Effect of early stabilization exercise focused on the scapulothoracic joint on the recovery of surgical patients due to rotator cuff tear

  • Purpose: This study examined the effects of early stabilization exercise focused on the scapulothoracic joint on the recovery of surgical patients due to rotator cuff tear. Methods: The subjects were 30 patients divided randomly into two groups. Group I consisted of 15 patients who practiced shoulder joint stabilization exercises, including glenohumeral joint movement. Group II consisted of 15 patients who practiced scapulothoracic joint stabilization exercises, excluding glenohumeral joint movement. The duration of stabilization exercise was 30 minutes for one day, five days a week, and five weeks. To measure the dependent variables, the Disabilities of the Arm, Shoulder and Hand (DASH) questionnaire, range of motion (ROM), dynamometer of measured grip were used. Results: The DASH, ROM, and grip power were compared. A significant difference was observed before the intervention in each group (p<.05), and there was no significant difference between group I and group II (p>.05). Conclusion: Both shoulder joint and scapulothoracic joint stabilization exercises were effective after the intervention compared to preintervention, but there was no difference between the two groups. Unlike many recent studies on the initiation of stabilization exercises, this study allays the concerns between the advantages of 'early exercise' and oppositions of previous studies about 'early exercise'. Nevertheless, further research regarding these subjects is needed.

Comparison of Scapular Position Between Operation and Non-operation Side to the Rotator Cuff Surgery (회전근개 수술 환자의 수술측과 비수술측 간에 견갑골 자세 비교)

  • Background: The purpose of this study was to compared of scapular position between operation side and non-operation side to the rotator cuff surgery. Methods: This study was carried out with a total 34 patients: male (n=14), female (n=20). Shoulder range of motion (ROM), the quadruple visual analogue scale (QVAS), the shoulder pain and disability index (SPADI), and the scapular index (SI) were used to assess shoulder posture and function. SI was the resting position of the scapular was determined by measuring the distance from the mid-point of the sternal notch (SN) to the medial aspect of the coracoid process (CP) and the horizontal distance from the posterolateral angle of the acromion (PLA) to the thoracic spine (TS) with a soft tape measure. The SI was calculated using the equation: [(SN to CP/PLA to TS) ${\times}$ 100]. Results: There were no significant difference in ROM, QVAS to rotator repair patients according to SI (p>.05). There were significant differences in SI between the operation side and the non-operation side (p<.01). Conclusions: Scapular position was operation side more internal rotation, protraction, abduction than non-operation side. Therefore, health professionals managing for rotator cuff tear repair patients should consider scapular position.

Effects of Modified Cross-body Stretching on Range of Motion in Glenohumeral Joint (수정된 크로스-바디 스트레칭이 오목위팔관절의 관절가동범위에 미치는 영향)

  • PURPOSE: The purpose of this study was to compare the effects of cross-body stretching (CBS) and modified cross-body stretching (MCBS) on the range of motion (ROM) of glenohumeral joint (GHJ) in healthy subjects. METHODS: Thirty subjects were randomly assigned to 1 of 2 groups: CBS group without stabilization of scapula (n=15) and MCBS group with stabilization of scapula (n=15). We measured horizontal adduction and internal rotation ROM of GHJ in pre- and post-intervention. The data were analyzed using the analyses of covariate (ANCOVA) and least significant difference (LSD) post hoc tests (p=.05). RESULTS: In the post-intervention, the MCBS group had a significantly greater increase in horizontal adduction ROM (mean ${\pm}$ SD, $11.46^{\circ}{\pm}0.83^{\circ}$) compared to the CBS group ($7.81^{\circ}{\pm}0.83^{\circ}$) (p=.007) and a greater increase in internal rotation ROM ($62.27^{\circ}{\pm}0.74^{\circ}$) compared to the CBS group ($59.20^{\circ}{\pm}0.74^{\circ}$) (p=.004). CONCLUSION: A single session application of an MCBS provides immediate more improvements in both horizontal adduction and internal rotation ROM than CBS. These results suggest that application of MCBS with stabilization of scapula may be a more useful to gain ROM of GHJ than CBS without stabilization of scapula.

Supraspinatus Tendon Full-Thickness Tear Patient's Pain and Range of Motion Improved by Korean Medicine Treatment with Aconitum ciliare Decaisne Pharmacopuncture: A Case Report (초오 약침시술을 병행한 한의학적 치료로 통증과 가동범위가 호전된 극상근건 전층파열 환자 치험 1예)

  • The objective of this study is to report the improved case of Korean medicine treatment with Aconitum ciliare decaisne pharmacopuncture for supraspinatus tendon full-thickness tear. We used Aconitum ciliare Decaisne pharmacopuncture with other Korean medicine treatments (acupuncture, electroacupuncture, and cupping) for this patient. This case is measured and assessed by the numerical rating scale (NRS), shoulder range of motion (ROM), shoulder pain and disability index (SPADI) and shoulder physical examination. As the result, we found the improvement of NRS, SPADI, ROM, and shoulder physical examination. This study suggested that Korean medicine treatment with Aconitum ciliare Decaisne pharmacopuncture is proved to be helpful to relieve pain and recover function for supraspinatus tendon full-thickness tear. But further clinical studies are needed to clarify the effect of Aconitum ciliare Decaisne pharmacopuncture therapy for patients with supraspinatus tendon full-thickness tear.

Effects of Flexi-bar Exercise on Shoulder Pain, Disfunction, Quality of Life and Range of Motion in Women with Breast Cancer Surgery (유방암 수술을 한 여성에게 Flexi-bar 운동이 견관절 통증, 기능장애, 삶의 질, 관절가동범위에 미치는 영향)

  • Background: The purpose of this study was to investigate the effects of flexi-bar exercise on shoulder pain, dysfunction, quality of life, and range of motion of women with breast cancer surgery. Methods: A total of 42 female patients who had breast cancer were included in this study. By drawing lots, subjects were divided into a flexi-bar exercise group (FBG, n=21) and a general breast cancer group (GBG, n=18). The effects of flexi-bar exercise on patients were evaluated in terms of the visual analogue scale (VAS), the shoulder pain and disability index (SPADI), the European organization for the research and treatment of cancer quality of life questionnaire (EORTC QLQ-C30), the breast cancer specific EORTC quality of life questionnaire (EORTC QLQ-BR23), and range of motion (ROM), all of which were measured pre- and post- intervention followed by evaluation. Results: A significant increase was pain, functional disability level, quality of life, ROM within the two groups (p<.01). There were pain, functional disability level, quality of life, ROM post test then invention were significant between the two groups(p<.01). Conclusion: The flexi-bar exercise has a positive treatment effect on breast cancer patients.

A Case Report on the Effect of Electroacupuncture at $LI_{15}$ and $TE_{14}$ for the Treatment of Shoulder Pain in Post-stroke Hemiplegia Patients (뇌졸중 후 편마비환자에게 발생한 견통에 대한 견우.견료혈 전침치료 증례보고)

  • Objectives : The objective of this case report was to investigate the effect of electroacupuncture for the management of shoulder pain caused by stroke. Methods : Electroacupuncture at $LI_{15}$ and $TE_{14}$ was performed to five patients who complained of shoulder pain after stroke. We have evaluated the efficacy of treatments by measuring the changes of pain rating score (PRS) and range of motion (ROM) of shoulder joint once a week. Results : PRS was significantly decreased after treatment ($p$<0.05). ROM of shoulder was also decreased but there was not statistical significance. Conclusions : These results suggest that electroacupuncture at $LI_{15}$ and $TE_{14}$ will be beneficial for the treatment of shoulder pain caused by stroke.

The effect of taping therapy on shoulder in hemiplegic patients (테이핑이 편마비 환자의 결관절 아탈구에 미치는 효과)

  • The purpose of this case study was to evaluate the taping therapy program designed to improve shoulder joint subluxation in hemiplegic patients. This study was 9 males who had hemiparesis with limitation of range of motion (ROM) and subluxation on affected side. Patients received proprioreptive facilitation using shoulder weight shifting and additional A type and B type taping. As a result of this study, Patients showed reduction of the subluxation on hemiplegic shoulder joint. but it was difficult to generalize. We concluded that proprioceptive facilitation using shoulder weight shifting and taping therapy was effective in reducing severe shoulder subluxation. Following studies must be evaluated its statistical significance.

  • Objective: There are some case reports on treatment of shoulder dislocation in Patients Undergoing Stroke. We report the electronic needle and the Bee venom Acupunure Theraphy about under using shoulder dislocation in patients Undergoing Stroke got a good remedial value. Methods : The patients diagnosed as shoulder dislocation and treated mainly with Bee venom Acupunture Therapy and electronic needle Results & Conclusions : Symptoms of the patientes such as shoulder pain, Range Of Motion(ROM) were improved after above treatments. The interval in X-ray film comes to be narrow. So, it is suggested that oriental medical treatment are effective on post stroke with shoulder dislocation.

The Clinical Study on the Effect of Acupotomy in Shoulder Disease (침도침 시술이 견관절 질환에 미치는 영향에 관한 임상적 고찰)

  • Objectives : The purpose of this study is to report the effect of acupotomy for patients with shoulder disease. Methods : We treated Five patients who have shoulder disease with acupotomy. To check the effect and satisfaction of acupotomy we used visual analog scale(VAS) and Five-point Likert scale. At the same time, we measured Change of shoulder range of motion(ROM). Results : After treatment pain were improved, VAS score decreased significantly to 2.5. The Likert scale point were checked 4.4, the patients were satisfied with the effect of acupotomy. Conclusions : This study shows acupotomy has useful effect on shoulder disease. An acupotomy is an alternative effective treatment modality for shoulder disease.

Arthroscopic Treatment of Stiff Elbow (주관절 관절경을 이용한 구축의 치료)

  • Purpose: Limitation of motion of the elbow joint due to stiffness affect on life quality of the patients. So contracture of the elbow should be treated as soon as possible. Among the many treatment modalities, we described the result of arthroscopic treatment. Materials and Methods: From Mar. 2000 to Mar. 2003, 40 patients, who received the arthroscopic treatment by author for contracted elbow, were the subjects. We estimated the range of motion (ROM) of elbow joint before and after surgery by goniometer. The clinical result was evaluated by Severance elbow scoring system. The final ROM was evaluated at the point of no further increasement of joint motion. Male ware 30 cases, female ware 7 cases, average 42.6 years old and mean follow up period were 31 months. During arthroscopic treatment we had done release of the joint capsule or resection, synovectomy, removal of loose bodies. We used traditional portals. Results: The avarage preoperative ROM of elbow joint was 72.5 degree(range, 5 - 132 degree) and the increasement of ROM was totally 49.3 degree in flexion 26.5 degree and extension 22.8 degree. There was no other complication. Conclusion: Arthroscopic treatment for contracted elbow permit early joint ROM and it decrease the secondary injury to the elbow joint. Also there are few complications. It is thought to be a good treatment modality in contracted elbow joint.
