A study of distribution, prevalence and relationship of the localized periodontitis of first and second molar root fusion (제 1, 2 대구치 치근융합의 발생빈도와 분포 및 국소적 치주염과의 관련성 조사)

  • The purpose of this study was to determine the distribution, prevalence and relationship of the localized periodontitis of root fusion in maxillary and mandibular molars. One hundred patients who had eight maxillary and mandibular molars(third molars excluded) were consecutively selected for the study subjects. The subjects provided a total of 800 molars, i. e., 400 maxillary and 400 mandibular molars. A decision about root fusion was made on the radiographic examination. Probing depth, plaque index, gingival index and mobility were measured. The results were as follows. 1. 15.9% of the molars had a fused root. 22% of the maxillary molars and 9.8% of the mandibular molars had a fused molars. 2. In maxillary molars, the results of probing depth, plaque index, gingival index, mobility are more higher in test group than in control group, and there was a significantly difference except plaque index of maxillary first molars group(p<0.01). 3. In mandibular molars, the results of probing depth, plaque index, gingival index, mobility are more higher in test group than in control group, and there was a significantly difference(p<0.01). As a result of this study, it can be concluded that, in management of molars with a root fusion, we should detect the molars through the precise radiographic examination, early periodontal treatment and systematic treatment plan should be chosen. And postoperative continuing supportive periodontal therapy is needed.

COMPARISON OF CANAL FILLING EFFICIENCY ON THE INTERNALLY RESORBED ROOT ACCORDING TO CANAL FILLING TEHNIQUES (근관충전방법에 따른 내흡수 치근의 근관충전 효율성 비교)

  • Canals with artificially made internal resorption were filled with 4 techniques(Lateral condensation, Ultrafil, Obtura II, Thermafil) to compare the efficacy of canal filling according to the filling techniques. After canal filling, radiographic examination, dye penetration through the apical portion and percentage of G-P filled area on the internal resorption area were evaluated. To examine the degree of crystal-like structure and the interface between filled G-P and canal wall, SM and SEM images were taken too. The results were as follows : 1. There was no statistically significant difference in apical microleakage among the 4 root canal filling techniques. 2. As a result of radiographic examination, Ultrafil was the best and Obtura II was acceptable but Lateral condensation and Thermafil showed unfavorable canal filling pattern similarly. 3. Ultrafil filled most of artificially made internal resorption area and Obtura II, Lateral condensation, Thermafil in that order filled unfavorably. 4. Degree of crystal like structure was the highest in the group filled with Ultrafil and those of Obtura II and Thermafil were similar and that of gutta percha used in Lateral condensation showed the lowest value. 5. Penetration of gutta percha into the dentinal tubules couldn't be seen in all groups. In the contact surface between the filled G-P and the canal wall, Lateral condensation showed relatively close sealing, Obtura II and Thermafil had irregular contact surface and Ultrafil showed regular filling pattern. 6. Contact surface between the core of Thermafil and the gutta percha showed close relationship without gap formation.

Clinical Characteristics of Trauma-Related Chronic Osteomyelitis in 3 Wild Raccoon Dogs (Nyctereutes procyonoides)

  • Osteomyelitis typically occurs because of the direct inoculation of bacteria or fungi after penetrating trauma or surgical contamination or, by extension, from soft tissue infection. Osteomyelitis is rarely reported in wildlife animals, though severe chronic osteomyelitis cases do exist in wildlife owing to the scarcity of medical support in the wild environment. This report describes three cases of chronic osteomyelitis in wild raccoon dogs related to trauma. The typical symptoms of three reported cases were ataxia, stiffness, muscle atrophy, and lethargy. All three cases were relevant to traumatic or severe external injury, and skin infestation caused by ectoparasites was apparent on an ocular inspection. In the radiographic examination, diffuse sites of osteolytic lesions and remarkable periosteal responses were demonstrated around the injured limb in all three cases. Apparent neutrophilia with a left shift, lymphocytosis, and monocytosis in hematological examinations generally indicated chronic infection as shown in case 1 and 3. Treatment was attempted with broad-spectrum antibiotics and non-steroidal anti-inflammatory drugs, such as amoxicillin/clavulanic acid, enrofloxacin, clindamycin, and meloxicam. These treatment options helped improve the overall prognosis of chronic osteomyelitis, but the outcomes did not meet the treatment goal entirely. Osteomyelitis can be extremely challenging to treat, particularly in wild animals, because of their distinctive traits, such as masking phenomenon and uncontrolled exposure to ectoparasites. Earlier diagnosis with a radiographic examination, hematological examinations, and careful patient monitoring, followed by prolonged antibiotic therapy and restricted exercise, are the key factors leading to a better prognosis.

A Study of Radiation Dose Evaluation and Optimization Methods for Intra Oral Dental X-ray in Pediatric Patient (소아 구내촬영 시 방사선량 평가 및 최적화 방안에 대한 연구)

  • Although intra oral dental x-ray is a lower dose than other radiological examinations, pediatric patients are known to have a higher risk of radiation damage than adults. For this reason, pediatric dental x-ray requires management of dose evaluation and imaging conditions during the examination. In this study, the dose calculation program ALARA-Dental(child/adult) was used to evaluate the organ dose and effective dose exposed to each examination site during intra oral imaging of children during dental radiographic examination, and dose analysis according to the imaging conditions was performed. As a result, the highest organ dose distribution was shown at 0.044 ~ 0.097 mGy in all are as of the mucous membrane of oral cavity except for the maxillary incisors and canines. Also, in the case of the thyroid gland, the maxillary canine and maxillary premolar examination showed 0.027 and 0.020 mGy, respectively, and the dose distribution was 15.4% to 70.0% higher than that of the mandibular examination. As for the effective dose calculated during intra oral imaging, the maxillary anterior and canine examinations showed the highest effective doses of 0.005 and 0.004 mSv, respectively, and the maxillary area examination showed a higher dose distribution on average than the mandible.

A CASE REPORT OF EWING′S SARCOMA OF THE MANDIBLE (하악에 발생된 Ewing 육종의 1증례)

  • The authors observed a 27-year-old male patient who came to the Infirmary of Kyungpook National University Hospital who had complained of dull pain in right mandibular angle area 1 month ago. As a result of careful analysis of clinical, radiological, and histopathological findings, the authors diagnosed it as Ewing's sarcoma and obtained the results as follows: 1. In clinical examination, main clinical symptoms were continuous dull pain and gingival swelling on the 2nd and 3rd molar area of the right mandible. 2. In radiographic examination, ill-defined radiolucent area was seen on mandibular right angle area, and apparent periosteal reaction of sun-ray spicule was emanated from the lingual cortex of mandibular angle area. And computed tomograph also revealed moderate-defined 3. soft tissue mass of the same area. In histopathological examination, small round-shaped, and closely packed cells with scanty cytoplasm were observed in this specimen, and positive reaction in PAS staining was also observed.

Ultrasonographic Evaluation of Surgically Induced Cranial Cruciate Ligament Rupture in the Dog (개에서 전십자 인대 단열의 초음파적 평가)

  • Ultrasonographic examination of the stifle joint four clinically and radiographically normal adult dogs was performed before and after inducing the cranial cruciate ligament (CrCL) rupture of the bilateral stifle surgically. At Pre- or post-surgery, the hyperechoic patella ligament and the echogenic infrapatella fat interfered the visualization of the CrCL by the plain scanning method. However when the stifle joint was imaged by the scanning method injecting normal saline within the joint capsule, the hyperechogenic ligament was visualized by the separation of the infrapatella fat and the CrCL and the contrast effect of anechoic saline. When the stifle joint was imaged by the real time scanning method after the induction of the surgical rupture, fluttering of the ligament and anechoic area between the bone and the CrCL was identified due to the lack of tonicity and continuity of the ligament. Enlargement of the ligament because of its increased diameter as well as fluttering of the CrCL and the increased joint space's thickness was visualized. Consequently, confirming fluttering of the CrCL, increased ligament diameter and anechoic region between the CrCL and the bone, ultrasonographic examination is a valuable diagnostic tool as well as cranial drawer test and radiographic examination.

Spontaneous Femoral Capital Physeal Fracture with Proximal Tibial Epiphyseal Dysplasia in a Cat

  • An 11-month-old, 5.6 kg, grade 4 on a 5-point scale body condition score, castrated male Korean short hair cat was presented with right hindlimb lameness for 3 days without trauma. On physical examination, severe pain with crepitation was elicited at the hip region but not at the stifle. Radiographic examination and computed tomography revealed a Salter-Harris type 1 fracture of the femoral capital physis and dysplasia of the proximal tibial epiphysis. Spontaneous femoral capital physeal fracture was suspected and femoral head and neck osteotomy was performed. Treatment for proximal tibial dysplasia was not performed. On histopathologic examination of femoral head, a cluster of chondrocytes in a proliferative zone at the epiphyseal plate was observed. Therefore, spontaneous femoral capital physeal fracture was diagnosed. The patient recovered his gait, and no related clinical signs were observed during 6 months of follow-up.

  • Carbon materials are widely used in many areas of our lives. A fiber having a carbon content of 90% or more obtained by heating an organic fiber precursor is referred to as a "carbon fiber". Carbon fibers are currently used in the medical market to manufacture radiation transmission device parts, artificial joints, and medical aids, as many developments have been made to utilize carbon fibers' characteristics such as light weight, radiation permeability, biocompatibility, high strength, high heat resistance, thermal conductivity, and electrical conductivity. In order to maintain body temperature and increase immunity in long-lasting nuclear medical examination and treatment through the idea of convergence of carbon materials and radiation technology, the quality of medical services can be improved by utilizing carbon materials. We should be aware of the domestic carbon-based medical device industry and make efforts to contribute to the development of medical devices. As a radiation expert, we should try to use our skills and experience to find items that can be fused with medical devices to develop various nuclear medical examination fields and radiographic examination fields that can be widely applied. We should actively engage in future technology development and carbon material research to strengthen the global competitiveness of the domestic medical device industry and improve the quality of medical services.

Computed Tomographic Features of Gallbladder Agenesis in a Golden Retriever

  • A two-year-old, intact female Golden Retriever had previously been diagnosed with a portosystemic shunt (PSS) during an ultrasonographic examination at a local animal hospital. The serum biochemistry revealed elevated liver enzymes and bile acid levels. The abdominal radiographic examination revealed mild serosal detail loss and microhepatica, while abdominal ultrasonography revealed mild ascites and high-velocity flow to the caudal vena cava (CVC) suspected as a PSS. The gallbladder was not observed within the hepatic parenchyma during ultrasonography. Computed tomography (CT) revealed an absent gallbladder and dilation of the common bile duct (CBD). Dilations of the gastroduodenal, splenic, colic and renal veins were also observed. A dilated left phrenico-abdominal vein that entered the CVC was previously misinterpreted as a PSS on the ultrasound examination. Based on the imaging examinations, the dog was diagnosed with congenital gallbladder agenesis associated CBD dilation.

Correlation of the Ultrasonography with the Physical Examinations and Simple Radiography in Measurement of Amount of Effusion of the Knee Joint (슬관절 활액 양 측정에서의 신체검사와 단순 방사선 검사, 초음파의 상관성)

  • Purpose: The goal of this study is to find out how useful ultrasonography is in measuring amount of effusion within the knee joint in comparison with physical examination and simple radiography. Materials and Methods: We evaluated total 60 cases which were suspicious for joint effusion. Average age was 61.3 years. The group was consisted of 22 males and 38 females, and 24 obese people and 36 non-obese people. Stroke test and patella floating test were done as physical examination. Amount of joint effusion was classified into 3 groups in simple lateral radiographic view and ultrasonography was used for recheck-up. Results: 39 cases were found positive in stroke test, and patella floating was shown in 20 cases. In simple lateral radiographic view, 47 cases of grade I, 7 cases of grade II and 3 cases of grade III were checked. Average $7.67{\pm}2.90mm$ amount of effusion was checked by ultrasonography. Amount of fluid in stroke test, patella floating test and simple lateral radiographic view showed close correlation with ultrasonography. Gender did not seem to affect physical examination, but strong correlation was found in female in simple radiography. Obese group showed closer relationship with all 3 exams compared with non-obese group. Physical examination was relatively effective with nonobese group, but not with obese group. And it was not easy to measure the amount of fluid with simple radiographic view, but we could obtain objective data from ultrasonography by measuring the amount of effusion dynamically. Conclusion: We concluded that more objective data was obtained with ultrasonography in comparison with physical examinations and simple radiography in the aspect of measuring amount of joint fluid.
