The Role of Enzyme Replacement Therapy in Fabry Disease in Cardiology Perspective

  • Fabry disease is a hereditary lysosomal storage disorder caused by the reduction or absence of lysosomal enzyme alpha-galactosidase A and the accumulation of glycosphingolipids, such as globotriaosylceramide (Gb3), in various organs, including the heart. The prevention of cardiac involvement in Fabry disease can only be achieved by enzyme replacement therapy (ERT), and the method of assessing the efficacy of ERT should be confirmed. Changes in the electrocardiogram, such as the shortening of PQ interval, prolongation of QTc and repolarization abnormalities as well as left ventricular hypertrophy in voltage criteria, can be used to identify Fabry disease patients; however, the usefulness of electrocardiograms for evaluating the efficacy of ERT is limited. The assessment of left ventricular hypertrophy using echocardiography has been established to evaluate the efficacy of ERT during long-term period. A new technique involving speckled tracking method might be useful for detecting early cardiac dysfunction and identifying the effect of ERT for a relatively short period. The estimation of left ventricular hypertrophy using cardiac magnetic resonance (CMR) is also useful for assessing the efficacy of ERT. Identifying late gadolinium enhancement in CMR may affect the effectiveness of ERT, and the new technique of T1 mapping might be useful for monitoring the accumulation of Gb3 during ERT. Histopathology in cardiac biopsy specimens is another potentially useful method for identifying the accumulation of GB3; however, the use of histopathology to evaluate of the efficacy of ERT is limited because of the invasive nature of an endomyocardial biopsy.

A Clinical Analysis of Surgically Managed Primary Spondylitis (수술 치료를 받은 원발성 척추염 환자의 임상적 고찰)

  • Objective : To assess the surgical outcome for patients with primary spondylitis who were treated surgically. Materials and Methods : We retrospectively analyzed the clinical characteristics of 19 patients who underwent surgical treatment from september 1997 to October 1999 in our department. Results : The 19 patients presented 13 tuberculous spondylitis and 6 pyogenic spondylitis. The male to female ratio was 1.4 : 1 and average age 48.4 years(range 15-68 years). The most prevalent location was thoracic region(47%) and paraparesis was frequently seen in patients with middle and lower spinal lesions. Operative approaches were either anterior(13) or posterior(6). All patients with neurologic deficits improved after surgery. Autogenous rib and/or iliac strut bone grafting was performed, followed by spinal instrumentation. Solid bone fusion was obtained in all patients. There was no need for prolongation of duration of antituberculous drug therapy and no increased incidence of secondary infection due to spinal instrumentation. Conclusion : From the results, it may be advised that patients of primary spondylitis who had neurologic deficit should receive an aggressive opeation in their early stage.

Antitumor and Immunological Activities of ${\gamma}$-ray Irradiated Lipopolysaccharide Extracted from proteus vulgaris RH-90 (Proteus vulgaris RH-90에서 추출하여 감마선 조사시킨 Lipopolysaccharide(LPS)의 항암 및 면역활성에 미치는 영향)

  • The aims of this study were investigated the antitumor effects and immunological activities of lipopolysaccharides (LPS) extracted from Proteus vulgaris RH-90 toward sarcoma-180 cells. LPS extracted from Proteus vulgris RH-90 was irradiated with gamma ray for detoxification. The tumor incidence of sarcoma-180 occurs all group which injected with gamma ray irradiated LPS and tumor of sarcoma-180 was necrotized with breeding in the injected group of l0$\mu\textrm{g}$ LPS. The inhibition ratio of tumor growth showed at the highest level of 60.88% when 5$\mu\textrm{g}$ gamma ray irradiated LPS was injected into mice. The prolongation ratio of life showed 20.72% when injected into mice with gamma ray irradiated LPS of 5$\mu\textrm{g}$. In the effect of immunological activity, the number of circurating leucocyte and peritoneal exudate cells were increased significantly in the treatment group than that control group, and dose-dependent response indicated by the increase of weights of immunorgans which revealed the improvement of immunity. The effect of macrophage on phagocytes, there were not found the differences between phagocytic and corrected phagocytic index.

  • This paper describes the information for quantitative simulation of weakly ionized plasma. We should grasp the meaning of the plasma state condition to utilize engineering application and to understand materials of plasma state. In this paper, the drift velocity of electron in $SF_6+O_2$ mixture gas calculated for range E/N values l~900[Td] at the temperature is 300[$^{\circ}K$] and pressure is 1[Torr], using a set of electron collision cross sections determined by the authors and the values of drift velocity of electrons are obtained for TOF, PT, SST sampling method of Backward Prolongation by two term approximation Boltzmann equation method. It has also been used to predict swarm parameter using the values of cross section as input. The result of Boltzmann equation, the drift velocity of electrons, has been compared with pure $SF_6$, pure $O_2$ and mixture gas.

Comparison of Adding Clonidine Versus Epinephrine into Local Anesthetics in Brachial Plexus Block (상박신경총 차단에서 첨가한 Clonidine과 Epinephrine의 비교)

  • The effects of either clonidine or epinephrine into local anesthetics administered into brachial plexus sheath were evaluated in 42 patients who underwent surgery of the upper limb. All patient received 0.5 ml/kg of 2:1 mixture of bupivacaine and lidocaine injected into the brachial plexus sheath, using the subclavian perivascular technique. The patients were randomly allocated into two groups; Group I(n=25) received $150{\mu}g$ of clonidine hydrochloride, and Group II(n=27) received $200{\mu}g$ of epinephrine. The duration of analgesia and the degree of sedation reflecting the systemic effect of clonidine were assessed. The block produced by the addition of clonidine was longer($100.3{\pm}469.8$ vs $648.8{\pm}192.1$ min) and superior to that by epinephrine(p < 0.05). The highest degree of sedation was achieved about 20 minutes after block, which roughly equals the time required for intramuscular clonidine to show the similar effect. The author concludes that the injection of clonidine mixed to local anesthetics into the brachial plexus sheath prolongs analgesia than that of epinephrine, but this prolongation may be due to the systemic effect of clonidine.

  • A review team has been set up in Yong Nahm central Bureau Dong Bang Life Insurance Company to study on yearly treands and Mortalities of life insureds according to the causes of deaths. The subjects included in this study were the Life insureds in these area offices and one Who died within 2 years after the admission during 17 months period between 1984-4-1 and 1985-8-31. In cases of the insurance for educational fund, the subjects were the applicants who pay the dues of insurances instead of the insured youngsters. 1) The most frequent cause of death was malignant newgrowth, the next was heart disease, the third was central nervous vascular system diseases, and the fourth was Hepatitis and Liver cirrhosis. 2) The previous Leading cause of deaths were infectious diseases, Parasitic disease or Respiratory diseases, Such as Pneumonia bronchitis, Tuberculosis, Since remarkable prolongation of the longevity of Korean population in recent years, this has been showed marked changes after 1980, this phenomenon would be in accordance with Bogue's theory. 3) The authors hoped to extend this study to get more clear views, because it covered only a short period and very limitted areas of subjects to make a difinite conclusion on this time.

Influence of Panax Ginseng on the Hypothermia in Rats Elicited by Various Drugs (인삼(人蔘)의 각종약물투여(各種藥物投與)로 인(因)한 체온하강(體溫下降)에 미치는 영향(影響))

  • Attempts have been made upon the temperature response of the rat induced by some central nervous system depressants as well as stimulants, so as to secure some hidden facets of Panax Ginseng acting upon central nervous system. Although considerable works have been done with regard to Panax Ginseng, it is quite apparent that neither definite implication in terms of its effective chemical constituents is with us nor its pharmacological activity thus far. The author could, however, arrive at some results through procedures preceded by intraperitoneal administration of various drugs in combination with Panax Ginseng in albino rats, that is: (1) Nembutal and chlorpromazine displayed a highly inhibitory effect upon temperature response in the presence of Panax Ginseng, while meprobamate, reserpine, phenacetin and aspirin exerted potentiation actions upon hypothermia. Phenobarbital, serotonin and histamine, on the contrary, did not appear to produce any effect of significance. (2) Nembutal with Panax Ginseng caused prolongation of hypnosis in rat, whereas sodium phenobarbital did not have any effect on it. (3) $LD_{-50}$ in each experimental group of administration of central nervous system stimulants such as strychnine, picrotoxin with Panax Ginseng, necessitated marked increase in the lethal doses. The observations from this study seemed to imply that the complicated mechanism of action of Panax Ginseng might be referred to both central nervous depressive action and influence to basal metabolic rate of mammalian.

  • Since the Committee for Proprietary Medicinal Products (CPMP) of the European Union issued in 1997 a "points to consider" document for the assessment of the potential for QT interval prolongation by non-cardiovascular agents to predict drug-induced torsades de pointes (TdP), the QT liability has become the critical safety issue in the development of pharmaceuticals. As TdP is usually linked to delayed cardiac repolarization, international guideline (ICH S7B) has advocated the standard repolarization assays such as in vitro IKr (hERG current) and in vivo QT interval, or in vitro APD (as a follow up) as the best biomarkers for predicting the TdP risk. However, the recent increasing evidence suggests that the currently used above biomarkers and/or assays are not fully predictive for TdP, but also does not address potential new druginduced TdP due to the selective disruption of hERG protein trafficking to the cell membrane or VT and/or VF with QT shortening. There is, therefore, an urgent need for other surrogate markers or assays that can predict the proarrhythmic potential of drug candidate. In this review, we provide an ideal pre-clinical strategy to predict the potentials of QT liability and lethal arrhythmia of the drug candidates with recent issues in this field in mind, not at the expense of discarding therapeutically innovative drugs.

Time-Resolved Photoexcitation Dynamics of Electrical Conductivity of Magnetic Organic Superconductor λ-(BETS)2Fe0.45Ga0.55Cl4

  • The time-resolved photoexcitation dynamics of electrical conductivity of the magnetic organic superconductor ${\lambda}-(BETS)_2Fe_{0.45}Ga_{0.55}Cl_4$ has been studied with a nanosecond visible laser pulse at its three different phases, i. e., metallic phase, superconducting phase and insulating phase. A transient increase of the resistance is induced by photoirradiation at all the temperatures measured for all three phases, but the decay profile shows a significant temperature dependence. The relaxation rate in the metallic and insulating phase are different from each other, and the decay time is relatively faster and almost constant in the metallic phase. However, a prolongation of the relaxation time is observed at temperature just around the narrow superconducting phase. Nonbolometric (nonthermal) origin of the observed photoresponse of the electrical conductivity is confirmed in the superconducting phase.

Antitumor and Immunopotentiating Effects of Manda Enzyme

  • This study was to evaluate the antitumor and immunopotentiation effects of Manda Enzyme (ME). Oral administration of ME (0.2ml/mouse) to tumor bearing mice significantly prolonged survival rate compared to the control group with the prolongation ratio of 40%. The inhibition ratios for the first and the second experiments were 51.8% and 26.4%, respectively. Only the spleen index was significantly increased in the MEF-treated group, but not in the control group. Gamma globulin level of the MEF-treated group was elevated when mice were injected with sarcoma-180 cells on the left groin. Activities of natural killer (NK) and lymphokineactivated killer (LAK) cells were observed by $^{51}Cr-release$ method. Activities of NK cell against YAC-1 cells were significantly increased in the MEF treated group. And LAK cell activities against P815 cells were also significantly increased in the experimental group. These observations, therefore, suggest that ME may have an anticancer effect and immunopotentiating effect in vivo.
