The Clinical Effects of Gamichuongsangboha-tang (Jiaweiqingshangbuxia-tang) extract in Asthmatic Patients Based on Severity (가미청상보하탕 엑기스제의 기관지천식환자의 중증도에 따른 임상효과)

  • Objectives : Based on the changes of quality of life and pulmonary function test, we aimed to identify the effects and side-effects of Gamichuongsangboha-tang and in asthmatic patients. Methods : The subjects consisted of 30 patients with asthma who were treated with Gamichuongsangboha-tang extract for four weeks. Gamichuongsangboha-tang extract is the extracted powder form of the modified herbal decoction Chuongsangboha-whan, which has been used as the traditional therapeutic agent for asthma. Pulmonary function test (PFT) was checked before and after 4 weeks of treatment. Quality of Life QuestionnaireQ for Adult Korean Asthmatics (QLQAKA) was checked before and after 2 and 4 weeks of treatment. Unpleasant symptoms were checked in all patients at 2 weeks and 4 weeks from the beginning of treatment. Results : Treatment with Gamichuongsangboha-tang extract for four weeks resulted in significant increase in $FEV_{1.0}%$, PEFR%, and QLQAKA. The total efficacy rate of QLQAKA in the patient group was 53.3% after 4 weeks. The QLQAKA of the step 2, step 3, and step 4 groups (n=18, 55.6%) classified by global initiative for asthma (GINA) consistently increased, and significant improvement of QLQAKA occurred in the step3 group. In the step 4 group, FEV1.0%, and PEFR% were significantly increased. A total of 15 patients experienced unpleasant symptoms during the first 2 weeks of treatment, but these symptoms disappeared in all but 2 cases with no need of further treatment during the 2nd 2-week period. Conclusions : This study shows that Gamichuongsangboha-tang extract has effects on improvement of pulmonary function and quality of life, especially in persistent symptomatic asthmatics. Obviously further study concerning all these is still necessary.

  • Objectives: This study evaluated the effects of traditional Korean medicine treatment on a patient with asthma. Methods: A patient was treated with acupuncture, herbal medicine, and cupping. The effect of treatment was evaluated via the quality of life questionnaire for adult Korean asthmatics (QLQAKA), the visual analog scale (VAS), and night awakening frequency. Results: After treatment, improvements were observed. The total QLQAKA score increased from 36 to 50. Conclusions: This study observed that traditional Korean medicine treatments are effective in caring for a patient with asthma.

  • Objectives : A 55-year-old male patient whose symptoms were not improved by western drugs(including steroids) was hospitalized in Korean medical hospital for 22 days. Methods : This patient was diagnosed as Han-po-yeol(寒包熱) and treated with herbal decoctions, acupuncture, and moxibustion. And he answered the quality of life questionnaire for adult Korean asthmatics (QLQAKA) at 1st, 15th, and 22th day of hospitalization. Results : After 2 weeks herbal medical treatment, score of QLQAKA was increased by 0.53 point. Conclusion : This result suggest that Korean medical treatment has the potential for asthma patients who did not responded to western drugs.

The Effects of Hyeongbangdojeok-san-gami on Pulmonary Function and Quality of Life in Asthmatic Patients (荊防導赤散加味方을 투여한 기관지천식 환자 치험 5례)

  • Objective: To examine the effects of Hyeongbangdojeok-san-gami (荊防導赤散加味方) which has been used in asthmatic patients in Korean Medicine. Methods: Five asthmatic patients were enrolled who had visited Semyung University Korean Medical Hospital from July 1, 2015 to June 30, 2016. They were examined by a pulmonary function test and completed the Quality of Life Questionnaire for adult Korean Asthmatics (QLQAKA) before and after taking Hyeongbangdojeok-san-gami for 3 weeks. All data were analyzed by paired t-tests. Results: After treatment with Hyeongbangdojeok-san-gami for 3 weeks, Forced Expiratory Volume 1 sec (FEV 1.0) improved significantly (p=0.01) and Peak Expiratory Flow (PEF) showed an increase of 20% in results examined by Pulmonary Function Test (PFT). The results of the QLQAKA indicated that the main improvements occurred in theitems associated with theactivity domain. Conclusions: This study suggests that Hyeongbangdojeok-san-gami is effective in improving pulmonary function and quality of life in patients with asthma. More extensive study is needed in the future.

A Case of Jaeumganghwa-tang and Gyeongok-go Korean Medicine Treatment for Asthma Patients (천식 환자에 대한 자음강화탕과 경옥고 한의치료 1례)

  • Objectives: This study aimed to report the effects of Jaeumganghwa-tang (Ziyinjianghuo-tang) and Gyeongok-go on a patient with asthma. Methods: A 54-year-old female patient was treated with herbal medicine, including Jaeumganghwa-tang, Gyeongok-go, acupuncture, and moxibustion treatment. The effects of treatments were evaluated using the modified medical research council dyspnea scale (mMRC), St. George's Respiratory Questionnaire (SGRQ), and Quality of Life Questionnaire for Adult Korean Asthmatics (QLQAKA). Results: Following treatment, the mMRC score decreased from 2 to 1, the SGRQ score decreased from 60.53 to 25.61, and the QLQAKA score increased from 44 to 72. In the SGRQ, symptom scores decreased from 70.67 to 57.49, activity scores from 36.22 to 25.96, and impact scores from 72.15 points to 15.44 points. In QLQAKA, symptom scores increased from 18 to 21, emotional scores from 7 to 13, environmental scores from 6 to 14, and activity scores from 13 to 24. Conclusion: The results suggest that Jaeumganghwa-tang and Gyeongok-go could be effective in treating patients with asthma.

The Clinical Effects Following Discontinuation of Chungsangboha-tang(Qingshangbuxia-tang) Treatment in Patients with Controlled Asthma (청상보하탕 치료중단이 기관지 천식환자에게 미치는 영향)

  • Background : Nowadays asthma is considered to be an inflammatory disease characterized by airway hyperresponsiveness and pulmonary eosinophilia. Production of cytokines by bronchial epithelial cells may contribution to the local accumulation of inflammatory cells in patients with bronchial asthma. Chungsangboha-tang is the herbal treatment of choice in persistent asthma patients. It has been recognized that cessation of treatment with Chungsangboha-tang evokes a recurrence of symptoms in patients with controlled asthma. This study was designed to evaluate the long-term effect of Chungsangboha-tang. Materials and Methods : The subjects consisted of 24 patients with asthma who had been treated with Chungsangboha-tang for four weeks. Chungsangboha-tang is an herbal decoction which has been used as the traditional therapeutic agent for asthma. PFT, QLQAKA, blood eosinophils, serum IgE, Serum IL-4, IL-5, $IFN-{\gamma}$ were checked before treatment, before withdrawal and 3 months after cessation of treatment with Chungsangboha-tang. Results : Treatment with Chungsangboha-tang for four weeks resulted in significant increase in FEV1.0%, PEFR%, and QLQAKA. The patients were treated with Chungsangboha-tang for four weeks with no significant difference in the blood eosinophils, serum IgE, IL-4 and IL-5. The serum $IFN-{\gamma}$ in asthmatic patients decreased significantly after 4 weeks of treatment. Discontinuation of treatment with Chungsangboha-tang resulted in significant drops in QLQAKA. Others measures in asthmatic patients 3 months after discontinuation of treatment with Chungsangboha-tang showed no significant difference. Conclusion : This study demonstrates that asthma can be exacerbated by discontinuation of treatment with Chungsangboha-tang in patients with asthma. Obviously further research concerning this is still necessary.

An Analysis of Therapeutic Effects of Gamichuongsangboha-tang in 30 Asthmatics Based on Criteria for Defiency-Excess Differentiating Syndromes of Asthma (허실변증(虛實辨證)과 가미청상보하탕(加味淸上補下湯)의 임상효과)

  • Objectives: The aim was to compare and analyse the clinical effects between excess syndrome and deficiency syndrome in asthmatics through treatment with herbal dicoction, Gamichuongsangboha-tang. Materials and Methods: The subjects consisted of 30 patients with asthma and were treated with Gamichuongsangbohatang for four weeks. All patients were divided into three groups as Excess Syndrome Group(ESG), Deficiency Syndrome Group(DSG) and Coexistence Syndrome Group(CSG). PHs were checked before and 4 weeks after treatments, and QLQAKAs were checked three times; before treatments, 2 and 4 weeks after treatments. The results of QLQAKA and PH were compared and analysed between ESG, DSG and CSG. Results: Treatment of Gamichuongsangboha-tang resulted in a significant increase of QLQAKA during the first two weeks in DSG and during the last two weeks in ESG. FEV1% and PEFR% significantly increased in both DSG and ESG. There were no significanctly changes of QLQAKA and PH in CSG. Conclusions: Observations suggest that asthma in DSG was more immediately and effectively managed through treatment with Gamichuongsangboha-tang than in ESG and CSG in ease of breading and pulmonary function.

The Steroid Sparing Effects of Cheongsangboha-tang in Asthmatic Patients (청상보하탕(淸上補下湯)의 기관지천식환자에 대한 스테로이드 절약효과)

  • Background : In recent years, the glucocorticoid hormone has become a fundamental medication for asthma. However, a long period of hormone administration will result. in general. side effects on many body parts as well as hormone dependence, which has become a serious problem for western physicians. Objectives : We aimed to identify the clinical effects of Cheongsanghoha-tang and the steroid sparing effects of Cheongsanghoha-tang on. Materials and Methods : A subject group consists of 36 asthmatics who had been treated with Cheongsanghoha-tang for four weeks. Cheongsanghoha-tang is a herbal decoction, which has been used of the traditional therapeutic agent of asthma. PFT, QLQAKA, blood eosinophil, serum IgE, Serum IL-4. IL-5, IFN-${\gamma}$ were checked before and 4weeks after the treatment. Results : The only FVC% in ICSG among asthmatic patients was increased significantly compared to NICSG. Treatment of Cheongsanghoha-tang for four weeks resulted in significant increase in QLQAKA. The NICSG treated with Cheongsangboha-tang for four weeks were no significant difference in the blood eosinophil, serum IgE, IL-4 and IL-5. The PFT and QLQAKA in NICSG were increased significantly after 4 weeks treatment. But the serum IFN-${\gamma}$ in NICSG was decreased significantly after 4 weeks treatment. Discontinuation of treatment with inhaled corticosteroid in ICSG resulted in insignificant changes in PFT, the blood eosinophil, the serum IgE, IL-4, IL-5 and IFN-${\gamma}$ and significant increase in QLQAKA. As a result. 8 of 13 cases were cured with hormones completely and the rest of ICSG reduced the dose of ICS. Conclusions : This study shows that Cheongsanghoha-tang has the effects on the improvement of pulmonary function and cures asthmatic patients. These findings demonstrate that Cheongsanghoha-tang has the steroid sparing effect. Some satisfactory therapeutic results have been obtained in treating hormone-dependent asthma by Cheongsanghoha-tang. However. the concept and mechanism of hormone-dependent asthma have not been fully defined yet, and the standard for judging therapeutic effects have not been established. Obviously further researches concerning all these are still necessary.

Clinical effects of AF-365 for maintenance therapy in chronic stages of asthmas (천식 완해기 유지치료로서 AF-365의 임상적 효과)

  • Backgrounds : Gamichungsangboha-tang extract has been known to improve pulmonary functions and quality of life within four weeks of treatment. Cessation of treatment causes recurrence of symptoms and regression of pulmonary function and poorer quality of life. Long-term herbal therapy is difficult to maintain due to economic burden and other reasons. AF-365 is made with reduced dosage of Gamichungsangboha-tang extract. This study was designed to evaluate the effectiveness of AF-365 for maintenance therapy. Materials and Methods : The subjects consisted of 27 patients with asthma who had beed treated with AF-365 for four weeks. First, patients were treated with Gamichungsangboha-tang extract for four weeks. Second patients stopped herbal medicine for four weeks and after that they took AF-365. Pulmonary function test(PFT) was checked before and after four weeks of treatment. Quality of Life Questionnaire for Adult Korean Asthmatics(QLQAKA) was checked every two weeks from the beginning. The effects of AF-365 were analysed throughout the last four weeks. Results : Treatment of AF-365 for four weeks resulted in significant increase in PEFR%, QLQAKA. The QLQAKA of the step3, step4 group classified by Global initiative for asthma(GINA) showed significant improvement, and the QLQAKA of the step2 group showed no significant difference. In step3 group, PEFR% increased significantly. Conclusions : This study supports a role for AF-365 in maintenance of pulmonary function and quality of life, especially in persistent symptomatic asthmatics.

  • Nowadays Asthma is considered to be an inflamatory disease characterized by airway hyperresponsiveness and pulmonary eosinophilia. Hominis Placenta is the dried placenta of a healthy women. It has correspondence to the meridians of lung and kindey. Hominis Placenta acupuncture therapy has effect on invigoration of vital energy nourishing blood and tonifying the essence. It can be applied to the disease as Asthma, pulmonary tuberculosis, chronic hepatitis, liver cirrhosis, degenerative change and cerebrovascular disease. We treated two patients of Asthma with CVA by Homins Placenta Aqua-Acupuncture. The effect of Homins Placenta Aqua-Acupuncture was assessed by analyzing the pulmonary function test(PFT) and Quality of Life Questionnaire for adult Korean Asthmatics(QLQAKA) in patients before and after treatment. Total score was increased. The patients are satisfied our treatment. But further research concerning this is still necessary.
