Dioxins and Health: Human Exposure Level and Epidemiologic Evidences of Health Effects (다이옥신과 건강: 인체 노출 수준 및 건강영향에 대한 역학적 연구)

  • General information is summarized, that is necessary to introduce a scientific assessment of the human health and exposure issue concerning dioxin and dioxin-like compound. Scientific literatures were reviewed to assess the background exposures to the dioxin-like compounds for normal residents. Epidemiologic studies were also reviewed to assess malignant and nonmalignant sweets of dioxins. In 1997, the International Agency for Research on Cancer classified 2,3,7,8-tetrachlorodibenzo-p-dioxin (TCDD) as a human carcinogen, primarily based on occupational cohort studies. The US Environmental Protection Agency made the same decision in it's Draft Dioxin Reassessment. Epidemiologic evidences point to a generalized excess of all cancers, without any pronounced excess at specific sites. Reported non-cancer effects included a range of conditions affecting most systems. Among them, chloracne, elevation in gamma glutamyl transferase(GGT), and alterations in reproductive hormones are related to TCDO, Other adverse outcomes, such as lipid concentrations, diabetes, circulatory and heart diseases, immunologic disorders, neurobehavioral effects, and developmental outcomes require further study before their respective relationships to TCDD can be more definitively assessed.

Regional disparities in healthy eating and nutritional status in South Korea: Korea National Health and Nutrition Examination Survey 2017

  • BACKGROUND/OBJECTIVES: Concerns about regional disparities in heathy eating and nutritional status among South Korean adults are increasing. This study aims to identify the magnitude of regional disparities in diet and nutritional status among Korean adults who completed the 2017 Korea National Health and Nutrition Examination Survey (KNHANES). SUBJECTS/METHODS: The participants were a nationally representative sample of Korean adults aged 19 years and older from the 2017 KNHANES (n = 6,126). We employed the svy commands in STATA to accommodate the complex survey design. The relative concentration index (RCI), absolute concentration index (ACI) and index of disparity were used to measure regional nutritional inequalities. RESULTS: Overweight and obese adults were more prevalent among the poor than among the rich in urban areas (RCI = -0.041; P < 0.05), while overweight and obese adults were more prevalent among the rich than among the poor in rural areas of South Korea (RCI = 0.084; P < 0.05). Economic inequality in fruit and vegetable intake ≥ 500 g per day was greater in rural areas than in urban areas in both relative size (RCI = 0.228 vs. 0.091, difference in equality = 0.137; P < 0.05) and absolute size (ACI = 0.055 vs. 0.023, difference in equality = 0.032; P < 0.05). CONCLUSIONS: This study provides useful information identifying opposite directions in the relative concentration curves between urban and rural areas. Adult overweight/obesity was more prevalent among the poor in urban areas, while adult overweight/obesity was more prevalent among the rich in rural areas. Public health nutrition systems should be implemented to identify nutritional inequalities that should be targeted across regions in South Korea.

Effectiveness of Obesity management programs: systematic review and meta-analysis (메타분석을 이용한 비만관리 프로그램의 효과분석)

  • As overweight and obese people have increased, obesity management programs have generated much concern in Korea. Many types of obesity management programs were operated, aimed at reducing weight, BMI, body fat percentage and so on. This study was conducted to review the features of studies in obesity management programs and systematically assess the effects which were published from 1995 to 2006 in Korea. Databases were systematically searched for published data in Korea. It were KISS (Korean studies information services system) and KERIS (Korean Education research information system), which were major literature search systems for all academic fields in Korea. Total 114 studies were initially identified, of which 21 satisfied our inclusion data. The effects of obesity management programs were assessed on the results in weight, BMI (body mass index), HDL (high density lipids), body fat percentage, self-efficacy. Effect size estimated on the equation of M1 - M2 / Sp, where M1 was mean of the experimental group, M2 was the mean of control group, and Sp was the pooled standard deviation. Magnitude of effect size was interpreted by using Cohen's definition. Cohen described small, medium, and large effect sizes as 0.2, 0.5, and 0.8 respectively. The studies about obesity management programs were the most published in 2005(26.1%). Obesity management programs were popularly operated as the type of 12 weeks intervention, exercise intervention(52.4%), quasi-experimental study(85.7%). Ten studies reported significant effects on weight, and nine studies reported significant effects on BMI and HDL. Only five and three studies reported significant effects on body fat percentage and self-efficacy respectively. The effects of each outcome were generally significant when the studies included these elements, intervention over 10 weeks, evaluation over 3 times, a comprehensive program (exercise, nutrition education, behavior modification) and reinforcing factors (self-monitoring, group discussion, one's goal setting, and etc.). Effective obesity management programs should contain these essential elements and objectives of obesity management programs must be set out evident at the beginning. The participants should be registered, educated and evaluated by continuous obesity management programs.

Analysis of factors related to suicidal ideation among people with and without disabilities in elderly living alone (장애인과 비장애인 독거노인의 자살생각 영향요인 분석)

  • Purpose: This study purposed to analyze of factors related to suicidal ideation among people with and without disabilities in elderly living alone Methods: The study examines the factors of suicidal ideation using over 65 years of age who participated in the 2017 National Survey of Older Koreans of the elderly living alone data. Subjects were 2,265 person among elderly living alone over 65 years of age. Data were analyzed using descriptive statistics, X2-test, t-test, logistic regression analysis with the SPSS win 26.0 program. Results: Suicidal ideation of the elderly living alone with disabilities were 13.6% and 9.0% of the elderly living alone without disabilities, and the elderly living alone with disabilities had higher suicidal ideation (p<.05). The significant predictors of the suicidal ideation in elderly living alone with disabilities were employment and depression. The significant predictors of the suicidal ideation in elderly living alone without disabilities were age, gender, number of chronic disease, economic state, SLCA(Society, leisure, culture, and activities) and depression. Depression was analyzed as a common factor related to suicidal ideation in disabled and non-disabled elderly living alone. Conclusion: Based on the results of this study, it is necessary to develop a program for people with and without disabilities. It is necessary to prepare various programs and community support systems to prevent and manage depression for people with and without disabilities in elderly living alone.

Two combined amino acids promote sleep activity in caffeine-induced sleepless model systems

  • BACKGROUND/OBJECTIVES: The aim of this study was to evaluate the biological and sleep-promoting effects of combined ${\gamma}$-aminobutyric acid (GABA) and 5-hydroxytryptophan (5-HTP) using caffeine-induced sleepless fruit flies, ICR mice, and Sprague-Dawley rats. MATERIALS/METHODS: Video-tracking analysis was applied to investigate behavioral changes of Drosophila melanogaster. Pentobarbital-induced sleep test and electroencephalogram (EEG) patterns were used for analysis of sleep latency, duration, and quantity and quality of sleep in vertebrate models. RESULTS: Administration of combined GABA/5-HTP could significantly reverse the caffeine induced total distance of flies (P < 0.001). Also, individually administered and combined GABA/5-HTP significantly increased the total sleeping time in the caffeine-induced sleepless ICR mice (P < 0.001). In the caffeine-induced sleepless SD-rats, combined GABA/5-HTP showed significant differences in sleep quality between individual amino acid administrations (P < 0.05). CONCLUSIONS: Taken together, we identified inhibitory effects of combined GABA/5-HTP in locomotor activity, sleep quantity and quality in caffeine-induced sleepless models, indicating that combined GABA/5-HTP may be effective in patients with insomnia by providing sufficient sleep.

A Study for Food Service Utilization and Service Satisfaction of the Elderly Participants (사회복지관 무료급식서비스 이용 노인의 특성 및 만족도에 관한 연구 : 충청북도를 중심으로)

  • The purposes of this research were 1) to investigate the characteristics of the meal service participants; 2) to evaluate the food service utilization of the elderly; 3) to identify major factors that affect food service satisfaction of the participants in Chungchongbuk-Do. For doing this, 309 subjects were selected and the survey research method was adopted. The characteristics of the meal service participants were similar to the characteristics of general elderly population in Korea. The public assistant recipients were under representative in this sample, that is, most of elderly people who participated in meal service consisted of non public assistant recipients. It meant that meal service was not provided to low-income elderly people. The health status of the elderly people was stable to be able to attend to the meal service organizations. The most important reason to use a meal service was to see their friends. It showed that meal services played an important role to provide support systems to the elderly people in community. Thus, meal services should be provided to the elderly people with social services. In order to analyze the factors influencing food service satisfaction, multiple regression analysis was employed. The results showed that physical activity, contact with friends who met in meal service program, and the number of social services were the important variables to predict meal service satisfaction in this study. These findings contributed to a better understanding of developing a meal service planning. Several improvement strategies were recommended to provide effective meal services. First, socio-economical characteristics of the elderly should be considered to provide appropriate services. Second, meal services should be provided to the elderly people with social services. Third, home delivery meal services should be available to the home-bound elderly.

  • Because personal information files held by educational institutions include sensitive information such as personal school affairs information or health information, damages resulted from personal information leakage of educational institutions are expected to be serious. In order to respond to this problem, the Ministry of Education has expanded information security education targeting (personal) information security officers in educational institutions. However, a number of personal information leakage cases of public institutions occurred at educational institutions. Thus, this study, targeting information security education centers, through an empirical research, tries to confirm whether information security education supply is being properly provided for (personal) information security officers in educational institutions, and suggest the appropriate balance between education supply and education demand as the implication for the educational direction of information security education centers.

Exploring the Satisfaction with COVID-19 Prevention Measures and Awareness of the Tourism Crisis for Residents' Tourism Attitude

  • Purpose: The researchers reviewed the previous studies on the psychological anxiety and economic damage during the recent COVID-19 pandemic to understand the satisfaction, awareness and attitude of the residents in Jeju, Korea. Research design, data and methodology: 215 samples based on self-reported survey were collected for path analysis focused on controlling variables in demographic characteristics in PLS-SEM using Smart PLS 3.0. Results: The researchers found the Satisfaction with COVID-19 Prevention Measures and the Awareness of Tourism Crisis affect Attitude toward Tourism of the residents with the explanatory power (R2 = 0.209). Age was found to be the only significant controlling variable in this model for the Awareness of Tourism Crisis to Attitude toward Tourism. Conclusions: The attitude toward tourism is influenced by the level of satisfaction of the prevention service for COVID-19 as well as the awareness of the crisis. Residents have different level of the tourism crisis awareness by the status of their economic activities while there is no differences in all age groups for satisfaction in the quarantine service. The study suggests that the importance of the public health service should be preferentially concerned for residents to have stable economic activities where the tourism is the major industry.

Health Care System Satisfaction and Reform Need of Medical Users (의료이용자의 보건의료제도 만족도와 변화 요구도)

  • This study was performed to investigate health care system satisfaction and reform need using the data from the '2019 Health Care Experience Survey'. For 8,349 data with experience in medical use, health care system satisfaction and reform need level was analyzed by t-testing and ANOVA by characteristics of the study subjects, and multiple regression was conducted. Research has shown that health care users' recognition of the health care system is relatively low compared to reliability and satisfaction. It is necessary to promote policies and health care systems for senior citizens, low education levels. Since the reliability has the biggest impact on the satisfaction of the health care system, government should establish policies that they can trust, and in the process, they should gather opinions from the public and secure credibility through social consensus. Medical users were sympathetic to the need for reforms in the health care system, and felt the need to support vulnerable areas and vulnerable groups the most.

Structural Relationships Among Factors to Adoption of Telehealth Service (원격의료서비스 수용요인의 구조적 관계 실증연구)

  • Within the traditional medical delivery system, patients residing in medically vulnerable areas, those with body movement difficulties, and nursing facility residents have had limited access to good healthcare services. However, Information and Communication Technology (ICT) provides us with a convenient and useful means of overcoming distance and time constraints. ICT is integrated with biomedical science and technology in a way that offers a new high-quality medical service. As a result, rapid technological advancement is expected to play a pivotal role bringing about innovation in a wide range of medical service areas, such as medical management, testing, diagnosis, and treatment; offering new and improved healthcare services; and effecting dramatic changes in current medical services. The increase in aging population and chronic diseases has caused an increase in medical expenses. In response to the increasing demand for efficient healthcare services, a telehealth service based on ICT is being emphasized on a global level. Telehealth services have been implemented especially in pilot projects and system development and technological research. With the service about to be implemented in earnest, it is necessary to study its overall acceptance by consumers, which is expected to contribute to the development and activation of a variety of services. In this sense, the study aims at positively examining the structural relationship among the acceptance factors for telehealth services based on the Technology Acceptance Model (TAM). Data were collected by showing audiovisual material on telehealth services to online panels and requesting them to respond to a structured questionnaire sheet, which is known as the information acceleration method. Among the 1,165 adult respondents, 608 valid samples were finally chosen, while the remaining were excluded because of incomplete answers or allotted time overrun. In order to test the reliability and validity of the assessment scale items, we carried out reliability and factor analyses, and in order to explore the causal relation among potential variables, we conducted a structural equation modeling analysis using AMOS 7.0 and SPSS 17.0. The research outcomes are as follows. First, service quality, innovativeness of medical technology, and social influence were shown to affect perceived ease of use and perceived usefulness of the telehealth service, which was statistically significant, and the two factors had a positive impact on willingness to accept the telehealth service. In addition, social influence had a direct, significant effect on intention to use, which is paralleled by the TAM used in previous research on technology acceptance. This shows that the research model proposed in the study effectively explains the acceptance of the telehealth service. Second, the research model reveals that information privacy concerns had a insignificant impact on perceived ease of use of the telehealth service. From this, it can be gathered that the concerns over information protection and security are reduced further due to advancements in information technology compared to the initial period in the information technology industry, and thus the improvement in quality of medical services appeared to ensure that information privacy concerns did not act as a prohibiting factor in the acceptance of the telehealth service. Thus, if other factors have an enormous impact on ease of use and usefulness, concerns over these results in the initial period of technology acceptance may become irrelevant. However, it is clear that users' information privacy concerns, as other studies have revealed, is a major factor affecting technology acceptance. Thus, caution must be exercised while interpreting the result, and further study is required on the issue. Numerous information technologies with outstanding performance and innovativeness often attract few consumers. A revised bill for those urgently in need of telehealth services is about to be approved in the national assembly. As telemedicine is implemented between doctors and patients, a wide range of systems that will improve the quality of healthcare services will be designed. In this sense, the study on the consumer acceptance of telehealth services is meaningful and offers strong academic evidence. Based on the implications, it can be expected to contribute to the activation of telehealth services. Further study is needed to assess the acceptance factors for telehealth services, such as motivation to remain healthy, health care involvement, knowledge on health, and control of health-related behavior, in order to develop unique services according to the categorization of customers based on health factors. In addition, further study may focus on various theoretical cognitive behavior models other than the TAM, such as the health belief model.
