WMD=-2.99; SE=0.96) and 3 mm (WMD=-0.78; SE=0.11 vs. WMD=-1.63; SE=0.40) from the initial crest height. No statistical analysis could be performed for the autogenic subgroup because it was not reported in sufficient numbers. Conclusions: Less vertical and horizontal bone reduction was observed when xenogenic graft materials were used than when allogenic graft materials were used; however, the loss of alveolar ridge dimensions could not be completely prevented by any graft material.

공진장치에 의한 지진해일파의 제어에 관한 수치시뮬레이션(II) (삼척항에 대해) (Numerical Simulation on Control of Tsunami by Resonator (II) (for Samcheok port))

  • 전편의 연구에서는 기존공진장치를 동해안의 묵호항과 임원항에 적용하여 1983년에 발생한 동해중부지진해일과 1993년에 발생한 북해도남서외해지진해일의 작용 하에 각 항내에서 지진해일고의 저감율을 수치적으로 검토하여 실해역의 지진해일파에 대한 기존공진장치의 유용성을 확인하였다. 후편의 본 연구에서는 전편과 동일한 지지해일파의 작용조건 하에 세 개의 신형공진장치 I, II-1 및 II-2를 동해안의 삼척항에 적용하여 지진해일고의 저감율을 각각 검토하였으며, 또한 수치해석에는 전편과 일관성을 갖도록 COMCOT 모델을 적용하였다. 삼척항에서 각 신형공진장치의 부착여부, 각 신형공진장치의 크기변화에 따른 최대지진해일고의 공간분포와 수위의 시간변동 및 공진장치의 유용성 등을 함께 논의하였다. 이로부터 신형공진장치를 부착함으로서 지진해일고를 최대 40% 정도로 저감시켰으며, 따라서 신형공진장치의 유용성을 확인할 수 있었다. 현장조건에 적합한 최적의 공진장치를 얻기 위해서는 지진해일파의 입사 방향 등을 고려하여 공진장치의 형상, 배치 및 크기에 관해 다각도적인 검토가 수행될 필요가 있는 것으로 판단된다.

Shaking Table Model Test of Shanghai Tower

  • Shaking table test is an important and useful method to help structural engineers get better knowledge about the seismic performance of the buildings with complex structure, just like Shanghai tower. According to Chinese seismic design guidelines, buildings with a very complex and special structural system, or whose height is far beyond the limitation of interrelated codes, should be firstly studied through the experiment on seismic behavior. To investigate the structural response, the weak storey and crack pattern under earthquakes of different levels, and to help the designers improve the design scheme, the shaking table model tests of a scaled model of Shanghai tower were carried out at the State Key Laboratory of Disaster Reduction in Civil Engineering, Tongji University, Shanghai, China. This paper describes briefly the structural system, the design method and manufacture process of the scaled model, and the test results as well.

하악골 과두 골절의 관혈적 정복술과 비관혈적 정복술의 예후에 관한 후향적 연구 (RETROSPECTIVE STUDY FOR PROGNOSIS AFTER OPEN AND CLOSED REDUCTION OF THE MANDIBULAR CONDYLE FRACTURES)

  • Condylar process of mandible, has the specialized anatomic structure compared with any other body structure, acts directly in connection with mastication and speech and so on. In general, mandibular condyle fractures have been managed by two methods as open and closed reduction. But, there are no reasonable consensus about the proper management of this injury. This study was designed for analysis of the prognosis of two methods of treatment, open and closed reduction, with positional change of fractured condyle and complications within 6 months post-intermaxillary fixation period. We conducted a retrospective analysis of 154 patients whose unilateral mandibular condyle fractures were treated by open or closed reduction in our department. The horizontal, sagittal, and coronal change of the condyle was examined using modified Towne's and panoramic radiographs before intermaxillary fixation(IMF), immediately after IMF, and at 6 months after IMF. Patients, whose mandibular condyle fractures were treated by closed reduction, had significantly shorter ramus height on the side of injury(P<0.05). But, fractured condylar fragments were displaced insignificantly with aspect to sagittal and coronal plane(P>0.05). The level of the fracture influenced the ramus length and the degree of coronal change in the closed reduction group(P<0.05). There was no significant correlation among the level of the fracture, treatment methods and complications(P>0.05). From the results obtained in this study, fractured mandibular condyles, were treated by closed reduction, had a tendency that continuous condylar displacement was occurred with aspect to horozontal and coronal plane in treatment period including intermaxillary fixation. And then there was a correlation between the level of the fracture and the position change in close reduction group statistically. These result suggested that care must be taken in basing treatment decisions on the degree of displacement of the condyle and in treating the mandibular condyle fractures for a long time.

수치해석에 의한 석회암 채굴 사면의 안정성 해석 (Stability Analysis of Rock Slope in Limestone Mine by Numerical Analysis)

  • 한라 석회석 광산에서 높이가 대체로 58m이고 사면의 경사각이 70$^{\circ}$인 암반 사면의 안정성 해석을 위하여 암반을 연속체로 가정한 FDM과 절리의 강도 특성 등을 주로 고려한 DEM 등 두 가지 수치해석법을 사용하였다. 수치해석법으로 사면의 안정성을 안전율로 표시하기 위하여 강도감소법을 이용하였다. 연속체 해석 결과에서는 사면의 안정이 유지되는 것으로 판단할 수 있으나 절리의 특성을 고려한 해석 결과 사면의 안전율이 1.3보다 크도록 사면을 안전하게 유지하기 위해서는 사면 높이 를 45m로, 경사를 55로$^{\circ}$로 변경 하는 것이 바람직 한 것으로 판단되었다.
