Objective : This study reviews the articles about 'Side effect' published in the Japanese Journal of Oriental Medicine to better understand about side effects of herbal medication and to encourage clinicians, the authorities and the public to establish side effects reporting system. Method : We searched articles published in the Japanese Journal of Oriental Medicine by using keyword '副作用(Side effect)' in the CiNii. Results : Among the 118 articles collected, 14 are clinical articles about side effect of prescriptions, 13 are reports about side effect of prescriptions presented from Japanese Ministry of Health, Labour and Welfare, 36 are clinical articles about relief of side effects derived from taking western medicine by taking a herbal prescription, 8 are studies about the effect of a herbal prescription combined with western medicine or comparative studies between a herbal prescription and western medicine, and so on. Conclusions : Lots of articles deal with side effects of Aconiti Lateralis Radix Preparata, Bupleuri Radix and so on. When side effects occurred, removing a certain herbal drug or quitting a herbal prescription is helpful to relieve or disappear the side effects. And Side effects reporting system should be established to guide safe medication use and treatment for patients. In order to establish side effects reporting system, the standardization of herbal drugs is needed.
Objective: This study reviewed recent clinical research trends regarding the effects of herbal medicine treatment containing diuretic herbs on nonalcoholic fatty liver disease (NAFLD) in China. Methods: We reviewed clinical studies on NAFLD from the China Academic Journal of the China National Knowledge Infrastructure from January 1, 2016, to September 10, 2018. Results: Thirty-nine papers from 86 studies were reviewed. The most commonly used herbal prescriptions included Alismatis Rhizoma(澤瀉), Salviae miltiorrhizae Radix (丹蔘), Crataegii Fructus (山査), Polia Sclerotium (茯?), Bupleuri Radix (柴胡), and Cassiae Semen (決明子). The treatment effective rate, liver function test, serum lipid test, radiological examination, and symptom comparisons were used to analyze the treatments. In 32 of the papers, the effective treatment rate was 76.9%-100%, and the effectiveness in the experimental group was significantly higher than that in the control group (p<0.05). The risk ratio for the treatment effective rate was 1.26. The mean difference of ALT was -13.44 U/L (95% confidential interval, -24.45 U/L, -2.44 U/L, p=0.0166). Conclusion: Herbal medicine containing diuretic herbs has been demonstrated to be an effective and useful treatment for NAFLD. Clinical studies that are more scientific and systematic should be actively conducted in the future, and the results of the current study could be used as basic data in future clinical studies on herbal medicine treatment for NAFLD.
Objectives: To examine the effect of complex Korean medical treatment on tic disorder in children who visited the neuropsychiatric clinic of Korean medicine. Methods: Medical records of 35 patients diagnosed with tic disorder based on DSM-V who received complex Korean medical treatment (herbal medicine, acupuncture and Korean psychotherapy) were reviewed. Tic disorder was then evaluated by Yale Global Tic Severity Scale (YGTSS) before and at 4, 8, and 12 weeks after treatment to determine whether their symptoms were improved. Results: 1) After the treatment, mean YGTSS was reduced from 35.54±14.77 to 23.20±12.65. There were statistically significant changes between scores according to the time of treatment (p< 0.05). 2) At the time of visit, symptoms of motor tics were in the order of eyes, neck and shoulders, mouth, head and arms & hands, nose, face, upper body and other parts, and legs and symptoms of vocal tics were in the order of sniffing, dry coughing and MMM, AA and whistling, and other sounds. 3) For 35 patients, herbal medicine was prescribed every two weeks. The frequency of prescriptions used was in the order of Shihogayonggolmoryo-tang, Gami-ondam-tang, and others. 4) The frequency of herbs used in 35 patients was in the order of Poria, Zingiberis Rhizoma Recens, Pinelliae Rhizoma and Scutellariae Radix, Bupleuri Radix, Ostreae Concha and Bovis Ossis Fossilia, and others. Conclusions: Complex Korean medical treatment including herbal medicine, acupuncture, and Korean psychotherapy is effective for reducing tic symptoms in children.

The purpose of this study was to review the research on treating depression in Traditional Chinese Medicine(TCM) over the last 5 years to set the foundation for further studies. We searched for and analyzed articles about depression in CNKI(China National Knowledge Infrastructure) from January 2010 to December 2014. The results were as follows : The most frequently used diagnostic criteria was CCMD-3(The Chinese Classification of Mental Disorders-3), and the most used evaluation criteria was HAMD(Hamilton Depression Rating Scale). Prescription of decoction of medical herbs was most frequently used as a therapeutic method. Acupuncture, traditional Chinese psychotherapy, and music therapy were also used for some studies. The most frequent TCM Syndrome Differentiation Type was stagnation of liver-QI. For decoction of Chinese herbs, Soyo-san(Xiaoyao-san) and Sihosogan-san(Chaihushugan-san) were most often prescribed, and Bupleuri Radix, Paeoniae Radix Alba, Curcumae Radix, Poria cocos wolf, Angelicae Gigantis Radix, Atractylodis Rhizoma Alba were mainly used for medical herbs. BaekHoi(GV20) and Yindang(Ex-HN3) were often used as acupuncture sites. Post-Stroke Depression was the most common case of intercurrent or secondary depression. According to the Jadad Quality Assessment Scale, the quality of the reports was not high as most of the reports had a score of 3 or below. Most systematic reviews on depression were conducted by Chinese researchers. The problem with Clinical research on depression, according to those reviews, was that there were no standardized criteria for the diagnosis and treatment and the trials were usually not randomized nor controlled. We found out there are various clinical methods for treating depression in TCM, and hope that this research could provide the preliminary data for designing and conducting clinical trials for depression.

The ${\gamma}$-irradiated medicinal herbs were examined the genotoxicological safety to consider the possibility of application of the irradiation technology for hygienic purpose. The three medicinal herbs -Glycyrrhigae Radix, Aurantii nobilis Pericarpium and Bupleuri Radix- were irradiated with ${\gamma}$ -rays at the practical dosage of 10 kGy. The hot water extracts of the irradiated herbs were examined in two short-term in vitro tests: (1) Ames test in Salmonella typhimurium TA98 and TA100, (2) Micronucleus test in cultured Chinese hamster ovary(CHO) cells. In the Salmonella reversion assays both with and without metabolic activation, the number of revertant colonies was not increased with each extract from the irradiated herbs, compared with negative controls. No significant difference in formation of the colonies was observed between non-irradiated and 10 kGy-irradiated herbs. These results indicated that no mutagenicity of the irradiated herbs was detected. In the micronucleus tests in cultured CHO cells, the incidences of micronucleus were not increased with irradiated herbs, and no significant difference in the incidences was observed between non-irradiated and irradiated herbs. These results indicated that no cytogenetic toxicity of the irradiated herbs was detected. The results of the two in vitro tests suggest that the irradiated herbs do not show mutagenic effects and cytogenetic toxicity. Further tests of in vivo genotoxicity and chronic toxicity are needed to determine the safety of the herbs irradiated with ${\gamma}$ -rays at practical doses.

Objectives : The purpose of this study is to investigate the trend of Tranditional Chinese Medicine for Otitis Media with Effusion(OME) in Chinese journals. Methods : Chinese National Knowledge Infrastructure(CNKI) and Wanfang med online were used to search Chinese studies which were published from January, 2010 to April, 2020. Results : Among Chinese studies, Exterior-releasing medicinal(解表藥) and Heat-clearing medicinal(淸熱藥) were the most frequently used. The herbs which used the most frequently are Bupleuri Radix(柴胡), Acori Graminei Rhizoma(石菖蒲). All of studies have reported that Tranditional Chinese Medicine is effective for Otitis Media with Effusion. Conclusions : In analysis of selected studies, Tranditional Chinese Medicine is more effective than Western Medicine Treatments. Recurrence rates and side effects of OME can be reduced by cotreatment of Tranditional Chinese Medicine and Western Medicine Treatments.
From crude drug preparation(Soshiho-Tang) ginseng sapogenins were identified by TLC and $ginsenoside-Rb_1$ was determined quantitatively by HPLC. Panaxadiol, pandaxatriol, acid-hydrolysates of ginseng saponin, were identified by TLC with benzene/acetone(4 : 1, v/v). Rf values of which were measured as 0.26 and 0.14, respectively. The content of $ginsenoside-Rb_1$ was determined by HPLC on $Lichrosorb-NH_2$ column with $CH_3CN/H_2O/n-BuOH$(80 : 20 : 10, v/v). Its recovery rate in the extract granules, was as relatively low as $19.8{\pm}1.4%$ compared to the content in raw red ginseng.
Objective This study identified useful herbal medicines and prescriptions for obesity treatment by analyzing herbal medicines and prescriptions that showed meaningful results in weight loss by changing the gut microbiota. Methods Using PubMed, we selected and studied 23 papers showing meaningful results on weight loss through changes in the gut microbiota using herbal medicines. Result Of the 23 selected papers, 17 were of studies using herbal medicines, and 6 of studies using prescriptions. Pomegranate peel, Flos Lonicera, Rehmannia glutinosa, Rhein, Coix seed, Platycodon grandiflorus, mulberry leaves, Poria cocos, asperuloside, Bupleuri radix, Astragalus polysaccharides, Ephedra sinica, Ganoderma lucidum, Hirsutella sinensis, Caulis spatholobi, aconite, and Bletilla striata were used as herbal medicine. Linggui zhugan-tang, Bofutsushosan, Shenling baizhu powder, Chowiseungcheng-tang, Daesiho-tang, and Yijin-tang were used as prescription. Conclusion Seventeen herbal medicines and six prescriptions associated with meaningful results in weight loss through changes in the gut microbiota, suggest the possibility of treatment and prevention of obesity through herbal medicine.
This study was carried out to find out favourable soil conditions to give good quality of Bupleuri Radix for Korean medicine. The major ingredients were saikosaponin a and c, which were determined by HPLC. The contents of saikosaponin a and c in the root were determined to be 0.141-0.479% and 0.042-0.094%, respectively. The content of saikosaponin a was negatively correlated with the total nitrogen contents in surface soils, but that of saikosaponin c was independent of physico-chemical properties of the soil. The contents of saikosaponin a and c were decreased in the root with high Ca and Fe.
This review deals briefly with the various medicinal components(mainly saikosaponins), their biological activities and the variation of their contents by different cultivation environment and plant parts in Bupleurum species. Bupleuri radix, a crude drug, is the root of Bupleurum falcatum L. (Korea, Japan), B. chinense(China), and their related species (Umbelliferae). There are over 120 species in Bupleurum genus throughout world, mainly Asian area, and over 5 species in Korea, investigated up to now. These plants contain many physiological active compounds and the principal components are saikosaponins. Major activities of this crude drug and saikosaponins are the anti-inflammatory and antihepatotoxic activities. Saikosaponins and their derivatives in Bupleurum spp. have been chemically studied, isolated and identified over 70 compounds in over 50 species. Other components, physiologically active ones, also have been investigated, which are the groups of lignan, flavonoid, essential oil, polyacetylene, polysaccharide, etc. Saikosaponins belong to the group of triterpenoid saponin chemotaxonomically and occur the accumulation and turnover in plant tissues through secondary metabolism, mevalonic acid pathway. The contents and kinds of saikosaponins and other components in Bupleurum spp. plants are various due to different species and growing environments, as the plant growth characters and yield are various. Most of medicinal plants as well as Bupleurum species are very useful as agricultural products and traditional medicines, and also are very valuable as genetic resources and natural products. So we need to collect, evaluate, preserve, and utilize various medicinal plants, and also to under-stand secondary metabolism and improve the breeding and cultivation techniques for the safe production of crude drugs with high quality and yielding.
