Factors affecting the Length of Stay in Patients with Total Knee Arthroplasty (슬관절 전치환술 환자의 재원일수에 영향을 주는 요인)

  • The purpose of this study was to analyze the claim data of Health Insurance Review & Assessment Service to determine what factors affected the length of stay in patients aged 65 and older and undergoing total knee arthroplasty due to the principal diagnosis of gonarthrosis by the type of medical institutions. As a result of making an analysis, gender, age, medical security type, severity, residential area and the number of sickbeds were identified as the factors that influenced the length of stay in each type of medical institutions. At this point in time when an increase in the elderly population triggered by population aging and another subsequent increase in medical expenses put a heavy strain on household and national economy, it's necessary to consider how to shorten the length of stay and how to ensure the efficient management of sickbeds based on the findings of this study. In addition, this study is of significance in that it could be used as basic data on quality life-care for elderly patients by the introduction of a systematic management system geared toward lessening patient burden for medical expenses.

On the QoS Support in Medium Access Control for Medical Sensor Networks (의료용 센서 네트워크에서 QoS 지원의 매체접속제어)

  • In line with the requirement of appropriate protocol support for such mission-critical wireless sensor network (WSN) applications as patient monitoring, we investigate the framework for designing medium access control (MAC) schemes. The data traffic in medical systems comes with inherent traffic heterogeneity as well as strict requirement of reliability according to the varied extents of devise-wise criticality in separate cases. This implies that the quality-of-Service (QoS) issues are very distinctly delicate requiring specialized consideration. Besides, there are features in such systems that can be exploited during the design of a MAC scheme. In a monitoring or routine surveillance application, there are degrees of regularity or predictability in traffic as coordinated from a node of central control. The coordinator thus takes on the role of marshaling the resources in a neighborhood of nodes deployed mostly for upstream traffic; in a collision-free scheme, it schedules the time slots for each superframe based on the QoS specifications. In this preliminary study, we identify the key artifacts of such a MAC scheme. We also present basic performance issues like the impact of superframe length on delay incurred, energy efficiency achieved in the network operation as obtained in a typical simulation setup based on this framework.

  • Providing Quality-of-Service (QoS) guarantee requires for each router on the path of a traffic flow not to violate the flow's delay budget allocated to itself. Since the amount of load being offered to the router is determined by the budget, some imbalance in load among routers on the path may be alleviated by means of adjusting the budget. The equal allocation applied to the resource reservation protocol (RSVP) is simple to implement, but it has the drawback of a poor resource utilization. A load balancing method in which the delay budget being allocated to a router depends on its load state was developed to improve the drawback, but it's too complex to apply to the RSVP. This paper proposes an intra-path load balancing method not only applicable to the RSVP but also more effective in improving the drawback. The proposed method first partitions the end-to-end delay bound of a flow to routers by the RSVP and then let them adjust their budgets according to both the bottleneck state of the path and their links' bandwidth availabilities. The results of the simulation applying the proposed method to an evaluation network showed that the proposed method may provide the gain of 4 ${\sim}$ 17 % compared to that in the legacy one in terms of the number of maximally admittable flows.

  • As the world's aging population increases, the aging problem in China, which has a huge population, has reached a serious situation. Therefore, this study intends to present the CCRC model of smart home care. Seven application factors of CCRC of smart home care and problems of CCRC of smart home care in China were derived through theoretical review and case analysis. Based on this, this study presented a value proposition for the 7 application elements of smart home care CCRC tailored to China's aging status and future technology demand. First, it improves the environmental atmosphere and increases the comfort of the spatial environment. Second, we value the physical and environmental safety of the elderly. Third, online convenience services are provided. Fourth, we guarantee the health and psychological health of the elderly. Fifth, it increases the efficiency of responding to unexpected situations. Sixth, predict the physical condition and surrounding environment of the elderly. Seventh, add more recreational rides. Through this, we expect to improve the quality of life by providing a safe and comfortable smart service environment for the elderly.

  • The level of transference of NGN is beginning as the operation of Access Gateway in korea at present, but NGN will keep developing continuously to the NGN integrated network until 2010. This research is finding the meaning and assignment of NGN QoS to deduct how to manage the control system and presenting the QoS control process and trial framework. The trial framework is the modeling of the QoS measurement metrics, the measurement time schedule, the section, hierarchy, instrument, equipment and method of measurement and the series of cycle & the methodology about analysis of the result of measurement. The objective standard of quality in communication service is guaranteed not by itself but by controlling and measuring continuously. Especially it's very important time to maintain the research about NGN QoS measurement and control because the big conversion of new network technology paradigm is now spreading.

  • In this paper, we propose coordinated scheduling and beamforming based on limited feedback to overcome the effects of inter-cell interference in multi-cell environment. The goal of this study is to improve a performance of cell-edge area significantly affected by the inter-cell interference and also to guarantee a performance of the entire system. Existing coordinated scheduling and beamforming based on the perfect channel state information generates an overhead for the channel state information feedback and exchange at the users and base stations, respectively. In this study, first we propose a novel codebook to obtain a null space of an interference channel efficiently which can be used to reduce the inter-cell interference. Using the proposed codebook, we propose coordinated scheduling and beamforming to improve the performance of cell-edge area and the entire system. Through the proportional fair scheduling, the performance of the proposed technique is shown as improvement of channel capacity for users located cell-edge area and entire system.

A Performance Analysis of the MAC Protocol Considering the Cell Structures and the QOS in Wireless ATM Networks (무선 ATM 망에서 셀 구조 및 QOS를 고려한 매체 접근 제어 프로토콜의 성능 분석)

  • Wireless channel has high error rate and limited bandwidth. In order to guarantee the quality of service for multimedia through wireless channel, wireless ATM network requires the cell structure with error control schemes and an efficient medium access control protocol. In this paprt, an efficient medium access control protocol, which is considered of cell structure and the QOS, is proposed for wireless ATM networks. Many kinds of ATM cell structures are analyzed in terms of cell error rate and cell loss rate to choose the proper cell structures for this proposed MAC protocol. And a bandwidth allocation algorithm is also designed to satisfy the priority of services. The performance of proposed MAC protocol are evaluated in terms of channel utilization, call blocking and packet discard probability accotding to different kinds of cell structure through simulation.

Long-term Policy Development for the Aged on Medical and Health Care Security (노인의료보장 및 건강관리를 위한 장$cdot$단기 대책)

  • The ageing problem of the population has been emerging in Korea since 1970's so that it is expecting the elderly 65 years and over among the total population from $4.5\%$ in 1988 to be $6.3\%$ in year 2000. This study was conducted to provide secure policy development in coming years for the aged on medical and health care aspects based on the examining current status of the aged problems and health care policies and systems. The study divided into four parts; The first part examined the medical insurance program and public assistance program of the health services in relation to the aged. The second part emphasized on reduction of medical care cost for the aged. The third part studied the regular health check-up program and health education for the aged. The fourth part examined the chronic disease management programs for the aged and strategies of the health care service quality improvement and specialized programs. The following recommendations made as the results of the study. 1. At present, the medical insurance program and public assistance program for the medically indigent is not appropriate to the elderly because it is a part of general medical insurance program so that Health Security Law for the Aged is proposed. 2. Medical cost will be increased due to the high occupancy rate of hospital beds and long stay of the elderly so that it is recommended to develop an early discharge program, home health care program, Health hospice and an althernative programs. 3. At present, a regular health check-ups for the elderly is not included in medical insurance program so that it is recommended to be included in the insurance program and at the same time health education program thoroughly developed for the aged. 4. To make proper medical and nursing services on chronic diseases for the elderly, it is recommended manpower development, specialized clinics or hospitals, nursing homes and an equivalent long term care facilitices should be established on the community based and a research institutions also to be related to supper the care programs.

Analysis of Delay Performance for QoS Support in Wireless Networks (무선 네트워크에서 Qos 보장을 위한 딜레이 성능 분석)

  • Providing quality of service (QoS) guarantees over wireless link requires thorough understanding and quantification of the interactions among the traffic source, the wireless channel, and the underlying error control mechanisms. In this paper, we account for such interactions in a network-layer model that we use to investigate the delay performance of a wireless channel. We consider a single ON/OFF traffic stream transported over a wireless link. The capacity of this link fluctuates according to a fluid version of Gilbert-Elliot's model. We derive the packet delay distribution via two different approaches: uniformization and Laplace transform. Numerical aspects of both approaches are compared. The delay distribution is further used to quantify the wireless effective bandwidth under a given delay guarantee. Numerical results and simulations are used to verify the adequacy of our analysis and to study the impact of error control and bandwidth allocation on the packet delay performance.   • MQTT provides three QoS levels on top of the TCP/IP layer to ensure message delivery reliability. However, in an environment where messages are concurrently transmitted and received in a node, messages are lost due to the delivery method for QoS level. In this paper, it construct an experimental environment in which MQTT generates concurrently messages, and confirm the message reliability according to QoS level. Therefore, we analyze the message reception ratio on the subscriber side by changing number of publisher and transmission cycle of message. Experimental results show that the message reception ratio of 1 and 2 except QoS level 0 decreases as the number of publisher increases or the message transmission cycle increases.
