측사위경두개방사선사진(Oblique Transcranial Radiograph)을 이용한 하악전돌증환자의 하악지 시상골절단술후 하악과두위치변화 분석 (OBLIQUE TRANSCRANIAL RADIOGRAPHIC ANALYSIS OF CHANGES IN CONDYLE POSITION FOLLOWING SAGITTAL SPLIT RAMUS OSTEOTOMY IN MANDIBULAR PROGNATHISM)

  • This study was intended to evaluate condyle position and the relationship of condyle position change and post surgical relapse following the sagittal split ramus osteotomy for mandible setback in 25 patients by paired t-test and multiple regression analysis. We used oblique transcranial and cephalometric radiographs taken before operation, immediate after operation, and at least 6 months post operatively. 1. In oblique transcranial view, posterior joint space was decreased immediate after operation and increased 6 months after operation. To compare the measurement before and 6 months after operation, there was no statistically significant change in over all joint spaces(P>0.05). 2. The joint spaces changed under the 0.2mm were 30%, 0.2mm to 1.0mm were 60.7%, above 1.0mm were 9.3%. This result reveals that condyle position was relatively reproduced to pre-operative state. 3. Statistically, the amount of mandible set back didn't influence the post operative relapse(P>0.05). 4. Statistically, the amount of mandible set back didn't influence the condylar displacement(P>0.05), and the amount of joint space change didn't influence the post operative relapse.(P>0.05) The changes in joint space is in the standard tracing error or within the adaptive capacity of the individual, it was too small to influence the stability of surgery.

하악전돌증의 악교정수술 후 연조직 형태변화에 대한 연구 (Changes in Soft Tissue Profile after Surgical Correction of Prognathic Mandible)

  • 악교정수술에 관한 치료계획은 치료결과에 관한 정확한 예측을 바탕으로 수립되는 것이 바람직하며 이는 심미적인 결과를 얻기 위한 가장 중요한 요소 중에 하나이다. 성인에서 골격성 하악전돌증의 치료는 하악골후퇴술을 이용한 악교정수술법이 주로 이용되는데, 술후 턱끝의 돌출도가 과도하게 감소되어 하악전진이부성형술을 추가로 시행해야 하는 경우 하악골과 턱 부위의 연조직 변화방향이 상이하게 나타나므로 술후 연조직 변화를 정확히 예측하는데 한계가 있다. 본 연구에서는 골격성 하악전돌증을 주소로 악교정수술을 시행한 성인을 대상으로, 상행지시상분할골절단술(BSSRO)만을 시행한 경우(A군) 11명 과 하악전진이부성형술(advancement genioplasty)도 같이 시행한 경우(B군) 9명의 경조직과 연조직 변화의 상관관계를 알아보기 위하여, 수술 8개월 이후에 채득된 측모두부계측방사선사진을 비교하여 다음과 같은 결과를 얻었다. 1. E-line에 대하여 상순은 더 돌출되고 하순은 더 후퇴되어 입술의 형태가 개선되었으며, 하악전진이부성형술을 병행한 경우 하순의 돌출도는 더 개선되었다. 2. 두 군 모두에서 상순은 후방이동하고 비순각은 증가하였다. 3. 술후 B점에 대한 연조직 POGs점의 변화율은 A군에서 $84\%$, B군에서 $66\%$로 나타났으며 두 군간에 통계적 유의성이 있었다. 4. 경조직과 계측점의 수직변화량은 A군에서 감소하였다.

파노라마방사선사진에 있어서 이차상에 관한 연구 (A STUDY ON SECONDARY IMAGES IN PANORAMIC RADIOGRAPH)

  • This study was performed to observe the secondary images and to analyse the relationships between the primary and secondary images in panoramic radiograph. Using the Moritta's Panex-EC panoramic x-ray machine and the human dry skull, the author analysed 17 radiographs which were selected from 65 radiographs of the dry skull that attached the radiopaque materials, and the attached regions of the radiopaque materials were the normal anatomical structures which were important and selected as a regions for the evaluation of the secondary images effectively. The results were as follows; 1. The cervical vertebrae showed three images. The midline image was the most distorted and less clear, and bilateral images were slightly superimposed over the posterior border of the mandibular ramus. 2. In mandible, the secondary image of the posterior border of the ramus was superimposed on the opposite ramus region, and this image was elongated from the anterior border of the ramus to the lateral side of the posterior border of the ramus. The secondary image of the condyle was observed on the upper area of the coronoid process, the sigmoid notch and the condyle in opposite side. 3. In maxilla, the posterior region of the hard palate showed the secondary image on the lower part of the nasal cavity and the medial wall of the maxillary sinus. 4. The primary images of the occipital condyle and the mastoid process appeared on the same region, and only the secondary image of the occipital condyle was observed symmetrically on the opposite side with similar shape to the primary one. 5. In the cranial base, the anatomical structures of the midsagittal portions like a inferior border of the frontal sinus, sella turcica, inferior borderr of the sphenoid sinus and inferior border of the posterior part of the occipital bone showed the simillar shape between the primary and secondary images symmetrically. 6. The petrous portion of the temporal bone showed the secondary image on the lateral side of the sella-turcica, and the secondary images of the posterior border of lesser wing, superior border of greater wing of the sphenoid bone and posterior border were observed on the anterior-superior and inferior region of the sella-turcica.

Sagittal split ramus osteotomy, intraoral vertical ramus osteotomy, and lateral corticectomy for asymmetric mandibular prognathism

  • Objectives: The purpose of this study was to evaluate the postoperative anteroposterior stability and improvements in facial asymmetry after performing LeFort I osteotomy in the maxilla, sagittal split ramus osteotomy (SSRO) and intraoral vertical ramus osteotomy (IVRO) in the mandible, and lateral corticectomy on the IVRO side. Materials and Methods: From July 2009 to October 2018, a retrospective analysis was performed on 11 subjects. Lateral cephalometric radiograph was performed preoperatively (T0), postoperatively (T1), and at 12 months of follow-up (T2), and the B point distance was measured. Posteroanterior cephalometric radiograph was performed preoperatively (S0) and at 12 months of follow-up (S1) and was used to measure five indicators (Ag angle, M-Ag, Co-Ag, Co-Me, and Ag-Me) of facial asymmetry. Results: The B point distances for T0 and T1 were significantly different (P=0.007), whereas those for T1 and T2 were not significantly different (P=0.1). In addition, there was a significant difference between the B point distances of T2 and T0 (P=0.026). Comparison of the facial asymmetry indicators before and after surgery showed a significant difference for all indicators between S0 and S1: the P-values of Ag angle, M-Ag, Co-Ag, Co-Me, and Ag-Me were 0.003, 0.003, 0.008, 0.006, and 0.004, respectively. The Z value was based on negative ranks. Conclusion: There was no significant difference in the B point distances from postoperation to the 12-month follow-up. However, there were significant differences in all five indicators related to facial asymmetry before and after surgery. The values for the five indicators of facial asymmetry all increased postoperatively.

하악전돌증 환자의 하악지시상분할골절단술 후 고정방법에 따른 안정성과 회귀율에 대한 분석 (COMPARATIVE STUDY OF STABILITY AND RELAPSE ACCORDING TO FIXATION METHOD AFTER BILATERAL SAGITTAL SPLIT RAMUS OSTEOTOMIES IN MANDIBULAR PROGNATHIC PATIENTS)

  • The purpose of this study was to compare the postoperative stability and relapse according to 2 different fixation methods after bilateral sagittal split ramus osteotomies in mandibular prognathic patients. Tweenty one patients with Class III dental and skeletal malocclusion who were treated with bilateral sagittal split ramus osteotomy were selected for this retrospective study. We classfied the patients into two groups according to the fixation methods of bony segments after osteotomies. Group W (n = 10) had the bone segments fixed with nonrigid wire and Group S (n = 11) had bicortical screws inserted in the gonial area through a transcutaneous approach. Cephalometric radiographs were taken preoperatively, immediate postoperatively and more than six months postoperatively in each patient. After tracing the cephalometric radiographs, various parameters were measured. Before surgery, both groups were balanced with respect to linear and angular measurements of craniofacial morphology. Mean posterior sagittal setback amounts of the mandibular symphysis was 8.6 mm in the wire group and 6.79 mm in the rigid group, Six months postoperatively, the wire group had 33.1% relapse of the mandibular symphysis and 22.8% in the rigid group relapse. Both groups experienced changes in the orientation and configuration of the mandible. It is thought that Rigid screw fixation is a more stable method than nonrigid wire fixation for maintaining mandibular setback after sagittal split ramus osteotomy.

외측 하악각 골절제술을 동반한 시상분할골절단술을 통한 골격성 3급 하악골 비대칭 환자의 치료 (THE CORRECTION OF CLASS III MANDIBULAR ASYMMETRY USING BILATERAL SAGITTAL SPLIT RAMUS OSTEOTOMY AND LATERAL ANGLE REDUCTION)

  • Purpose: The aim of this study is to identify the usefulness of unilateral mandibular angle ostectomy, so-called "Lateral Angle Reduction", in asymmetric prognathism patients by the assessment of postoperative stability and esthetic results Patients and methods: For the retrospective study, 10 skeletal class III mandibular asymmetry patients who were performed SSRO and unilateral mandibular angle ostectomy, Lateral Angle Reduction, was selected. Lateral and posterioanterior cephalogram was taken before surgery (T0), 1day after surgery (T1) and 6month after surgery (T2). To know the esthetic results the facial width and lateral facial contour were examined on posterioanterior cephalogram and to know the postoperative stability B point and Incisor inferius was examined on lateral cephalogram. Statistical analysis was performed. Results: From T0 to T1, Intergonial width was significantly decreased, dominantly at shortened side but no significant changes at lengthened side. Those were well-maintained during 6 months. Lateral facial angle and Ramus angle was significantly decreased on only shortened side from T0 to T1. As a result, after surgery, there were no significant differences in all measurements between shortened side and lengthened side. Ramus deviation angle in shortened side and ramus angle in lengthened side which reflect the angulation of ramus on frontal plane didn't show significant changes after surgery and during postsurgical periods. Lower dental midline showed no statistical changes during postsurgical period. The relapse rate on B-point was 11.92%. Conclusion: Unilateral "Lateral angle reduction" in the asymmetric mandible is valuable to obtain the narrow lower face and symmetric facial contour with a good stability.

New Treatment in Facial Nerve Palsy Caused by Sagittal Split Ramus Osteotomy of Mandible

  • A 25-years-old woman with mandibular prognathism underwent a mandibular setback by way of mandibular sagittal split ramus osteotomy (MSSRO). After 2 days of operation, she developed difficulty of closing her right eye. The blink reflex test and motor nerve conduction study of the right orbicularis oris muscle were revealed right facial neuropathy of unknown origin and House-Brackmann facial nerve grading system (HBFNGS) grade V. For treatment, we initially prescribed oral prednisolone and nimodipine including physical therapy. The samples consisted of 11 facial nerve palsy patients caused by MSSRO and were analysed about onset of facial nerve palsy, postoperative HBFNGS, final HBFNGS, treatment method and recovery time. At 10 weeks of treatment of nimodipine, she had completely regained normal function (HBFNGS grade I) of the right facial nerve. The clinical results lead to assume a fast recovery of facial nerve function by the nimodipine medication, whereas average time of recovery is 16.32 weeks in references. Despite of the limited one patient treated, the result was very promising with respect to a faster recovery of the facial nerve function. Considering the use of nimodipine treatment for peripheral facial nerve palsy following a surgical approach with an anatomically preserved nerve can be recommended.

하악에 발생된 법랑모세포 암종 (Ameloblastic carcinoma of the mandible)

  • The ameloblastic carcinoma is an extremely rare, aggressive odontogenic neoplasm of the jaws. It is described as an ameloblastoma in which there is histologic evidence of malignancy in the primary or recurrent tumors, regardless of whether it has metastasized. We report an aggressive case of ameloblastic carcinoma of the mandible. A 68-year-old man with the complaint of the left facial swelling and intermittent pain was referred to our hospital. Serial images of panoramic radiograph, computed tomograph, and magnetic resonance imaging showed an ill-defined destructive radiolucent lesion of the left mandible. The lesion had typically aggressive behavior with extensive local destruction of bone and extended to the adjacent soft tissues. Bone scan revealed increased uptakes in the left mandibular body and ramus regions. Histological features were generally resembled with those of an ameloblastoma but with cytologic features of epithelial malignancy.

편측성순구개열자의 하악골성장에 관한 연구 (A STUDY ON THE MANDIBULAR GROWTH IN SURGICALLY REPAIRED UNILATERAL CLEFT LIP AND PALATE)

  • This study was undertaken to analyze the growth of mandible in surgically repaired unilateral cleft lip and palate. The subjects consisted of 63 unilateral cleft lip and palate individuals, 60 class III malocclusions and 60 normal occlusions ranging from 6 to 15 years old. Each group was divided into two age groups. (6-10 Y and 11-15 Y) The results obtained from UCLP compared with other groups were as follows: 1. The anteroposterior position of the chin was similar to that of the normal occlusions. 2. The shape of the mandible was similar to that of the class III malocclusions. 3. In mandibular size, ramus height was the smallest among three groups, but body length and overall mandibular length were similar to those of the normal occlusions. 4. The lower border of the mandible was the steepest among three groups and strong vertical or clockwise growth tendency was indicated. 5. The position of condyle in relation to the cranial base showed little difference in three groups. 6. In older age group , vertical growth tendency of the mandible decreased more or less.

저작근 공간을 침범한 하악골내 신경섬유종 (Intraosseous neurofibroma of the mandible involving masticator space)

  • Neurofibroma is a benign, heterogenous peripheral nerve sheath tumor arising from the connective tissue of peripheral nerve sheaths, especially the endoneurium. Its intraoral occurrence is uncommon and its occurrence within mandible is extremely rare. A case of solitary intraosseous neurofibroma of the mandible involving masticator space in a 8-year-old male is reported. He was referred from a private local clinic with a chief complaint of limitation in opening of the mouth. Panoramic and cone-beam computed tomographic images showed unilocular radiolucent lesion with scalloped border at the right mandibular ramus, connected posteriorly to the enlarged mandibular foramen and anteriorly to the mandibular canal. T1-weighted magnetic resonance images showed soft tissue mass of isointensity compared with muscles. Contrast-enhanced T1-weighted images showed peripheral enhancement and T2-weighted images showed the heterogeneous hyperintense mass with extension between lateral and medial pterygoid muscles. The tumor was surgically removed under general anesthesia and diagnosed to be neurofibroma at the biopsy.
