중소병원의 의료서비스 품질, 지각된 위험, 평판, 고객만족의 구조적 관계 연구 (A Study on the Structural Relationship between Quality of Medical Service, Perceived Risk, Reputation and Customer Satisfaction in Small and Medium Hospitals)

  • Purpose - This study attempted to construct and validate a structural model of the relationship between the quality of medical services, perceived risk, reputation and customer satisfaction, which is the main concept of the relationship between large hospitals as well as small and medium hospitals and medical consumers. Through this verification, the small and medium hospitals are to find the way for wise coping in competitive situation with large hospitals. Research design, data, and methodology - This research developed a hypothesis by constructing a structural equation that reaches the satisfaction and the relationship between reputation of perceived risk and perceived risk of service quality perceptions of customers of small and medium hospitals. Research data were collected through a questionnaire survey of respondents who had medical service experience from small and medium hospital. A total data of 252 respondents were used as the sample for the final analysis and analyzed using SPSS 23.0 and AMOS 23. Results - As a result, the relationship of quality of medical service, reputation, and customer satisfaction among small and medium hospitals was consistent with the results of precedent studies, and the perceived risk has a significant impact on reputation, so the greater the perceived risk, the higher the preference for reputable medical institutions as large hospitals. In addition, it was found that the direct route from perceived risk to customer satisfaction was not significant, and reputation was found to have a full mediating effect on perceived risk and customer satisfaction. Customers who use small and medium hospitals prefer to use reputable medical institutions if their perceived risk is high, which is different from risk perception when specific targets are specified. Conclusions - In terms of the effect from customer satisfaction, not only the path of perceived risk → reputation → customer satisfaction, but also the quality of service quality → reputation → customer satisfaction. These findings suggest that small and medium hospitals are appropriately responding to competition with large hospitals, rather than focusing on the perceived risks and reputation of customers in establishing and utilizing competitive strategies to create new customers and preserve existing customers.

국내 의료기관의 사명진술문과 조직성과 분석 (Analysis of Mission Statements and Organizational Performance of Hospitals in South Korea)

  • Purpose: The purpose of the study was to examine mission statements and their elements and to investigate correlations between mission statements and organizational performance. Methods: The current research was a descriptive study based on the examination of mission statements of 353 hospitals that posted mission statements on their webpage and 92 hospitals that made their income statements public. Results: The most common mission element was 'identification of principal services', which accounted for 92.6%. Mission statements of hospitals included the average of 4.82 mission elements out of 9, and the objective of medical quality improvement was 0.81 among 6 objectives of IOM (Institute of Medicine). Net profit of hospitals with mission statements that have above average number of mission elements were significantly higher (t=2.71, p=.008) than those of other hospitals. Net profit was significantly correlated with mission statements (r=.26, p<.001), and mission elements (r=.29, p<.001). Conclusion: The results of the study empirically reveal that mission statements in the hospital affect organizational performance. That is, better organizational performance is shown for hospitals with better, more diversified, and more firmly stated mission statements which include identification of target customers, identification of principal services, contribution to society as a non-profit organization, and concern for employees.

지역사회 정신보건 서비스를 이용하는 불면증을 동반한 만성 조현병 환자의 수면의 특징 (The Sleep Characteristics of Chronic Schizophrenia Patients with Insomnia in Community-based Mental Health Services)

  • 목 적 : 본 연구의 목적은 불면증과 정신병적 증상이 지속되는 조현병 환자의 수면 장애와 연관된 요인과 수면의 특징을 평가하는 것이다. 방 법 : 지역사회 정신보건 시설에 거주 중이며 불면증과 정신병적 증상이 동반된 조현병 환자들이 참여하였다(n = 63). The Korean version of Insomnia Severity Index (ISIK)와 the Korean Version of Pittsburgh Sleep Quality Index (PSQI-K)와 같은 수면 척도가 평가되었고, ISI-K 점수가 15점 이상인 대상자들이 연구에 참여하였다. 정신병적, 불안, 우울 증상은 각각 Brief Psychotic Rating Scale (BPRS), the Korean Version of Anxiety Sensitivity Index (K-ASI) 그리고 the Korean Version of Beck Depression Inventory-I(K-BDI)로 평가하였다. 사회인구학적 자료와 ISI-K 그리고 PSQI-K 사이의 상관관계를 확인하기 위해 Pearson correlation analysis를 시행하였다. ISI-K와 PSQI-K에 영향을 미치는 각각의 요인을 확인하기 위하여 다중회귀분석으로 분석하였다. 결 과 : ISI-K와 PSQI-K의 평균 점수는 각각 $18.1{\pm}2.6$점과 $12.0{\pm}2.2$점이었다. Pearson correlation analysis 결과에서 발병 연령과 ISI-K사이의 음의 상관관계와 BPRS와 PSQI-K 사이, K-ASI와 모든 수면 척도 사이의 양의 상관관계를 확인하였다. ISI-K와 PSQI-K에 대해 K-ASI와 발병연령, K-ASI와 BPRS를 각각 단계적으로 입력한 모든 다중회귀분석에서 K-ASI만이 유의한 요인으로 남았다. 결 론 : 본 연구는 조현병 환자의 불면 증상이 우울 또는 정신병적 증상과 무관한 반면, 불안 증상과 관련이 있다는 점을 제안한다.

병원 외래 방문 환자의 만족도 평가 및 관련 요인에 대한 연구 - 한 3차 병원 방문 환자를 중심으로 - (Measurement of Ambulatory Patients' Satisfaction and Its Influencing Factors in a Tertiary Hospital)

  • 환자 만족도의 증진은 의료 제공의 궁극적인 목적의 하나이며, 의료의 질적 수준 향상을 위해서는 환자의 만족도 평가가 매우 중요하다. 따라서 판자의 만족도를 타당도와 신뢰도가 높은 도구를 이용하여 측정하고, 환자의 만족도에 영향을 미치는 요인을 규명할 필요가 있다. 이 연구는 우리 나라 실정에 적합한 표준화된 설문서의 개발에 필요한 기초자료를 제공하기 위하여 환자의 만족도를 평가하는 설문지를 개발하고 한 3차 병원 외래를 방문한 827명의 환자를 대상으로 이의 신뢰도와 타당도를 평가하였으며, 환자의 만족도에 영향을 미치는 요인을 파악하였다. 외래 방문 환자가 질을 평가하는 차원은 인자 분석을 통하여 병원 환경 및 편의 시설에 관련된 요인, 원무관리 및 보조 서비스에 관련된 요인과 의사진료에 관련된 요인으로 분류되었으며, 이 3개의 병원내 과정에 대한 척도들은 비교적 높은 신뢰도와 타당도를 나타내고 있었다. 전반적인 진료 결과를 나타내는 3개 문항은 신뢰도가 낮아, 서로 다른 별도 차원의 개념임을 의미하고 있었다. 로지스틱 회귀분석 결과 전반적인 만족도에 영향을 미치는 요인은 최근의 건강 상태, 진료비에 대한 인식과 진료에 따른 건강상태의 변화였고, 타인에게 병원을 추천할 의향에 영향을 미치는 요인은 환자의 성, 초 재진 여부, 진료에 따른 건강상태의 호전에 대한 기대, 진료비에 대한 인식과 진료에 따른 건강상태의 변화였다. 앞으로의 환자 만족도에 대한 연구에서는 이 연구 결과를 참고로 하여 조사 대상의 확대, 포괄적인 설문 문항의 개발, 조사 방법의 보완 등이 필요할 것이다. 이 연구에 사용된 설문지의 입수를 원하시는 분은 저자에게 개인적으로 연락하여 주시면, 설문지를 보내드리겠습니다.

간호서비스 연구개발 분야에서의 우선순위과제 도출 (Development of Agenda Priority for Nursing Service Research and Development)

  • Purpose: The purpose of this study was to suggest agenda priorities for nursing service R&D (Research and Development) related policies development. Methods: Two steps in developing the agendas and priorities were performed in this descriptive study. First, nursing service R&D agendas were extracted through needs assessment of nursing researchers and practitioners. Then, the priority of agendas was set by Analytic Hierarchy Process by ten experts who were representatives of nursing and other healthcare professionals. Results: Six core areas and forty-six nursing service R&D agendas were developed. The priority of agendas was different according to the evaluation criteria depending on weight value of nursing services. Conclusion: In order to select and promote nursing service R&D projects within national healthcare policy, nursing service R&D policy should be proposed with consideration to the importance of the criteria in reflecting characteristics of nursing care. By strengthening R&D capabilities for quality improvement and sensitive awareness of national directions for healthcare R&D policies, nursing service R&D can be appropriately promoted.

분류회귀나무를 이용한 의료서비스 적기처리 예측모형 (A Prediction Model of Timely Processing on Medical Service using Classification and Regression Tree)

  • 의학적 진단을 내리기 위해 시행되는 검사의 소요시간(turnaround time, TAT)은 환자대기시간과 직결되며 중요한 의료서비스 평가항목 중 하나이다. 본 연구에서는 주요 영상의학검사를 대상으로 TAT를 측정하고, 그 결과가 의료기관이 설정한 기준치를 달성하는지 여부를 분석하였다. 분류회귀나무 알고리즘을 이용한 예측 결과, "진료과", "상병", "검사종류", "실시월"이 적기처리 달성에 가장 큰 영향을 주는 요인으로 확인되었다. 본 연구는 의료서비스의 적기처리를 예측하는 모형을 통하여 의료서비스 지연을 사전에 조치할 수 있는 수단을 제공하였다는 데에 큰 의미가 있다.

병원 내 심폐소생술의 자세별 피로도와 가슴압박 정확도 비교 분석 (Comparative analysis of cardiopulmonary resuscitation accuracy and fatigue by posture in hospitals)

  • Purpose: This study aimed to identify effective cardiopulmonary resuscitation methods by comparing the fatigue of rescuers according to various positions in cardiopulmonary resuscitation situations conducted on beds in hospitals. Methods: An experimental study of students in the department of emergency medical service in H University, G Metropolitan City was conducted in four positions for applying chest pressure on mannequins on beds. Results: As a result of measuring the muscle fatigue of four muscle attachments according to the four positions conducted on the bed, the average was 3.4%, the P was significant at 0.001, and the fatigue difference was confirmed to occur depending on the attachment. An analysis of pressure depth by pose revealed that P1, P2, P3, and P4 have a depth of 58.3, 55.1, 56.4, and 56.3 mm, respectively, with P4 having the deepest depth. Conclusion: Among the various postures of the rescuer during cardiopulmonary resuscitation performed on the bed in the hospital, P1 is thought to be the most tiring, although its associated CPR quality is good.

119 구급대원의 곤충교상환자에 대한 응급처치 현황 및 개선방안 (Prehospital care and improvement of 119 emergency medical technician for the insect bite patients)

  • Purpose : This study aimed to improve quality of prehospital emergency care for the insect bite patients by figuring out its current situations and problems. Methods : This study was conducted to 219 insect bite patients who were transferred to the hospital by 119 ambulance in D-City from July 1, 2009 to June 30, 2011. This study was a descriptive study by 119 run sheets. A total of 171 emergency medical technicians(EMTs) working in D-city and C-province completed the questionnaire. All of data were analyzed by SPSS 18.0. Results : Among the 219 patients, bee sting accounted for 62.6% and wasp sting accounted for 20.1%. Some patients complained of dizziness, headache, dyspnea, and disturbance of consciousness. The 119 EMTs initially treated the patients with oxygen supply to 57 patients (31.1%). Knowledge toward insect bite was statistically significant (p=.000) between advanced EMTs and basic EMTs. The obstacle to the prehospital care, many EMTs answered that the obstacle was due to the lack of 119 EMTs. Conclusion : It is necessary to differentiate the patients' symptoms and provide the better prehospital emergency care to verify the exact symptoms and signs.

소방 119구급대원에 의해 시행된 전문기도관리 현황 및 법적 고찰 (The current status and legal review of advanced airway management implemented by 119 EMTs)

  • Purpose: This study aimed to analyze the data from public information disclosure about pre-hospital advanced airway management and identify the problem by considering domestic laws and guidelines. Methods: Data were collected between 2017 and 2018 and analyzed using SPSS 25.0. Then, the problems of the analysis results based on the relevant laws and practical guidelines were reviewed. Results: The review of domestic laws and practice guidelines revealed that ambulance nurses can implement supraglottic airway device only under the following three conditions: ① smart advanced life support pilot project area, ② trained to insert I-Gel, and ③ member of a special ambulance. In total, 21,574 cases of advanced airway management (endotracheal intubation: 2,428, I-Gel: 18,502, LMA : 499, KING AIRWAY: 144) were reported. In many cases, advanced airway management was performed by ambulance nurses who did not meet the above conditions, which was in violation of laws and guidelines. In addition, the prognosis of intubated patients was not followed up. Conclusion: The Korea National Fire Agency must stop all unlicensed medical practice by untrained, uneducated, and uncertified nurses and demand quality control programs for intubated patients.

한국 호스피스 완화의료: 과거, 현재 그리고 미래 (Hospice Palliative Care in South Korea: Past, Present, and Future)

  • Hospice palliative care (HPC) in Korea has developed steadily since its introduction in 1965. Currently, HPC in Korea is targeted only towards terminal cancer patients and their families, and the national health insurance scheme covers only inpatient hospice care for said patients. In recent years, healthcare professionals and policy makers began to recognize the need for HPC services in diverse settings including outside hospital boundaries, and for all terminally-ill patients. A law on HPC passed in January 2016 allows terminally-ill patients to refuse life-sustaining treatments, and will likely facilitate further development of HPC services. It is critical for the government and all interested parties in the medical, academic and social sectors to collaborate to ensure its success once it takes effect in 2017. This article will briefly review the half-century history of HPC in Korea, and discuss how to prepare for and cope with death and, thereby, improve the quality of death.
