Stages of Smoking Cessation among Malaysian Adults - Findings from National Health Morbidity Survey 2006

  • Increasing the rate of smoking cessation will reduce the burden of diseases related to smoking, including cancer. Understanding the process of smoking cessation is a pre-requisite to planning and developing effective programs to enhance the rate of smoking cessation.The aims of the study were to determine the demographic distribution of smokers across the initial stages of smoking cessation (the pre-contemplation and contemplation stages) and to identify the predictors of smoking cessation among Malaysian adult smokers. Data were extracted from a population-based, cross-sectional survey carried out from April 2006 to July 2006. The distribution of 2,716,743 current smokers across the pre-contemplation stage (no intention to quit smoking in the next six months) or contemplation stage (intended to quit smoking in the next six months) was described. Multivariable logistic regression analysis was used to examine the relationship between socio-demographic variables and the stages of smoking cessation. Of the 2,716,743 current smokers, approximately 30% and 70% were in the pre-contemplative and contemplative stages of smoking cessation respectively. Multivariable analysis showed that male gender, low education level, older age group, married and those from higher income group and number of cigarettes smoked were associated with higher likelihood of pre-contemplation to cease smoking in the next six months. The majority of current smokers in Malaysia were in the contemplative stage of smoking cessation. Specific interventions should be implemented to ensure the pre-contemplative smokers proceed to the contemplative stage and eventually to the preparation stage.

남성 관상동맥질환자의 금연모형 구축 (Smoking-cessation Model for Male Patients with Coronary Heart Disease)

  • purpose : The purpose of this study was to find out the influencing factors of smoking-cessation behavior of patients with coronary heart disease and to suggest the model of smoking-cessation behavior which was based on the relationship between influencing factors and then to test its fitness empirically. method : This study was based on the Theory of Reasoned Action and a hypothetical model was constructed with fifteen paths in consideration of main predictive factors of smoking-cessation behavior such as biological factor, disease-related characteristics, self-efficacy, supportive factor, environmental factor, disease-related perception factor, intention-to-quit, and psychological factor. The validity of a smoking- cessation model was tested to 264 patients with coronary heart disease by using SPSS 8.0 and Window LISREL 8.12a. results : 1. Seven of the 15 paths of smoking-cessation behavior proved to be significant. 2. The final model excluded three paths in the hypothetical model was demonstrated to be improved by $x^2$=44.31 (df=38, p=.22), Goodness of Fit Index (GFI)=.98, Adjusted Goodness of Fit Index (AGFI)=.96, Non-Normed Fit Index(NNFI)=1.00, Normed Fit Index(NFI)=1.00, and Root Mean Square Residual(RMR)=.24. 3.The smoking-cessation behavior was influenced directly by biological factor, self-efficacy, supportive factor, environmental factor, intention-to-quit, and psychological factor. The smoking-cessation behavior was accounted for 82% of variance by these factors. conclusion : although the adolescents' smoking behavior can be predicted by only smoking intention, it is hard to predict the adults' smoking-cessation behavior by only this factor. Therefore, intention-to-quit, self-efficacy, supportive factor should be improved because these are promotive factors for smoking-cessation behavior. Biological factor, environmental factor, and psychological factor are inhibitive factors, so nicotine replacement therapy is helpful to the high nicotine-dependents, and ex-smokers avoid other smokers in their environment and also patients should learn and practice the stress coping-skills.

대학생을 위한 캠퍼스 금연교육 프로그램의 장기 효과에 관한 연구 (Long-term Effect of Campus Smoking Cessation Program for College Students)

  • 본 연구는 캠퍼스 금연 교육 프로그램에 참여하고 금연에 성공한 대학생들의 장기금연 효과를 파악하기 위하여 실시되었다. 연구 대상자들은 2016년부터 2018년까지 서울시 소재 일개 대학에서 36주에 걸쳐 진행되는 캠퍼스 금연 교육 프로그램의 2차 자료와 참여 후 금연에 성공한 전체 금연성공자중, 연구에 참여하기로 동의한 30명이었다. 캠퍼스내에서의 흡연이 전면 금지되어 있는 서울시내 일개 대학교의 금연 교육 프로그램은 금연관련 교육과 상담, 주1회 소변 내 Cotinine 검사, 금연장학금 지급으로 구성되었다. 참여자의 평균 금연 성공률은 1차시 50.17%, 2차시 44.16%, 3차시 77.30%로 나타났으며, 2차 성공이후에 금연이 고착화되는 경향이 관찰되었다. 본 연구에서 금연에 성공한 이후, 12개월 이상 금연을 유지하고 있는 참여자는 51.22%, 흡연 재발한 참여자는 무응답 흡연자를 포함하여 48.78%이었다. 금연유지의 가장 큰 성공요인은 금연의지(28.57%)였으며, 금연유지의 가장 큰 실패요인은 스트레스(66.67%)로 나타났다 대학생을 위한 캠퍼스 주도적 금연 교육 프로그램은 대학생 흡연자의 금연유도 뿐 아니라 금연을 습관화하여 장기금연 유지자로 전환시키는데 효과적인 프로그램임을 확인할 수 있었다.

보건소 금연클리닉의 금연 성공률과 금연상담사의 직무만족도 영향요인 (Predictors of Institution Level of Smoking Cessation Rate and Job Satisfaction of Counselors in Smoking Cessation Clinics of Community Health Centers)

  • The purposes of this study were to identify factors of institution-level 6-month quit rates and job satisfaction of counselors in smoking cessation clinics of community health centers. The study was a mixed-method study including descriptive study and secondary data analysis and the data collection period was April 3, 2020, to October 31, 2020. Data were collected using a self-report survey with counselor representatives in 267 smoking cessation clinics in the nation. Part of the data regarding institutional and community characteristics was obtained from the national database. Most survey respondents were middle-aged 42.4±9.2 years old women (96%). The institutional level of 6-month quit rates was 35.3±0.1%, and the job satisfaction of survey respondents was 7.4±1.7/10 on average. Work competency was the only statistically significant contributor of both institution-level 6 month quit rate (aOR=3.57, 95% CI=1.65, 7.74, p=.001) and job satisfaction of counselors (aOR=4.64, 95% CI=2.01 10.74, p<.001). More studies are needed on the work competency of smoking cessation counselors and the development of interventions to improve their competency.

원주시민과 춘천시민의 지역사회 내 금연프로그램 이용 격차가 금연 시도에 미치는 영향 (Impact of the Utilization Gap of the Community-Based Smoking Cessation Programs on the Attempts for Quitting Smoking between Wonju and Chuncheon Citizen )

  • 이 연구는 강원도 원주시와 춘천시에 거주하고 있는 만 19세 이상 성인 600명을 대상으로 두 지역 간 흡연 상태에 차이가 있는지를 탐색하고, 지역 내 금연프로그램 경험이 금연 시도에 영향을 미치는지를 평가하였다. 연구결과 원주시는 춘천시보다 현재흡연율이 더 높고, 흡연기간은 더 길었으며, 흡연 시작 연령은 춘천시보다 더 낮았다. 원주시의 현재흡연율이 춘천시보다 더 높았음에도 불구하고 관할지역 내 보건소 등에서 운영하는 금연 교육프로그램의 이수 경험률이 원주지역에서 더 낮았고(OR=0.52, 95% CI=0.33~0.81), 금연클리닉의 이용률도 더 낮았지만 유의하지는 않았다. 지역 내 금연프로그램 경험이 금연 시도에 미치는 영향을 분석한 결과, 원주시는 금연교육 이수자와 금연클리닉 이용자가 그렇지 않은 대상자에 비해 각각 OR=2.31, OR=2.29로서 금연 시도 경험이 더 높았고, 춘천시의 경우도 금연지원서비스를 인지하고 있는 대상자의 금연 시도 경험이 그렇지 않은 대상자에 비해 OR=2.26배 더 높았다. 하지만 두 지역 모두 적은 표본수로 인하여 통계적 유의수준에 도달하지는 못하였다. 이 연구결과를 바탕으로 해당보건의료기관은 지역주민의 금연지원서비스에 대한 인식 제고와 흡연자의 금연 시도를 높이기 위한 지역 내 보다 실용성 있는 금연 교육프로그램을 개발하고 지역-밀착형 홍보로 접근성을 제고하여 현재흡연율의 지역 격차를 해소할 수 있는 중재전략을 마련할 필요가 있다.

서울 시내 일부 중년층 남성의 금연 결정요인에 관한 연구 (A Study on the Determinants of Stop Smoking by Some Middle-aged Men in Seoul)

  • The purpose of this study was to investigate determinants of stop smoking. The data on which the analysis was based come from a survey of 355 middle-aged men in Seoul. The data was analyzed using proportion, x²-test, Pearson's Correlation Coefficient and Stepwise Multiple Regression. The following were the results; 1. Success rate of stop smoking is 19.9% of 229 persons who attempted to quit smoking. 2. As for the number of quit attempts, 33.3% of ex-smokers tried once; 25.4% of current smokers tried twice. 3. As for methods of quit attempts ex-smokers showed higher tendency in using will power and books/guides than current smokers who in using will power alone. 4. For the attitudes on smoking of family, the majority of ex-smokers and current smokers were opposed absolutely. 5. The main opposite to smoking were wives in cases of both ex-smokers and current smokers, but More ex-smokers answered that the main opposite to smoking were others than current smokers. 6. As for the measures of smoking in the office, more ex-smokers answered inviting than current smokers and more current smokers assigned a smoking spot than ex-smokers. 7. Majority of ex-smokers approve of restriction at public spot strongly. 8. As for the attitude about caution on a cigarette case, more ex-smokers insisted than current smokers. 9. There was a relation between the degree of exposure about knowledge and the degree of knowledge. 10. Stepwise Multiple Regression portray that following factors influence stop smoking in order named. (1) attitude on the smoking restriction at public spot, (2) methods of quit attempt, (3) attitude about caution on cigarette case. Even so, it turned out that these factors alone can explain only 20% of self-examination. Therefore study for the other factors ought to be continued. I submit following suggestions ending this study. 1. Continuous study of the other factors affecting stop smoking must be carried on. 2. Since there was a relation the degree of exposure about knowledge and the degree of knowledge, efficient health education is required using campaign and mass media.

의과대학생들의 흡연 실태 및 다른 건강 행태와의 관련성 (Smoking Behaviors and Its Relationships with Other Health Behaviors among Medical Students)

  • Objectives : This study was conducted to examine the smoking behaviors and the relationship between smoking and other health behaviors among medical students. Methods : A self-administrated questionnaire was administered to a sample of 1,775 students from four medical schools between April and May 2003. Due to the small number of female smokers, the characteristics of smoking behaviors were analyzed only for males. Results : A total of 1,367 students (920 males and 447 females) completed the questionnaires, with an overall response rate of 77.7%. The smoking rates for males and females were 31.5, and 2.2%, respectively. Among the male smokers, 70.7% smoked daily, and 39.0% smoked one pack or more per day. Male students on medical course were more likely to smoke daily, and one pack or more per day, than those on premedical course. Male daily smokers desired to quit smoking less than occasional smokers, and 65.0% of male daily smokers were not ready to quit compared with 37.8% of the occasional smokers. Among the male daily smokers, 29.6% were severely nicotine dependent. The most common reason for not to quit smoking among male smokers was 'no alternative stress coping method' (44.4%), followed by 'lack of will power' (25.4%), and 'no need to quit' (19.4%). Compared with male non-smokers, male smokers were more likely to drink alcohol more often and in larger amounts, take coffee more often, eat breakfast less regularly, and be overweight or obese. Conclusions : The results of this study suggest that many male medical students were serious smokers, especially those on medical course. It is necessary to install a smoking prevention program for pre-medical students, provide effective smoking cessation methods for smokers, teach positive stress coping methods, and make the school environment suitable for coping with stress.

의사의 금연 건강지도의무와 의료과오책임 (Doctor's Failure to Provide Effective Treatments for Smokers and the Legal Responsibility of Medical Malpractice)

  • Tobacco has become the world's leading cause of deaths and diseases. And !be tobacco use and dependence itself is a kind of diseases, so-called "mental and be-havioural disorders due to use of tobacco" in "International Statistical Classification of Diseases and Related Health Problems(ICD-10)" and "Korean Standard Classification of Diseases". The tobacco use and dependence is a chronic disease that requires repeated clinical interventions and multiple attempts to quit. But effective treatments to the tobacco use and dependence are developed and exist that can significantly increase the rate of long-tenn smoking abstinence. So the physicians should warn smoking patients about the dangers of smoking to the health and the life, and the clinicians ought to provide one of more of the treatments which have been proven effective in helping smokers quit to smoke. It has been concluded that if a doctor failed to provide effective treatment for smokers, and the smokers subsequently died of the smokers-related conditions(tobaccosis) or became incapacitated by the tobaccosis the smokers were considered in the medical malpractice. Thus the smokers could sue the physician for medical malpractice, claiming that the doctor's legal responsibility of appropriate treatments including smoking-cessation which the physician deliberately or negligently breached.

뇌졸중 흡연노인의 금연의도 예측요인 (Predictors of Intention to Quit Smoking in Elderly Smokers following a Stroke)

  • Purpose: This study was done to identify factors based on the theory of planned behavior that predict smoking cessation in elderly smokers following a stroke. Method: Between March and May 2013, data related to intention, attitude toward the behavior, subjective norm, and perceived behavioral control, were collected through questionnaire from 223 elders who smoked and who had a stroke. Data analysis was performed using PASW (SPSS) 18.0 program, and included t-test, one-way ANOVA, Pearson correlation coefficients, and multiple stepwise liner regression analysis. Results: Results showed a positive correlation between intention and attitude toward the behavior (r=.494, p<.001), subjective norm (r=.390, p<.001), and perceived behavioral control (r=.189, p=.005). These variables such as perceived behavior control, subjective norm, number of cigarettes per day, and usefulness of quitting smoking explained 50% of intention to quit smoking. Conclusion: The results of this study indicate that there is a need to develop smoking cessation nursing strategies for elderly smokers following a stroke in order to provide effective counseling on subjective norm and perceived behavioral control.
