The Clinical Study on 20 Cases with Sudden Sensorineural Hearing Loss in Korean Medical Hospital Last 3 Years (최근 3년간 돌발성 난청으로 한방병원에 입원한 환자 20례에 대한 임상적 고찰)

  • Objectives : The purpose of this study is to report 20 patients who have been diagnosed with sudden sensorineural hearing loss(SSNHL) in the Korean medical hospital for last 3 years. Methods : 20 patients who have been hospitalized with a SSNHL in Korean medical hospital from June 2014 to June 2017 were analyzed by reviewing the patients' charts. Results : In the study group, Pure tone audiometry was performed in 17 patients. 3 of them showed 'Marked improvement' in pure tone audiometry. And 6 of them showed 'Slight Improvement.' On the other hand, 9 of them showed 'No response' or even got worse. However, the symptoms accompanied with hearing loss like tinnitus, aural fullness, vertigo improved most of patients. Conclusions : This study shows that the Korean medical treatment is effective in the SSNHL.

Three Cases of Sudden Hearing Loss Improved after East-West Medical Combined Treatment through Cooperation in a Hospital (동일기관 내 협진을 통한 돌발성 난청의 한·양방 병용치험 3례)

  • Objectives : The purpose of this study is to report the effect of east-west medical combined treatment of sudden sensorineural hearing loss(SSNHL) through cooperation in a hospital. Methods : We treated three patients diagnosed as SSNHL by combination of herbal medicine, acupuncture, pharmoacupuncture, moxibustion, cupping, laser therapy and conventional medications. We evaluated the results of this treatment with pure tone audiometry, word recognition score(WRS), the changes in distortion product otoacoustic emissions(DPOAE) and visual analogue scale. Results : One of them showed a meaningful improvement in the hearing level of the low frequency region. The others showed 'Complete recovery' in pure tone audiometry and WRS. The subjective symptoms including tinnitus and ear fullness improved in three patients. Conclusion : This study suggests that east-west medical combined treatment is effective on SSNHL patients.

It is now widely used in clinical audiology and it plays an important role in otology. Recently there was the literature stated on normal value of various test c Impedance. This paper is dealing with the clinical evaluation of type c in comparison with following subjects as; 1. Comparison with the otoscopic finding. 2. Correlation with the pure tone audiometry. 3. Correlation with the stapedial reflex. 4. Correlation between pathologic negative pressure range and middle ear fluid.

  • The author presents clinical study for 111 cases of tinnitus with pure tone audiometry from Jan. 1, 1978 to Dec. 31, 1980 in department of otolaryngology, Ewha Womans University Hospital. The results were as follows; 1) Male to Female ratio was as 1 : 1, and peak age incidence was in the age group of 21 to 30. 2) Most frequent duration was 1 month to 1 year in the cases of the tinnitus. 3) The patient who complained tinnitus only was 10.0% and the patient who complained tinnitus with hearing impairment was 72.0% in the cases of tinnitus. 4) The affected site in the tinnitus, in order of frequency were; Both ear 38.7% Left ear 32.4% Right ear 28.9% 5) The drum finding, in order frequency were; Normal 48.0% Retraction 33.1% Perforation 8.4% 6) The degree of hearing loss in the audiometry were; Normal 29.2% Moderate severe 22.7% 7) The shape of hearing impairments were; High tone loss 46.9% Flat 33.0% Low tone loss 11.9% 8) The occurrence of $C_{5}$ dip was 11.0% in cases of the tinnitus. 9) The classification of hearing loss were; Sensorineural hearing loss 46.9% Mixed hearing loss 33.1 % Conductive hearing loss 20.1%

Sudden Hearing Loss Treated by Chuna Manual Therepy; A Case Report (돌발성 난청 환자의 경추부 추나요법 증례보고)

  • The purpose of this study is to report a clinical progress of sudden hearing loss through korean medicine. Between 26th Mar. 2014 and 19th Apr. 2014, she was treated with Chuna manual therapy every day and observed by Pure Tone Audiometry (before and after) and visual analogue pain scale (VAS). A patient was diagnosed with sudden hearing loss on 23th Dec. 2013 in Severance Hospital, she had sudden hearing loss on left side. She had 19 times treatments. At the first of treatment, her audibility was 20 dB on right side, 74 dB on left side and she had tinitus and ear fullness (VAS 8 / VAS 9). At the middle of treatment (2nd Apr. 2014), symptoms of tinitus and ear fullness started improving. State was tinitus VAS 6, ear fullness VAS 6. At the end of treatment (21th Apr. 2014), state was tinitus VAS 4, ear fullness VAS 4, and recorded 35 dB on right side, 80 dB on left side in pure tone audiometry. Chuna manual therapy is estimated to be good for sudden hearing loss especially on tinitus and ear fullness. More cases are required to develop treatment of sudden hearing loss.

  • Lowering of auditory caused by noise is increased, these days. Especially, people who have noise induced hearing loss by permanent exposure to noise are increased according to spread out of multimedia and improvement of information equipment. The pure tone audiometry used in the hospital presently inspect auditory manually up to 8,000 Hz bandwidth. So the case of noise induced hearing loss which suffered from extended high frequency over 8,000 Hz, there is a problem to prevent hearing loss by precognition. In this paper, we proposed a method to prevent noise induced hearing loss by using extended high frequency bandwidth from 12,000 Hz to 20,000 Hz. We have got a experimental result to fifty of twenties who are often used to earphones through portable equipment. As a result, 36% of twenties show lowering of auditory caused by noise and 2% of them shows severe loss of hearing.

  • Cortical deafness is a clinical rarity whereby a patient is unresponsive to all types of sounds despite the preserved integrity of the peripheral hearing organs. In this study, we present a patient who suddenly lost his hearing following ischaemic infarcts in both temporal lobes with no other neurological deficits. The CT confirmed damage to the primary auditory cortex (Heschl's gyrus) of both hemispheres. Initially, the patient was unresponsive to all sounds, however, he regained some of the auditory abilities during 10 months follow up. Pure tone threshold improvement from complete deafness to the level of moderate hearing loss in the right ear and severe in the left was observed in pure tone audiometry. Otoacoustic emissions, auditory brainstem responses, and acoustic reflex findings showed normal results. The middle and late latency potential results confirmed objectively the improvement of the patient's hearing, however, after 10 months still, they were somewhat compromised on both sides. In speech audiometry, there was no comprehension of spoken words neither at 3 nor at 10 months. The absent mismatch negativity confirmed above mentioned comprehension deficit. The extensive auditory electrophysiological testing presented in this study contributes to the understanding of the neural and functional changes in cortical deafness. It presents the evolution of changes after ischaemic cerebrovascular event expressed as auditory evoked potentials starting from short through middle and long latency and ending with event-related potentials and supported by neuroimaging.

A Case of Korean Medical Treatment of Sudden Hearing Loss with Severe Vertigo (심한 현훈을 동반한 돌발성 난청 치험 1례)

  • Objectives : Sudden sensorineural hearing loss(SSNHL) is defined as an unexplainable decline in hearing which o㏄urs within a short period of time. This study describes a case of SSNHL accompanied by many unfavourable prognosis factors including severe vertigo. Methods : We treated 1 SSNHL patient who had many unfavourable prognosis factors including severe vertigo. After Korean medical treatment for 23 days, we measured the hearing recovery and decrease of vertigo. For diagnosis, pure tone audiometry was done. The degree of improvement was evaluated by Siegel's Criteria. Decrease of vertigo was assessed by Vertigo Scale. Results & Conclusions : The pure tone 5 frequency(0.25, 0.5, 1, 2, $4_{KHz}$) average improved by 25dB(slightly recovered). Vertigo score decreased from 4 to 1. We may expect that the Korean medical therapy improves the hearing recovery in the SSNHL patients who have unfavourable prognosis factors such as vertigo.

Association between air conduction hearing threshold and blood viscosity in normal adult males (정상 성인남자에 있어서 기도청력역치와 혈액점도와의 연관성에 관한 연구)

  • This is cross-sectional study to evaluate a potential relationship between air conduction hearing threshold and blood viscosity in normal adult males(n=1677). We measured hearing threshold in frequency level at 500, 1000, 2000, 4000Hz by pure-tone audiometry and RBC profiles containing red cell number, hemoglobin, hematocrit. Blood viscosity are replaced by hematocrit that are one major factor of influencing blood viscosity. PTAs(pure-tone averages) are measured by hearing threshold averages level at 500Hz, 1000Hz, 2000Hz. Grades of PTAs(pure-tone averages) are divided three groups that are less then 10.0dB group, between 10.0-19.9dB group and excess 20.0dB. The results shows significant association among hematocrit, red cell number and hearing loss(age adjust by ANACOVA).
