The diagnosis of peripheral T cell lymphoma is difficult due to the varying size and shape of the neoplastic lymphoid cells and the frequent admixture of nonneoplastic mature lymphyocytes, histiocytes, eosinophils, and plasma cells. We report a case of peripheral T cell lymphoma, lymphoepithelioid ceil type, which was difficult to differentiate from tuberculous lymphadenitis due to the aggregates of epithelioid histlocytes mimicking granuloma and the past history of pulmonary tuberculosis. Fine needle aspiration cytology of the inguinal lymph node in a 63-year-old male was characterized by hypercellular aspirates composed of a mixture of small and intermediate-size lymphoid cells and large lymphoid cells with background of confluent epithelioid histiocytes. The neoplastic lymphocytes demonstrated significant nuclear irregularity with protrusion and indentations of the nuclear membrane, prominent nucleoli, and frequent mitotic figures. The diagnosis of peripheral T cell lymphoma was confirmed by histological and immunohistochemical studios.

Primary malignant lymphomas of the lung are rare neoplasms representing 3.6% of all extranodal lymphomas and less than 1% of pulmonary malignancies, with its distinct clinicopathological features. A 60-year-old symptomatic man, who was a former non-smoker, had a cough and sputum. Chest roentgenogram and computed tomographic scan showed the presence of a posterior mass in the basal segment of the right lung. Transthoracic needle aspiration cytology was inadequate for the diagnosis of cancer. A subsequent right thoracotomy was carried out, which showed a primary pulmonary lymphoma of the bronchus-associated lymphoid tissue.

Lymphomatoid granulomatosis (LYG) is an angiocentric and angiodestructive neoplastic proliferation of B and T lymphocytes commonly involving the lungs. Epstein-Barr virus is commonly detected in lesional cells. We report a case of a 54-year-old female with underlying monoclonal gammopathy of unknown significance who presented with a 4 week history of dyspnea and cough. Computed tomography scan of the chest showed multiple lung nodules as well as endobronchial narrowing causing atelectasis at the left upper lobe. Bronchoscopic findings revealed obstruction at the lingula segment due to endobronchial mass as a rare presentation. Bronchoscopic biopsy was diagnosed with LYG grade 1. After treatment, the endobronchial mass and lung lesions were completely resolved. However, the patient eventually evolved to malignant lymphoma after 1 year.

저자들은 악성 임파종의 항암요법후 갑자기 진행되는 폐침윤(ground glass & consolidation) 소견을 보인 Pneumocystis carinii(PCP)와 Cytomegalovirus(CMV)의 복합 폐염 환자를 치료하였으나 계속 진행되는 호흡 부전으로 사망하였다.

Primary malignant lymphoma of the lung is a very rare disease, which consists of 0.34% of entire malignant lymphoma. The majority are low-grade B-cell tumors, and because of their morphological peculiarities and overall excellent prognosis, many cases, like many other extranodal lymphomas, have been mislabelled as "pseudolymphomas" in the past. For these reasons their true incidence is difficult to estimate. An incidentally discovered mass in the right middle lobe of a 36-year-old woman was operated on November 9, 2001 at Hanyang University Hospital. A right upper lobectomy was done and the pathologic diagnosis of extranodal marginal zone B-cell lymphoma of MALT type was made.

저자들은 비호즈킨 림프종 환자에서 bleomycin, doxorubicin, prednisolone, cyclophosphamide의 복합 항암화학요법을 시행하던 중, bleomycin 축적 용량 90단위에서 중증의 폐독성이 빠르게 진행하는 급성호흡부전의 형태로 발생하여 기계호흡 등의 보존적 치료 후 폐 섬유화 소견을 남기고 회복하는 증례를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

폐의 원발성 림프종은 드문 질환으로 저등급 B세포 림프종이 주를 이루며 경과와 예후가 양호한 반면 고등급 B세포 림프종은 경과와 예후가 저등급 B세포 림프종에 비해 좋지 않다. 그리고 그 치료에 대해서는 아직 정확히 제시된 바가 없고 치료경험들에 대한 증례 보고들이 몇몇 있었다. 본 저자는 폐내 종괴와 흉막삼출이 있었던 환자에서 흉막침범 소견이 있었으며 흉막생검을 통해 비교적 급속한 진행경과를 보이는 diffuse large B cell 림프종을 진단하였으며 cyclophosphamide, epirubicin, vincristine 그리고 prednisolone의 복합화학요법을 6회 시행후 종괴가 소실된 부분관해를 치험하였기에 문헌고찰과 함께 증례보고 하는 바이다.

원발성 폐 림프종은 매우 드문 질환으로 전세계적으로 보고된 바가 흔하지 않고 특히 폐 실질의 침범 없이 기관에서 원발성으로 발생한 경우는 극히 드물며 그 대부분은 BALT 기원의 저등급 B-세포 림프종으로 알려져 있다. BALT 림프종은 오랜 기간 동안 한 곳에 국한되어 존재하고, 예후가 좋고, 얌전한 조직학적 소견을 보이는 것이 특징이다. 아직 전세계적으로 그 예가 많지 않아 표준적인 치료지침은 정해져 있지 않으나 외과적 절제, 국소 방사선 치료나 항암제 치료 등이 있으며 특히 국소성 방사선 조사는 확실히 전이가 되지 않았을 때 일차적 치료로 선택할 수 있다. 저자들은 호흡곤란을 주소로 내원한 환자에서 경기관지 조직 검사상 기관 원발성 BALT 림프종을 진단 후 경기관지 레이저 소작술 및 국소 방사선 조사치료를 시행하고 2년간의 추적관찰 기간동안 재발의 소견없이 안정적인 임상경과를 보이고 있는 기관 원발성 BALT 림프종 1예를 경험하였기에 문헌고찰과 함께 이를 보고 하는 바이다.

Pulmonary mucormycosis is an uncommon opportunistic fungal infection associated with diabetis mellitus, leukemia, lymphoma & other debilitating diseases. Their clinical presentations depend on the associated underlying disorders. Generally patients with leukemia and lymphoma often have rapidly progressive clinical course, and may cause diffuse parenchymal disease refractory to medical and surgical therapies. However, some diabetics with pulmonary mucormycosis have a striking tendency to develop a localized endobronchial disease in major airway which is amenable to therapy with surgery and Amphotericin B. We have experienced a case of endobronchial mucormycosis complicating lung abscess associated with diabetic ketoacidosis, which was cured without specific antifungal therapy or surgery. A brief review of the literature was given.
