Evaluation of the effects of co-culture system of human dental pulp stem cells and epithelial cells on odonto/osteogenic differentiation capacity

  • Background: In healthy dentin conditions, odontoblasts have an important role such as protection from invasion of pathogens. In mammalian teeth, progenitors such as mesenchymal stem cells (MSCs) can migrate and differentiate into odontoblast-like cells, leading to the formation of reparative dentin. For differentiation using stem cells, it is crucial to provide conditions similar to the complex and intricate in vivo environment. The purpose of this study was to evaluate the potential of differentiation into odonto/osteoblasts, and compare co-culture with/without epithelial cells. Methods: MSCs and epithelial cells were successfully isolated from dental tissues. We investigated the influences of epithelial cells on the differentiation process of dental pulp stem cells into odonto/osteoblasts using co-culture systems. The differentiation potential with/without epithelial cells was analyzed for the expression of specific markers and calcium contents. Results: Differentiated odonto/osteoblast derived from dental pulp tissue-derived mesenchymal stem cells with/without epithelial cells were evaluated by qRT-PCR, immunostaining, calcium content, and ALP staining. The expression of odonto/osteoblast-specific markers, calcium content, and ALP staining intensity were significantly increased in differentiated cells. Moreover, the odonto/osteogenic differentiation capacity with epithelial cells co-culture was significantly higher than without epithelial cells co-culture. Conclusions: These results suggest that odonto/osteogenic differentiation co-cultured with epithelial cells has a more efficient application.

Expression of DSPP mRNA During Differentiation of Human Dental Pulp-derived Cells (HDPC) and Transplantation of HDPC Using Alginate Scaffold

  • Tissue stem cells are used for the regenerative medicine. In previous study we observed hard tissue formation of human dental pulp-derived cells using alginate scaffold. In this study, we explore the ability to differentiate of the 13th passage cells with glycerol 2-phosphate disodium salt hydrate (${\beta}-GP$) which accelerate calcification. Reverse transcriptase Polymerase Chain Reaction (RT-PCR), transplants using alginate scaffold and histological examination were performed. We observed the expression of DSPP mRNA on day 10 cultured cells with ${\beta}-GP$. In conclusion, the 13th passage cells still have an ability to differentiate into odontoblast-like cells and alginate supports the differentiation of cultured cells in the transplants.

Development of a Software Program for the Automatic Calculation of the Pulp/Tooth Volume Ratio on the Cone-Beam Computed Tomography

  • Purpose: The aim of this study was to develop an automated software to extract tooth and pulpal area from sectional cone-beam computed tomography (CBCT) images, which can guarantee more reproducible, objective and time-saving way to measure pulp/tooth volume ratio. Methods: The software program was developed using MATLAB (MathWorks). To determine the optimal threshold for the region of interest (ROI) extraction, user interface to adjust the threshold for extraction algorithm was added. Default threshold was determined after several trials to make the outline of extracted ROI fitting to the tooth and pulpal outlines. To test the effect of starting point location selected initially in the pulpal area on the final result, pulp/tooth volume ratio was calculated 5 times with different 5 starting points. Results: Navigation interface is composed of image loading, zoom-in, zoom-out, and move tool. ROI extraction process can be shown by check in the option box. Default threshold is adjusted for the extracted tooth area to cover whole tooth including dentin, cementum, and enamel. Of course, the result can be corrected, if necessary, by the examiner as well as by changing the threshold of density of hard tissue. Extracted tooth and pulp area are reconstructed three-dimensional (3D) and pulp/tooth volume ratio is calculated by voxel counting on reconstructed model. The difference between the pulp/tooth volume ratio results from the 5 different extraction starting points was not significant. Conclusions: In further studies based on a large-scale sample, the most proper threshold to present the most significant relationship between age and pulp/tooth volume ratio and the tooth correlated with age the most will be explored. If the software can be improved to use whole CBCT data set rather than just sectional images and to detect pulp canal in the original 3D images generated by CBCT software itself, it will be more promising in practical uses.

Formocresol이 치수조직(齒髓組織)에 미치는 영향(影響)에 관(關)한 실험적(實驗的) 연구(硏究) (EXPERIMENTAL STUDY ON THE EFFECT OF FORMOCRESOL TO THE PULP TISSUE)

  • The purpose of this study was to evaluate the pulpal response against calcium hydroxide and zinc oxide containing various concentration of formocresol (0.1, 0.5, 1, 5, 10, 20 and 30%). The experiment was performed on dog's teeth (75 teeth from 5 dogs: Table 1.) and the teeth were routinly treated in laboratory procedures. Followings are the results obtained through microscopical examination. 1. In zinc oxide group, intlammatory reaction was severe in low concentrated formocresol and the (higher the concentration of formocresol the milder the inflammatory reaction was more evident. 2. In zinc oxide group, inflammatory change was milder at 3 weeks than 1 week, and proliferation of young connective tissue was seen at 3 weeks. 3. In calcium hydroxide group, inflammatory change in relation to the concentration of formocresol was not noticeable. 4. In calcium hydroxide group, repair process with decreased inflammatory reaction and fibrosis, and dentin bridge like layer was found at 3 weeks.

광중합 시 수종의 심미적 수복재와 이장재의 사용에 따른 치수내 온도변화 (TEMPERATURE CHANGES IN THE PULP ACCORDING TO VARIOUS ESTHETIC RESTORATIVE MATERIALS AND BASES DURING CURING PROCEDURE)

  • Polymerization of light-activated restorations results in temperature increase caused by both the exothermic reaction process and the energy absorbed during irradiation. Within composite resin, temperature increases up to 2$0^{\circ}C$ or more during polymerization. But, insulation of hard tissue of tooth lowers this temperature increase in pulp. However, many clinicians are concerned about intrapulpal temperature injury. The purpose of this study was to evaluate temperature changes in the pulp according to various restorative materials and bases during curing procedure. Caries and restoration-free mandibular molars extracted within three months were prepared Class I cavity of 3$\times$6mm with high speed handpiece fissure bur. 1mm depth of dentin was evaluated with micrometer in mesial and distal pulp horns. Pulp chambers were filled with 37.0$\pm$0.1$^{\circ}C$ water to CEJ. Chromium-alumina thermocouple was placed in pulp horn below restorative materials for evaluating of temperature changes. This thermocouple was connected to temperature-recording device(Multiplication analyzer MX, 6.000, JAPAN). Temperature changes was evaluated from initial 37.$0^{\circ}C$ after temperature changes to 37.$0^{\circ}C$. Tip of curing unit was placed in the center of prepared cavity separated 1mm from restorative materials. Curing time was 40s. The restorative materials were used with Z 100, Fuji II LC, Compoglass flow and bases were used with Vitrebond, Dycal. Resrorative materials were placed in 2mm. The depth of bases were formed in 1mm and in this upper portion, resin of 2mm depth was placed. This procedure was performed 10 times. The results were as follows. 1. All the groups showed that the temperature in pulp increased as curing time increased 2. The temperature increase of glass ionomer was significantly higher than that of Resin and Compomer during curing procedure (P<0.05). 3. The temperature increase in glass ionomer base was significantly higher than that of Calcium hydroxide base during Resin curing procedure (P<0.05).

인산(燐酸) ESTER계(系) 접착성(接着性) 시멘트의 치수반응(齒髓反應)에 관(關)한 실험적(實驗的) 연구(硏究) (AN EXPERIMENTAL STUDY ON PULPAL RESPONSE TO THE PHOSPHORIC ESTER CEMENT IN DOGS)

  • It is considered that etching solution or material itself of phosphoric ester cement will induce not a little pulpal irritation, if applied directly onto unsealed dentinal tubules. This study was designed to confirm above consideration by comparing two different conditions between $Ca(OH)_2$-based and non-$Ca(OH)_2$-based group. Posterior teeth of 15 male dogs were selected for this experiment. One experimental group was filled with cement after $Ca(OH)_2$-basing and enamel-etching, the other experimental group after enamel etching without $Ca(OH)_2$-basing. And both of two experimental groups were observed at 2 hours, 15 hours, 3 days, 1 week, 2 weeks, 4 weeks, and 6 weeks after filling. The findings reached to the following conclusions histologically. 1. In both groups, the damaged odontoblasts were atrophied and eventually disappeared. 2. In non-based group at early stage, odontoblasts were severely atrophied and defective areas were appeared between odontoblast cell layers. However, in based group, the odontoblasts were arranged slight irregularly. 3. In non-based group, a small number of undifferentiated cells below the odontoblast cell layers started to appear at 1 week after filling. However, in based group, the undifferentiated cells were appeared at 15 hours after filling. 4. In non-based group, formation of reparative dentin was not begun until late stage of experiment. However, in based group, formation of reparative dentin matrix was begun at 2 weeks after filling and very thickened reparative dentin was formed at 6 weeks after filling. 5. In odontoblast cell layers of both groups, dilated capillaries were observed. 6. Argyrophilic fibers were reticularly condensed in zone of Weil, and they were connected to the pulp tissue and dentin.

New trends of root canal disinfection and treatment strategies for infected root canal based upon evidence-based dentistry

  • The main objectives of root canal therapy are cleaning and shaping and then obturating the root canal system in 3 dimensions to prevent reinfection. Many instrumentation techniques and devices, supported by an irrigation system capable of removing pulp tissue remnants and dentin debris, have been proposed to shape root canals. But current regimens in chemomechanical debridement using instrumentation and irrigation with NaOCl are not predictably effective in root canal disinfection. These findings are not surprising because the root canal system is complex and contains numerous ramifications and anatomical irregularities. The microorganisms in root canals not only invade the anatomic irregularities of the root canal system but also are present in the dentinal tubules. Therefore further disinfection with an effective antimicrobial agent may be necessary and it well1mown that use of intracanal medication will lower bacterial count in infected root canals. Calcium hydroxide has a long history of use in endodontics, and more attention has been given to the use of calcium hydroxide as intracanal dressing for the treatment of infected pulp. However, when treatment is completed in one visit, no intracanal medications other than intracanal irrigants are used. Recently, a mixture of a tetracycline isomer, an acid, and a detergent(MTAD), has been introduced as a final rinse for disinfuction of the root canal system. It has been shown that MTAD is able to remove the smear layer with minimal erosive changes on the surface of dentin, and is effective against Enterococcus faecalis, a microorganism resistant to the action of other antimicrobial medications. In another study, the ability of MTAD was investigated to disinfect contaminated root canals with whole saliva and compared its efficacy to that of NaOCl Based on the results, it seems that MTAD is significantly more effective than 5.25% NaOCl in eradicating bacteria from infected root canals. In the cytotoxicity evaluation, MTAD is less cytotoxic than engenol, 3% $H20_2,\;Ca(OH)_2$ paste, 5.25% NaGCl, Peridex, and EDTA and more cytotoxic than 2.63%,1.31% and 0.66% NaOCl. Is it promising or transient?

  • 개의 치수에 MTA, calcium hydroxide 및 calcium sulfate로 치수복조 후 치수 반응을 서로 비교하여 MTA와 calcium sulfate가 임상적으로 치수복조제로서 사용 가능할 것인가를 구명하고자 본 연구를 시행하였다. 8개월 된 2마리 개의 24개의 치아가 본 연구에 사용되었다 전신 마취하에 고속 핸드피스를 사용하여 멸균된 #2 round bur로 치경부에 와동을 형성한 후 치수를 노출시켰다. MTA, calcium hydroxide 및 calcium sulfate를 치수노출부에 도포하였다. 와동 부위는 IRM으로 가봉하고 광중합레진으로 수복하였다. 처리 2개월 후, 전신 마취하에 희생시킨 후 조직학적으로 관찰하였다. MTA처리군에서는 치수 노출부위에 경조직의 상아질교가 형성되었으며 새로 형성된 상아질교 하방에 조상아세포가 새로 형성되었다. 치수충혈과 함께 국소적 인 혈관 증식 이 나타났으며 치수에 염증반응은 나타나지 않았다. Calcium hydroxide로 처리한 군은 상아질교 하방에 조상아세포가 관찰되지 않았으며 만성 염증반응이 다양하게 나타났다. Calcium sulfate로 처리한 군은 경조직의 상아질교가 관찰되었으며, 상아질교 하방에 조상아세포 층이 새로 관찰되었다. 몇몇의 중성구 침윤과 함께 미약한 정도의 만성염증반응이 관찰되었다. 이상의 결과에서 MTA가 calcium hydroxide및 calcium sulfate에 비해 치수에 생체친화적임을 시사하며 기계적 치수노출시 치수복조제로 사용할 수 있음을 시사한다.

The Role of SDF-1𝛼-CXCR4/CXCR7 in Migration of Human Periodontal Ligament Stem Cells

  • Background and Objectives: Regenerative endodontic procedures (REPs) are a research hotspot in the endodontic field. One of the biggest problems of REPs is that it is difficult to realize regeneration of pulp-dentin complex and functional reconstruction. The reason is still not clear. We hypothesize that the migration may be different in different dental stem cells. Periodontal ligament stem cells (PDLSCs) may migrate faster than stem cells of apical papilla (SCAPs), differentiating into cementum-like tissue, bone-like tissue and periodontal ligament-like tissue and, finally affecting the outcomes of REPs. Hence, this study aimed to explore the mechanism that regulates the migration of PDLSCs. Methods and Results: After isolating and culturing PDLSCs and SCAPs from human third molars, we compared the migration of PDLSCs and SCAPs. Then we investigated the role of SDF-1𝛼-CXCR4/CXCR7 axis in PDLSC migration. We further investigated the impact of Porphyromonas gingivalis (P. gingivalis) lipopolysaccharide (LPS) on PDLSC migration and the potential mechanism. PDLSCs showed better migration under both noninflammatory and inflammatory conditions than SCAPs. SDF-1𝛼 can promote the migration of PDLSCs by elevating the expression of CXCR4 and CXCR7, increasing the interaction between them, promoting expression of 𝛽-arrestin1 and activating the ERK signaling pathway. P. gingivalis LPS can promote the migration of PDLSCs toward SDF-1𝛼 through increasing the expression of CXCR4 via the NF-𝜅B signaling pathway, promoting the expression of 𝛽-arrestin1, and activating the ERK signaling pathway. Conclusions: This study helped elucidate the potential reason for the difficulty in forming pulp-dentin complex.

The effect of dentinal tubule erosion by application of 17% EDTA solution on the apical leakage

  • I. Objectives 17% EDTA is known as an effective solution to remove smear layer. When it's applicated for more than 1 minute, it shows dentinal erosion of intertubular and peritubular dentin. This study is to investigate the effect of dentinal tubule erosion with different time application of 17% EDTA solution on the apical leakage. II. Materials and Methods 35 recently extracted human teeth with single canal, straight root, and closed apex were used in this study. Crowns were removed and the pulp tissue remnants were removed with a barbed broach.(omitted)
