The following influencing variables were selected based on the model developed by Anderson and Newman(1973): predisposing factors, enabling factors, need factors. The result shows that in both group the possibility of service utilization is high in case of having a spouse, more family members, feeling one's health ill, no chronic disease. And service use level is high in case of having no spouse, lower income in general group, higher income in low-income group, more family members, feeling one's health ill, having a chronic disease. In addition, the higher service use level is associated in low-income group with men rather than women, younger age, low education, the unemployed rather than the employed, urban residents, low life satisfaction. These findings provide implications and suggestions that each income group has various characteristics and needs, so it is necessary to provide comprehensive and appropriate services through the investigation for potential service recipients' needs.

  • The purpose of this study is to explore the family diversity-related service experiences of Family Center practitioners under the main policy goal of recognizing family diversity set out in the Fourth Master Plan for Healthy Families. To this end, a focus group interview was conducted with 12 Family Center practitioners in multiple cities and districts who had extensive family service experiences. The focus group interview data were analyzed using thematic analysis. The analysis found that Family Center practitioners understood the concept of family diversity according to the changes in the family policy environment and applied it in various forms to the administration of their respective centers. However, while administering the family support services considering the aspect of family diversity, they experienced a lack of resources for various family service programs, conflicts between each family service goal and the family diversity-related program goal, and difficulties in identifying the recipients. To expand family diversity-related services in the future, the interviewees emphasized the need for education programs about family diversity targeted at Family Center practitioners, and family support services that reflect regional characteristics. They also stressed the need for changes in program operation methods, such as expanding family diversity education, and developing family service programs appropriate for diverse families. Lastly, they highlighted the need for mounting a publicity campaign for Family Centers, widening the target recipients of family programs, and expanding the budget for the administration of the family diversity project. The results of this study can serve as a basis for restructuring family services for the application and expansion of family diversity practices, and as educational material that can be used to strengthen the capacity of family service practitioners to plan and execute family support programs that take family diversity into consideration.

Method: The sample 492 older adults participated in the study. The QOL was measured using the scale of QOL of Ware and other data were collected through face-to-face interviews from September to August, 2009. Results: The level of QOL was moderate (Mean 24.4, SD 7.4). The QOL was poorer in older adults (p<.05) and in those living alone (p<.01) compared to older adults and those living with couple or family respectively. The QOL was positively correlated with a sense of mastery (r=.213, p<.05), connection of health (r=.160, p<.05) and a cognitive function (r=.119, p<.01), and negatively correlated with Activities of Daily Living (r=-.266, p<.01) and Instrumental Activities of Daily Living (r=-.339, p<.01). • Objectives: In Korea, the top 10% of Medical Aid recipients represent nearly 60% of total payment, with the costs for those disabled for over 365 days representing approximately 30% of total payment. The purpose of this study was to compare Medical Aid use of the disabled with non-disabled recipients, and to identify contributing factors to the total payment in the top 2% of recipients identified as Medical Aid overusers. Methods: Subjects (n=2,211) selected were ${\geq}18$-years-of-age and received >1000 days of co-payment-free type I Medical Aid. Case managers (n=200) conducted interviews in December 2006, and collected data from Health Insurance Review & Assessment Service. Amounts over the 9 months from January September 2006 were analyzed descriptively and using Chi-square, ANCOVA, and robust multiple linear regression. Results: Disabled individuals (mean age 61.3 years) composed 36.6% of subjects; 44.8% of the disabled were male. On a monthly basis per capita, the disabled group averaged 10.5 outpatient days, total payment of 523,000 Korean Won(₩), inpatient payment of ₩359,000, and outpatient payment of ₩183,000. All values exceeded the monthly average for non-disabled individuals. Contributing factors were identified as male gender (₩82,000), elementary school or lower educational level (₩64,000), residence in a small city (₩82,000), lack of family support (₩61,000), kidney disability (₩673,000), intellectual disability (₩151,000), and multiple disabilities (₩119,000). Conclusions: The identification of contributing factors to Medical Aid use by those defined as disabled supports the adoption of comprehensive alternative policies such as strengthening of education and consultation services, provision of alternative facilities, and promotion of self-care.
