An analysis of contributing factors to financial status of regional health insurance (지역의료보험조합의 재정 상태에 영향을 미치는 요인분석)

  • Finances of health insurance can be explained by factors determining benefit expense and premium collection. This study was conducted to analyze factors contributing to the financial status of rural health Insurance. Nationwide 134 health insurance associations except the six pilot project counties were analyzed and obtained the followings. 1. In univariate analysis, statistically significant variables that explain 1) outpatient benefit expenditures include public health center utilization, proportion of pregnant women. premium and collection rate of premium 2) inpatient benefit expenditures include public health center utilization, Proportion of old age, proportion of pregnant women, premium and collection rate of premium 3) profits include public health center utilization, proportion of old age, proportion of pregnant women and collection rate of premium. 2. In multiple regression analysis, statistically significant determinants in 1) outpatient benefit include premium and public health utilization 2) inpatient benefit include premium 3) profit include public health center utilization, premium and collection rate of premium.

Cost Effectiveness of Interventions to Promote Screening for Colorectal Cancer: A Randomized Trial

  • Objectives: Screening for colorectal cancer is considered cost effective, but is underutilized in the U.S. Information on the efficiency of "tailored interventions" to promote colorectal cancer screening in primary care settings is limited. The paper reports the results of a cost effectiveness analysis that compared a survey-only control group to a Centers for Disease Control (CDC) web-based intervention (screen for life) and to a tailored interactive computer-based intervention. Methods: A randomized controlled trial of people 50 and over, was conducted to test the interventions. The sample was 1224 partcipants 50-70 years of age, recruited from Kelsey-Seybold Clinic, a large multi-specialty clinic in Houston, Texas. Screening status was obtained by medical chart review after a 12-month follow-up period. An "intention to treat" analysis and micro costing from the patient and provider perspectives were used to estimate the costs and effects. Analysis of statistical uncertainty was conducted using nonparametric bootstrapping. Results: The estimated cost of implementing the web-based intervention was $40 per person and the cost of the tailored intervention was $45 per person. The additional cost per person screened for the web-based intervention compared to no intervention was $2602 and the tailored intervention was no more effective than the web-based strategy. Conclusions: The tailored intervention was less cost-effective than the web-based intervention for colorectal cancer screening promotion. The web-based intervention was less cost-effective than previous studies of in-reach colorectal cancer screening promotion. Researchers need to continue developing and evaluating the effectiveness and costeffectiveness of interventions to increase colorectal cancer screening.

Opinion Survey of Health Center Officers on Rural Health Service Improvement Project (농어촌의료(農漁村醫療)서비스 개선사업(改善事業)에 대한 보건소(保健所) 공무원(公務員)의 인식도(認識度))

  • This study was performed investigate the opinion of civil servants in Health center about Rural Health Service Improvement Project The survey by mail was carried out for 447 servants of 25 health centers in Kyungsangbuk-do and the data were collected through self-administered questionnaires to servants about need, participation, concern, and comprehension for the project and satisfied with current facility and equipment of health center. The results were as follows. Generally considered, 48.2% of the improved health center servants was satisfied with health center building and 14.0% or 24.1% of the improving or unimproved center was. About the location of health center, 37.7% of the improved health center servants was satisfied, 25.9% of the unimproved center was. Of the improved health center servants, 43% was satisfied with the medical equipment but in unimproved place, the dissatisfaction was appeared higher than any other place. 49.7% of respondents was participated in making out the Rural Health Service Improvement Project. 50.6% was interested in this project. In the improved area. 65.5% of health center servants replied that the mayor's or county executive's concern about this project was high and 46.5% in councilors but in the unimproved area. their concern was low. About the contents of the project. 24,6% of the servants in the improved center, only 15.2% in unimproved center replied that they had known well. After making out the plan, 13.6% of respondents was unsatisfied with this plan and 17.1% replied that the estimating method of selecting the project area was not good. After the improvement of institution and equipment, 86.1% of health center servants answered that the medical service provided by health center would increase but 59.2% replied that the residents' utilization rate of private medical facility would decrease. The servants of the improved health center replied that the recognition about the developing will of health service(91.2%), the efficiency(91.2%), the quality of health and medical service(93.0%), the amount of health project(91.2%) were improved. In health center which had already improved the institution and equipment, 88.5% of servants replied that the residents' utilization for health center was increased. So, this project should be continuously carried out for health center and health center must develope new project to fit region condition.

  • The purpose of this study is to suggest the possibility of complexity use these facilities for elderly welfare facility or public health facility and community center in rural. For this purpose this article researched into elderly welfare facilities, public health facilities and social welfare centers. And we studied of village community center, life pattern in old people in rural. With a rapidly increase in population of older people in rural, there is a great demand for the construction of complexes facilities. Particularly, there was highly demand for the public health facilities and the welfare programme of physiotherapy facilities on village community center in rural. However, there is not a intersection of space composition and specification function between elderly welfare facilities and public healthcare facilities and community welfare centers in this time. Accordingly, in the future plan for community center in rural, it is necessary to consider integration with public health facility. And it is necessary to compose the community center in consideration of the spatial organization of complexity as a possibility of community welfare activities.

The past and future of health promotion program in health center (보건소 건강증진사업의 과거와 미래)

  • This paper reviews the past of community-based health promotion program through public health center to suggest the direction of future. The Minister of Health and Welfare has implemented health promotion demonstration programs at 18 public health centers in September 1998. Health promotion programs were spread nationwide in 2005. Now, 251 public health centers have performed health promotion programs. Health promotion includes both actions towards changing determinants, within the more immediate control of individuals, including individual health behavior, and those factors largely outside the control of individuals, including social, economic and environmental conditions. Direction of health promotion programs in public health is divided into two categories: creating environment for healthy lifestyle and health promotion services. The result of this paper will be able to act as a guide for future operation plan in health center.

Survey on the development of the public dentist's professional education by the participating dentists of the 2008 public dentist's education (2008년도 직무교육참여 공공기관치과의사 대상 전문교육발전방안에 관한 설문조사 연구)

  • This study was conducted to investigate the effects of the public dentist's professional education for 81 dentists employed at the public health centers and dental clinics in the National hospitals. The purpose of this education was to improve public dentist's abilities to perform their duties. The 3days education was from 28 May until 30 May in 2008. The total 81 participants consisted of 8 medical position dentists, 20 employment position dentists, and 53 public health dentists. The survey provided appraisal of the education and suggestion of the development after this education. The obtained results were as followings : l. The public dental health project for the disabled people(91.4%) was a top priority. The second rank was the public dental health education project (87.6%) and the public dental health project for the aged people(86.4%) was followed. 2. The participants agreed that public dentist's professional educational programs were necessary(87.6%). 3. The participants did not have difficulties in attending this education(56.8%). 4. The participants who participated in 2008 public dentists' professional education were satisfied with this educational programs(81.4%). 5. Not only did the public dentists need the preventive dentistry programs and the treatment of the disabled people but also needed the health administrative programs and the health statistical programs in the future.

A Study on Health and Public Health Center Utilization Behavior for lower Income Family in Korea (전국 차상위계층 주민의 건강행위와 보건소 이용행태에 관한 연구)

  • The purpose of this study was to analyze the relation between health behavior and public health center utilization among lower income families who earned half of the average Korean family income. The cross-sectional descriptive survey research we conducted was a nationwide randomization sampling among lower income families. The data was collected from July 12 to August 7. 1999 and the total sample was 5.819 household members 1,735 households). There were, three major findings according to these studies. 1. In health behaviors, 26.5% of respondents had a health examination. Among them, 41.5% were in a poor condition of health. In health promotion behaviors, only 37.3% of respondents were doing activities for health maintenance or promotion. 2. In the results of the public health center utilization for the past year. 57.4% of respondents visited one or more times. In addition, 46.2% of respondents wanted to use a visiting nursing service. 3. In the results of multiple logistic regression, we found that less education, larger family size, and medicaid affects more utilization of public health center. In addition, older age, living without a spouse, less education, larger family size, lower family income. and public health center usage affects requests for the visiting nurse service.

A Study on the Management and Effect of Dysmenorrhea Care Program by Korean Traditional Medicine (한방 월경통 개선교실의 운영 및 효과에 관한 연구)

  • Objectives: The aim of this study was to evaluate the effectiveness of dysmenorrhea care program by Korean traditional medicine on dysmenorrhiec juveniles. Methods: 47 adolescent dysmenorrhiec patients in local girls' high school took part in dysmenorrhea care program by Korean traditional medicine. The subjects were treated by acupuncture, ear-acupuncture, acupressure education, herbal extract medication and qigong exercise. The results were investigated by visual analogue scale(VAS), multidimensional verbal rating scale(MVRS) and verbal rating scale (VRS). Data was collected every three months from March, 2011 to March, 2012. Additionally satisfaction survey was conducted. Results: VAS score was reduced after treatment, but rebounded back to baseline after 6 months of discontinued dysmenorrhea care program. There was no significant difference of time and group interaction in linear mixed model analysis. MVRS and VRS outcomes showed similar pattern. Conclusions: Dysmenorrhea care program by Korean traditional medicine is effective in juveniles with dysmenorrhea for several months but not for long as nine months after treatment. However, the effect can last for three months at the very least.
