Is It Possible to Replace Microendoscopic Discectomy with Percutaneous Transforaminal Discectomy for Treatment of Lumbar Disc Herniation? A Meta-Analysis Based on Recurrence and Revision Rate

  • Objectives : Due to recent developments and the wide application of percutaneous transforaminal discectomy (PTED) in China, we herein compare its clinical effects with microendoscopic discectomy (MED) for the treatment of lumbar disc herniation in terms of recurrence and revision rates. Methods : Six databases, namely, PubMed, EMBASE, Cochrane Library, Ovid, China National Knowledge Infrastructure and Wanfang, were searched by computer. The literature was screened according to inclusion and exclusion criteria, and the quality of the included literature was evaluated. After extracting the data from the papers, Review Manager 5.2 software (Cochrane Collaboration, Oxford, UK) was applied to analyze these data. Finally, sensitivity and publication bias analyses of the results were conducted. Results : A total of 12 studies consisting of 2400 patients were included in this meta-analysis. A comparison of PTED with MED revealed higher postoperative recurrence and postoperative revision rates for PTED (odds ratio [OR] recurrence, 1.60; 95% confidence interval [CI], 1.01 to 2.53; p=0.05 and OR revision, 1.77; 95% CI, 1.18 to 2.64, p=0.006). Conclusion : PTED has a number of advantages because it is a minimally invasive surgery, but its recurrence and revision rates are higher than MED. Therefore, MED should not be completely replaced by PTED.

A Study on the Retrieval Effectiveness of KoreaMed using MeSH Search Filter and Word-Proximity Search (검색용 MeSH 필터와 단어인접탐색 기법을 활용한 KoreaMed 검색 효율성 향상 연구)

  • This study examined the method for adding related to "stomach neoplasms" as filters to the Medical Subject Headings (MeSH) for search as well as a method for improving the search efficiency through a word-proximity search by measuring the distance of co-occurring terms. A total of 8,625 articles published between 2007 and 2016 with the major topic terms "stomach neoplasms" were downloaded from PubMed article titles. The vocabulary to be added to the MeSH for search were analyzed. The search efficiency was verified by 277 articles that had "Stomach Neoplasms" indexed as MEDLINE MeSH in KoreaMed. As a result, 973 terms were selected as the candidate vocabulary. "Gastric Cancer" (2,780 appearances) was the most frequent term and 7,376 compound words (88.51%) combined the histological terms of "stomach" and "neoplasm", such as "gastric adenocarcinoma" and "gastric MALT lymphoma". A total of 5,234 compounds words (70.95%), in which the co-occurring distance was two words, were found. The matching rate through the MEDLINE MeSH and KoreaMed MeSH Indexer was 209 articles (75.5%). The search efficiency improved to 263 articles (94.9%) when the search filters were added, and to 268 articles (96.7%) when the 13 word-proximity search technique of the co-occurring terms was applied. This study showed that the use of a thesaurus as a means of improving the search efficiency in a natural language search could maintain the advantages of controlled vocabulary. The search accuracy can be improved using the word-proximity search instead of a Boolean search.

Effects of Exercise on Endothelial Progenitor Cells in Cardiovascular Disease Patients: A Systematic Review (운동중재가 심혈관질환자의 혈관내피전구세포에 미치는 영향: 체계적 문헌고찰)

  • In this study, we performed a systematic review and meta-analysis to identify the effects of exercise on endothelial progenitor cells (EPCs) in patients with cardiovascular disease (CVD). We conducted database searches (Cochrane Library, PubMed, EMBASE, ScienceDirect, CINAHL, Scopus, KoreaMed, KISS, RISS, KMBASE) for the effect of exercise on cardiovascular disease, using heart disease, coronary artery disease, heart failure, cardiovascular disease, exercise, motor activity, rehabilitation, and endothelial progenitor cells as the keywords. Of the 539 studies identified, 9 met the inclusion and exclusion criteria. Comprehensive Meta-Analysis version 2.0 was used to analyze the effect size and the publication bias was checked with a funnel plot. Exercise was found to improve the VEGF (vascular endothelial growth factor), CD34+KDR+, and endothelial function, assessed via FMD (flow-mediated dilation), in the exercise vs. control groups, viz. 2.008 (95% CI 0.204-3.812), 1.399 (95% CI 0.310-2.489), and 1.881 (95% CI 0.848-2.914), respectively. Exercise improved the VEGF, number of EPCs, and endothelial function in the CVD patients. Considering the increasing prevalence and mortality rates for cardiovascular disease in Korea, the findings of this study that analyzed the effects of exercise on EPCs might provide guidelines for planning exercise interventions for patients with CVD.

Somatic Symptoms after Psychological Trauma (심리외상 이후의 신체증상)

  • Objectives : Somatic symptoms after the exposure of psychological trauma frequently developed. However, the somatic symptoms are not covered under the diagnostic criteria of posttraumatic stress disorder(PTSD) in detail, although they are often associated with social and occupational functioning and patient-doctor relationships. The aim of this article is to highlight the potential mechanisms, the common manifestations, and the treatment of the somatic symptoms. Methods : This article studied the somatic symptoms searched using academic search engines like PubMed, Scopus, Google Scholar, KoreaMed and KISS from the earliest available date of indexing to March 31, 2016. Results : The mechanism of somatic symptoms after the exposure was described as psychological and physiological aspects. Psychological mechanism consisted of psychodynamic theory, cognitive behavioral theory, and others. Physiological mechanism involved changes in neuroendocrine and immune system, autonomic nervous system and central nervous system. Somatization associated with psychological trauma manifested various health conditions on head and neck, chest, abdominal, musculoskeletal, and dermatological and immune system. Few studies described the standardization of treatment for the somatic symptoms. Conclusions : Clinicians and disaster behavioral health providers should think of the accompanying somatic symptoms during intervention of psychological trauma and PTSD. Further studies are needed on the somatic symptoms seen in psychological trauma and PTSD.

Occupational Therapy for Community Mobility in Stroke Patients : Systematic review (뇌졸중 환자의 지역사회이동을 위한 작업치료 중재: 체계적 고찰)

  • Objective : The purpose of this study was to analyze occupational therapy intervention on the community mobility for stroke patients, and to provide evidence of intervention in the clinical fields. Methods : A systematic review was executed according to the PRISMA checklist. The accessed database was PubMed, EMBASE, Cochrane Library (CENTRAL), ProQuest Dissertations & thesis (PQDT), RISS, and KoreaMed. We included the articles published from 2005 to September 2018. RoBANS checklist was used to evaluate the quality of the articles. Included articles, totally eight, were categorized according to the type of intervention. Results : The study design of the literature was varied from two-group randomized trial, quasi-experimental study, case-control trial, one group pre-post comparison study, and cross-sectional study. In the evidence level, 6 articles were included in level II (75%). The percentage of low risk of bias in each article ranged from 52.5%~87.5%. Four studies (50%) provided intervention based on virtual reality or virtual environment. The three (37.5%) provided intervention based on the protocol, and the other (12.5%) did wheelchair training. All studies reported significant effects of the intervention. Conclusion : This systematic review provided evidences to use proper intervention in the clinical fields. Various type of studies should be conducted to prove the effect of occupational therapy intervention for community mobility.

Effect of Helicobacter pylori Eradication on the Prevention of Gastric Cancer in the General Population with or without Precancerous Lesions: A Meta-analysis (일반 인구 집단에서 전암성 병변 유무에 따른 헬리코박터 제균 치료의 위암 예방 효과 연구: 메타분석)

  • We performed a comprehensive meta-analysis on whether Helicobacter pylori (H. pylori) eradication can prevent gastric cancer in individuals with precancerous lesions (chronic atrophic gastritis, intestinal metaplasia, and dysplasia). The studies evaluating the effect of H. pylori eradication on gastric cancer were searched from PubMed, KoreaMed, EMBASE, and Cochrane Library. Subgroup analyses by precancerous lesions, regions and follow-up time were performed. Six randomized clinical trials were included. In the overall population, H. pylori eradication significantly decreased the risk of gastric cancer (risk ratio [RR]=0.56; 95% confidence interval [CI]: 0.41-0.77, p<0.01). Eradication therapy significantly reduced the risk of gastric cancer in Asian and group who followed up more than 10 years (RR=0.51; 95% CI: 0.35-0.73, p<0.01; I2=0%, p=0.46). However, subgroup analysis according to the precancerous lesions did not show any significant results (p>0.05). In the general population especially in Asian population, H. pylori eradication can reduce the risk of gastric cancer. The effect of the eradication treatment is remarkable upon the long-term follow-up. In contrast, there is insufficient evidence whether the eradication should be recommended for the individuals with precancerous lesions.

Systematic Review on the Effect of Forest Healing Activities on the Elderly (산림치유 활동이 노인에게 미치는 효과에 대한 체계적 문헌고찰)

  • Life expectancy is increasing due to the aging of the population, which is in turn exacerbating problems such as the prevalence of various geriatric diseases. This study was established to provide basic data for the expansion of forest healing activities for the elderly by systematically analyzing the literature on how such activities affect this age group. For the collection of studies, the Korean databases RISS, KISS, Korea Med, and Science On were used, while PubMed, Cochrane Central, MDPI, and Google Scholar were used to identify reports published elsewhere. To assess the quality of the methodology used in the collected studies, the risk of bias was analyzed using Cochrane's RoB2 and RoBANS. Among 1,856 reports initially identified, 21 were finally selected for analysis in this study, which were limited to research papers on forest healing activities for the elderly published between 2000 and January 2022. In this review, the subjects were those aged 60 or older, with a total of 750 participants, ranging from at least 7 to a maximum of 88 per study. The analysis showed that the most frequently performed tests in each category were on depression as a psychological indicator in 7 studies, MMSE(Mini Mental State Examination) as a cognitive indicator in 2 studies, on blood pressure as a physiological indicator in 4 studies, on melatonin as a biochemical indicator in 2 studies, and on body fat and muscle strength as physical indicators in 3 studies. Of the 21 studies, 19 used two or more test items, with psychological indicators being most commonly measured. For the future application of forest healing activities for the elderly, various forest healing programs to prevent cognitive function decline should be developed and distributed, and follow-up studies should be continuously presented to provide the basis for forest healing activities.

Systematic Review of the Research Evidence Examining the Effectiveness of Occupational Therapy Using a Sensory Integration Approach (감각통합에 기반을 둔 작업치료효과에 관한 체계적 고찰)

  • Objective : This study aimed to systematically reviewed to identify, evaluate, synthesize the research literature and suggested to the research direction on the effectiveness of occupational therapy using a sensory integration approach. Methods : We analyzed 10 studies that identified the effectiveness of occupational therapy using a sensory integration approach for children and adolescents offering in MEDLINE/PubMed and http://ajot.aotapress.net between 1993 and 2011. Results : The subjects were the sensory modulation disorder and autism spectrum. The different intervention strategies were similar to a previous studies. Intervention dosage was over 20 sessions, 2~3days per week more than 10weeks. Occupational therapy using a sensory integration approach may result in positive outcome in motor performance, behavioral outcomes, academic and psychoeducational outcomes and tried to currently identify the effect of intervention outcomes in sensory processing and occupational performance using the GAS. Conclusion : Clinicians have a minimal idea planing a research design and a evidence running a study.

Review on Cohort Projects in Domestic and Overseas and Establishment of Cohort on Korean Medicine (국내외 코호트 사업 고찰 및 한의학 코호트 구축)

  • Objectives : In this study, we investigated the trends of cohort project in domestic and overseas, and suggested the necessity of cohort on korean medicine and the direction of research. Methods : The papers which include cohort in the title and published by 2015 were searched using PubMed. In order to study the history of cohort in domestic and overseas, research trend was investigated based on the websites of each cohort projects and traditional medicine searching engine named OASIS. Results : Using PubMed, 42,802 papers were searched. Most of the studies over 50 % of total articles, however, were conducted in these 6 years. Nevertheless, cohort studies are being actively proceed, the korean medicine based on cohort studies are nonexistent. Therefore, further discussion about cohort study on korean medicine is necessary to gather physiological index and clinical index of korean medicine consistently. Conclusions : Systematically constructed cohort on korean medicine would be the foundation which collects information, such as gene information, bio information, dietary habit, life style, and Pattern identification (辨證), and integrates them. The analysis of massive clinical materials could suggest the basis of Prevention and treatment of korean medicine considering the difference between personal.

The Latest Trends of Acupuncture Therapy on Obstetrics (Pub Med 검색을 통한 침구치료의 산과적 응용에 대한 최근 연구 고찰)

  • Purpose: We aimed at reviewing the studies of the acupuncture treatment for women related to pregnancy. Methods: We referred to PubMed by searching words of pregnancy and acupuncture and limited experimental method to randomized control trial. Results: We found 52 articles in 32 journals, which were divided into 3 parts: treatment for symptoms during pregnancy, treatment for symptom related to delivery and treatment for symptoms related to surgical operation for IVF-ET. In this study, articles on hypermesis gravidarum were the greatest in number and acupuncture for pain control related to pregnancy are most frequently appearing in terms of symptoms. Besides acupuncture treatment, acupressure band and acupressure were used for hyperemesis gravidarum, common acupuncture for low back pain in pregnancy and acupuncture or moxibustion to resolve breech presentation. Conclusion: In this study, acupuncture seems to be an efficacious means of reducing symptoms related to pregnancy and improving the quality of life of pregnant women.
