  • Hallux valgus is a complex deformity which contains static subluxation of the first metatarsophalangeal joint with lateral deviation of the great toe and medial deviation of the first metatarsal bone. Among the several parameters, radiographic indices are the important ones in the choice of the treatment method. Even though, the normal values of the western peoples are already estabilished, there was no standard values for Korean who has different genetics, and cultural background, till now. So, we tried to estimate the noraml values of the HVA (hallux valgus angle ), IMA ( Intermetatarsal angle) and DMAA (Distal Metatarsal Articular surface Angle) in a normal adult. Fifty normal adult (each 25 male and female) were chosen and took 100 cases of foot radiograph and radiologic indices of the hallux valgus were measured. Average age at the investigation was 34.5 years old. The average HVA is $13.3^{\circ}$, IMA is $8.5^{\circ}$ and DMAA is $34.6^{\circ}$.

  • The purpose of this article was to determine the amount of cranio-facial asymmetry in normal subject before the analysis of the cranio-facial asymmetry as the result of internal derangement in T.M.J. dysfunction. The author has conducted analysis using Cephalometric P-A reviews of 54 males and 51 females. Following the Grayson's method of measurement, the standard value of cranio-facial asymmetry in normal subject was obtained. The following results were obtained: 1. Compared with right and left width, asymmetry could be identified in normal subject, although the degree of the difference appears to be small. 2. In male, asymmetric value of contact point of the upper central incisors is 0.76±0.84㎜ that of the lower central incisors is 0.86±0.86㎜, and that of center of genial tubercle is 0.87±1.06㎜. In female, asymmetric value of contact point of the upper central incisors is 1.03±1.29㎜, that of lower incisors is 1.11 ±1.18㎜, and that of center of genial tubercle is 1.45±2.15㎜. 3. Cranio-facial saymmetry in female is somewhat greater than that of male.

  • Objectives : We investigated the possibility of cervical spine misalignment caused by scalenus anticus syndrome to find out how it affects cervical spine misalignments. Methods : 28 patients with scalenus anticus syndrome (sample group), along with 21 participants without neck pain (control group), who attended the Dept. of Oriental Rehabilitation Medicine, College of Oriental Medicine, Dong-guk University from the 20th of June to the 30th of November, 2006 were investigated. After researching misalignments through neutrality, flexion and extension lateral x-ray examination views, we measured the difference of each length of cervical spine misalignment. We analyzed the relationships among the neutrality lateral, flexion lateral and extension lateral positions. Results : We found with statistical significance that there were differences in length of cervical spine misalignments between the sample and control groups. Furthermore, we found that C3 and C4 vertebra bodies were shown in lateral neutrality position, only C3 vertebra body in flexion lateral position, and C2, C3, and C4 vertebra bodies in extension lateral position. Conclusions : It is considered that scalenus anticus syndrome could increase cervical spine misalignment which could be a factor in causing cervical spine disease.

  • Regular radiographic examination has been considered as an essential diagnostic method for the osseointegrated dental implantation. This study investigated the marginal bone loss through the measurement on the periapical radiograph around 46 endosseous dental implants. Marginal bone loss have been observed for 5 years after delivery of prostheses. The results were as follows : 1. According to the experimental periods, average marginal bone loss in total implant was 1.821mm in the first year, 1.833mm in the second, 1.892mm at third, 1.910mm at forth and 1.957mm at fifth after delivery of prostheses(P<0.05). 2. The average bone loss was 1.832mm in the maxillae and 1.819mm in the mandible in three years but there was no significant difference between the upper and lower jaw(P>0.05). 3. The average bone loss was 1.824mm in males and 1.818mm in females in five years but there was no significant difference according to the sex(P>0.05). These results indicate that the most of the alveolar bone loss occur within the first year after delivery of dental prostheses resulting in the exposure of polished neck positions, and the bone level stabilizes at first thread portion of the implant fixtures.

  • The purpose of this study are to determine the proper radiographic measurements for hip deformity in spastic cerebral palsy patients, and the correlation of hip deformity with neurological involvement, ability of ambulation. Thirty children with cerebral palsy(22 males, 8 females) were evaluated by measurement of the migration index, acetabular index, center edge angle from bilateral hip APs(anterior posterior view). The result are as follows; 1. The incidence of hip dislocation, among the thirty children fifteen children were found to be dislocated, and more significantly high in non-walking group than in walking group(p<0.05) and in quadriplegia than in diplegia(p<0.05). 2. When compared to normal hip and dislocation hip, the migration index was significantly higher(p<0.01) and the center edge angle was lower in the dislocation hip than in the normal hip. 3. Correlation of radiological findings in right and left hip, the migration index and the center edge angle were highly correlated(p<0.01). We recommand regular intervals X-ray study for early diagnosis and management of hip dislocation in spastic cerebral palsy.

  • Purpose: Pain caused by osteoarthritis is primarily nociceptive pain; however, it is considered that a component of this pain is due to neuropathic pain (NP). We investigated the effects of total hip arthroplasty (THA) in patients with NP diagnosed by the PainDETECT questionnaire. Materials and Methods: One hundred sixty-three hips (161 patients) were evaluated. All patients were asked to complete the PainDETECT questionnaire based on their experience with NP, and clinical scores were evaluated using the Japanese Orthopaedic Association (JOA) Hip Score before and after THA. Results: The patients of 24.5% reported NP before THA; 5.5% reported NP 2 months after THA. Prior to THA, there was no significant correlation between the PainDETECT score and the radiographic severity; however, there was a significant correlation between the PainDETECT score and JOA score. NP at 2 months after THA was not significantly correlated with pain scores at 1 week after THA; however, a significant correlation was observed between the preoperative pain score and NP at 2 months after THA. Conclusion: THA was useful for relieving nociceptive pain and for relieving NP in patients with hip osteoarthritis. Preoperative pain was a risk factor for NP after THA. Controlling preoperative pain may be effective for reducing postoperative NP.

  • PURPOSE: This study was conducted to compare different standing postures with the use of standing aids for lumbar spine posture and muscle activity, and to identify the most desirable standing posture. METHODS: The lumbopelvic angle was assessed based on static radiographic measurement on the sagittal plane. Lumbar lordosis, lumbosacral lordosis, and the intervertebral joint angle at L1/L5 and L5/S1 were measured using radiography in three standing postures (standing on level ground, standing with one foot on a platform, and standing on a sloped surface). In addition, muscle activity was measured using surface electromyography to examine the co-contraction of the lumbar and hip muscles. RESULTS: Lumbar lordosis, lumbosacral lordosis, and L5/S1 intervertebral joint flexion occurred with one foot on the platform. No significant differences were found between standing on a sloped surface and standing on level ground. However, muscle co-contraction was reduced with the use of standing aids. CONCLUSION: This study demonstrated that standing with a foot on a platform induced lumbar lordosis, but that there was no significant difference between standing on a sloped surface and standing on level ground. However, muscle co-contraction was reduced with the use of standing aids. Based on the motor control pattern as a predictor of LBP, the use of standing aids would help workers during prolonged standing.

  • Purpose. This intra-patient retrospective study of up to 10 years evaluated the clinical success and risk factors of 6- and 8-mm long implants and their respective prostheses. Materials and Methods. The sample consisted of patients treated at a Military Polyclinic dental service, who received both 6- and 8-mm long tissue level implants in the posterior region of the same arch. Data were collected from the dental charts, clinical and radiographic exams, self-report of sleep bruxism, measurement of maximum occlusal force, and clinical crown-to-implant (C/I) ratio. Data were analyzed by descriptive and inferential statistics with univariate and hierarchical multivariate models, at the 0.05 significance level. Results. The 30 patients (27 women) had 85 implants and 83 prostheses. Two implants were lost before prosthesis installation (implant survival: 97.6%). Ten events of prosthetic complication (screw tightening loss) occurred in five patients (success rate: 87.9%) in a single moment. Only the variable C/I ratio had a significant effect for repairable prosthesis complication (P<.05). Conclusion. The results suggest that 6- and 8-mm long implants have similar long-term clinical success for implants and prostheses.

  • The Law method is observing the temporal bone. There are two types of methods: the double angle method, which manipulates the center ray angle of the tube twice, and the single angle method, which manipulates once. The purpose is to increase the reproducibility of the image by making vertical incidence by making an assistive device using a 3D printer. Two assistive devices with a wedge-shaped 8.5 × 10 × 2.3 cm, an inclined surface of 7.5 cm, and an inclination angle of 15° were fabricated. Assistive devices can be combined with each other in the form of grooves, and PLA (Poly Lactic Acid) is used as a material. In the first experiment, 10 examiners operated the tube 15° in the caudad direction and 15° in the anterior direction, and measured it with a protractor to conduct a reproducibility experiment. Second, two examiners acquired vertically incidence images using the existing law method and assistive devices, and measured the distance between each measurement point to evaluate the reproducibility. The tube center ray angle reproducibility experiment was not statistically significant, but the angle difference was up to 9° between examiners. The reproducibility experiment of radiographic images was not statistically significant with the conventional method, and the method using an assistive device was statistically significant. Therefore, regardless of skill level, an image capable of securing reproducibility, which is the advantage of vertical incidence, could be obtained.

  • Purpose: The goal of this study is to find out how useful ultrasonography is in measuring amount of effusion within the knee joint in comparison with physical examination and simple radiography. Materials and Methods: We evaluated total 60 cases which were suspicious for joint effusion. Average age was 61.3 years. The group was consisted of 22 males and 38 females, and 24 obese people and 36 non-obese people. Stroke test and patella floating test were done as physical examination. Amount of joint effusion was classified into 3 groups in simple lateral radiographic view and ultrasonography was used for recheck-up. Results: 39 cases were found positive in stroke test, and patella floating was shown in 20 cases. In simple lateral radiographic view, 47 cases of grade I, 7 cases of grade II and 3 cases of grade III were checked. Average $7.67{\pm}2.90mm$ amount of effusion was checked by ultrasonography. Amount of fluid in stroke test, patella floating test and simple lateral radiographic view showed close correlation with ultrasonography. Gender did not seem to affect physical examination, but strong correlation was found in female in simple radiography. Obese group showed closer relationship with all 3 exams compared with non-obese group. Physical examination was relatively effective with nonobese group, but not with obese group. And it was not easy to measure the amount of fluid with simple radiographic view, but we could obtain objective data from ultrasonography by measuring the amount of effusion dynamically. Conclusion: We concluded that more objective data was obtained with ultrasonography in comparison with physical examinations and simple radiography in the aspect of measuring amount of joint fluid.
