Directions for Muti-institutional Cooperation in Follow-up Management of Prostatic Disease Screening for the Low-income Group in Gangwon-do (강원도 저소득층의 전립선질환 검진사업과 사후관리를 위한 유관기관 협력방안)

  • Objectives: This study aimed to investigate factors related to prostate disease and follow-up procedure on health examination program in Gangwon-do. Methods: The studied population was 16,501 male aged 40 or more, who underwent screening tests for prostatic disease in 2011 health examination program designed for the low income residents in Gangwon-do. The screening tests included prostate specific antigen (PSA), international symptom score (IPSS), and maximal flow rate (Qmax). We conducted chi-square test for trend, two independent samples t-test, and binary logistic regression to find out the relationship between prostate cancer or benign prostatic hyperplasia and risk factors such as age, PSA level, and IPSS. Results: The number of subjects who turned out positive for PSA level (> 3.8ng/ml) was 971 men accounting for 5.9%. The result of post survey on them showed that 511 subjects (52.6%) got the urology check-up: 156 (30.5%) were diagnosed as normal, 286 (56.0%) as benign prostatic hyperplasia, and 69 (13.5%) as prostate cancer. The detection rate for prostate cancer was 8.8% when PSA level was between 3.81 and 9.99g/ml, and was 30.6% when PSA level was more than 10.00ng/ml. The rates of further testing varied depending on the communities from 26.7% to 68.2%, which was mainly affected by the role of local health center. Conclusions: Our findings indicate that enhanced quality control of local health center and empowerment of screening agency would increase the effort of coupled institutions between public and private sectors.

Effects of providing procedural information to patients undergoing prostate biopsy on anxiety, depression and sleep quality (전립선 조직검사 사전정보교육이 전립선비대증 환자의 불안, 우울 및 수면의 질에 미치는 효과)

  • Purpose: This study aimed to investigate the effects of providing prostatic hypertrophy patients with information about an upcoming prostate biopsy procedure on their anxiety, depression, and sleep quality. Methods: Sixty-two participants were divided equally into an experimental and control group. Experiments were conducted from July 31, 2015, to March 30, 2016. After providing information, we evaluated anxiety, depression, and sleep quality using structured questionnaires. Data were analyzed using chi-square tests, Fisher's exact tests, t-tests, and ANCOVA using SPSS. Results: The experimental group demonstrated significantly lower levels of anxiety and depression than the control group. The experimental group also demonstrated significantly higher sleep quality. Conclusion: Information on an upcoming prostate biopsy improved psychological outcomes in patients with prostatic hypertrophy. This education should be incorporated into nursing practice.

  • Ultrasonography of benign prostatic hypertrophy(BPH) has been used a lot to determine the size of the prostate with a biopsy. In this study, we measured the size of the prostate in a proportion of the transition zone and the peripheral zone quantitatively, we propose a method that can be diagnosed with BPH automatically ultrasound image.

Albizzia julibrissin Suppresses Testosterone-induced Benign Prostatic Hyperplasia by Regulating 5α-Reductase Type 2 - Androgen Receptor Pathway

  • Albizzia julibrissin (AJ) is an herbal medicine that shows low toxicity, promotes promoting blood circulation and mitigates the inflammation and has mild side effects. Benign prostate hyperplasia (BPH) is one of the most common diseases that occurs in older males and often results in lower urinary tract symptoms. This study was conducted to evaluate the protective effect of AJ against BPH using LNCaP cells and Sprague Dawley rats treated with testosterone. Treatment with AJ extract reduced the expression of androgen receptor (AR) and prostate-specific antigen (PSA) in vitro. In vivo, rats were divided into 6 groups: 1 (Normal Control); 2 (Testosterone propionate (TP) alone); 3 (TP + finasteride); 4 (TP + AJ 10 mg/kg); 5 (TP + AJ 50 mg/kg); 6 (TP + AJ 300 mg/kg). The groups treated with AJ showed reduced the relative prostate weights and BPH-related proteins were altered, with decreased AR, PSA and proliferating cell nuclear antigen (PCNA) observed by western blot. Histopathological analysis revealed the therapeutic effect of AJ, with a decreased thickness of epithelial cells and reduced level of PCNA and $5{\alpha}$-reductase type 2. These results suggest that AJ extract could ameliorate testosterone-induced benign prostatic hyperplasia.

The Relationship of Late-Life Function and Disability(LLFDI) with Quality of Sleep in Older Men with Prostatic Hypertrophic Symptoms (노인의 전립샘 비대증상, 일상생활 수행능력 및 수면의 질)

  • Purpose: The purpose of this study was to investigate the relationships between LLFDI and quality of sleep in older men with prostatic hypertrophic symptoms. Methods: The secondary analysis of data collected for the 2nd stage BK(Brain Korea)21 project was performed. The original data were collected using a cross-sectional, face-to-face, and private interview method. The questionnaires consisted of prostatic hypertrophic symptoms, late-life function and disability, quality of sleep, and demographic profiles. Results: A total of 112 older men included in this study. The major findings of this study were as follow; 1) 29.5% of participants had more than moderate symptoms of prostatic hypertrophy. 2) There were a significant positive correlation of prostatic hypertrophic symptoms with subjective sleep quality, sleep latency, sleep disturbances, use of sleeping medication, and daytime dysfunction found, whereas there was a negative relationship with frequency dimension. 3) The LLFDI is significantly associated with urgency, weak stream, and intermittency. The quality of sleep is significantly associated with weak stream which explained 11.2% of variance. Conclusion: These results may contribute to a better understanding late-life function and disability, quality of sleep in older men with prostatic hypertrophic symptoms. Therefore, health programs for prompting older men's health should be planned based on results of the study.

Meta-analysis of the Korean Literatures for Developing Clinical Practice Guidelines of Benign Prostatic Hyperplasia (전립선비대증의 진료지침 개발을 위한 한국문헌의 메타분석)

  • This study is to provide evidence-based recommendations for the most-effective treatments of benign prostatic hyperplasia based on patient preference or clinical need, and to meta-analyze the Korean literatures for the development of BPH treatment guidelines. For these analyses, extensive literature searches (208 articles), with priority given to the Korean Journal of Urology, were conducted from 1960 to August, 1996. Meta-analysis, like all statistical analysis, has two main functions: data summarization (qualitative meta-analysis) and smoothing o. pattern recognition (quantitative meta-analysis). As well, critical reviews and syntheses with the mean and 90-percent confidence intervals for the likelihood were used to evaluate empirical evidence and significant outcomes of the BPH treatment literatures (106 articles). For this task, the Methodologic Panel for BPH Guidelines was composed of multidisciplinary experts in the field. The results of the study were summarized as follows: For all that watchful waiting is an appropriate treatment strategy for the majority of patients with prostatism, we couldn't find the Korean literatures which carried this article. The literatures on alpha-1-adrenergic receptor blockers provide no evidence to suggest that any one alpha blocker is more effective than another. The finasteride reduces the size of the prostate, on average, and leads to a small yet perceptible reduction in sysptoms. Of all treatment options, prostate surgery with transurethral resection of the prostate (TURP), transurethral incision of the prostate (TUIP), and so on, offers the best chance for symptom improvement. However, surgery also has the highest rates of significant complications. Therefore, surgery need not always be a treatment of last resort. Balloon dilation of the prostatic urethra is clearly less effective than surgery in relieving symptoms, but it is associated with fewer complications. Emerging technologies for treating BPH include lasers, coils, stents, thermal therapy and hyperthermia. Established technologies will also be reanalyzed as results of new trials are reported. Although this study has some limitations due to lacking for good quality literatures, ' it provides a cornerstone for our medical research. It represents the most current scientific knowledge regarding the clinical epidemiology including treatment of BPH. It will be revised and updated as needed.

  • We fabricated a sensor for measuring the water level and water temperature using GMR-SV (giant magnetoresistance-spin valve) device, simultaneously. It could be applied to the medical cooling system of the potassium titanylphosphate KTP) laser system for the therapy of a benign prostatic hyperplasia. The middle point of GMR-SV device was set to the near position of a high magnetic sensitivity with 2.8%/Oe. The sensitivity for the water level and water temperature of the fabricated sensor were $400\;m{\Omega}/mm$ and $100\;m{\Omega}/^{\circ}C$, respectively.

Ameliorative Effect of Yacon Containing Herbal Mixture against Benign Prostatic Hyperplasia Symptoms (야콘을 포함한 수종 천연물 처방의 전립선비대 증상 개선효과)

  • We evaluated the benign prostatic hyperplasia (BPH) with Yacon pill, the preparation composed of Smallanthus sonchifolius, Torilis japonica and Acorus gramineus. A total of 45 rats were divided into five groups. One group was used as a control and the other groups received subcutaneous injections of testosterone for 4 weeks to induce BPH. Yacon pill extract (200 or 400 mg/kg) was administered daily for 4 weeks to two groups by oral gavage concurrently with testosterone. The rats euthanized, the prostate and body weights were recorded, and tissues were subjected to hormone assay. In addition, we investigated proliferating cell nuclear antigen (PCNA) expression in the prostate using immunoblotting. Rats with BPH showed significantly increased prostate weights, increased dihydrotestosterone (DHT) levels in the serum and increased PCNA expression in the prostate; however, Yacon pill extracts treated rats showed significant reduction of prostate weights, DHT levels and PCNA expression compared with the BPH group. Conclusively, Yacon pill showed the possibility as ameliorable agents of BPH symptoms.

  • Difficulties in treating chronic prostatitis cause physical pain and mental problems not only for the treating doctor, but also for the patient. Accordingly, the device of the present research was developed to help patients suffering from prostate symptoms due to the vibration effect of shock wave energy caused by thermal and mechanical interaction of the external shock wave. The purpose of this study is to provide an effective treatment means for prostatitis and BPH(Benign prostatic hyperplasia) by devising an extracorporeal shockwave therapy device that effectively applies the heat and vibration effects of concentrated energy by MCU and spiral coil module.
