Study on the Realities of Utilization and Consciousness of Behavior with Dental Clinics (치과(齒科) 이용실태(利用實態)및 행동의식(行動意識)에 관(關)한 연구(硏究))

  • The purpose of this study is to examine the effect of parents knowledge on oral health status toward their children. This study was carried out to investigate and analyze the cognitive degree of dental subjects and general public who have an experience to be treated for their dental knowledge, operative dentistry, and dental prothesis in the oral health care through the questionnaires. The results are as follows : 1 : In questioning the frequency in the visitation of dental clinic, 68.8% usually go to see dentists, if children's teeth have trouble. It was the reason that economical standard and educational level were enhanced as before. 57.6% took a kind of medical measures. 2 : In surveying the preference of dentists’ age or gender, 56.9% preferred younger dentists to older or female dentists. On the contrary, 56.9% children want younger dentists, and 64.8% children want female dentist doctor. As a conclusion, the preference of dentists’ age or gender is relevant with a view to remove big horror, which can be evoked by dental measures. 3 : In questioning the possession amount of decayed teeth, 45.8% have more than four. Usually those parents who have children filled their decayed teeth with artificial or installed with dental prosthesis. As a conclusion, age is relevant in this study in p<0.05 level. 4 : In asking for efficient starting-year of tooth brushing, 42.6% said that 2 or 3 year age is the most efficient, and especially 80.6% teach children to learn brushing themselves. (P<0.05). As a conclusion, the voluntary brushing can be good behaviour for good dental health. 5 : In surveying the way they obtained dental information, 38.9% were given form mass media, but 90% were once not delivered with the education of dental health. Because parents have been known narrow and deficient information of dental health through mass media, experts in dental health have to deliver help their parents to be delivered with systematical dental information. The conclusion of this study can be summarized that parents' role is very important to their children's tooth health and the education program for systematical dental information should be delivered to their parents.

The Associated Factors of Health Examinations Behaviors among Some Elderly Persons in Urban and Rural Areas (일부 도시·농촌지역 고령자의 건강검진 수진행동에 관련된 요인)

  • Objectives: We investigated the factors related to health examination behaviors, sociodemographic aspects and lifestyles of elderly persons with different social backgrounds, and compared sexual and regional differences in urban and rural elderlies. Methods: The total study subjects(464 individuals) from urban(236) and rural areas(228), recruited by a stratified cluster random sampling were interviewed and examined about their sociodemographic profiles, daily lifestyles, subjective health status, conditions concerning use of medical resources, hearing acuity, visual acuity and ADL(activity of daily living), and whether they receive health examination or not. For statistical analysis, Chi-square test was used for sexual and regional comparisons among the groups who have been given a health examination and the one who have not. Results: In urban areas, the rate of having underwent health examination was 54.5% in men and 46.9% in women, and in rural areas, it was 59.8% in men and 42.7% in women, showing its higher rate in men than in women in both areas. For regional differences between the group who have taken a health examination and the one who have not, there was a significant difference in terms of age, family pattern, current job, monthly household income, owning a house, drinking status, eating habit, subjective health status, whether they have taken outpatient medical service for the recent 3 months or not, anxiety for the health, and IADL conditions according to whether the community is rural or urban. In multiple regressions, the influential factors on the health examination behaviors were selected such as having their own house, their family doctor, amnesia, urinary incontinence and chronic disease in urban districts. But in rural districts, the variables were selected such as having or not of their family doctor, urinary incontinence, anxiety for the health, educational level, their own house and chronic disease. Conclusions: It is suggested that the approach to the health examination of an older patient requires substantial consideration of highly variable individual sociodemographic characteristics involving regional attributes as well as their daily life styles, subjective health status, status of performing health examination, physical health status and ADL conditions.

A Study on the Medical Ethics Education at Colleges of Korean Medicine (한의대 교과목으로서 의료윤리에 관한 고찰)

  • Objectives : According to the data from the 2016 Yearbook of Traditional Korean Medicine, 10 out of 12 medical schools of Korean medicine are offering medical ethics. Medical ethics has become essential in Korean medicine education, but there has been no agreement on the content of education yet, so initial discussions are necessary with respect to the content and methods of education. Methods : In this study, basic data were collected by searching papers, reports, books, and media articles on medical ethics related to Korean medicine education, and by studying the website of medical schools of Korean medicine nationwide. Based on the collected data, the status of medical ethics lectures were determined and compared with the current state of medical ethics lectures by western medical schools. The contents suitable for medical ethics education at medical schools of Korean medicine were discussed. Results : The topics of the medical ethics include: the basic concepts of medical ethics, the ethics of birth, the ethics associated with genetics, the ethics associated with death, and the ethics regarding doctor-patient interaction, the ethics of medical research, medical rationing ethics, ethics between medical staffs, medical law and ethics, philosophical base of medical ethics, ethics of doctor as professional, and moral personality formation of doctor. The contents of medical ethics in traditional Korean medicine reflected views on the human body and life based on "Huangdi's Internal Classic"and medical ethics from the viewpoints of Buddhism and Confucianism. Conclusions : Medical schools of Korean medicine are medical training institutions, and medical ethics education is essential to become a Korean medicine doctor as professional worker, medical practitioner, and biomedical researcher. There is no fundamental difference in the basic principles of medical ethics in both western and Korean medicine, and there are differences in contents depending on the clinical practice. The contents of medical ethics on clinical practice should be modified for Korean medicine doctors, and traditional Korean medicine ethics would be set up upon that. In the national licensing examination, medical ethics needs to be added as one part so that all the ethical problems related to the clinical situation can be solved.

  • Traditionally, the Supreme Court has held that medical treatment agreements covered by national health insurance should be distinguished from other medical treatment agreements which are viewed as a consummation of the autonomous free will between doctor and patient. Namely, the Supreme Court views medical treatment agreements covered by national health insurance to be bound by the National Health Insurance Law with the intent to promote the applicability and comprehensiveness of the national health insurance scheme. Yet, issues of voluntary non-reimbursable treatments are triggered not only by the mistakes or moral hazard of medical care institutions but also by systemic limitations of national health insurance coverage criteria. Thus, there is a need for legislative measures that allow certain medical treatments to be included or reflected in the national health insurance coverage system so that patients may receive prompt and flexible medical treatments. To reflect such concerns, the Supreme Court made an exception for voluntary non-reimbursable treatments and developed a strict test to be applied in such cases in Supreme Court Judgment 2010 Doo 27639, 27646 (ruled on June 8, 2012 by the Grand Bench). Such judgment, however, is not a fundamental overturn of the Supreme Court's prior rulings that voluntary non-reimbursable treatments are not allowed under the law. It is only a slight revision of its previous stance for cases in which there is a lack of legislative measures to make coverage of a new yet valid medical treatment possible under the current national health insurance coverage system.

Development of National Liscensing Examinaion for Improvement of Pharmacist (약사직능수행 향상을 위한 약사국사시험 개선에 관한 연구)

  • To maximize the services of phatmacists, we studied the best national liscensing examination for improvement of pharmacist. Among 650 Pharmacist who are posed the questions, 282 pharmacist such as professor in college of pharmacy(46), pharmacists in drug store and hospital (136), administrators in The Ministry of The Health and Welfare(48) and researchers and phamacists in pharmaceutical company (52) replied it. By analyzing these questionares, the results were as follows: In the Korean national liscensing examination for medial care team, pharmacist has been examed over two times the subjects as compared with another national examinations for medical and public health liseneses and 6-year curriculum of medical school. Now, the national examination for pharmacy liscence have been tested the basic subjects such as pharmaceutical analytical chem-istry, medicinal chemistry, biochemistry, etc., but almost demander, pharmacist who has been engaged about over 80% in the drug stores and hospital pharmacy, want to have exam pharmacy practics which are clinical pharmacy, pathohysiology and pharma-cotherapy of diseases like The United State of America, but not basic chemisty and biology. If the Korean liscensing examination for pharmacy will be changed like America, pharmacy education in the college of phar-macy will be progressed in detail. There are no clerkships and internships in the pharmaceutical education program in Korea, and so most of pharmacist want to have 6-year curriculum for pharmacy education and to test the pharmacy practice for Doctor of Pharmacy(Pharm. D.) but not basic science.

Survey on Private Health Insurance Awareness of Korean Medicine Doctor (실손의료보험에 대한 한의사 인식도 조사)

  • Objectives : This study aimed to investigate the recognition of coverage for Korean medicine treatments in private medical insurance among Korean medicine doctors (KMDs). Methods : Questionnaire reviewed by experts was performed to KMDs who were registered in the Association of Korean Medicine. The survey targeted awareness of private health insurance for Korean medicine, appropriate coverage, cost, and frequency. Results : Data were collected from 932 respondents out of 28,234 Korean medicine doctors. Most KMDs were aware of coverage for Korean medicine in private medical insurance, and they responded that pharmacopuncture and herbal decoction were covered first. KMDs also responded that the coverage should be provided without limited number of times, except herbal medicine. Conclusion : Most KMDs responded that Korean medical private medical insurance was essential. To promote the developing insurance for Korean medicine, survey for public and insurance company will be performed.

Factors associated with changes in pharmaceutical expenditures of outpatient care in clinic setting : Focusing on the incentive scheme to reduce total prescribed drug expenditure and the drug utilization review system (의원 외래환자의 약품비 변화 관련요인: 처방총액 절감 인센티브제도와 DUR 제도 시행 전후를 중심으로)

  • This study was performed in order to compare a change in pharmaceutical expenditures per outpatient of clinic and to analyze factors relevant to a systems as part of evaluating policies for the incentive scheme to reduce total prescribed drug expenditure and for the drug utilization review system("DUR system" hereafter). For this, it had finally analytical subjects as 21,320 clinics nationwide without a change in location, clinics symbol and signed subject during both terms of the first half of 2010 and the first half of 2011. As a result, the odds ratio with reduction in pharmaceutical expenditures of clinic was statistically higher significantly in the shorter year number of opening clinic, in the larger number of doctors, when the classification of establishment is other, not individual, and when the signed subject is surgical division. Also, the odds ratio was significantly higher in the less patient number of clinic and in the lower ratio of patients aged over 65. Finally, the odds ratio was significantly high when a clinic had been located in DUR system demonstrative project area. Through this, a case of policy for improvement in doctor's autonomous prescription behavior like DUR system can be known to be effective for reduction in pharmaceutical expenditures. A future research on evaluation of policy for pharmaceutical expenditure management system will need to be performed in-depth analysis in consideration of diverse characteristics on the participatory entities.

Professional Job Perception, Job Stress and Job Satisfaction of Westerm Doctors and Oriental Doctors Practicing at Local Clinic (개원의사와 개원한의사의 전문직업성, 직무스트레스 및 직업만족도)

  • Objectives : This study was conducted in order to investigate the professionalism, job stress and job satisfaction in western and oriental doctors in Korea. Methods : The authors conducted a survey using a self-administered questionnaire, conducted between August and September, 2001. The study subjects were 457 western doctors and 161 oriental doctors practicing at local clinics in Daegu City. Results : For the professionalism scale, the score for belief in service to the public of western doctors was significantly higher than that of oriental doctors (p<0.05) whereas the scores for sense of calling to the field and feeling of autonomy were higher in oriental doctors (p<0.01). For the job stress scale, the scores for work factor, and clinical responsibility/decision factor of western doctors were significantly higher than those of oriental doctors (p<0.01). Of the western doctors, 59.7% expressed satisfaction with their job as a doctor, 69.5% responded that the role of a doctor was appropriate to their aptitude, and 61.8% answered that they wouldn't consider other kinds of job even if offered. In case of oriental doctors, these responses were 83.1%, 82.5%, and 82.3%, respectively. Conclusions : The job stress scores of western doctors were higher than those of oriental doctors, but the job satisfaction was lower. The reasons for the above results are not clear. Further studies are required to understand the characteristics of job stress and job satisfaction in doctors.

Korean Medicine Doctor's Perception in Korean Medicine Clinical Practice Guideline and Korean Medicine Cliniical Pathways (한의 표준임상진료지침 및 한의 표준임상경로에 대한 한의사의 인식)

  • Objectives : This study is aimed to survey Korean Medicine Doctors's perceptions in Korean Medicine Clinical Practice Guideline and Standard Korean Medicine Clinical Pathways for its spread(to increase utilization in clinical sites). Methods : The research population was set at 14,831 Korean medical institutions registered with the Health Insurance Review and Assessment Service for sampling representative of Korean Medicine Doctors, and the final 2,007 subjects were selected in consideration of the research period and budget. This survey was conducted based on a telephone survey, and in some cases, a fax or e-mail survey was also conducted together. Six questions were asked about the perception of 'Korean Medicine Clinical Practice Guideline and Standard Korean Medicine Clinical Pathways', and three questions about the characteristics of the respondent. Results : The rate of recognizing Korean Medicine Clinical Practice Guideline was 36.1%. Those who worked at Korean medicine hospitals, were under 39 years of age, and had less than 11-20 years of experience as an Korean medical doctor were more likely to be aware of it. Regarding the experience of using Korean Medicine Clinical Practice Guideline, the rate of 'not used in the past and not used now' was very high at 82.9%, but the intention to use it in the future was high at 60.7%. About the Korean Medicine Clinical Pathways, 79.9% of respondents answered that they did not know. 80.6% of respondents recognized the need for the development of clinical manuals such as Korean Medicine Clinical Practice Guideline and Standard Korean Medicine Clinical Pathways. Conclusion : Their low awareness of Korean Medicine Clinical Practice Guideline seems to affect its low utilization. Therefore, active education and public relations are required in the future.

A Study of Knowledge, Attitude, and Practice Relative to Maternal and Child Health Among Women Residing in Apartments at Yonsei Community Health Area (연세지역 아파트 주민의 모자보건에 관한 실태조사)

  • A study of the knowledge, attitude and practices about the maternal and child health of 305 married women residing in apartments at the Yonsei Community Health area was conducted during the period from November to December 1970 using designed questionnaire with well trained interviewers. The results and findings obtained from the study are summarized as follows: A. Pregnancy and Birth Questions were asked about their last child. 1. 16.4% of the women were pregnant. 2. Among 281 women who had experienced delivery, 48.0% were assisted by doctor or midwisves for their last delivery, while the rest of women delivered their last baby at home without any professional's assistance. The higher the level of education or the greater exposure to mass communication, the more the deliveries were assisted by doctors or midwives. Those women who were born and raised in cities had more deliveries assisted by doctors and midwives than those who were not. 3. Kinds of delivery sheets used. Among 141 cases of home delivery 68% used cement bag paper or vinyl sheets. Three% used nothing and remained used unsterile materials. 4. Among 141 cases of home delivery, 70.2% used scissors. The rest of them used other methods. 5. 47.3% of the women had a rest for one month or more after birth. The higher the level of education, the longer the period of rest was observed. 6. 52.4% of the women fed the colostrum to their babies. This was not related to the mother's education. 7 About half(42.9%) of the women had poor knowledge about a proper diet for the pre and post natal period. B. Child Health 1. Knowledge and practice regarding to the immunization for their children: Most of the women (93.2%) could name at least one kind of immunization. 20.3% could name 6 kinds of immunization. Mothers education level did not influence their ability to name immunizations. 85.2% of children had been immunized at least once. 2. Morbidity of last born children: 48.1% of their last born children were found to have been sick during the last year. Less than half(41.5%) of the sick children were seen by doctor. 3. Counselling at well baby clinic: Most of the women(76.5%) had no counselling for their children. Registration rate at the well baby clinic at the Severance Hospital was 13.2%. 45.9% wanted to visit to the well baby clinic at the Severance Hospital. 4. Weaning Period: 44.6% said that the beginning of the weaning for their last born children was from 6 months to twelve months of age. The most important reason of weaning was the health of both mothers and children. 5. Knowledge and Practice regarding birth and death Registration: 64.6% of the women could name correctly the Ku-office as the place for the registration. Only 29.2% registered the birth of their last born children within 14 days. C. Knowledge, Attitude and Practice regarding to family planning Most: of the women accepted the idea of family planning. 97.7% could name at least one contraceptive method. 35.4% were found to be current users of contraceptive methods. The ideal number of children was 3.1 in average.
