Professional Self Concept of Psychiatric Mental Health Nurse Practitioner and General Nurse in Psychiatric Ward (정신병동의 정신보건간호사와 일반간호사의 전문직 자아개념)

  • Purpose : This study was conducted to investigate the extent to which the professional self concept between the psychiatric mental health nurse practitioner (PMHNP) and general nurse in psychiatric ward was comparable. The results were expected to provide basic data for developing the construct of professional self concept and making awareness of the importance of the program increasing professional self concept. Method : The subjects of this study were 227 PMHNP and 436 general nurse in psychiatric ward. The instruments used for this study were PSCNI by Arthur (1990), PSI by Heppner and Peterson(1982) and the index of work satisfaction by Slavitt et al.(1978). With the aid of the SAS, t-test. two-way ANOVA and stepwise multiple regression were conducted. Result : 1. The average item score of PSCNI of PMHNP was $2.82\pm0.27$. and that of general nurse was $2.66\pm0.27$. Statistically significant difference between two groups was found(p=0.0000) 2. There was statistically significant difference between two groups in the score of professional practice(p=0.0000), satisfaction(p=0.0024), leadersbip(p=0.0000) , flexibility(p=0.0000) and skill (p=0.0000). 3. Statistically significant differences between the two groups were observed in terms of age(p=0.0003), marital status(p=0.0001). education(p=0.0005), religion(p=-.0144), motive (P=-.0001), length of service as a nurse(p=0.0121), the length of service in psychiatric unit(p=0.0143). However there were no significant interaction effect with group and age, marital state. education, religion, motive, length of service as a nurse, length of service in psychiatric unit. 4. Job satisfaction (JS) and problem solving inventory score(PS) were found to be the highest factor predicting the professional self concept between the PMHNP and general nurse. JS and PS accounted for $43.4\%$ in the professional self concept of PMHNP, whereas PS, JS, age and religion accounted for $53\%$ in the professional self concept of general nurse in psychiatric ward. In conclusion, this study suggested that we need to develop programs and polices to increase the professional self concept of nurse, particularly of psychiatric mental health nurse practitioners.

A Study of Cognitive Function and Psychiatric Disturbance in Normal Adults (정상인의 연식기능과 신경증경향에 관한 연구)

  • In an attempt to take Comprehensive nursing care the author tested cognitive function and psychiatric disturbance produced by normal adults. Mini-mental state and simple reaction time were tested and general health questionnaire was conducted two hundred subjects. The data were analyzed statistically and the results follow: 1. The score of mini-mental state of the elderly group was less significant than that of the younger group in the tests of cognitive dysfunction. The score of MMS of the female group, the illiteracy group less significant than that of other groups in the test of cogntive dysfunction. 2. Reaction time of the elderly group was slower than that of the younger group. Reaction time of the female group was slower than that of the male group. Reaction time of the illiteracy group was the slowest & the unskilled group was slower than that of the other groups. 3. As a result of the results of psychiatric disturbance the score of general health questionnaire of the elderly group was significantly higher than that of the younger group in the tests of psychiatric disturbance. The test of psychiatric disturbance sho-wed that the score of general health questionnaire of the female group was significantly higher than that of the male group in the tests of psychiatric disturbance. The illiteracy group was the highest in the psychiatric disturbance among other groups and the unskilled group was significantly higher than the other groups. 4. Education had higher correlation to mini-men-tat state, while age negative correlation in comparison with other variables age had higher correlation to reaction time, while education had negative correlation in comparison with other variables. Sex had low correlation in these tests.

Relationships between Perceived Stress, Mental Health, and Social Support in Community Residents (일 도시지역 주민의 스트레스 지각, 정신건강 및 사회적 지지)

  • Purpose: The study investigated the relationships among perceived stress, mental health, and social support in community residents. Method: A self-reporting questionnaire was completed by 302 community residents aged 19-64-years-of-age from October 7 to November 30, 2008. Assessment tools were an The established perceived stress scale, standardized mental health scale, and established social support scale. The data was analyzed by descriptive statistics, t-test, one-way ANOVA, Scheffe's test, and Pearson's correlation coefficient using the SPSS WIN 12.0 program. Result: Significantly negative correlations were evident between perceived stress and social support, and between social support and mental health. But, there was a significantly positive correlation between perceived stress and mental health. Conclusion: It is necessary to develop a strategy to decrease perceived stress, improve mental health, and increase social support for community residents.

An Integrative Review of Intervention for School-bullying Perpetrators (학교폭력 가해 아동·청소년을 위한 중재의 통합적 문헌고찰)

  • Purpose: This study was intended to integrate the evidence of intervention for child and adolescent perpetrators of school violence through an integrative literature review. Methods: Using combinations of the terms 'bullying', 'school violence', and 'intervention' as key words, the researchers searched eight electronic databases for relevant studies. Fifteen studies were selected through full-text screening of related research published in academic journals before June 2018. The framework was used to identify the selected studies' intervention patterns and classify the various intervention components. The extracted intervention components were grouped into potential themes to determine whether the researchers clearly showed the interventions in the studies. Results: The intervention components of 15 selected studies were categorized into five themes: 1) Utilizing intervention techniques for voluntary participation, 2) Enhancing self-awareness, 3) Strategies to improve emotional intelligence, 4) Promoting interpersonal skills, and 5) Emphasis on responsibility through future vision experience. Conclusion: As a result of analyzing interventions for children and adolescent perpetrators of school violence, five components were derived. It is suggested that these components should be considered in the field, and intervention programs development and research using them are needed.

Improving Empathy in Nursing Students: A Comparative Longitudinal Study of Two Curricula

  • Purpose: This study aimed to examine changes of empathy levels of nursing student in two different curricula structures, one called "traditional" and the other called "integrated" curricula. The study was a longitudinal design to follow a cohort of nursing students to examine the magnitude of changes in empathy in their education years. Methods: The study was conducted in a public school of nursing giving a baccalaureate degree, which had a fundamental change in their curricula. In all, 81 students from the traditional curricula and 66 students from the integrated curricula completed the study, and data from a total of 147 students were analyzed between 2003 and 2008. The Empathic Communication Skills Scale and the Empathic Tendency Scale were given to the students in the beginning of their freshman year and at the end of the fourth year just before graduation. Results: Although both of the curricula were seemed effective at improving empathic skills of students, especially the scores of students who completed the integrated curricula were higher than the scores of the other group attending the traditional curricula (p<.05). However, the empathic tendency scores of students in both curricula decreased at the end of fourth year. Conclusion: Although undergraduate nursing curricula either traditional or integrated improved empathic skills, it seemed that integrated curricula were more effective than traditional curricula in increasing empathic skills. The more hours and more experiential methods contributed to improved empathy. The decrease in empathic tendency requires further attention of educators and nurse managers.

Recovery of People Living with Mental Illness: A Concept Analysis (정신질환의 회복[Recovery] 개념분석)

  • Purpose: The purpose of this study is to analyze the concept of recovery in relation to those living with mental illness and provide a better understanding to the definition, perspectives, and paradigm of recovery in phenomenon, as a conceptual knowledge. Methods: A literature review was conducted to define the concept of recovery from a mental illness by using key words, "recovery", "mental health and illness", "concept analysis" and "recovery-oriented nursing", and searching the Cumulative Index to Nursing and Allied Health Literature, PubMed, Cochrane library and RISS4U database. Concept analysis of recovery was done, by using the Walker and Avant's framework of concept analysis. Results: Attributes of recovery for those living with mental illness included regain, life reconstruction, hope, adjustment, and health. Antecedents of recovery from mental illness included instilling hope, recovery vision, belief, peer support, recovery-oriented services, empowerment, personal accountability, education, human rights and culture. The consequences, as meanings of recovery included self-esteem, hopeful life, positive adjustment, and healthy life. Conclusion: Concept of recovery is important for a nurse to understand when caring for a person living with mental illness. This concept of recovery from mental illness may apply to future studies to develop a recovery-oriented nursing intervention.

Factors Influencing Suicidal Ideation in People with Mental Disorder (만성정신질환자들의 자살사고에 영향을 미치는 요인)

  • The purpose of this study was to identify the effects of depression, self-efficacy and social support on suicidal ideation in people with mental disorder. The participants for this study were 165 patients from psychiatric hospital, day hospital, community mental health center and nursing home, located in Busan. A self-report survey was used for data collection, which was carried out from May 1 to 30, 2011. The collected data were analyzed using SPSS21.0 program. According to results, Suicidal ideation was influenced by age, number of admission, psychiatric diagnosis and history of suicidal attempts. This study showed that suicide ideation had positive correlation with depression, negative with social support. History of suicidal attempt, depression, self-esteem and social support accounted for 52.8% regarding suicidal ideation. Based on the outcomes of this study, it is necessary to design an intervention program that emphasizes decreasing depression and suicidal attempt and increasing the self-esteem and social support in order to decrease suicidal ideation.

Self-evaluative Awareness of Problem Solving Ability In Psychiatric Nurse Practitioners (정신보건간호사의 문제해결능력에 대한 인식정도)

  • This study was conducted to investigate the degree of PSI (self-evaluative awareness of problem solving ability) in PMHNP (psychiatric mental health nurse practitioners) and to identified the factors predicting problem solving ability of them, in order to provide basic data for the development of the specific education programs to improve problem solving ability. Data was collected from 355 subjects who have the certificates of PMHNP working in hospitals and public centers. Chun Seok Kyun's instruments (1993) based on the Heppner and Petersen's Problem Solving Inventory (1982) was used to determine PMHNPs' PSI. The results were as follows; 1. The average score of PSI of PMHNP was $2.81\pm0.23$. Of the three factors. there marked the lowest score in the factor 3. personal control $(2.56\pm0.43)$, and the highest score was in the factor 1. problem-solving confidence $(2.95\pm0.34)$. 2. The differences in PSI were significant for degree of education and career in psychiatric settings (p=0.000l, p=0.0187). 3. Job satisfaction was the highest factor predicting PSI of PMHNP$(17.9\%)$. When degree of education, career in psychiatric setting and marital status were added, the total predictors explained $24.3\%$. As a result, it is necessary to conduct further research in relation to the problem-solving process, the development of education program. skill acquisition and measurement of problem solving in nursing practice.

The Relationship between Family Burden and Mental Health Service Needs of Family for Home-based Chronic Mental Patients in Community (지역사회 재가 만성정신질환자 가족의 부담감과 정신보건서비스요구와의 관련성)

  • = ABSTRACT = Objectives: This study was carried out to identify the relationships of family burden and mental health service needs of chronic mental patients in community. Methods: Objects of the study were 153 chronic mental patients in community of P. city in korea. Data were collected from December, 2007 to February, 2008 using structured questionnaire. Research tools of this study were family burden tool developed by Pai & Kapur (1981) and mental health service needs tool developed by Kim (2003). Results: The average grades for family burden was 1.62 points. And the biggest part of family burden was economic burden(1.74), followed by interrupt of daily life(1.67), interrupt of family relationship(1.64), interrupt of family leisure (1.57), effects of mental health(1.50), and effects of physical health(1.43). The average grades for mental health service needs was 2.72 points. And the biggest part of mental health service needs was rehabilitation service(3.09), followed by social service(2.87), and Psychiatric medical service(2.21). Positive correlation showed between all parts of family burden. And, positive correlation showed between psychiatric medical service and interrupt of daily life(r=.281, p<.01), psychiatric medical service and effects of physical health(r=.355,p<.01), social service and effects of mental health(r=.213,p<.01). Conclusion: The family burden for care giver of mental patients was related with all parts of family burden and mental health service needs of family. Thus, these results should be considered to reduce family burden for care giver of mental patients in community.
