Health impact assessment in South Korea is carried out within the framework of Environmental Impact Assessment (EIA). And, public health and hygiene items in EIA as an evaluation of health impact items were designed to calculate the emission level of hazardous air pollutants from specific development projects, and the exposure concentration at major sites, and to assess the health risk associated with these projects. But, the assessment of the receptor characteristics around the development project is limitedly considered. The purpose of this study is to propose the method of calculation of potential exposed population and the use of it for public health and hygiene items in EIA. The data for the calculation of potential exposed population in South Korea were identified. And the calculation algorithm based on them was proposed. After that, in a development of industrial complex, a case analysis was carried out assuming the release of hazardous air pollutants. This study suggested that the concept of population risk should be applied in risk assessment utilizing the potential exposed population and as a future study, a standard of assessment of health effects for population risk should be established. Finally, the possibility of using this study as a location-limiting tool for the development project was presented through the method of calculating the potential exposed population. It is meaningful that this study presented a method for performing risk assessment from the perspective of receptors around a development project.
1. Introduction Community medicine with the concept of comprehensive medical care and an ideal medical care delivery system not only for an individual or family but for the whole community has emerged. In April 1970, the Presbyterian Medical Center started a hospital based community health service project in order to improve the health of the people in rural areas. Prior to commencing a comprehensive medical care system, a family survey was needed. The major objective of this survey was to obtain information concerning the people and their environment so as to be able to plan and implement a comprehensive medical care program in Soyang-Myun. 2. Survey Method An interview using a family record form was carried out for each household. This family record form was designed to get information about demography, family planning, environmental sanitation and vital statistics. Prior to beginning, the members of the survey team were trained in interviewing techniques for three days. The team consisted of a public health nurse, four nurse-aides, a sanitarian and four health extension workers who are working in our project, The survey was carried out during the period November 1971 to March 1972. 3. The economic feasibility analysis including benefit-cost ratio and internal rate of return of a land consolidation and on-farm development project was mainly depended upon the direct benefits and costs arising during project life. Therefore the projects were neglected in allocating the government's financial funds and loans on account of the low economic rate of return of the project. Accordingly the extended benefit-cost analysis method should be introduced and adapted to cover not only the benefit s such as non-market values of environmental and food security fun ct ions of the project but also market values of the project outputs. The main purposes of this study are (1) to prepare a guide line for economic feasibility analysis based on the IBRD and ADB guidelines, (2) to estimate the benefits from productivity increase, labor saving, savings of O&M cost of farm mechanization and project facilities, savings of farmer's burden for their public health, increasing environmental and public functional value of paddy fields, improving food security condition and formation of peaceful and uncontroversial rural society by implementing the land consolidation and on-farm development, (3) to introduce the newly adapted analytical method as the extended benefit cost analysis which could make possible to be included the values of non-market goods such as the food security and the environmental public function of paddy fields. To carry out this study, the existing publications on the guidelines for economic agricultural projects were reviewed and consultation was made with a For the post evaluation study of the land consolidation and on-farm development project, field survey in Jigok and other comparative area were made to get available information. According to the project completion report, Jigok land consolidation and on-farm development project has 55ha of benefit area out of 69ha of gross area. The project was started in November 11th, 2000 and completed october 30th, 2001. The total project costs were amounted to 2,548 million won and the annual project benefits were estimated at 335million won evaluated by domestic market prices. The ERR(Economic Rate of Return) and SRR(Social Rate of Return) of the project based on the shadow pricing system were estimated at 4.4% and 16% respectively. On the other hand, the ERR and SRR based on the domestic market value system were amounted to 6.37% and 14.62%. In conclusion, Korean land consolidation and on-farm development projects have not carried out from the view point of economic rate of return under shadow pricing system but from the view point of domestic pricing system. For the future feasibility studies on land consolidation projects have to be carried out including the non-market values as environmental and food security function of the projects.
Objectives: The main purpose of this review is to introduce the current status of environmental health indicators of Korea and to share our experience of the developmental process of Korean version of environmental health indicators. Methods: This paper describes 1) the background of environmental health indicators; 2) the ways how we developed environmental health indicators of Korea; 3) the current status of selected indicators; and 4) suggestions for the further policy development. Results: The 6-year long project for the development of environmental health indicator systems in Korea could provide outputs on three major distinct aspects on the indicator issues. Firstly, we have developed the rational process/manual so that the government can select and advocate the potential indicators with a relatively objective manner. Secondly, we have suggested the potential candidate indicators which can be implemented immediately. We also pooled all indicators in order to evaluate the summary index which we expect to tell the status of environmental health. Third, we provided suggestions on the further utilization of this indicator system. Conclusion: The mission of environmental health policy is to resolve the public health problems occurring because of hazardous environment. In order to identify the environmental determinants of the community health problems and to evaluate the effectiveness of the policy implemented, these environmental indicators can be used. Therefore, the government should implement this ready-prepared system of environmental health indicators.

This study was conducted to grasp the progress of the health management program in small scale industries with phenomenological approach. The industrial health care system compose of manpower, devices, and facilities such as medical resources, organization, service delivery system, financial resources, payments, and management system is important for the industrial health. Especially health management program should be provided feasible conditions to workers. The data collection period was 2 months from September 1 to October 30, 1997. The indepth interview results for health monitor, labors, and occupational health nurses were analyzed by Giorgis' phenomenological method. The major results were as follows: 1. The workers, health monitors and nurses felt that the subsidiary program of health management in small scale industries were necessary. This project for small-size industry can be set-up through complementary education for health monitors and resolvement of nurses' six suggestions. It is necessary to provide followings ; 1) Properly devision of industry 2) More clear guidance for health management at visiting time 3) Legitimate incentive system 4) Health education materials and devices 5) Change of fee and material payments at visiting 6) Budgets and system for medication and vaccination at visiting 2. Above all, it is suggested that the strategics of the health management program should be developed.
Ever since the foundation of the Korean Society for Preventive Medicine in 1947, members of the Society had made remarkable contributions to the public health development and national health promotion. They had played key roles in establishing national health system, improving environmental hygiene, controlling infectious and chronic diseases, promoting family planning, improving industrial and environmental health, and developing health service management. However, the Society had less actively responded to the changes in health service needs of the population that were caused by a rapid epidemiologic transition in last a few decades. Early detection and treatment of chronic diseases including cancer and cardiovascular diseases and risk reduction by the life style modification are major approaches to the contemporary national health problems and they are the core contents of preventive medicine. The author proposed to develop the clinical preventive medicine specialist who will have additional training in clinical medicine for health screening and life style modification to the current preventive medicine training program and thus will be able to provide comprehensive preventive medical services. Another area that the Society may take the initiative is training preventive medicine specialist in the disaster, including bioterrorism, preparedness and management. The Society should be more active in proposing health policy and health service program and also participate collectively in a large scale health research project of the government. These approaches may not only contribute more effectively to the national health promotion but also improve the identity of the Society.
Objectives: This case - study was conducted to assess the oral health status change of the elderly through visiting oral care interventions based on a community health care project. Methods: Professional dental hygiene treatment and oral health education, including brushing using interdental toothbrushes and sponge brushes, were performed on three senior citizens who received home visiting oral health care benefit. Results: The subject's periodontal conditions improved including gingival inflammation and bleeding. The gingival color tured pink by controlling the dental plaque. Conclusions: Visiting oral health care contributed to the improvement of oral health of the elderly. Therefore, based on the characteristics of the elderly with various systemic diseases, it is necessary to discover various cases that can perform professional and customized visiting oral health care programs.
The main objectives of this study were to know the prevalence of cerebrovascular diseases, to find the important risk factors of cerebrovascular diseases. This study is a part of Eumseong Community Health Project supportes by GTZ(West German Goverment). 116 perceived cases of cerebrovascular diseases were first screened by health interview and examinations and 80 cases were diagnosed as a cerebrovascular group. For comparison, 80 cases were matched with their neighbor controls of the same sex and the similar age. 1. The prevalence rate for cerebrovascular diseases was 476.3 per 100,000 population which is considered to be very high compared with that of other countries. Age adjusted rate for cerebrovascular diseases was 261.6 per 100,000. 2. Among the risk factors examined for the association with cerebrovascular diseases by case-control study, and analysed by paired marginal test(McNemar's $X^2$-test) and odds ratio, only hypertension showed high significant statistical association.
Focusing on complex diseases of public health significance, strategic issues regarding the on-going Korean Genome Cohort were reviewed: target size and diseases, measurements, study design issues, and follow-up strategy of the cohort. Considering the epidemiologic characteristics of Korean population as well as strengths and drawbacks of current research environment, we tried to tailor the experience of other existing cohorts into proposals for this Korean study. Currently 100,000 individuals have been participating the new Genome Cohort in Korea. Target size of de novo collection is recommended to be set as between 300,000 to 500,000. This target size would allow acceptable power to detect genetic and environmental factors of moderate effect size and possible interactions between them. Family units and/or special subgroups are recommended to parallel main body of adult individuals to increase the overall efficiency of the study. Given that response rate to the conventional re-contact method may not be satisfactory, successful follow-up is the main key to the achievement of the Korean Genome Cohort. Access to the central database such as National Health Insurance data can provide enormous potential for near-complete case detection. Efforts to build consensus amongst scientists from broad fields and stakeholders are crucial to unleash the centralized database as well as to refine the commitment of this national project.

Purpose: The obesity prevention and management program led by public health centers are important in the community. This study aims to identify the current status of obesity prevention and management programs at public health centers and perceptions regarding facilitators and barriers when implementing programs. Methods: This study used a concurrent mixed methods design. A survey was conducted to investigate the current status and infrastructure of the obesity prevention and management program at eighty-three public health centers nationwide. Nine program managers and six local residents with experience in the program were interviewed by using a semi-structured questionnaires. Results: Most of the infrastructure facilities for the program were inadequate, and insufficient budgets and lack of professional staff were identified as barriers. Facilitators included diversification of program delivery methods, operator competence, and visible outcomes and rewards. For the effectiveness of the program, it is vital to have adequate assistants, a sufficient budget, various promotional methods, and connections with various institutions in the community. On this basis, it is acknowledged that the public health center serves as a platform for preventing and managing obesity in the community. Conclusion: It is expected that infrastructure improvements in public health centers and the link with community resources are needed. In addition, there is an urgent need to set standards for obesity prevention and management programs implemented in public health centers to 'resolve regional disparities'.
