Objective : This study was to report a clinical trial in which acupuncture treatmnt on 會陰(Huiyin, CV1) was effectively applied to a patient who had suffered from teasing pruritus ani for 38 years. pruritus ani was thought to be initiated by dystonia. Methods : The acupoint, 會陰(Huiyin, CV1) was selected, because needling on perineal muscles was mainly used to alleviate pelvic pain or pruritus ani in the light of MPS(Myofascial Pain Syndrome) theory, on which 會陰(Huiyin, CV1) was located and also had pelvic pain and pruritus ani in its indications. On the acupoint, the following maneuver was used. (1) Make a patient stand and bend forward (2) Insert a needle on 會陰(Huiyin, CV1) (3) Repeat lifting and pushing (4) Draw a needle immediately without retaining needle on the acupoint Results : The patient with Pruritus ani recovered after above treatment two times and had no recurrence up to Feb. 2001. Conclusion : In this study, Needling on 會陰(Huiyin, CV1) proved to be effective in the aspect of MPS and acupuncture treatment as well, but more clinical trials are expected to follow this study.

Pruritus is the most prominent symptom of atopic dermatitis and other skin diseases. It is not easily improved and is a major factor that threatens the quality of life of patients and their families. The purpose of this study was to evaluate the effect of HyungGae-Water, which was bath preparation made from Korean medicine herbs, for children and adolescents suffering pruritus. The participants were 50 boys and girls aged from 2 to 15 years old. The HyungGae-Water was used once a day for 6 weeks, and we were followed up three times every two weeks. Of 50 participants enrolled, 47 completed the study. The outcome variables were dermatology quality of life scales in children, pruritus VAS, and investigator global assessment (IGA). The results of the study show that the dermatology quality of life is significantly improved and the VAS of pruritus has meaningful changes. In the IGA, moderate or severe improvement was observed in 48.9% and slight improvement in 17%. There were seven suspected or less relevant adverse events using HyungGae-Water. The present results suggest that the HyungGae-Water has alleviating effect for the pruritus in children and adolescents.
Background: Nowadays, epidural morphine is commonly used in postoperative pain control. But epidural morphine may produce some side-effects, e.g. pruritus, nausea, vomiting, urinary retention and respiratory depression. Especially, pruritus is the most common complaint in pain-controlled patients by epidural morphine. So we evaluated whether addition of epidural butorphanol affects the degree of pruritus and pain score in pain controlled patients who by epidural morphine after hysterectomy. Methods: Group 1(N=15) received postoperative epidural 0.1% bupivacaine 100ml plus morphine 10 mg, group 2(N=15) received the mixture of butorphanol 2 mg with same regime as in group 1, group 3(N=15) received the mixture of butorphanol 4 mg with same regime as in group 1. All of the three groups received these solutions by infusion pump, 1 ml/hour, for postoperative 4 days. all groups received additional morphine 1.2 mg in 0.2% bupivacaine 6ml epidurally when the peritoneum was closed under general anesthesia. The severity of pain, pruritus, nausea and vomiting was estimated by 10 cm VAS(visual analogue scale) and somnolence by positive or negative during postoperative 4 days. Results: Severity of pruritus, but not nausea and vomiting was decreased in group 2 and 3 compared with group 1(p<0.05). Pain score was increased in group 3 at postoperative day(POD) 0 and 2 compared with group 1(p<0.05). Incidence of somnolence in group 1, 2 and 3 were $2.7{\pm}0.7,\;5.3{\pm}0.7$ and $10.0{\pm}1.0$ respectively. Conclusion: These results suggest that butorphanol reduce the degree of pruritus, the most common side effect of morphine, but increase the incidence of somnolence.

This study examined pruritus to provide the data based on burn patient pruritus. A self-report questionnaire survey was conducted among ninety five burn survivors, who were over 18 years of age with admission periods longer than two weeks. The results indicated that the average burn range was $18.95{\pm}17.10%$, severity was $48.83{\pm}23.96$, and distress was $46.42{\pm}25.80$. A significant difference in pruritus consequences was observed according to frequency. The patients consistently experienced pruritus, even if it happened less often, difficulty in falling asleep (F=4.817, p=.004), poor mood (F= 5.955, p=.015)), and loss of concentration (F=6.483, p<.000). A significant difference in the pruritus consequences according to duration was observed, lasting for 30 minutes for all groups, more than the population and less than 30 minutes, difficulty in falling asleep (F=7.452, p<.000), poor mood (F=3.655, p<.015), and loss of concentration (F=12.564, p<.000). The frequency, duration (r=.29, p=.004), severity (r=.45, p<.000), distress (r=.44, p<.000), and consequences of pruritus in burn patients (r=.29, p=.004) showed a positive correlation. The results of this study could be used as a base material for the care of burn patients experiencing pruritus.
Atopic dermatitis is a common skin disease affecting up to 10% of children and approximately 2% of adults. Atopic dermatitis exhibits four major symptoms, including intense itching, dry skin, redness and exudation. The "itch-scratch-itch" cycle is one of the major features in atopic dermatitis. The pathophysiology and neurobiology of pruritus is unclear. Currently there are no single and universally effective pharmacological antipruritic drugs for treatment of atopic dermatitis. Thus, controlling of itch is a very important unmet need in patients suffering from atopic dermatitis. This article will update progress during the past 10 years of research in the field of pruritus of atopic dermatitis, focusing on aspects of pruritogens (including inflammatory lipids, histamine, serotonin, proteinases, proteinase-activating receptors, neurotransmitters, neuropeptides, and opioid peptides), antipruritic therapies, and emerging new targets. Based on recent progress, researchers expect to identify exciting possibilities for improved treatments and to develop new antipruritic drugs acting through novel targets, such as histamine H4 receptor, gastrin-releasing peptide receptor, MrgprA3, thromboxane A2 receptor and the putative SPC receptor.

Objectives : The purpose of this report is to know the effect of Korean medical treatments on pemphigus vulgaris with generalized pruritus. Methods : We treated a 44-years-old woman patient with pemphigus vulgaris on the whole body with Korean medicine. After Korean medical treatment for 17 weeks, we measured the extinction of blisters and decrease of pruritus. We recorded pictures of changes on symptoms. Results & Conclusions : The symptoms of pemphigus vulgaris were significantly improved. The blisters and itching were decreased and skin damages were almost recovered. Thus Korean medical treatments are effective on pemphigus vulgaris.

Purpose: This experimental study was to verify the effects of the mineral oil on skin under casts for relief of skin dryness and pruritus. Method: Participants in the study were inpatients at C university hospital in D city who had long leg casts. The mineral oil was applied to the skin under the casts in the experimental group of 18 patients and was not applied to the control group, also 18 patients. During the experimentation, the participants in the experimental group were informed about how to apply the mineral oil and 1 week later were responsible for applying the oil themselves. The oil was applied three times a day. Data collection was done from July 28, 2002 to September 28, 2002. The analyses were carried using frequencies, percentages, $x^2$-test, t-test, and repeated measures ANOVA with the SPSS WIN 10.0 program. Results: Hypothesis 1, 'The experimental group using the mineral oil application showed remarkably less dryness in the skin compared to the control group' was accepted (F=16.39, p=0.000). Hypothesis 2, 'The experimental group using the mineral, oil application showed remarkably less pruritus compared to the control group' was accepted (F=34.01, p=0.000). Conclusion: These results demonstrate that mineral oil application to skin under casts was effective in treating skin dryness and pruritus. Accordingly, it is concluded that mineral oil application to skin under casts can be an effective nurse intervention to treat skin dryness and pruritus.
Background: Postoperative pruritus following the administration of epidural narcotics is a very common and undesirable side effect. Therefore, we evaluated the use of a combination of naloxone and sufentanil via patient controlled epidural analgesia to determine if the incidence of pruritus was decreased when compared to the use of sufentanil alone. Methods: Patients scheduled for subtotal gastrectomy under general anesthesia were enrolled in a prospective, double-blinded and randomized trial. All patients received a $20{\mu}g$ epidural bolus of sufentanil in 5 ml of 0.2% ropivacaine. Following administration of the epidural, patients in the sufentanyl group (S) received a continuous epidural comprised of sufentanil ($0.75{\mu}g/ml$) in 0.2% ropivacaine, whereas patients in the naloxone group (N) received an epidural infusion comprised of naloxone ($4{\mu}g/ml$) and sufentanil ($0.75{\mu}g/ml$) in 0.2% ropivacaine. The infusion rate, demand dose and lockout interval were 5 ml/hr, 0.5 ml and 15 minutes respectively. Next, the occurrence of postoperative analgesia and side effects were evaluated by blinded observers. Results: The incidence of pruritus (47.4% versus 20.0%, P = 0.013) and nausea (42 .1 % versus 20.0%, P = 0.043) were lower in group N than in group S. In addition, there were no significant differences observed in the visual analogue scale, the incidence of vomiting or the incidence of sedation. Furthermore, epidural infusion of naloxone at $0.25-0.4{\mu}g/kg/hr$ did not affect the requirement for postoperative sufentanil. Conclusions: Epidural naloxone reduces epidural sufentanil induced pruritus and nausea without reversing its analgesic effects.

Alcohol affects human body very seriously by causing different types of disease. Therefore, a great deal of studies on alcohol addiction have been conducted. There is little, however, on skin problem in relation to alcohol abuse in domestic researches. This study, with this reason, has been conducted on alco holic women, especially who have sensitive skin, with pruritus. From May 1st until 20th, 2013, surveys using same questionnaires had been carried out for the two different groups. one with 114 female alcoholics from four different clinics for acoholics through the nation, the other with 106 residents without alcoholic problems in Cheong-ju and Seong-nam city. From the comparative study of those surveys, alcohol addicts showed higher scores on pruritus than those without alcoholic problems. In addition, Audit-K score had positive correlation with pruritus score, and drinking cycle and quantity, too, had been found related to pruritus. It is expected that the findings will be used as a fundamental data for treating alcohol addicts with skin disease and leading more active researches on this field for the future.

Background: It is axiomatic that average life span is prolonged in proportion to the advance of socio-economic improvements and medical science. Consequently, the number of elderly patients with skin diseases is increasing. Objective: This study was performed to examine the patterns of skin diseases in elderly patients that had visited Dongguk Oriental Medical hospital, Dept. of Dermatology. We also compared these patients to the young generational groups. Methods: We studied 67 elderly patients with complaints of skin diseases who had visited our hospital from March 2000 to February 2002. We analysed their skin diseases and compared them with the younger group's. Results: The results were as follows; 1. The common disease groups were eczema(32.8$\%$); pruritus(23.9$\%$); viral infections (13.4$\%$); psoriasis(9.0$\%$); fungal infections and urticaria(6.0$\%$). 2. The disorders prevalent in men were eczema(30.0$\%$); herpes zoster & pruritus(23.3$\%$); fungal infection(13.3$\%$); psoriasis (3.3$\%$); melasma(6.7$\%$). 3. The common diseases of women's group were observed in the following order; eczema(35.l$\%$); pruritus(24.3$\%$); psoriasis(13.5$\%$); urticaria(10.8$\%$); other dermatoses(8.1$\%$); herpes zoster(5.4$\%$). 4. The most frequent skin disease for those in their 50's was eczema(32.3$\%$); 60's, pruritus(36.8$\%$); 70's, eczema(50.0); and over eighties., eczema and pruritus(40.0$\%$). Conclusion: This study suggests that dermatologic problems in the elderly are very common, especially eczema, pruritus. It is a very different pattern from the younger group's. And we have to better consider more effective management and treatment for them; especially further dermatologic studies including substantial medical care. Usually Oriental medicine is good at treating chronic diseases and less harmful. So we expect it to be generally better for elderly patients than western medicine.
