진행성 폐암에서 광역동 치료로 호전된 5례에 관한 임상적 경험 (Clinical Experience of Photodynamic Therapy in Five Patients with Advanced Lung Cancer)

  • 연구배경 : 폐암의 말기에 관찰되는 기도 폐쇄는 호흡곤란, 기침, 객혈 및 발열 등을 일으키고 반복적인 호흡기계 감염의 원인이 되기도 한다. 그러나 대부분의 경우 방사선 치료, 항암화학요법 또는 수술적 치료를 받았기 때문에 더 이상 치료를 할 수 없는 어려움이 있다. 따라서 기도 폐쇄를 호전시키기 위한 보조적인 방법으로 광역동 치료가 한 방법임이 알려져 있다. 방 법 : 기도 폐쇄가 관찰되는 폐암 환자에게 기관지 내시경 시행 48시간 전에 광감작물질인 porfimer sodium (Photofrin$^{(R)}$)을 kg당 2mg으로 정맥주사 후 일광화상 및 광과민반응을 방지하기 위해 직사광선을 차단시켰다. Light source는 diode laser를 이용하였고, light dose는 180-200 $joul/cm^2$, 방사하는 빛의 강도는 400 $mW/cm^2$ 으로 하였으며 interstitial irradiation 및 cylindrical diffuser 를 이용하여 630nm의 레이져를 조사하였다. 시행 48시간 후 기관지내시경을 시행하여 괴사된 조직을 제거하고 남아있는 종양 부위에 대해 재조사를 시행하였다. 결 과 : 폐암 수술 후 수술부위의 국소적인 재발을 보인 경우가 1예, 기도 폐쇄로 인한 호흡곤란을 호전시키기 위해 시행한 경우가 4예였다. 광역동 치료를 통해 종양조직의 선택적인 괴사를 관찰할 수 있었고 괴사조직을 제거한 후에는 기도 폐쇄가 4예에서 호전되었다. 광역동 치료 종결 후 호흡곤란 및 활동도가 호전된 경우가 3예에서 관찰되었고 폐쇄성 폐렴으로 인한 객담 및 발열의 소견 및 기관지암에 의한 객혈의 경우에도 광역동 치료 후 호전되었다. 결 론 : 기도 폐쇄가 관찰되는 폐암 환자에서 보존적인 치료 방법으로 기관지내시경을 이용한 광역동 치료는 호흡곤란, 반복적인 호흡기계 감염 및 객혈 등을 호전시키기 위한 효과적인 방법 중의 하나이다.

정상 노년층의 호흡 및 발성 특성 (Characteristics of Respiration and Phonation in Normal Health Elderly)

  • Korea does not have a certain criteria on the respiratory ability and phonation of the normal aged, and also has no clear standard to examine the boundaries of geriatric diseases. This study analyzed the characteristics in respiration and phonation of the aged in normal healthy elderly from diverse angles with different variables. Thirty-three participants in total, seven males and eight females in the age group 55-64 participated in the study. Seven males and eleven females in the age group 65-74 were selected for the respiration and phonation experiments, and 10 different variables such as FVC, $FEV_1$, $FEV_1$/FVC, MPT, MFR, Psub, f0, jitter, shimmer and NHR were comparatively analyzed for each group of different age and gender. To see the difference in respiration and phonation by age and gender, the study conducted a two-way ANOVA. First, from the result of the analysis on respiratory ability, FVC of male appeared to be significantly greater than female. In both age groups of 55-64 and 65-74, male displayed greater FVC than female did. Second, as for $FEV_1$, $FEV_1$/FVC, the age group of 55-64 showed greater values than the values measured in the age group of 65-74. Third, MFR showed a significant difference by gender. In both age groups, male showed significantly higher MFR than female. Fourth, for different gender, a significant difference in MFR was observed. In both age groups of 55-64 and 65-74, male exhibited higher MFR than female. This study has a clinical implication in that it analyzed the criteria on respiration and phonation in normal healthy elderly according to gender and age. The normal aged showed a difference in their respiratory and phonatory functions by age and gender, it was closely related to the decline of pulmonary function due to the physical aging and the weak respiration coming from weakness of respiratory muscles. Also, the physical differences in height, weight, and the muscles in laryngeal and respiratory system between males and females had an influence on the performance. These results from this study might be a helpful guideline for the clinical criterion in the future.

최대환기능의 간접측정법에 관한 연구 (Studies on the Indirect Measuring Method of the Maximum Voluntary Ventilation)

  • The maximum voluntary ventilation (MVV) is one of the most widely used pulmonary function test, but its measuring method was very difficult and unreliable. However, it is need to get more easy and simple measuring method of MVV. Therefore, this study was attempted to get more easy and simple measuring method of MVV by means of the forced expiratory volume $(FEV_{T})$. The young and healthy 1,000 Korean students(592 male and 408 female) were cheesed for this purpose and whose ages were from 8 to 20 years. A spirometer (9L, Collins Co.) was used for the MVV and FEV, and they were measured 3 times at standing position, and the highest value was used. In the measurements, the subjects for MVV were asked for the breath as fast and deeply as possible for 12 seconds, and for FEV were asked for the rapid and forceful exhalation after a maximal inhalation (forced expiratory curve). In the FEV measurements toward the end of the expiration, the subjects were exhaused to continue the effort until no further gas was expired. During these measurements, the investigator stood by the subject to give a constant encouragement. FEV were calculated in the volume exhaled during the one-half $(FEV_{0{\cdot}5,}\;ml)$, the first second $(FEV_{1{\cdot}0,}\;ml)$ and the percentage of the total vital capacity exhaled during the one-half second $(FEV_{0{\cdot}5,}\;%)$. The results are summarized as follows: 1) The values of MVV were increased linearly with ages until 20 in both sexes. The values of male at the age of 20 was $168.2{\pm}2.5L/min$, and female at the age of 17 was $112.3{\pm}3.0L/min$, respectively. 2) The values of FEV (ml) were increased linearly with ages until 20 in both sexes. The values of $FEV_{0{\cdot}5}$ were $2,797{\pm}65.7ml$ in the male of 20 years and were $2,088{\pm}54.6ml$ in the female of 17 years, and of $FEV_{1{\cdot}0$ were $4,119{\pm}68.2ml$ in the male of 20 years and were $2,897{\pm}65.9ml$ in the female of 17 years, respectively. 3) The correlation coefficients between MVV and $FEV_{0{\cdot}5}\;or\;FEV_{1{\cdot}0$ (ml) were 0.82 or 0.85 in the male, and 0.77 or 0.79 in the female, respectively. 4) The prediction formulae for MVV to be derived from above results were: For male: MVV (L/min) =7.19+$0.05{\times}FEV_{0\cdot5}(ml)$, MVV (L/min)=11.25+$0.04{\times}FEV_{1\cdot0}(ml)$ For female: MVV (L/min)=16.03+$0.05{\times}FEV_{0\cdot5}(ml)$, MVV (L/min)=9.47+$0.03{\times}FEV_{1\cdot0}(ml)$.

기관절제 및 단단문합술에 의한 소아 기관협착증의 치료 (Tracheal Resection and End to End Anastomosis for Tracheal Stenosis in Children)

  • Background and Objectives : In children with tracheal stenosis, operative management remains a challenging problem due to difficulties of operative techniques and postoperative care. The purpose of this study was to determine the effectiveness of tracheal resection with end to end anastomosis as operative management for tracheal stenosis in children. Materials and Methods : 6 children with severe tracheal stenosis underwent tracheal resection with end to end anastomosis. Causes of stenosis were trauma in 1 case and prolonged intubation or tracheotomy in 5 cases. The diagnoses were made by radiologic evaluation (plain X-ray, CT, 3-Dimensional CT) and confirmed by direct laryngoscopy and ventilating bronchoscopy under general anesthesia. Thyroplasty and unilateral arytenoidectomy were performed in 1 case. Suprahyoid release was done in 1 case with severe adhesion. Decanulation was achieved following postoperative endoscopic examination and pulmonary function test. Postoperative physical and radiologic examinations were given at regular intervals. Results : Stenosis were improved from grade III grade I in 4 cases and from grade II to grade I in 2 cases. Decanulation was achieved on average postoperative 6 months in 5 cases, and 10 years in 1 case due to exertional dyspnea. There were 1 each case of immediate postoperative subcutaneous emphysema, pneumothorax and wound infection. Postoperative granulomas at anastomosis site were treated with laser vaporization under suspension laryngoscope and bronchoscope in 3 cases. There was 1 each case of delayed postoperative vocal cord palsy, aspiration pneumonia and loss of cough reflex. Conclusion In tracheal stenosis of children, tracheal resection with end to end anastomosis has good result with preservation of normal airway. Preoperative evaluation of local factors such as swallowing, vocal cord movement and cough reflex and general condition was important for successful treatment. As the cases in adults, authors considered this operation to be a curable operative management for tracheal stenosis.

Transforming Growth Factor β Inhibits MUC5AC Expression by Smad3/HDAC2 Complex Formation and NF-κB Deacetylation at K310 in NCI-H292 Cells

  • Airway mucus secretion is an essential innate immune response for host protection. However, overproduction and hypersecretion of mucus, mainly composed of the gel-forming MUC5AC protein, are significant risk factors for patients with asthma and chronic obstructive pulmonary disease (COPD). The transforming growth factor β (TGFβ) signaling pathway negatively regulates MUC5AC expression; however, the underlying molecular mechanism is not fully understood. Here, we showed that TGFβ significantly reduces the expression of MUC5AC mRNA and its protein in NCI-H292 cells, a human mucoepidermoid carcinoma cell line. This reduced MUC5AC expression was restored by a TGFβ receptor inhibitor (SB431542), but not by the inhibition of NF-κB (BAY11-7082 or Triptolide) or PI3K (LY294002) activities. TGFβ-activated Smad3 dose-dependently bound to MUC5AC promoter. Notably, TGFβ-activated Smad3 recruited HDAC2 and facilitated nuclear translocation of HDAC2, thereby inducing the deacetylation of NF-κB at K310, which is essential for a reduction in NF-κB transcriptional activity. Both TGFβ-induced nuclear translocation of Smad3/HDAC2 and deacetylation of NF-κB at K310 were suppressed by a Smad3 inhibitor (SIS3). These results suggest that the TGFβ-activated Smad3/HDAC2 complex is an essential negative regulator for MUC5AC expression and an epigenetic regulator for NF-κB acetylation. Therefore, these results collectively suggest that modulation of the TGFβ1/Smad3/HDAC2/NF-κB pathway axis can be a promising way to improve lung function as a treatment strategy for asthma and COPD.

Early Outcomes of Robotic Versus Video-Assisted Thoracoscopic Anatomical Resection for Lung Cancer

  • Background: We compared the safety and effectiveness of robotic anatomical resection and video-assisted thoracoscopic surgery (VATS). Methods: A retrospective analysis was conducted of the records of 4,283 patients, in whom an attempt was made to perform minimally invasive anatomical resection for lung cancer at Seoul National University Hospital from January 2011 to July 2020. Of these patients, 138 underwent robotic surgery and 4,145 underwent VATS. Perioperative outcomes were compared after propensity score matching including age, sex, height, weight, pulmonary function, smoking status, performance status, comorbidities, type of resection, combined bronchoplasty/angioplasty, tumor size, clinical T/N category, histology, and neoadjuvant treatment. Results: In total, 137 well-balanced pairs were obtained. There were no cases of 30-day mortality in the entire cohort. Conversion to thoracotomy was required more frequently in the VATS group (VATS 6.6% vs. robotic 0.7%, p=0.008). The complete resection rate (VATS 97.8% vs. robotic 98.5%, p=1.000) and postoperative complication rate (VATS 17.5% vs. robotic 19.0%, p=0.874) were not significantly different between the 2 groups. The robotic group showed a slightly shorter hospital stay (VATS 5.8±3.9 days vs. robotic 5.0±3.6 days, p=0.052). N2 nodal upstaging (cN0/pN2) was more common in the robotic group than the VATS group, but without statistical significance (VATS 4% vs. robotic 12%, p=0.077). Conclusion: Robotic anatomical resection in lung cancer showed comparable early outcomes when compared to VATS. In particular, robotic resection presented a lower conversion-to-thoracotomy rate. Furthermore, a robotic approach might improve lymph node harvesting in the N2 station.

Changes in Forced Expiratory Volume in 1 Second after Anatomical Lung Resection according to the Number of Segments

  • Background: Although various methods are already used to calculate predicted postoperative forced expiratory volume in 1 second (FEV1) based on preoperative FEV1 in lung surgery, the predicted postoperative FEV1 is not always the same as the actual postoperative FEV1. Observed postoperative FEV1 values are usually the same or higher than the predicted postoperative FEV1. To overcome this issue, we investigated the relationship between the number of resected lung segments and the discordance of preoperative and postoperative FEV1 values. Methods: From September 2014 to May 2020, the data of all patients who underwent anatomical lung resection by video-assisted thoracoscopic surgery (VATS) were gathered and analyzed retrospectively. We investigated the association between the number of resected segments and the differential FEV1 (a measure of the discrepancy between the predicted and observed postoperative FEV1) using the t-test and linear regression. Results: Information on 238 patients who underwent VATS anatomical lung resection at Kyung Hee University Hospital at Gangdong and by DH. Kim for benign and malignant disease was collected. After applying the exclusion criteria, 114 patients were included in the final analysis. In the multiple linear regression model, the number of resected segments showed a positive correlation with the differential FEV1 (Pearson r=0.384, p<0.001). After adjusting for multiple covariates, the differential FEV1 increased by 0.048 (95% confidence interval, 0.023-0.073) with an increasing number of resected lung segments (R2=0.271, p<0.001). Conclusion: In this study, after pulmonary resection, the number of resected segments showed a positive correlation with the differential FEV1.

Transcriptome profiling identifies immune response genes against porcine reproductive and respiratory syndrome virus and Haemophilus parasuis co-infection in the lungs of piglets

  • Background: Co-infections of the porcine reproductive and respiratory syndrome virus (PRRSV) and the Haemophilus parasuis (HPS) are severe in Chinese pigs, but the immune response genes against co-infected with 2 pathogens in the lungs have not been reported. Objectives: To understand the effect of PRRSV and/or HPS infection on the genes expression associated with lung immune function. Methods: The expression of the immune-related genes was analyzed using RNA-sequencing and bioinformatics. Differentially expressed genes (DEGs) were detected and identified by quantitative real-time polymerase chain reaction (qRT-PCR), immunohistochemistry (IHC) and western blotting assays. Results: All experimental pigs showed clinical symptoms and lung lesions. RNA-seq analysis showed that 922 DEGs in co-challenged pigs were more than in the HPS group (709 DEGs) and the PRRSV group (676 DEGs). Eleven DEGs validated by qRT-PCR were consistent with the RNA sequencing results. Eleven common Kyoto Encyclopedia of Genes and Genomes pathways related to infection and immune were found in single-infected and co-challenged pigs, including autophagy, cytokine-cytokine receptor interaction, and antigen processing and presentation, involving different DEGs. A model of immune response to infection with PRRSV and HPS was predicted among the DEGs in the co-challenged pigs. Dual oxidase 1 (DUOX1) and interleukin-21 (IL21) were detected by IHC and western blot and showed significant differences between the co-challenged pigs and the controls. Conclusions: These findings elucidated the transcriptome changes in the lungs after PRRSV and/or HPS infections, providing ideas for further study to inhibit ROS production and promote pulmonary fibrosis caused by co-challenging with PRRSV and HPS.

Risk Factor Analysis of Morbidity and 90-Day Mortality of Curative Resection in Patients with Stage IIIA-N2 Non-Small Cell Lung Cancer after Induction Concurrent Chemoradiation Therapy

  • Background: Major pulmonary resection after neoadjuvant concurrent chemoradiation therapy (nCCRT) is associated with a substantial risk of postoperative complications. This study investigated postoperative complications and associated risk factors to facilitate the selection of suitable surgical candidates following nCCRT in stage IIIA-N2 non-small cell lung cancer (NSCLC). Methods: We conducted a retrospective analysis of patients diagnosed with clinical stage IIIA-N2 NSCLC who underwent surgical resection following nCCRT between 1997 and 2013. Perioperative characteristics and clinical factors associated with morbidity and mortality were analyzed using univariable and multivariable logistic regression. Results: A total of 574 patients underwent major lung resection after induction CCRT. Thirty-day and 90-day postoperative mortality occurred in 8 patients (1.4%) and 41 patients (7.1%), respectively. Acute respiratory distress syndrome (n=6, 4.5%) was the primary cause of in-hospital mortality. Morbidity occurred in 199 patients (34.7%). Multivariable analysis identified significant predictors of morbidity, including patient age exceeding 70 years (odds ratio [OR], 1.8; p=0.04), low body mass index (OR, 2.6; p=0.02), and pneumonectomy (OR, 1.8; p=0.03). Patient age over 70 years (OR, 1.8; p=0.02) and pneumonectomy (OR, 3.26; p<0.01) were independent predictors of mortality in the multivariable analysis. Conclusion: In conclusion, the surgical outcomes following nCCRT are less favorable for individuals aged over 70 years or those undergoing pneumonectomy. Special attention is warranted for these patients due to their heightened risks of respiratory complications. In high-risk patients, such as elderly patients with decreased lung function, alternative treatment options like definitive CCRT should be considered instead of surgical resection.
