Prostate cancer is the secondary most frequently diagnosed cancer in the world. Although numerous prospective randomized trial have been conducted to guide the management of patients with localized or locally advanced prostate cancer, few clinical trials targeting node-positive prostate cancer have been reported. Therefore, there are still controversies in the optimal management of node-positive prostate cancer. Recently, efficacy of multimodality treatment, including radiation therapy (RT), for such patients has been reported in several articles. The results indicate potential benefit of RT both in adjuvant therapy after prostatectomy and in definitive therapy for node-positive prostate cancer. The aim in this article was to summarize the current evidence for RT and evaluate the role in multimodality treatment for patients with node-positive prostate cancer.
Radiotherapy has an important role in the treatment of prostate cancer. Three-dimensional conformal radiation therapy (3D-CRT), intensity modulated radiation therapy (IMRT) and volumetric modulated arc therapy (VMAT) techniques are all applied for this purpose. However, the risk of secondary radiation-induced bladder cancer is significantly elevated in irradiated patients compared surgery-only or watchful waiting groups. There are also reports of risk of secondary cancer with low doses to normal tissues. This study was designed to compare received volumes of low doses among 3D-CRT, IMRT and VMAT techniques for prostate patients. Ten prostate cancer patients were selected retrospectively for this planning study. Treatment plans were generated using 3D-CRT, IMRT and VMAT techniques. Conformity index (CI), homogenity index (HI), receiving 5 Gy of the volume (V5%), receiving 2 Gy of the volume (V2%), receiving 1 Gy of the volume (V1%) and monitor units (MUs) were compared. This study confirms that VMAT has slightly better CI while thev olume of low doses was higher. VMAT had lower MUs than IMRT. 3D-CRT had the lowest MU, CI and HI. If target coverage and normal tissue sparing are comparable between different treatment techniques, the risk of second malignancy should be a important factor in the selection of treatment.

The record of radiation therapy cases during a five year period at the University of Michigan Hospitals has been analyzed. Of a total of 73 adenocarcinoma of prostate, the majority belonged to Stage B and C which represented 49 and 20 cases, respectively. The mortality rate after irradiation was clearly related to the tumor stage. Local irradiation resulted in $88\%$ of the local control of well·differentiated adenocarcinomas. Stage C cases had $50\%$ mortality, whereas that of Stage B patients was $14\%$. Results of this study are in general agreement with previous data in terms of the local disease control after irradiation and provide a basis for conservative radiotherapy regimen as an approach in the treatment of localized prostatic carcinomas.
Purpose: The purpose of this study was to describe treatment patterns of radiotherapy (RT) for prostate cancer in Korea. Materials and Methods: A questionnaire about radiation treatment technique and principles in 2013 was sent to 83 radiation oncologists and data from 57 hospitals were collected analyzed to find patterns of RT for prostate cancer patients in Korea. Results: The number of patients with prostate cancer treated with definitive RT ranged from 1 to 72 per hospital in 2013. RT doses and target volumes increased according to risk groups but the range of radiation doses was wide (60 to 81.4 Gy) and the fraction size was diverse (1.8 to 5 Gy). Intensity-modulated radiation therapy was used for definitive treatment in 93.8% of hospitals. Hormonal therapy was integrated with radiation for intermediate (63.2%) and high risk patients (77.2%). Adjuvant RT after radical prostatectomy was performed in 46 hospitals (80.7%). Indications of adjuvant RT included positive resection margin, seminal vesicle invasion, and capsular invasion. The total dose for adjuvant RT ranged from 50 to 72 Gy in 24-39 fractions. Salvage RT was delivered with findings of consecutive elevations in prostate-specific antigen (PSA), PSA level over 0.2 ng/mL, or clinical recurrence. The total radiation doses ranged from 50 to 80 Gy with a range of 1.8 to 2.5 Gy per fraction for salvage RT. Conclusion: This nationwide patterns of care study suggests that variable radiation techniques and a diverse range of dose fractionation schemes are applied for prostate cancer treatment in Korea. Standard guidelines for RT in prostate cancer need to be developed.
본 연구에서는 전립샘암 환자에게 방사선 치료법인 3차원 입체조형법과 세기조절치료법을 각각 적용 할 경우 선량분포의 차이를 관찰하여 치료기법의 우수성을 평가하고자 하였다. 실험대상자 10명의 컴퓨터 단층 모의치료영상을 얻어 종양학과 전문의가 종양용적 및 정상장기를 구분하고 종양용적에 흡수선량을 80 Gy로 설정한 후 각각 다른 치료계획을 수립하였다. 그 결과 선량분포윤곽은 세기조절치료법이 우수 하였고 종양조직의 흡수선량은 세기조절치료법이 처방선량에 근접(100.2%)하였으며 정상조직 흡수율(방광, 직장, 소장, 좌 우 대퇴골두) 또한 우수하였다. 즉, 전립샘암의 방사선 치료시 세기조절방사선치료가 입체조형치료법보다 선량적인 면에서 양호한 것으로 분석되었다.
The results of CBCT was obtained using image guided radiation therapy for radiation therapy in 5 prostate cancer patients. Using these results, we compared and evaluated the dose changes according to the treatment plan depending on the volume and position of bladder, rectum, and prostate. The 28 images of CBCT were acquired using On-Board Imaging device before radiotherapy. After the outline of bladder, rectum, and PTV, pCT images and CBCT images for radiotherapy were treated respectively. The volume of the bladder was increased by 105.6% and decreased by 45.2%. The volume of the rectum was increased by 30.5% and decreased by 20.3%. Prostate volume was increased by 6.3% and decreased by 12.3%. The mean dose of the rectum was higher in the CBCT than in the pCT, and V40 (equivalent to 40 Gy) of the bladder showed a reduction in all treatment regimens in the CBCT than in the pCT. Conformity treatment and homogeneity index of PTV showed better results in all treatment regimens using pCT than CBCT. It was found that the dose distribution of the pelvic internal organs varied greatly according to the patient 's condition and pretreatment.
최근 들어 새로운 방사선치료법인 강도변조방사선치료가 많은 종류의 종양치료에 적용되어 치료에 따른 부작용을 줄이고 치료율을 향상시키려는 노력들이 이루어지고 있다. 특히 전립선암에서 강도변조방사선치료법을 적용하여 삼차원입체조형치료에 비해 향상된 선량분포와 이로 인한 주위 정상조직의 방사선량을 줄임으로써 더욱 많은 양의 방사선을 전립선에 투여할 수 있고 이로 인하여 치료율의 상승을 기대할 수 있게 되었다. 저자들은 강도변조방사선치료를 전립선암에 적용하였기에 이에 대한 문헌고찰과 함께 치료과정을 보고하고자 한다.
목 적 : IMRT는 일반적인 3차원 방사선 치료방법보다 Prostate 주변 중요장기(OAR)에 대한 방사선 부작용을 줄이고 Prostate Cancer에 더 많은 선량을 투여할 수 있기 때문에 Prostate Cancer Therapy에 광범위하게 사용되었다. 장비 및 치료 기술의 많은 진보로 인하여 최근에는 체적 변조 아크 치료(VMAT)가 널리 사용되어지고 있으며, VMAT은 IMRT에 비해 치료 시간을 최대 55 % 단축시킬 수 있어 치료 중 움직임에 대한 오차를 최소화 할 수 있는 장점이 있다. 대상 및 방법 : 본 실험에서는 Prostate Cancer 환자 10명에 대해 LN가 포함된 치료 Group 5명과, LN가 포함되지 않는 Group 5명으로 1 Arc와 2 Arc 치료 계획 방법을 총 4 Group으로 분류하여 MU, DVH값들을 비교 분석 하였고, ArcCHECK과 MapCHECK을 사용하여 DQA 관련 측정을 수행했다. 결 과 : Prostate patients의 Target과 OAR 선량분포 결과는 다음과 같다. $D_{max}$는 4개 Group이 100~110 % 범위로 나타났으며, Hot spot인 110 % 이상은 보이지 않았다. LN가 없는 Only-Prostate($P_1$, $P_2$)의 Target $D_{98%}$ 분포도는 1 Arc plan($P_1$)보다 2 Arc plan($P_2$)에서 조금 더 좋은 결과 값을 보였다. LN-Prostate($P_{L1}$, $P_{L2}$) Group에서의 Target $D_{98%}$ 분포도는 1 Arc plan($P_{L1}$)보다 2 Arc plan($P_{L2}$)이 더 좋은 결과 값을 보였고, 1 Arc plan($P_{L1}$)에서는 Target $D_{98%}$의 처방 선량 값에 도달하지 못했다. 또한, OAR에서는 Only-Prostate($P_1$, $P_2$) Group에서 1 Arc와 2 Arc Plan 모두 처방 선량 값에 만족했고, LN-Prostate 1 Arc($P_{L1}$) Plan은 OAR의 처방 선량치보다 높은 선량 값을 보였다. DQA 측정에서 계산된 ArcCHECK과 MapCHECK의 Gamma evaluation pass rate는 99 %보다 조금 더 높았고, CC04 Ion-chamber를 이용한 점 선량(Point Dose) 측정값의 평균 오차 범위는 1 % 미만이었다. 결 론 : 본 연구에서 분석한 결과, Only-Prostate($P_1$, $P_2$) Group의 경우는 두 plan의 선량이 비슷했지만, 치료시간과 MU값 등을 고려했을 때 1 Arc 치료 방법($P_1$)이 더 적합하였으며, LN-Prostate($P_{L1}$, $P_{L2}$) Group에서는 Target $D_{98%}$와 OAR의 처방 선량에 만족하는 2 Arc 치료 방법($P_{L2}$)이 더 적합한 결과를 보였다.
Purpose: To evaluate the effect of common three photon energies (6-MV, 10-MV, and 15-MV) on intensity-modulated radiation therapy (IMRT) plans to treat prostate cancer patients. Materials and Methods: Twenty patients with prostate cancer treated locally to 81.0 Gy were retrospectively studied. 6-MV, 10-MV, and 15-MV IMRT plans for each patient were generated using suitable planning objectives, dose constraints, and 8-field setting. The plans were analyzed in terms of dose-volume histogram for the target coverage, dose conformity, organs at risk (OAR) sparing, and normal tissue integral dose. Results: Regardless of the energies chosen at the plans, the target coverage, conformity, and homogeneity of the plans were similar. However, there was a significant dose increase in rectal wall and femoral heads for 6-MV compared to those for 10-MV and 15-MV. The $V_{20Gy}$ of rectal wall with 6-MV, 10-MV, and 15-MV were 95.6%, 88.4%, and 89.4% while the mean dose to femoral heads were 31.7, 25.9, and 26.3 Gy, respectively. Integral doses to the normal tissues in higher energy (10-MV and 15-MV) plans were reduced by about 7%. Overall, integral doses in mid and low dose regions in 6-MV plans were increased by up to 13%. Conclusion: In this study, 10-MV prostate IMRT plans showed better OAR sparing and less integral doses than the 6-MV. The biological and clinical significance of this finding remains to be determined afterward, considering neutron dose contribution.

Purpose: To study the long-term outcomes and tolerance in our patients who received dose escalated radiotherapy in the early salvage post-prostatectomy setting. Materials and Methods: The medical records of 54 consecutive patients who underwent radical prostatectomy subsequently followed by salvage radiation therapy (SRT) to the prostate bed between 2003-2010 were analyzed. Patients included were required to have a pre-radiation prostate specific antigen level (PSA) of 2 ng/mL or less. The median SRT dose was 70.2 Gy. Biochemical failure after salvage radiation was defined as a PSA level >0.2 ng/mL. Biochemical control and survival endpoints were analyzed using the Kaplan-Meier method. Univariate and multivariate Cox regression analysis were used to identify the potential impact of confounding factors on outcomes. Results: The median pre-SRT PSA was 0.45 ng/mL and the median follow-up time was 71 months. The 4- and 7-year actuarial biochemical control rates were 75.7% and 63.2%, respectively. The actuarial 4- and 7-year distant metastasis-free survival was 93.7% and 87.0%, respectively, and the actuarial 7-year prostate cancer specific survival was 94.9%. Grade 3 late genitourinary toxicity developed in 14 patients (25.9%), while grade 4 late genitourinary toxicity developed in 2 patients (3.7%). Grade 3 late gastrointestinal toxicity developed in 1 patient (1.9%), and grade 4 late gastrointestinal toxicity developed in 1 patient (1.9%). Conclusion: In this series with long-term follow-up, early SRT provided outcomes and toxicity profiles similar to those reported from the three major randomized trials studying adjuvant radiation therapy.
