근전도와 저항 센서를 이용한 보행 단계 감지 (Gait Phases Detection from EMG and FSR Signals in Walkingamong Children)

  • 본 연구에서는 근전도 신호를 활용하여 정상인의 보행과 관련된 상지와 하지 근육의 신호를 확인하고 저항센서를 이용하여 정상적인 보행 패턴을 확인하였다. 대학생 15명을 대상으로 정지해 있을 때와 평지를 보행할 때, 상지의 4부위(대흉근과 승모근)와 하지의 10부위(대퇴직근, 대퇴이두근, 내측광근, 외측광근, 반막양근, 반건양근, 가자미근, 장비골근, 내비복근과 외비복근)에 전극을 부착하여 근전도를 측정하였다. 저항센서는 양측 발바닥의 8부위에 센서를 부착하여 보행시 발에 가해지는 압력을 측정하였다. 그 결과, 근전도 신호는 정지상태에 비하여 보행 시에 허벅지의 외측광근과 반건양근을 제외하고 모든 근육에서 유의하게 높은 진폭을 가졌다. 또한 보행주기의 두 단계인 입각기와 유각기와 관련된 근육을 확인하였다. 저항 센서의 신호 분석 결과, 평균 보폭 주기 동안 크게 입각기와 유각기의 두 주기와 세부적으로 여덟 단계 - 초기 접지기, 하중 반응기, 중간 입각기, 말기 입각기, 전 유각기, 초기 유각기, 중간 유각기, 말기 유각기 - 의 보행 주기를 확인할 수 있었다.

경호무도를 수련한 경호원의 우발 상황 시 경호자세의 반응시간 및 EMG 패턴 분석 (Reaction time and EMG pattern analysis of Body guards who have trained martial arts to accidental situations)

  • 본 연구의 목적은 경호 중 우발 상황 시 경호원들의 자세를 분석하여 객관적이고 정량적인 기초자료를 얻는데 있다. 이러한 목적을 달성하기 위하여 경호무도 수련한 4명을 대상으로 하였으며, 경호자세의 반응시간 및 근육의 활성도를 분석하기 위하여 EMG시스템을 활용하여 분석한 결과 다음과 같은 결론을 얻었다. 경호자세의 전체반응시간은 평균 $1.38{\pm}0.07$초 소요되었다. 신호에 대한 1구간의 반응시간은 구간은 평균 $0.22{\pm}0.02$초였으며, 신호의 확인 이후 2구간의 반응시간은 평균 $0.62{\pm}0.10$초로 추진력을 얻기 위해 가장 긴 시간이 소요되었으며, 그다음 3구간은 $0.29{\pm}0.02$초, 4구간은 $0.26{\pm}0.02$초로 점점 시간이 단축되는 것을 알 수 있었다. 경호자세의 각 근육의 활성도는 1구간에서도 좌우 전경골근, 2구간에서는 좌우 전경골근과 우측 비복근, 3구간에서는 우측 전경골근과 비복근, 4구간에서는 좌우 대퇴이두근과 우측 대퇴직근의 활성도가 크게 나타내보였다. 또, 1구간과 2구간은 주로 하퇴근육을 많이 사용하였으며, 3구간과 4구간에서는 대퇴와 하퇴근육을 즉, 하지 전체를 근육을 사용하는 것을 알 수 있었다.

슬관절 탄력테이핑 적용이 슬관절 굴곡과 신전의 근력, 근파워, 근지구력에 미치는 영향 (Effect of Kinesio Taping on Muscular Strength, Power, and Endurance of the Extension and Flexion of the Knee Joint)

  • 본 연구는 스포츠 현장에서 부상예방으로 많이 사용되고 있는 탄력 테이핑을 슬관절 굴곡과 신전에 적용하여 근력, 근파워, 근지구력에 미치는 영향을 검증하기 위해 진행하였다. 연구 대상자는 슬관절의 굴곡과 신전 운동에 이상이 없는 남자 대학생 10명으로 하였다. 측정방법은 등속성근기능측정 장비를 활용하여 슬관절 굴근과 신근에 키네시오 테이핑을 적용 전, 후로 각속도 60°/sec 5회, 180°/sec 5회, 240°/sec 25회를 측정하였다. 연구결과는 슬관절 신전 시 키네시오 테이핑 적용 전보다 후에 우측 각속도 180°/sec에서 Average Power가 유의하게 증가하였다(p=.010). 슬관절 굴곡 시 키네시오 테이핑 적용 전보다 후에 우측 각속도 180°/sec에서 Average Power가 유의하게 증가하였다(p=.016). 또한 각속도 240°/sec에서는 Total Work Done에서 좌(p=.002), 우(p=.002) 모두 유의하게 증가하였다. 대퇴직근과 대퇴이두근에 키네시오 테이핑 적용이 슬관절 운동 시 근파워과 근지구력을 향상시키는 보조적인 영향이 긍정적으로 나타났다.

신경근전기자극치료가 암 환자의 근육 손실에 미치는 영향: 체계적 문헌고찰 (Effects of Neuromuscular Electrical Stimulation on Muscle Wasting with Cancer Patient: A Systematic Review)

  • Objectives The purpose of this study is to systematically review the effects of Neuromuscular Electrical Stimulation treatment on muscle wasting in cancer patients. Methods Randomized controlled trials (RCTs) were searched from seven online databases (PubMed, Cochrane Library, EMBASE, China National Knowledge Infrastructure, Oriental Medicine Advanced Searching Integrated System, Korean studies Information Service System, Research Information Sharing Service. The selected RCTs were evaluated for methodological quality through the Cochrane RoB. Results A total of 126 articles were identified, and 4 randomized controlled trials were selected for systematic review. In one study, it was found that there were statistically significant improvements in the Health-Related Quality of Life (FACTH&N total score, p<0.001). Additionally, significant effects were observed in measurements that represented the size of the quadriceps muscle (cross-sectional area of Vastus lateralis and Rectus femoris, p=0.004), maximum muscle strength, the twitch response of resistance muscles, and voluntary activation (p<0.001). However, no significant differences were observed between the intervention and control groups in terms of quadriceps muscle strength in two other studies (p>0.05). Lastly, while one study showed no significant differences in muscle fiber characteristics between the two groups, it did report significant improvements in measurements related to mitochondria within muscle tissue and muscle strength in the intervention group (p<0.05). Conclusions Neuromuscular electrical stimulation can be a method used to improve muscle strength in muscle wasting of cancer patients, but it is difficult to see its effects as significant compared to other treatments.

스트레칭의 유형에 따른 근활성도 및 최대 수의적 등척성 수축력의 유지기간 비교 (Comparison of Maintenance Period on Muscle Activity and MVIC according to Type of Stretching)

  • 본 연구는 넙다리곧은근에 적용한 정적 및 탄성적 스트레칭의 유지기간을 알아보기 위하여 남성 22명을 대상으로 정적 스트레칭 군 11명과 탄성적 스트레칭 군 11명을 스트레칭 적용 전, 적용 후 30초, 1분 후, 3분 후, 5분 후, 10분 후에 중앙주파수 및 최대 수의적 등척성 수축력의 변화를 분석하였다. 중앙주파수는 시간에 따른 변화와 군 간에서 유의한 차이를 보였다. 정적 스트레칭군과 탄성적 스트레칭군 모두에서 3분후까지 감소되었다가 증가되었다. 최대 수의적 등척성 수축력은 시간에 따른 변화와 군 간에서도 유의한 차이를 보였다. 정적 스트레칭군은 3분후까지 감소되었다가 그 이후부터 증가되었으며, 탄성적 스트레칭군은 5분후까지 감소되었다가 증가되었다. 결론적으로 스트레칭 적용 후 시간에 따라 변화를 보였으며, 정적 스트레칭보다 탄성적 스트레칭의 효과가 조금 더 길게 유지되는 것을 알 수 있었다.

Motion Analysis and EMG Analysis of the Pelvis and Lower Extremity according to the Width Variation of the Base of Support

  • The purpose of this study is to identify which width of the base of support(BOS) is safer and more effective in lifting by comparing muscle activations and body sways when lifting objects under the width variation of the BOS. A total of fifteen healthy adults participated in this study. For the width variation of the BOS, the participants changed the width between their feet into three different types(10cm, 32cm, 45cm) and lifted a 10kg four times in each type after going up on a force plate. In order to measure body sways according to the width variation of the BOS, a motion analysis system was used. In addition, in order to measure the muscle activations of lower extremities, including the erector spinae, gluteus maximus, rectus femoris, and tibialis anterior, an electromyogram(EMG) analysis was employed. In addition, the Borg's scale was drawn by quantifying the subjective discomfort levels felt from each width of the BOS. In conclusion, no statistically significant differences according to the width variation of the BOS were observed(p=.295, .308)(p>.05). However, a statistically significant difference was exhibited between the Borg's scale, which indicates the discomfort levels from lifting performances, and the width variation of the BOS (p=$.000^*$).

Surgical implications of anatomical variation in anterolateral thigh flaps for the reconstruction of oral and maxillofacial soft tissue defects: focus on perforators and pedicles

  • Objectives: To gain information on anatomical variation in anterolateral thigh (ALT) flaps in a series of clinical cases, with special focus on perforators and pedicles, for potential use in reconstruction of oral and maxillofacial soft tissue defects. Materials and Methods: Eight patients who underwent microvascular reconstructive surgery with ALT free flaps after ablative surgery for oral cancer were included. The number of perforators included in cutaneous flaps, location of perforators (septocutaneous or musculocutaneous), and the course of vascular pedicles were intraoperatively investigated. Results: Four cases with a single perforator and four cases with multiple perforators were included in the ALT flap designed along the line from anterior superior iliac spine to patella. Three cases had perforators running the septum between the vastus lateralis and rectus femoris muscle (septocutaneous type), and five cases had perforators running in the vastus lateralis muscle (musculocutaneous type). Regarding the course of vascular pedicles, five cases were derived from the descending branch of the lateral circumflex femoral artery (type I), and three cases were from the transverse branch (type II). Conclusion: Anatomical variation affecting the distribution of perforators and the course of pedicles might prevent use of an ALT free flap in various reconstruction cases. However, these issues can be overcome with an understanding of anatomical variation and meticulous surgical dissection. ALT free flaps are considered reliable options for reconstruction of soft tissue defects of the oral and maxillofacial area.

태권도 선수와 일반인의 등척성 무릎신전 토크-각도 관계 특성 비교 분석 (Comparison of Isometric Knee Extension Torque-Angle Relationship between Taekwondo Athletes and Normal Adults)

  • Objective : In order for Taekwondo athletes to perform destructive kicking performance, they are expected to have Taekwondo-specific muscle properties such as high muscle strength and power. The purpose of this study was to investigate the joint angle-dependent force-producing property of Taekwondo athletes' knee extensor muscles, which is one of the primary muscle groups involved in kicking performance. Method : Ten Taekwondo male athletes (age: $19.9{\pm}0.7yrs$, height: $180.6{\pm}6.2cm$, body mass: $75.9{\pm}8.9kg$, career: $9.2{\pm}2.9yrs$.) and 10 healthy male non-athletes (age: $26.3{\pm}2.6yrs$, height: $174.2{\pm}4.8cm$, body mass: $72.8{\pm}7.7kg$) participated in this study. Subjects performed maximum isometric knee extension at knee joint angles of $40^{\circ}$, $60^{\circ}$, $80^{\circ}$, and $100^{\circ}$ (the full knee extension was set to $0^{\circ}$) with the hip joint angles of $0^{\circ}$ and $80^{\circ}$ (the full extension was set to $0^{\circ}$). During the contractions, knee extension torque using an isokinetic dynamometer simultaneously with muscle activities of the rectus femoris (RF), and the vastus lateralis (VL) and vastus medialis (VM) using surface electromyography were recorded. Based on the torque values at systematically different knee-hip joint angles, the joint torque-angle relationships were established and then the optimal joint angle for the knee extensor was estimated. Results : The results of this study showed that the isometric knee extension torque values were greater for the Taekwondo athletes compared with the non-athlete group at all hip-knee joint angle combinations (p<.05). When the hip joint was set at $80^{\circ}$, the peak isometric torque was greater for the Taekwondo athletes compared with the non-athlete group ($313.61{\pm}36.79Nm$ and $221.43{\pm}35.92Nm$, respectively; p<.05) but the estimated optimum knee joint angles were similar ($62.33{\pm}5.71^{\circ}$ and $62.30{\pm}4.67^{\circ}$ for the Taekwondo athletes and non-athlete group, respectively). When the hip joint was set at $0^{\circ}$, the peak isometric torque was greater for the Taekwondo athletes compared with the non-athlete group ($296.29{\pm}45.13Nm$ and $199.58{\pm}25.23Nm$, respectively; p<.05) and the estimated optimum knee joint angle was larger for the Taekwondo athletes compared with the non-athlete group ($78.47{\pm}5.14^{\circ}$ and $67.54{\pm}5.77^{\circ}$, respectively; p<.05). Conclusion : The results of this study suggests that, compared with non-athletes, Taekwondo athletes have stronger knee extensor strength at all hip-knee joint angle combinations as well as longer optimum muscle length, which might be optimized for the event-specific required performance through prolonged training period.

호흡운동을 병행한 몸통 근력운동과 심부 안정화 운동이 배근육 두께와 호흡에 미치는 영향 (Effect of Trunk Strength Exercise and Deep Stabilization Exercise Combined with Breathing Exercise on Abdominal Muscle Thickness and Respiration)

  • Purpose : The purpose of this study is to compare the effects on abdominal muscle thickness and breathing by applying trunk strength exercise and deep stabilization exercise along with breathing exercise, which is the main respiratory muscle during breathing, to present an efficient exercise method with diaphragm breathing. Methods : This study was performed on normal 6 females and 14 males subjects. They were divided into 2 groups which trunk strength exercise and deep stabilization exercise group. The trunk strength exercise group (TSE) attended prone press-up, crunch and pelvic tiling. The deep stabilization exercise group (DSE) attended abdominal drawing, horizontal side-support and bridging exercise. Breathing exercise was performed for each set break time for 1 minute. Results : First, in the comparison of the change in the thickness of the abdominal muscle between the trunk strength training group and the deep stabilization group before and after exercise, there was a statistically significant difference in the comparison of transverse abdominis (TrA), rectus femoris (RF), external oblique (EO), internal oblique (IO) (p<.05). However, there was no significant difference in any comparison between groups (p>.05). Second, in the comparison of changes in respiratory function between the trunk strength exercise group and the deep stabilization exercise group before and after exercise, there were statistically significant differences in the exerted forced vital capacity (FVC), forced expiratory volume at one second (FEV1), peak expiratory flow (PEF) in the comparison before and after the experiment (p<.05). However, there was no significant difference in any comparison between groups (p>.05). Conclusion : As a result of this study, it can be said that both trunk strength exercises and deep stabilization exercises along with diaphragm breathing are exercises that strengthen deep and superficial muscles, and have a positive effect on breathing function as well as muscle strength. However, it is not known which exercise was more effective, and because it was combined with breathing exercise, the interference effect appeared.

슬관절의 등속성 최대 반복 신전시 Hilbert-Huang 변환과 AR 모델을 이용한 근피로 평가 (Muscle Fatigue Assessment using Hilbert-Huang Transform and an Autoregressive Model during Repetitive Maximum Isokinetic Knee Extensions)

  • In the working population, muscle fatigue and musculoskeletal discomfort are common, which, in the case of insufficient recovery may lead to musculoskeletal pain. Workers suffering from musculoskeletal pains need to be rehabilitated for recovery. Isokinetic testing has been used in physical strengthening, rehabilitation and post-operative orthopedic surgery. Frequency analysis of electromyography (EMG) signals using the mean frequency (MNF) has been widely used to characterize muscle fatigue. During isokinetic contractions, EMG signals present strong nonstationarities. Hilbert-Haung transform (HHT) and autoregressive (AR) model have been known more suitable than Fourier or wavelet transform for nonstationary signals. Moreover, several analyses have been performed within each active phase during isokinetic contractions. Thus, the aims of this study were i) to determine which one was better suitable for the analysis of MNF between HHT and AR model during repetitive maximum isokinetic extensions and ii) to investigate whether the analysis could be repeated for sequential fixed epoch lengths. Seven healthy volunteers (five males and two females) performed isokinetic knee extensions at $60^{\circ}/s$ and $240^{\circ}/s$ until 50% of the maximum peak torque was reached. Surface EMG signals were recorded from the rectus femoris of the right thigh. An algorithm detecting the onset and offset of EMG signals was applied to extract each active phase of the muscle. Following the results, slopes from the least-square error linear regression of MNF values showed that muscle fatigue of all subjects occurred. The AR model is better suited than HHT for estimating MNF from nonstationary EMG signals during isokinetic knee extensions. Moreover, the linear regression can be extracted from MNF values calculated by sequential fixed epoch lengths (p> 0.0I).
