내시경 점막하 박리법을 시술 받은 위종양 환자에서 심리적 요인이 시술 후 통증에 미치는 영향 (The Effect of Psychological Factors on Postoperative Pain in Gastric Tumor Patients after Endoscopic Submucosal Dissection)

  • 연구목적 암 환자에게 있어 통증은 삶의 질과 예후에 큰 영향을 주는 중요한 요소로 통증을 줄이기 위한 시도가 활발히 이루어지고 있다. 이러한 관점에서 위종양 환자에게 많이 시행되는 내시경 점막하 박리법 후 통증을 줄이기 위한 노력이 계속되고 있는데, 시술 후 통증에 영향을 미치는 생물학적 요인이 많이 알려진 것에 비해 심리적 요인에 대해서는 연구가 거의 이루어지지 않았다. 본 연구의 목적은 내시경 점막하 박리법을 시술 받은 위종양 환자가 시술 후 겪는 통증에 영향을 미치는 심리적 요인을 알아보고자 하였다. 방 법 본 연구는 2015년 5월부터 2016년 6월까지 국민건강보험 일산병원에 내원하여 내시경 점막하 박리법을 시술 받은 91명의 위종양 환자를 대상으로 시행되었다. 내시경 점막하 박리법을 시술 받기 전에 사회인구학적 요인과, 불안, 우울, 회복탄력성을 평가하였다. 환자들이 호소하는 통증의 정도가 시각적 유추척도 3점 이상인 군(고통증군)과 3점 미만인 군(저통증군)으로 나누었고, 시술 후 통증에 영향을 주는 요인에 대해 독립표본 T검정 및 카이제곱 검정을 시행 후 유의한 차이를 보인 요인에 대해 로지스틱 회귀분석을 시행하였다. 결 과 고통증군이 저통증군에 비해 음주량이 낮았고 우울 점수가 높게 나타났다. 또한 고통증군이 저통증군에 비해 회복탄력성 점수가 낮았으며, 회복탄력성의 하위 항목 중 자기조절능력, 긍정성 항목에서 점수가 낮았다. 다변량 로지스틱 회귀분석 결과 회복탄력성 중 자기조절능력(OR, 0.911 ; 95% CI, 0.854-0.971, p=0.004)이 낮을수록 내시경 점막하 박리법 시술 후 겪는 통증이 큰 것으로 나타났다. 결 론 본 연구 결과 내시경 점막하 박리법을 시술 받는 환자의 회복탄력성 하위 항목인 자기조절능력이 낮을수록 시술 후 더 큰 통증을 호소하는 것을 확인하였다. 따라서 본 연구에서 대상으로 한 심리적 요인 중 개인의 회복 탄력성이 통증에 영향을 주는 것을 확인하였다.

간호대학생의 학업탄력성에 영향을 주는 요인 (Factors Affecting Academic Resilience of Nursing Students)

  • 학업장면에서 과도한 위협으로 볼 수 있는 치명적이거나 만성적인 역경을 극복해내는 개인의 역량인 학업탄력성은 스트레스를 극복할 수 있는 심리적 능력이다. 이에 본 연구는 간호대학생의 학업탄력성에 영향을 주는 요인은 파악하고자 시행된 조사연구이다. 총 554명의 간호대학생을 대상으로 대인커뮤니케이션 능력과 행복감이 학업탄력성에 미치는 효과를 설문조사하였으며 SPSS version 21.0을 이용하여 분석하였다. 그 주요 결과는 다음과 같다. (1) 학업탄력성은 학년, 주거형태, 대인관계, 성격유형과 학업성적에 따라 차이가 있었다. (2) 학업탄력성은 대인의사소통 능력(r=.46, p<.001, 행복감(r=.35, p<.001)과 유의한 상관관계를 나타냈다. (3) 학업탄력성의 유의한 영향요인은 학년(p=.002), 주거형태(p=.001), 학업성적(p<.001), 대인의사소통 능력(p<.001), 행복감(p<.001)이었으며 설명력은 36.9%이었다. 본 연구결과 대인커뮤니케이션 능력과 행복감은 간호대학생의 학업탄력성에 긍정적인 영향요인으로 나타났다. 더 나아가 학업탄력성을 향상시키기 위해 대인커뮤니케이션 능력과 행복감을 증진시킬 프로그램 개발에 관한 연구를 제안한다.

구성원들의 침묵이 창의성과 혁신행동에 미치는 영향에 관한 연구: 회복탄력성의 조절효과를 중심으로 (A Study on the Employees' Silence Influencing on Creativity and Innovation Behavior: Focusing on Moderating Effect of Resilience)

  • 본 연구는 변화하는 환경에 대응하기 위해 현대 기업이 고려해야 할 여러 가지 요인 중 인적자원의 중요성에 대해 알아보고 이를 검증하고자 한 연구이다. 독립변수로는 구성원들이 느끼는 동기에 의한 체념적 침묵과 방어적 침묵을 구성하였으며, 결과변수로는 창의성과 혁신행동을 설정하였다. 이러한 연구를 통해서 먼저 조직의 구성원들과 관리자간의 소통의 중요성을 알아보고, 이를 효율적으로 관리하고자 한다. 두 번째로 이들 간의 관계에서 회복탄력성의 조절효과에 대해 확인하고, 구성원들의 심리적 회복의 중요성을 알아보고자 한다. 이는 결국 인적자원을 관리함에 있어서 구성원들의 심리적 관리와 회복의 중요성을 이야기하고자 한다. 결과적으로 체념적 침묵은 창의성과 혁신행동에 부정적인 영향을 미치고 방어적 침묵의 경우 창의성과 혁신행동에 긍정적인 영향을 미치는 것으로 나타났다. 또한 회복탄력성의 조절효과의 경우 방어적 침묵과 창의성 및 혁신행동의 관계에서는 모두 유의미한 관계가 나타나지 않았으며, 체념적 침묵의 경우 혁신행동만 유의미한 관계가 있는 것으로 나타났다. 이는 체념적 침묵과 방어적 침묵을 선택하게 되는 구성원들의 성향의 차이와 회복탄력성의 고유한 특징이 결합되면서 나타난 결과이다.

The Effect of Evaluative Concerns Perfectionism on Resilience: The Joint Moderating Effect of the Social Support and Emotional Intelligence of the Leader

  • In a competitive organizational environment, members view completing tasks to perfection without mistakes as the requirement for success and good evaluation from others. However, unrealistically strong perfectionism can have negative effects on members' psychological states and organizational performance. This study focuses on evaluative concerns perfectionism, the negative aspect of perfectionism, based on positive psychology theory to explore the positive mechanism that comes into place in the process of strengthening organization members' resilience. For this purpose, we performed a moderated moderation analysis to investigate whether the moderating effect of leaders' social support (primary moderator) is moderated by their emotional intelligence (secondary moderator). To verify our research model, we conducted a two-part survey of 252 employees in various industries with a time interval to minimize the common method bias. Job rank and positive affectivity were set as control variables to identify the clear causal relationship among variables. Our findings showed that first, evaluative concerns perfectionism reduced resilience; second, leaders' social support positively moderated the relationship between evaluative concerns perfectionism and resilience; and third, the leaders' emotional intelligence positively moderated the two-way interaction between evaluative concerns perfectionism and the leaders' social support, confirming a moderated moderation. Our findings suggest the need for leaders to assume multidimensional roles to enhance the positive traits of organization members, especially in today's complex business environment. The implications of our findings are further discussed in relation to knowledge management and the development of organization members who exhibit evaluative concerns perfectionism, along with suggestions for future research.

임상간호사의 성역할갈등, 회복탄력성, 간호조직문화가 간호업무성과에 미치는 영향 (Influence of gender role conflict, resilience, and nursing organizational culture on nursing work performance among clinical nurses)

  • Purpose: The purpose of this study was to identify the factors influencing nursing work performance among clinical nurses in the hospital. Methods: This descriptive correlational study collected data from 122 clinical nurses who were working in three tertiary referral hospitals in Busan. The data were collected, using self-reported questionnaires, from September 2 to October 14, 2019. The data were analyzed using descriptive statistics, t-test, ANOVA, Pearson's correlation coefficient, and multiple regression using SPSS/WIN 25.0 program. Results: Nursing work performance was significantly associated with resilience, innovation oriented culture, relationship oriented culture, and hierarchy oriented culture. The factors significantly influencing nursing work performance of the participants included relationship oriented culture (β=.27, p=.015), hierarchy oriented culture (β=.25, p=.003), resilience (β=.19, p=.035), and total clinical career (β=.18, p=.030), which explained 27.3% of nursing work performance (F=8.59, p<.001). Conclusion: The results indicate that there is a need to improve communication skills among multidisciplinary team members and to harmonize hierarchical and relational cultures in order to enhance nursing work performance. In addition, this study suggests developing interventions for enhancing the resilience of clinical nurses.

부모, 또래, 지역아동센터 교사와의 관계가 지역아동센터 이용 저소득층 청소년의 학교적응에 미치는 영향: 자아탄력성의 매개효과 (The Effects of Relationships with Parents, Peers, and Community Child Center Teachers on School Adjustment of Low-Income Adolescents Attending Community Child Centers: The Mediating Effect of Resilience)

  • This study aimed to examine the direct effects of relationships with parents, peers, and community child center teachers on the school adjustment of low-income adolescents attending community child centers and the indirect effects mediated through adolescents' resilience. The second-term panel data from the fourth-year Community Child Center Survey was used to analyze 198 adolescents (103 boys, 95 girls) who were first graders at middle schools. SPSS 20.0 and AMOS 20.0 were used to analyze the data through structural equation modeling. The results of the study were as follows: Parental monitoring and peer trust had direct effects on the adolescents' school adjustment, while satisfaction with community child center teachers did not. Parental monitoring, peer trust, and satisfaction with community child center teachers had indirect effects on adolescents' school adjustment mediated through resilience. The school adjustment of low-income adolescents has received attention from both researchers and politicians. The results of this study are notable in that both the important relationships and resilience of low-income adolescents can significantly contribute to their school adjustment in their first year at middle school. Given the developmental needs of low-income adolescents, interventions for school adjustment that consider adolescents' important social contexts and psychological characteristics should be developed.

간호대학생의 혐오민감성, 임상실습스트레스, 회복력이 전문직 자아개념에 미치는 영향 (Factors Influencing Disgust Sensitivity, Clinical Practice Stress, and Resilience in Professional Self-Concept among Nursing Students)

  • Purpose: This study aimed to identify factors affecting nursing students' professional self-concept. Methods: A survey was conducted from August to October 2022, targeting 110 students in the 3rd and 4th grades at three universities. Data were analyzed using descriptive statistics, t-tests, ANOVA, Pearson's correlation, and multiple linear regression. Results: There were significant differences in professional self-concept according to application motivation (F=5.43, p=.006), school record (F=3.24, p=.043), major satisfaction (t=-6.38, p<.001), desired occupation (F=9.30, p<.001), participant health status (F=4.31, p=.016), character (F=25.29, p<.001), clinical practice satisfaction (t=-5.36, p<.001), and change of thinking about career path after clinical practice (t=2.78, p=.006). Professional self-concept showed significant correlations with clinical practice stress (r=-0.37, p<.001) and resilience (r=0.66, p<.001). The variables affecting nursing students' professional self-concept were resilience (β=.38, p<.001), character (β=.32, p<.001), major satisfaction (β=.28, p<.001), and clinical practice stress (β=-.18, p=.007); the explanatory power was 65%. Conclusion: A professional self-concept enhancement program should be developed to promote the establishment of a positive professional self-concept in nursing students. As a strategy, it should reflect individual personality tendencies, improve major satisfaction and resilience, and reduce clinical practice stress.

응급실 간호사와 응급구조사의 외상성 사건의 경험, 외상후 스트레스장애에 대한 지식, 태도 및 회복탄력성 (Experiences of traumatic events, knowledge and attitudes concerning post-traumatic stress disorder, and resilience among nurses and paramedics working in emergency department)

  • Purpose: This study aimed to assess exposure to traumatic events, knowledge and attitudes concerning post-traumatic stress disorder (PTSD) and the level of resilience among nurses and paramedics working in emergency departments. Methods: Data were collected from May 22 to June 12, 2022, using a self-administered survey questionnaire. The participants comprised 135 nurses and 80 paramedics working in emergency departments. The collected data were analyzed with descriptive statistics, a t-test, and an analysis of variance with Scheffé's test. Results: Compared with emergency room nurses, paramedics were more positive about the government's spending on job opportunities for people with PTSD. There were no significant differences in attitude regarding government strategies and people with PTSD between nurses and paramedics. Paramedics had higher scores on the effective treatment for PTSD, while emergency room nurses showed higher scores on effective psychotherapy. General knowledge of PTSD differed according to sex (t=-2.33, p=.021) and education level (F=3.21, p=.042). Resilience scores differed significantly according to sex (t=2.02, p=.045), education level (F=4.10, p=.018), self-reported economic state (F=10.34, p<.001), and self-reported health (F=11.57, p<.001). Conclusion: The findings support that emergency department nurses and paramedics are in need of self-care programs to support their mental health and indicate that intervention programs should be developed to enhance resilience in emergency department professionals.

여성 류마티스 관절염 환자의 건강관련 삶의 질 구조모형 (Health-related quality of life in female patients with reumatoid arthritis: a structural equation model)

  • Purpose: This study aimed to construct a structural equation model to explain and predict factors affecting the health-related quality of life (QoL) in female rheumatoid arthritis (RA) patients based on the health-related QoL model by Ferrans et al. (2005) and a literature review. Methods: Patients (N=243) who were either registered members of an internet cafe composed of patients with RA or rheumatology outpatients at two tertiary general hospitals in Busan, Korea, were recruited via convenience sampling. Data were collected from July 2 to September 9, 2021, and the survey was conducted using a web-based questionnaire. The data were analyzed by SPSS and AMOS 26.0. Results: The goodness-of-fit statistics of the final model exhibited good results (χ2/degree of freedom=2.68, Turker-Lewis index=.94, comparative fit index=.96, standardized root mean-squared residual=.04, root mean- square error of approximation=.08), and 11 out of 14 paths of the model were supported. The squared multiple correlation, which reflected the explanatory power of the environmental characteristics, symptoms, functional status, and perceived health status on health-related QoL, was 80%. In the hypothesis model, 10 paths had significant direct effects, 6 paths had significant indirect effects, and 12 paths had significant total (direct and indirect) effects. Conclusion: Considering that factors directly affecting the health-related QoL of female patients with RA were social support, symptoms (fatigue and depression), resilience, and perceived health status, and that resilience was the most influential factor, clinicians can encourage resilience. Hence, to improve the health-related QoL of female patients with RA, continuing management is necessary, using various intervention methods that focus on enhancing resilience from the early stage to the end of treatment for RA.

동료교수활동이 간호학과 신입생의 셀프리더십, 심리역량 및 학업만족에 미치는 영향 (Effects of peer teaching on self-leadership, psychological capitals and learning satisfaction among nursing freshmen)

  • 본 원시실험연구의 목적은 동료교수활동이 간호학과 신입생의 셀프리더십, 심리역량 및 학업만족에 미치는 효과를 파악하기 위함이다. 30분간 12회의 동료교수활동 전후 측정된 설문조사결과를 SPSS 21로 기술통계, t-test, ANOVA와 paired t-test를 시행한 결과, 처치 전후 주관적 학교성적이 낮은 집단의 학업만족과, 토론 선호도가 보통 이하인 집단의 낙관성이 유의하게 증가하였다. 처치 전후 셀프리더십과 심리자본에 유의한 차이가 있었는데(p=.002, p=.044), 심리자본의 하위영역 중 자신감과 복원력이 유의하게 증가되었다(p=.006, p=.015). 결론적으로 동료교수활동은 간호학과 신입생의 셀프리더십과 심리역량을 증가시켰다. 따라서 간호교육과정에서 다양한 형태의 동료교수활동을 적용하는 추후 연구를 제언한다.
