Effect of Clinical Improvement of Schizophrenic Symptoms on $^{99m}Tc$-HMPAO Brain SPECT (정신분열병 환자의 임상적 증세 호전에 따른 $^{99m}Tc$-HMPAO 뇌 SPECT 소견의 변화)

  • This study investigated regional blood flow changes of frontal, temporal, and basal ganglia in eleven schizophrenic patients on DSM-IV criteria to examine the relationship between rCBF and clinical improvement of symptoms. Single-photon emission computed tomography imaging with $^{99m}Tc$-HMPAO was peformed in baseline and sixth weeks after the treatment, and concurrently psychopathology was assessed by PANSS. Antipsychotics wash-out period was more than 2 weeks, and three patient were drug naive. All patients were finally divided into two groups, the improved or not improved. We examined the difference of the amount of rCBF changes between two groups. Finally, frontal activity shows no significant difference between two groups but both groups show decreased frontal blood flow after antipsychotic treatment. However, the change of right temporal rCBF had positive correlation with the change of the total PANSS score, and the change of left temporal lobe activity was greater in the improved group than in the not improved group. Our results suggest that the temporal lobe activity has relation to the underlying schizophrenic symptoms.

Nonlinear Dynamic Analysis of EEG in Patients with Positive and Negative Schizophrenia (양성 및 음성 정신분열증 환자 뇌파의 비선형 역동 분석)

  • Objectives : The hypothesis that the brain is a nonlinear dynamical system exhibiting deterministic chaos has offered new perspectives to the investigation of information processing in the brain of schizophrenic patients. It seemed worthwhile to estimate nonlinear measures of the electroencephalogram (EEG) in positive and negative schizophrenics, because nonlinear measures might serve as indicators of the specific brain function in schizophrenia according to specific psychopathologies. Method : Previous studies which estimated the chaoticity in the brain of schizophrenia with nonlinear methods recorded the EEGs at limited electrodes, so we tried to record EEGs from 16 channels for nonlinear analysis in 8 positive and 9 negative schizophrenics and 8 healthy control subjects. We employed a new method to calculate the nonlinear invariant measures. For limited noisy data, this algorithm was strikingly faster and more accurate than previous ones. Results : Our results showed that the patients with negative schizophrenia had lower the first positive Lyapunov exponents ($L_1$) than the positive schizophrnics and control subjects at $T_3$ lead. Positive symptoms were positively correlated with $L_1$ in $C_3,\;O_1$ leads, and negatively correlated with $C_4$ lead. Conclusion : These results suggest that if clinical variables such as psychopathology or neuroleptic medications would be well controlled, the nonlinear analysis of the EEGs in patients with schizophrenia seems to be a useful tool in analyzing EEG data to explore the neurodynamics.

COMORBID PSYCHOPATHOLOGY AND PARENTAL BEHAVIORS IN TIC DISORDER CHILDREN (틱 장애아(障碍兒)들의 동반 정신병리(同伴 精神病理)와 부모양육태도(父母養育態度)에 관(關)한 연구(硏究))

  • Clinical Characteristics of 83 tic patients referred to child psychiatric clinics were studied including the patients' sexes, birth orders and onset ages. We compared the differences between patients and normal control regarding the co-morbidity, and mothering attitude using CBCL(Child Behavioral Check List) and MBRI(Mothering Behavior Rating Instrument). And we also evaluated the influence of types of tic disorder and the presence of behavioral characteristics of the tic patients. The following results were obtained. 1) The sex ration was 5.9:1, male dominant. 2) The eldest children were being twice as many as the youngest children(eldest : youngest : single=4.7 : 1.7 : 1). 3) The eldest children tended to have earlier onset than others. 4) The tic children as a whole had more accompanying behavioral problems than the normal children according to the CBCL scales' scores. 5) The mothers of tic children had more negative view of their children, more rejecting and more hostile attitude toward their children. 6) The types of tic disorder(the Tourette disorder vs chronic moter tic disorder) did not make a difference in the incidence of behavior problems. 7) Those who had attentional problems regardless the types of tic had more behavioral problems than those who had not. 8) Those who had familial loadings of tic disorder tended to have more likely Tourette disorders than chronic tic disorders.

  • Objects : There is considerable interest in the role of serotonin(5-HT) in the pathophysiology of schizophrenia. Cimetidine, $H_2$ antagonist, produces transient increase in serum prolactin(PRL) levels by indirect serotonergic mechanism in man following intravenous administration. Therefore the authors investigated the effects of cimetidine on serum PRL levels of male unmedicated schizophrenics. Method : Baseline serum prolactin level and psychopathology were measured at 9:00 AM. in the two groups(12 positive schizophrenics, 7 negative schizophrenics) and $T_{30}$ levels were measured 30 minutes after intravenous injection of cimetidine (ie, 9:30 AM) Results: 1) Baseline prolactin levels were not different in the three groups. 2) Prolactin levels of 30 minutes after intravenous injection of cimetidine($T_{30}$) compared with baseline prolactin levels were increased all in the three groups. 3) Degrees of interval change from baseline to $T_{30}$ were significantly different between normal control and negative schizophrenics(p<0.05). Conclusion : The prolactin response to cimetidine was significantly blunted in negative male schizophrenics than normal control. These data are consistent with the hypothesis of an abnormality of serotonergic activity, including down-regulation $5-HT_2$ receptors, in male negative schizophrenics.

  • Human being can be defined in several ways. These are biological, psychological, social and religious, ethical and philosophical aspects. The first two viewpoints refers to the intrapersonal aspects and the last two viewpoints deals with the interpersonal aspects. There have been numerous thoeries and hypotheses to explain the human behavior and psychopathology, but the author does not find any theory or hypothesis to integrate the biological and psychological viewpoints. In addition, The author does not find any theory to integrate the intrapersonal and interpersonal aspects in interpersonal relationships. Thus, the author proposed one model to explain the human behavior. In proposing this theory, the author has the following hypotheses:1) All human being can not exist by oneself. 2) The ultimate goal of all human being is to achieve 'Survival'. 3) There are four kinds of survivals and they are biological, psychological, social and religious, ethical and philosophical survivals. 4) Power and control are two prerequisites for survival and there are four kinds of power and control. 5) The four elements of power and control are biological, psychological, internal and external ones. 6) The conflict between buman being can also be explained in terms of power, control and survival. 7) Normality and abnormality of human behavior can be explained in terms of the harmony and balance among the powers and controls.

Internet Game Addiction and Emotional and Behavioral Characteristics in Upper Grade Elementary School Students and Middle School Students (인터넷 게임 중독에 있어서 초등학생 고학년군과 중학생군 간의 정서 및 행동특성의 차이)

  • Objectives : This study was aimed at investigating the relationship between internet game addiction and emotional and behavioral characteristics of students both in the 5th and 6th grade of elementary school and in middle school. Methods : Two elementary schools and two middle schools were chosen to participate. The students completed selfreport questionnaires designed by the authors, called the Internet Game Addiction Scale, Korean youth self-report (K-YSR). Results : Compared to previous studies, the rates of those in the obvious game addiction group were much lower in this study. We defined the upper 10% of internet game addiction scores as a high risk user group and the lower 10% as a control group. There were significant differences between the groups with respect to the the ages at which internet gaming began, the frequency of game play per week, and the average length of each game playing session. Significant associations were also found between the level of internet game addiction and the withdrawn and delinquency subscales of the K-YSR. However, the association between game addiction and the withdrawn subscale was found only in middle school students. Conclusion : The findings from this study suggested that withdrawn and delinquent behaviors could be predictors of internet game addiction. Results also suggested the importance of early childhood intervention for preventing the development of more severe psychopathology in early adolescence.

Systematic Review on the Use of Sasang Personality Questionnaire in Traditional Korean Medicine (사상성격검사(SPQ)의 한의학적 활용 현황에 대한 체계적 문헌 분석)

  • Objectives: Operational definition and its validated measure of traditional Korean medicine (TKM) are pivotal for research studies and reliable clinics. There have been ten years of clinical studies on Eum-Yang (Yin-Yang) and Sasang typology using Sasang Personality Questionnaire (SPQ) of integrative biopsychology. The objective of this study was to perform a systematic review on the use of SPQ to provide insights and foundation for its further studies and clinical application. Methods: Clinical studies on SPQ were searched using five domestic and foreign research databases. Attained research articles were then subjected to predefined systematic reviews. Demographic characteristics of participants and research topics of articles were described. Psychological and physical features of SPQ and its subscales were reviewed and compiled to reveal biopsychological characteristics. Results: There were 33 articles with 15,085 participants on Eum-Yang (n=19) and Sasang typology (n=14). The SPQ-total score was in an increasing order of So-Eum, Tae-Eum, and So-Yang. Eum-Yang groups were categorized at a 3:4:3 ratio using %ile score of SPQ-total. The SPQ-total showed a significant positive correlation with Novelty-Seeking and a negative correlation with Harm-Avoidance in psychological perspective, while it was not significantly correlated with Body Mass Index or Ponderal Index in physical perspective. The SPQ-total and subscales explained psychopathological characteristics of stress recognition, negative emotion, problem behavior, and depression. Conclusions: The SPQ was found to be an objective biopsychological measure of TKM for Sasang typology and psychopathology. It would provide foundation for 'Korean Psychology' and multidisciplinary studies.

Psychiatric Manifestations of Anti-NMDA Receptor Encephalitis: A Case Report (항-NMDA 수용체 뇌염의 정신증상: 증례보고)

  • Anti-N-methyl-D-aspartate receptor (Anti-NMDAR) encephalitis is a neuroinflammatory disease mediated by autoantibodies to NMDAR. In the initial clinical stages of anti-NMDAR encephalitis, psychiatric symptoms like delusions, perceptual disturbances, and disorganized speech or behaviors are pronounced even without obvious neurological symptoms. Early treatments like immunotherapy and/or tumor removal are central to good clinical outcomes. Hence, it is important to diagnose early anti-NMDAR encephalitis, distinguishing it from mental disorder. In the present case study, the authors described psychiatric symptoms assessed with Positive and Negative Syndrome Scale (PANSS) of Ms. A, a 26-year-old woman, in the early phase of anti-NMDAR encephalitis. We will discuss the characteristic psychopathology of anti-NMDAR encephalitis toward prompt diagnosis and treatment. Ms. A showed a higher negative subscale score than positive one on the PANSS. Compared with mental disorder, negative symptoms and cognitive impairment would be more prominent in the early stage of anti-NMDAR encephalitis. Rituximab and teratoma removal were effective, and quetiapine showed good tolerability. It is recommended to evaluate anti-NMDAR encephalitis when negative symptoms, cognitive impairment, catatonia, changes in consciousness level, and neurological symptoms are observed, especially in young women.

Research Trends of Temperament and Character Inventory in Korean Journals Published from 1998 through 2018 (기질 및 성격 검사(Temperament and Character Inventory: TCI)의 국내 연구 동향: 1998-2018년 학술지 게재 논문을 중심으로)

  • The purpose of the present study was to investigate the research trends of published articles in Korean journals dealing with Temperament and Character Inventory (TCI) from 1998 through 2018. A total of 185 papers were analyzed about an affiliation of corresponding author, type and dimension of TCI, and research contents considering the major publication regarding the Korean version of the TCI and major event in Korea. The results were as followed. First, most researchers belonged to university (53%) and hospital (44%). Second, both temperament and character dimensions (65%) were often used rather than temperament dimension only (34%). Third, regarding the subjects, adults (52%), children and adolescents (38%), and preschoolers (10%) appeared the most frequently used in the order. Fourth, we found that psychopathology (36%), personality (22%), health (16%), addiction (10%), assessment (8%) were the most popular areas of concern. Based on these results with the focus of analysis on the relationships between research subjects and research contents, implications and directions for future research and clinical settings regarding the treatment effect using the TCI character dimensions and diverse age groups were discussed.

A Review and Suggestion for Korean Depression study from perspective of Cultural Psychology (한국인 우울 연구를 위한 문화심리학적 고찰과 제언)

  • This study is performed to fine a way to illuminate the direction of depression study suitable to Korean culture. To do so, it is found that from when we started to use the term of 'depression' and how we Koreans conceptualize depression. And cultural psychological perspectives to psychopathology including depression are reviewed. Finally, based on these findings, the ways to study depression with the perspective of cultural psychology are suggested. Specifically speaking, at the first, it is found that the term of depression has not been used before colonialized times by Japan, but terms of 'Ul-Uep', 'Kwang-Zeung', and 'Jun-Kwanng' are used from at least times of King SeJong. Term of depression was actually used after liberation from Japan and spread after introduction of DSM to Korea via Japan. Second, the way to compromise universalist and relativist perspectives is recommended and the perspective of absolutist is not recommended. Lastly, in Korea depression shares with 'HwaByung' in some area, but not with 'Han'. In conclusion, it is desirable to combine etic factors(universal perspective) and emic factors(culture-relative perspective) to study depression suitable for Korean culture.
